노력성 호흡운동에 의한 편마비환자의 보행 비대칭율 개선 (The improvement of gait asymmetry ratio for hemiplegic patients by forceful respiratory exercise)

  • The regain of independent ambulatory ability is a important goal in the rehabilitation program of hemiplegic patient. Not only the function of lower extremity muscles, but also trunk muscles which stabilize extremities and pelvis, are important factors in normal gait. Therefor, it is necessary to develop an effective program which can improve muscle strength and symmetric activity of trunk muscles. The purpose of this study was to evaluate the influence of trunk muscle strengthening by forceful respiratory exercise on the gait asymmetry ratio in hemiplegic patient. 45 Hemiplegic patients due to stroke was randomized in 3 groups, forceful expiratory training(FET), forceful inspiratory training(FIT) and control group. In the experimental groups, ordinary physical therapy with forceful expiratory training and forceful inspiratory training for 20 minutes duration 3 times per week for 6 weeks were respectively performed. In the control group, only ordinary physical therapy was done. Before and after experiments, temporal-spatial gait parameters was measured in all patients. The data of 28 patients who carried out the whole experimental course were statistically analysed. The results of these experiment are as follows : 1. In comparison of difference of single support time asymmetry ratio among 3 groups, the FET group was significantly decreased than the control group (p<.05). 2. In comparison of difference of step length asymmetry ratio among 3 groups, the FIT group was significantly decreased than the control group (p<.05). Based on these results, it is concluded that the forced respiratory exercise program for 6 weeks can be improve the gait asymmetry ratio in hemiplegic patients. Therefore, the forced respiratory exercise is useful to improve the walking ability in hemiplegic patients. Since this study dealt only with the patients who could walk more than 3 meters in distance on floor independently, the further study for evaluating the influence of the forceful respiratory exercise on patients with acute stage stroke and also the development in various methods of use are expected.

만성기침환자에서 비강최대흡기유속 측정의 의의 (Clinical Significance of Nasal Peak Inspiratory Flow Rate in Patients with Chronic Cough)

  • 연구배경 : 만성기침환자에 있어서 비폐색과 같은 주관적인 상기도 증상을 객관적인 지표로 나타내어 비교할 수 있게 하기 위해, 최근 비교적 높은 재현성을 보이는 것으로 보고된 비강최대흡기유속을 측정하여 만성기침환자들에서 비폐색을 객관적으로 측정해보고자 하였다. 방 법 : 정상인 7명에서 하루 2회씩 3일 동안 휴대용 폐활량 측정기를 이용하여 비강최대흡기유속과 폐기능검사를 시행하였다. 이들 7명을 포함한 총 32명의 정상인들과 만성기침환자 32명을 선별하여 최초 첫 방문 시에 비강최대흡기유속과 폐기능검사를 시행하였다. 결 과 : 정상인 7명에 대해서는 비교적 일관된 비강최대흡기 유속과 $FEV_1$ 및 FVC를 얻을 수 있었다. 만성기침환자는 남자 14명, 여자 18명으로 총 32명이었으며 나이는 $41.4{\pm}15.9$세였으며, 정상인은 남자 22명, 여자 10명으로 총 32명이었으며 나이는 $39.8{\pm}18.6$세였다. 만성기침환자군과 정상인군 사이의 연령 및 폐기능검사 상의 차이는 없었다. 만성기침환자들에서 비강최대흡기유속은 $2.25{\pm}0.68\;L/sec$, 정상인들에서는 $2.75{\pm}1.00\;L/sec$로 만성기침환자에서 의미 있게 낮았다(p=0.02). 만성기침환자들 중, 후비루종후군환자들이 27명으로 가장 많았으며, 비강최대흡기유속은 $2.18{\pm}0.66\;L/sec$로 정상인 $2.75{\pm}1.00\;L/sec$ 보다 더욱 의미 있게 낮았다(p=0.006). 결 론 : 비강최대흡기유속은 만성기침환자들에 있어서 주관적인 비폐색 증상을 객관적으로 보여주는 지표이다. 비강최대흡기유속은 비교적 재현성 있는 지표이며, 정상인에 비해서 만성기침환자, 특히 원인 질환이 후비루 증후군인 환자들에서 특히 감소되었다. 하지만, 양군간의 값이 중복되는 범위가 많아서 진단 및 감별진단의 지표로서의 가치는 좀 더 많은 대상에 대한 연구에 의하여 확인되어야 할 것으로 생각된다.

  • Purpose : Stabilization exercise and respiratory muscle training are used to train trunk muscles that affect postural control and respiratory function. However, there have been no studies that combine stabilization exercise and respiratory muscle training. The purpose of this study is to investigate effects of stabilization exercise with and without respiratory muscle training on respiratory function and postural sway. Methods : Fifteen healthy adults were recruited for this experiment. All the subjects performed stabilization exercise with and without respiratory muscle training. For stabilization exercise with respiratory muscle training, the subjects sat on a gym ball wearing a stretch sensor. The subjects inspire maximally as long as possible during lifting one foot off the ground, alternately for 30 seconds. The stretch sensor was placed on both anterior superior iliac spine (ASIS), and the stretch sensor was used to monitor inspiration. For stabilization exercise without respiratory muscle training, the subjects sat on a gym ball and lifted one foot off the ground, without respiratory muscle training. Kinovea program used to investigate postural sway tracking during exercise. The maximum inspiratory pressure (MIP) and maximum expiratory pressure (MEP) were measured using a spirometer to investigate changes of respiratory muscle strength before and after exercise. A paired t-test was used to determine significant differences postural sway tracking, MIP, and MEP between stabilization exercise with and without respiratory muscle training. Results : There were significantly lower a distance of postural sway tracking during stabilization exercise with respiratory muscle training, compared with stabilization exercise without respiratory muscle training (p<.05). The MIP and MEP were significantly increased after stabilization exercise with respiratory muscle training compared with before stabilization exercise with respiratory muscle trianing (p<.05). Conclusion : The results of this study suggest that stabilization exercise with repiratory muscle training would be recommended to improve postural control and respiratory muscle strength.

정상 아동과 뇌성마비 아동의 감소된 횡격막 움직임 및 호흡 기능의 비교 (Comparisons of Diaphragm Movement and Pulmonary Function Between Normal Children and Children With Cerebral Palsy)

  • Background: Research efforts to improve the pulmonary function of children with cerebral palsy (CP) need to focus on their decreased diaphragmatic ability compared to normal children. Real-time ultrasonography is appropriate for demonstrating diaphragmatic mechanisms. Objects: This study aimed to compare diaphragm movement, pulmonary function, and pulmonary strength between normal children and children with CP by using ultrasonography M-mode. The correlation between general characteristics, diaphragm movement, pulmonary function, and pulmonary strength was also studied. Methods: The subjects of this study were 25 normal and 25 CP children between five and 14 years of age. Diaphragm movement was measured using real-time ultrasonography during quiet and deep breathing. Pulmonary function (such as forced expiratory volume in one second; FEV1 and peak expiratory flow; PEF) and pulmonary strength (such as maximum inspiratory pressure; MIP and maximum expiratory pressure; MEP) were measured. A paired t-test and Spearman's Rho test, with a significance level of .05, were used for statistical analysis. Results: The between-group comparison revealed that normal children had significantly greater diaphragm movement, FEV1, PEF, MIP, and MEP (p<.05) than CP children. The results showed that general characteristics were significantly related to FEV1, PEF, MIP, and MEP (p<.05). Conclusion: In clinical settings, clinicians need to concern decreased diaphragm movement, pulmonary function, and pulmonary strength in CP group compared to normal children.

정상 성인의 신체조성과 폐 기능의 연관성 (Correlation between Body Composition and Lung Function in Healthy Adults)

  • Purpose : We investigated the correlation between body composition and lung function in healthy adults. Methods : This study included 204 healthy adults in whom all measurements were obtained once, and all data were analyzed using the SPSS software for Windows, version 22.0. Pearson's correlation analysis was performed to determine the correlation between body composition (represented by the total body water, protein mass, soft lean mass, mineral mass, basal metabolic rate, fat-free mass, skeletal muscle mass, and body fat percentage) and lung function (represented by the forced vital capacity [FVC], forced expiratory volume in 1 second [FEV1], the FEV1/FVC ratio, maximum voluntary ventilation [MVV], maximum expiratory pressure [MEP], and the maximum inspiratory pressure [MIP]). All measurements were obtained by two investigators to improve reliability. A significance level of α=.05 was used to verify statistical significance. Results : Among the lung function measurements obtained in both men and women, the FVC, FEV1, MVV, and MIP were positively correlated with the total body water, protein mass, soft lean mass, mineral mass, basal metabolic rate, fat-free mass, and skeletal muscle mass in men (p<.05). The FEV1/FVC ratio was negatively correlated with the total body water, soft lean mass, mineral mass, basal metabolic rate, fat-free mass and the body fat percentage (p<.05). Notably, the FVC, FEV1, and MVV were positively correlated with the total body water, protein mass, soft lean mass, mineral mass, basal metabolic rate, fat-free mass, and skeletal muscle mass in women (p<.05). Conclusion : This study showed a significant correlation between body composition and lung function in healthy adults. In combination with future studies on lung function, our results can provide objective evidence regarding the importance of prevention of lung disease, and our data can be utilized in rehabilitation programs for patients with respiratory diseases.

호흡운동이 뇌졸중 환자의 흉곽 확장과 폐 기능에 미치는 영향 (The Effect of Chest Expansion and Pulmonary Function of Stroke Patients after Breathing Exercise)

  • Purpose: This study examined whether breathing exercises might increase the chest expansion and pulmonary function of stroke patients. Methods: Twenty four patients with stroke were assigned randomly into two groups: a combination of diaphragmatic resistive breathing and pursed-lip breathing exercise (CB) group (n=10) and control group (n=14). The CB group completed a 4-week program of diaphragmatic resistive breathing and pursed-lip breathing exercise. The subjects were assessed using the pre-test and post-test measurements of the chest expansion (length for resting, deep inspiration, deep expiration, deep expiration-inspiration) and pulmonary function (forced vital capacity (FVC), forced expiratory volume at one second (FEV1), FEV1/FVC, peak expiratory flow (PEF), vital capacity (VC), tidal volume (TV), expiratory reserve volume (ERV), inspiratory reserve volume (IRV)). Results: A comparison of the chest expansion between the pre and post tests revealed similar rest, deep inspiration, deep expiration, and deep expiration-inspiration lengths in the CB and control groups (p>0.05). A comparison of the pulmonary function between pre and post tests, revealed significant improvements in the FVC, FEV, PEF, VC, IRV, and ERV in the CB group (p<0.05). There was a significant difference in the FVC, FEV1, PEF, VC and IRV between the 2 groups (p<0.05). Conclusion: These findings suggest that breathing exercise should help improve the pulmonary function, such as the volume and capacity. This suggests that the pulmonary functions of stroke patients might be improved further by a continued respiratory exercise program.

안면견갑상완형 근이영양증 환자에 대한 한의학적 치료 이후 호흡기능 호전에 대한 임상 증례 (A Clinical Case Report on the Improvement of Respiratory Function of a Fascioscapulohumeral Muscular Dystrophy Patient after Korean Medical Treatment)

  • Objectives : The purpose of this study is to report on the improvement of respiratory function of a fascioscapulohumeral muscular dystrophy patient after receiving Korean medical treatment. Methods : This study was carried out on a 60 year-old male patient who suffered from fascioscapulohumeral muscular dystrophy. While we carried out Korean medical treatment, we observed $EtCO_2$ (end-tidal carbon dioxide), Vte (exhaled tidal volume) and PIP (peak inspiratory pressure). Results : $EtCO_2$ count decreased from 34 mmHg to 24 mmHg during the treatment period, and PIP count became stable. Vte count also did not worsen in this study. Conclusions : Based on this study, we concluded that Korean medical treatment could be effective in treating fascioscapulohumeral muscular dystrophy patients with respiratory failure.

  • PURPOSE: This study was conducted to determine correlations between grip and lower limb muscle strength and pulmonary function and respiratory muscle in children with cerebral palsy. METHODS: Subjects were 17 children with cerebral palsy. Inclusion criteria for participation were having GMFCS from I to III grade and ability to independently blow into a spirometer. Pulmonary function and respiratory muscle were measured with a spirometer. All subjects performed maximal expiratory flow maneuvers using a spirometer in order to determine their forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), peak expiratory flow (PEF) and FEV1/FVC, and maximum inspiratory pressure (MIP) and maximum expiratory pressure (MEP). Muscle strength was measured in terms of grip strength and lower limb muscle strength in terms of knee extension strength with a dynamometer and manual digital muscle tester respectively. Data were analyzed using Person product correlation. RESULTS: Grip strength significantly positively correlated with FVC (r=0.95, p<0.01), FEV1 (r=0.95, p<0.01), PEF (r=0.84, p<0.01), MIP (r=0.65, p<0.01) MEP (r=0.71, p<0.01) and lower limb strength with FVC (r=0.72, p<0.01), FEV1 (r=0.69, p<0.01), PEF (r=0.54, p<0.05), and MEP (r=0.69, p<0.01). CONCLUSION: Grip and lower limb muscle strengths of children with cerebral palsy were positively correlated pulmonary function and respiratory muscle.

  • PURPOSE: This study assessed the effect of exercise intervention methods on diaphragm movement, pulmonary function, and pulmonary strength in children with cerebral palsy (CP). METHODS: A total of 28 children with CP were randomly allocated to the general exercise group (n=9, GEG), respiratory exercise group (n=10, REG), and intensive exercise group (n=9, IEG). The exercise intervention was performed for 12 weeks. This study measured diaphragm movement, pulmonary function, and pulmonary strength under two different conditions before and after each exercise. Ultrasonography was used for measuring diaphragm movement, and Pony Fx was used to measure the forced expiratory volume in one second ($FEV_1$), peak expiratory flow (PEF), maximum inspiratory pressure (MIP), and maximum expiratory pressure (MEP). A Mann-Whitney test and ANOVA with a significance level of .05 were used for statistical analysis. RESULTS: Significant change was observed between the REG and the IEG as well as between the GEG and the IEG (p<.05). No significant difference was observed between the GEG and the REG. The diaphragm movement, $FEV_1$ PEF, MIP, and MEP were most improved in the IEG (p<.01). CONCLUSION: This study confirmed that intensive exercise is the most effective treatment method for improving diaphragm movement and respiratory function in CP children.

뇌성마비 아동에서 노력성 폐활량에 영향을 미치는 요인 분석에 관한 연구 (Predictive Factors Affected to Forced Vital Capacity in Children with Cerebral Palsy)

  • Purpose: Children with cerebral palsy generally have a high incidence of respiratory problem, resulted from poor coughing, airway clearance problem, respiratory muscle weakness, kyphoscoliosis and so forth. The purpose of this study is to investigate the possible factors that can be affected to forced vital capacity (FVC) in children with cerebral palsy. Methods: Total thirty six children with diplegic and hemiplegic cerebral palsy were recruited in this study. They were evaluated by general demographic data (i.e., age, gender, body mass index (BMI)) and variables related to respiratory functions (i.e., chest mobility, waist mobility, maximal phonation time, and maximum inspiratory/expiratory pressure (MIP/MEP)). The correlation between forced vital capacity and the rested variables were analyzed, and multiple regression with stepwise method was conducted to predict respiratory function, in terms of FVC as the dependent variable, and demographic and other respiratory variables as the independent variable. Results: FVC showed a significant correlation with waist mobility (r=0.59, p<0.01), maximal phonation time (r=0.48, p<0.05), MIP (r=0.73, p<0.01), and MEP (r=0.60, p<0.01). In addition, the multiple regression analysis model indicated that FVC could be predicted by the assessment of each waist mobility and MIP. Conclusion: These finding suggest that respiratory function is related to body size and respiratory muscle strength, and that BMI, waist mobility, and MIP can be predictable factors to affected respiratory function in term of FVC.
