Background: Lynch Syndrome (LS) is a familial cancer condition caused by germline mutations in DNA mismatch repair genes. Individuals with LS have a greatly increased risk of developing colorectal cancer (CRC) and it is therefore important to identify mutation carriers so they can undergo regular surveillance. Tumor DNA from LS patients characteristically shows microsatellite instability (MSI). Our aim here was to screen young CRC patients for MSI as a first step in the identification of unrecognized cases of LS in the Saudi population. Materials and Methods: Archival tumor tissue was obtained from 284 CRC patients treated at 4 institutes in Dammam and Riyadh between 2006 and 2015 and aged less than 60 years at diagnosis. MSI screening was performed using the BAT-26 microsatellite marker and positive cases confirmed using the pentaplex MSI analysis system. Positive cases were screened for BRAF mutations to exclude sporadic CRC and were evaluated for loss of expression of 4 DNA mismatch repair proteins using immunohistochemistry. Results: MSI was found in 33/284 (11.6%) cases, of which only one showed a BRAF mutation. Saudi MSI cases showed similar instability in the BAT-26 and BAT-25 markers to Australian MSI cases, but significantly lower frequencies of instability in 3 other microsatellite markers. Conclusions: MSI screening of young Saudi CRC patients reveals that approximately 1 in 9 are candidates for LS. Patients with MSI are strongly recommended to undergo genetic counselling and germline mutation testing for LS. Other affected family members can then be identified and offered regular surveillance for early detection of LS-associated cancers.

A cup burner experiment was performed to investigate the effect of the oxidizer velocity and concentration on flame instability near extinction. Heptane was used as a fuel and air diluted by nitrogen and carbon dioxide was used in the oxidizer stream. Two types of flame instabilities at the flame base and at axial downstream were observed near extinction. The instability at the flame base could be characterized by cell, swing, and rotation modes, and the cell mode changed to the rotation mode through the swing mode as the oxidizer velocity increased. To assess the parameters for the flame instability, the initial mixture strengths, Lewis number, and adiabatic flame temperature were investigated under each condition. The Lewis number might be the most important among them, but it is impossible to generalize because of the insufficient number of cases. Furthermore, the axial periodic flickering motion disappeared at low and high oxidizer velocities near extinction. This resulted from the fact that low oxidizer velocity induced evaporated fuel velocity below the critical velocity and high velocity made the reacting fuel velocity comparable.
A fiber-phase-noise compensating system was constructed for a 1.4 GHz reference frequency transferred through a 13-km-long fiber spool. The transfer instability was dependent on the temperature variation of the compensating system. With the room temperature variation stabilized within $0.3^{\circ}C$, the transfer instability was $4.6{\times}10^{-14}$ at 0.8 s of average time and $2.5{\times}10^{-16}$ at 1000 s of average time with the fiber phase noise compensated. However, with the room temperature changed by $3.5^{\circ}C$, the transfer instability was $6.8{\times}10^{-14}$ at 1.2 s of average time and $3.0{\times}10^{-15}$ at 1000 s of average time. From this result, the temperature stability condition for the experimental setup could be determined to obtain a transfer instability of $10^{-16}$ at 1000 s of average time.

Objectives: Parkinson's disease is the neurodegenerative disease that affects both motor and non-motor function, including postural instability. Camptocormia is an abnormal condition in which the thoracolumbar spine bends forward during walking or standing. However, the treatment options are limited and often not effective. The purpose of this study was to report on the Korean medical treatment of a Parkinson's disease patient with postural instability who presented with camptocormia. Methods: We used Korean medical treatment including herbal medicine (Jemageopung-tang), acupuncture and pharmacopuncture therapy to the patient who was admitted to the hospital for 21 days. The clinical symptoms were assessed with the Unified Parkinson's Disease Rating Scale (UPDRS) and walking time without assistance. Results: After treatment, the UPDRS Parts 2 and 3 scores were decreased from 5 to 3 and 20 to 9, respectively. Also, the walking time without assistance was improved. Conclusion: This study suggested that Korean medical treatment could be an effective option for treating Parkinson's disease with postural instability.
The purpose of this study was to compare the trunk and lower extremity muscle activity induced by six different conditions floor, intensity 0, 1, 3, 5 of whole body vibration (WBV), and Swiss ball during bridging exercise. Surface electromyography (EMG) was used to measure trunk and lower extremity muscles activity. Ten elderly women were recruited from Hong-sung Senior Citizen Welfare Center. The collected EMG data were normalized using reference contraction (during floor bridging) and expressed as a percentage of reference voluntary contraction (%RVC). To analyze the differences in EMG data, the repeated one-way analysis of variance was used. A Bonferroni's correction was used for multiple comparisons. The study showed that EMG activity of the rectus abdominis, external oblique, internal oblique, erector spinae and rectus abdominis muscles were not significantly different between six different conditions of during bridging exercise (p>.05). However, there were significantly increased EMG activity of the rectus femoris (p=.034) in the WBV intensity 0, 1, 3, and 5 conditions compared with the floor bridging condition. EMG activity of the medial gastrocnemius were significantly increased in the WBV intensity 0, 1, 3, 5 and Swiss ball conditions compared with the floor bridging condition. Future studies are required the dynamic instability condition such as one leg lifting in bridging.

Characteristics of cryogenic single jet flow were investigated. Liquid nitrogen was injected into a high-pressure chamber and formed single jet. Ambient condition around jet was changed from subcritical to superctirical condition of nitrogen. Injector geometries also were changed. A shape of the jet and core diameter were measured by flow visualization technique, and core spreading angle was calculated. Flow instability was found at atmospheric pressure condition. As ambient pressure increased, core spreading angle was increased and maintained after certain pressure.

The similarity in internal flow of solid and hybrid rocket suggests that hybrid rocket combustion can be susceptible to instability due to vortex sheddings and their interaction. This study focuses on the evolution of interaction of vortex generated in pre-chamber with other types of vortex in the combustor and the change of combustion characteristics. Baseline and other results tested with disks show that there are five different frequency bands appeared in spectral domain. These include a frequency with thermal lag of solid fuel, vortex shedding due to obstacles such as forward, backward facing step and wall vortices near surface. The comparison of frequency behavior in the cases with disk 1 and 3 reveals that vortex shedding generated in pre-chamber can interact with other types of vortex shedding at a certain condition. The frequency of Helmholtz mode is one of candidates resulting to a resonance when it was excited by other types of oscillation even if this mode was not discernable in baseline test. This selective mechanism of resonance may explain the reason why non-linear combustion instability occurs in hybrid rocket combustion.
The concentrations of inert gases ($N_2$, Ar, $CO_2$ and He) required to induce the flame instabilities such as swing, rotation, lifted and blow-out were measured in a cup burner nonpremixed flames for $CH_4$ and $C_3H_8$ fuels. Quantitative differences in the extinguishing concentration with fire suppression criteria (i.e. blow-out or onset of flame instability) were also examined. It was found that the difference in extinguishing concentration was increased with the appearance of lifted flame and the low extinguishing performance of inert gaseous. The maximum difference in extinguishing concentration with the suppression criteria was approximately 35% at the highest fuel velocity condition (1.3 cm/s) for the $C_3H_8$-air nonpremixed flame. It can be also expected that the extinguishing concentration by the criteria based on the onset of flame instability will provide the useful information from the viewpoint of the accurate and economical design concentration.

Although the stall stagnation phenomena have often been experienced in site and also analytically in numerical experiments in surges in systems of compressors and flow paths, the fundamental causes have not been identified yet. In order to clarify the situations, behaviours of infinitesimal disturbance waves superposed on a main flow were studied in a simplified one-dimensional flow model. A ratio of the amplifying rate of the system instability to the characteristic slope of the compressor element was surveyed as the instability enhancement factor. Numerical calculations have shown the following tendency of the factor. In the situation where both the sectional area ratio and the length ratio of the delivery flow-path to the suction duct are sufficiently large, the enhancement factors are greater in magnitude, which means occurrence of ordinary deep surges. However, in the situation where the area ratio and/or the length ratio is relatively smaller, the enhancement factor tends to lessen significantly, which situation tends to suppress deep surges for the same value of the characteristic slope. It could result in the stall stagnation condition. In the domain of area ratio vs. length ratio of the delivery duct to the suction duct, contour-lines of the enhancement factor behave qualitatively similar to those of the stall stagnation boundaries of a fan analytically obtained, suggesting that a certain range of the enhancement factor values could specify the stagnation occurrence. The significant decreases in the factors are observed to accompany appearances of phase lags and travelling waves in the wave motions, which macroscopically suggests breaking down of the complete surge actions of filling and emptying of the air in the delivery duct. The strength of the action is deeply related with acoustic interferences and is evaluated in terms of the volume-modified reduced resonance frequency proposed by the author. These observations have shown the fundamental cause and the sequence of the stall stagnation in principle.
Since the kinetic energy is dissipated through plastic deformation energy generated in expanding process of the tube by a die. In order to successfully absorb the kinetic energy there should be no buckling in the expansion tube during expanding process. The buckling instability of the expansion tubes is affected by the initial boundary conditions, tube thickness and length. In this study, the effects of the tube thickness except length and initial boundary condition on the buckling instability are studied using a finite element method. In addition, Analysis procedure for nonlinear post-buckling analysis of expansion tube is established. There are three kinds of finite element analysis procedures for buckling analysis of expansion tube, quasi-static analysis, linear buckling analysis and nonlinear post-buckling analysis. The effect of the geometry imperfections defined as linear superimposition of buckling modes is considered in the nonlinear post-buckling analysis. The results of finite element analysis indicate that the buckling load increase with increase of thickness of tube and geometry imperfection. Finial buckling shapes are changed with respect to the geometry imperfection.
