Oxaliplatin-Induced Peripheral Neuropathy and Quality of Life in Patients with Digestive System Cancer

  • Purpose: This study aimed to identify the levels of oxaliplatin-induced peripheral neuropathy (OXLIPN) and the quality of life (QOL) related to OXLIPN in patients with digestive system cancer. Methods: A total of 83 patients with chemotherapy-induced peripheral neuropathy (CIPN)-related symptoms participated in this study. Data were collected through self-reported questionnaire which were constructed to include general and clinical characteristics, EORTC QLQ-C30, Patient Neurotoxicity Questionnaire (PNQ), and EORTC QLQ-CIPN20. Results: The average scores of OXLIPN upper and lower extremity scale were 30.01 and 29.16, respectively. The average scores of PNQ sensory and motor scale were 2.11 and 1.70, respectively. The mean score of the QLQ-C30 global health status was 54.85, and the range of mean score of the functional and symptom subdomains was 34.85~73.29 and 17.67~53.54, respectively. The CIPN-related symptoms positively correlated with the global health status scale and all subdomains of functional scale, respectively and negatively correlated with fatigue, pain, dyspnea, insomnia, and financial problem subdomains of the symptom scale, respectively. Conclusion: Oncology nurses should pay attention and provide remedies for CIPN symptoms reported by their patients. Nursing interventions should be developed for patients with digestive system cancer to alleviate CIPN and enhance their QOL.

Effect of Light Therapy on Sleep Disturbance and Depression in Climacteric Women (빛 요법이 갱년기 여성의 수면장애와 우울에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of light therapy on sleep disturbance and depression in climacteric women with menopausal symptoms. Methods: The research design was a nonequivalent control group pre test-post test design. Data were collected from September 29, 2013 to November 11, 2013. Participants included 17 climacteric women in an artificial light therapy group, 17 climacteric women in a sun light therapy group and 16 climacteric women in a control group. Measures consisted of the sleep disturbance, depression, melatonin, and serotonin. Results: There was a statistically significant difference of Korean Sleep Scale A (F=53.87, p<.001), and melatonin (F=31.19, p<.001) among three groups. There was a statistically significant difference of Self-Rating Depression Scale (F=121.86, p<.001), and serotonin (F=102.37, p<.001) among three groups. Conclusion: Artificial and sun light therapy can be applied as a supportive nursing intervention to subjects with sleep disturbance and depression in climacteric women with menopausal symptoms. Artificial and sun light therapy is expected to be a complementary alternative intervention for health management of the subjects with sleep disturbance and depression in climacteric women with menopausal symptoms.

Factors Influencing Quality of Sleep of Disturbance Factors Sleep by Inpatients (입원환자의 수면 방해 요인이 수면의 질에 미치는 영향)

  • Purpose. The purpose of this study was to identify the factors affecting quality of sleep and sleep disturbance among inpatient. Methods. A descriptive correlational study was conducted Participants were 200 inpatient in a hospital located in B city. A structured questionnaire was used for data collection. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation, and stepwise multiple regression. Result. Hospitalized patient's score of sleeping quality was 38.26 on an overall scale of 60, indicating it to be higher than 'Medium (30 points). Almost hospitalized patient in this study suffer from sleep disturbances. As results of univariate analyses, Educational level, Departments, Why hospitalized, Insomnia / depression, Chronic Disease, Scale of pain, physical symptoms, environmental factors, emotional factors, and sleep promoting behavior, disturbed sleep among hospitalized. However, the result of stepwise multiple regression analysis identified that physical symptoms, environmental factors, emotional factors, sleep promoting behavior, and Why hospitalized disturbed sleep and were significant score of sleeping quality for hospitalized and these sleep disturbance factors accounted 46.8% of variance of sleeping quality among hospitalized. Conclusion. These findings suggest that hospitalized patients with poor sleep quality should have their health carefully screened for physical symptoms, environmental factors, emotional factors, sleep promoting behavior, and why hospitalized. In addition, we recommend the development of a nursing program for improving sleep quality.

A Literature Review of Korean Medical Studies of Amyotrophic Lateral Sclerosis (ALS) (근위축성 측삭경화증의 한방치료에 관한 문헌적 고찰연구)

  • Objectives : There have been many reports that show that Korean Medical Treatment of ALS patients can maintain or improve their major and minor symptoms, but this has not been reviewed in literature. The aim of this study is to review studies of ALS patients treated with Korean Medicine. Methods : A search was completed of studies containing word ALS in English and in Korean from KIOM OASIS. Results : There were no cases of ALS patients that were fully cured. However with Korean Medical treatment, there were articles that reported improvement or maintenance of major symptoms, recovery of minor symptoms like pain, chest discomfort, insomnia, cough, sputum, etc, and success in the long term care of ALS patients. Conclusions : Although the current study has limitations, our findings support the potential use and effectiveness of Korean Medical Treatment for ALS patients.

Manic Patient with Meningioma Treated with Low dose Risperidone and Valproic Acid (저용량 리스페리돈과 발프로에이트로 치료된 뇌수막종에 의한 조증)

  • We describe the case of a 73 year-old female patient, YSG, who initially presented with a manic episode without any previous psychiatric history and was later diagnosed as having a meningioma in the left frontal lobe. YSG's symptoms were characterized by hyperactivity, insomnia, aggressive behavior with an auditory hallucination. She showed no abnormal signs on a complete neurologic examination. A gadolinium-enhanced MRI study showed a huge, extra-axial mass with homogenous enhancement in the left high convexity of the frontal lobe. Her manic symptoms subsided after administration of risperidone 1mg and valproic acid 500mg daily, for three weeks without surgical resection of the tumor. These findings suggest that YSG's mania might have resulted from the left-sided frontal tumor, and that her symptoms were treated rapidly by small doses of risperidone combined with valproic acid. Medical staff who care for manic patients should be aware of this possibility of a organic lesion without evidence of neurologic disease.

  • UL-syndrome(鬱證) is a syndrome of emotional depression and various physical symptoms generally due to chronic psychological stress, without constitutional abnormality. These are cases of female patients suffering from headache, chest discomfort, heart burning, insomnia, dyspepsia, anorexia, fatigue and some other minor symptoms due to chronic stress. We diagnosed these cases as UL-syndrome and treated them with oriental medicine, specifically Gamisoyosan(加味逍遙散). This paper describes the symptoms that the patients suffered, the treatment used, and identifies stages in the development of UL-syndrome, and suggests a stage in which Gamisoyosan is efficacious.

  • Objective: This study reports on a treatment case of Soojeom-san plus Jeungmiyijin-tang (SJJI) on gastroesophageal reflux disease with hiatal hernia. Method: We considered a male patient suffering from gastroesophageal reflux disease with hiatal hernia because of blood stasis and damp-heat of the spleen and stomach pattern and prescribed SJJI. The progress was evaluated using the frequency scale for the symptoms of gastroesophageal reflux disease (FSSG) score, and the visual analogue scale for abdominal pain, sore stomach, and other symptoms. Results: The patient's symptoms, which included abdominal pain, sore stomach, dyspepsia, anorexia, insomnia, etc., almost disappeared. Conclusion: SJJI can be used for patients who have a chronic condition of gastroesophageal reflux disease with hiatal hernia because of blood stasis and a pattern of damp-heat of the spleen and stomach.

A Case Study of a Taeeumin Patient with Meniere's Disease Treated with Cheongsimyeonja-tang (청심연자탕으로 호전된 메니에르병 환자 치험 1례)

  • Objectives The aim of this study was to report significant improvement of vertigo, tinnitus and aural fullness after treatment with Cheongsimyeonja-tang in a Taeeumin patient with Meniere's disease. Methods The patient was diagnosed with Taeeumin Dry-heat symptomatic pattern and treated with Cheongsimyeonja-tang and acupuncture. The patient's subjective symptoms of vertigo, tinnitus and aural fullness were observed using Global Assessment Scale (GAS) during the treatment period. Results & Conclusions The symptoms of vertigo, tinnitus and aural fullness decreased from GAS 100 to GAS 0 for seven weeks. Furthermore, the patient's symptoms of insomnia, urinary frequency, chest discomfort and fatigue were reported to be improved after treatment. In conclusion, this study shows that Sasang Constitutional Medicine can be effective treatment for Taeeumin patient with Meniere's disease diagnosed as Dry-heat symptomatic pattern.

  • Alcohol withdrawal syndrome includes two or more of the following symptoms: autonomic hyperactivity, increased hand tremor, insomnia, nausea or vomiting, hallucinations of illusions, psychomotor agitation, anxiety, grand mal seizures. Symptoms are usually relieved by administering brain depressant. In oriental medicine, Ju sang correspond to alcohol withdrawal syndrome. Galhwahaesung-tang has been used for the treatment of Ju sang. The authors treated 2 cases of alcohol withdrawal syndome with Galhwahaesung-tang and symptoms are markedly improved. So, We report these case with a brief review of related literatures.

A Case Report of the Traditional Korean Treatment of Female Patients with Both Functional Dyspepsia and Chronic Atrophic Gastritis (기능성소화불량과 만성 위축성 위염이 병발된 여성 환자에 대해 한방치료 경험 1례)

  • Objective: The aim of this study was to introduce gastric dysmotility as a common cause in patients with concurrent functional dyspepsia and chronic atrophic gastritis. Method: Dyspeptic symptoms, the Rydoraku score, gastric motility (electrogastrography, bowel sound analysis), gastric mucosa (gastroendoscopy), and blood and blood chemistry were all evaluated. For the treatment method, Pyengwi-san (solution) and Banwhasashim-tang (extract) were used as herbal drugs. Both ST36 electrical stimulation and simple immersion stimulation of CV11, 12, and 13 in the abdomen were applied. Results: Dyspeptic symptoms including indigestion, headache, and insomnia were all relieved. Gastric myoelectrical activity and gastric pyloric function were additionally improved. The condition of the gastric mucosa was changed from atrophic to erosive. Other side-effects of the treatment were not noted. Conclusion: The traditional Korean treatment showed effectiveness in the relief of dyspeptic symptoms and mucosal improvement of chronic atrophic gastritis. Gastric dysmotility is a common cause of the condition being concurrent with both functional dyspepsia and chronic atrophic gastritis without Helicobacter pylori infection.
