폐쇄성 수면무호흡증 환자의 주간 졸림증 및 불면증에 대한 임상 분석 (Clinical Analysis of Daytime Sleepiness and Insomnia in Patients with Obstructive Sleep Apnea)

  • Background and Objectives: Sleep disturbances and excessive daytime sleepiness (EDS) are the major symptoms of obstructive sleep apnea (OSA). This study aimed to investigate clinical implications of insomnia and EDS in patients with OSA using the Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS). Materials and Method: We evaluated 131 subjects with suspected OSA who were undergoing polysomnography (PSG) and performing the PSQI and ESS surveys. OSA was diagnosed when the apnea-hypopnea index was five or more. EDS was defined when ESS score was 11 points or higher. Detailed history and questionnaire were used to categorize insomnia. We compared clinical variables and PSG results in subgroups with or without insomnia and EDS. Results: There were no significant differences of PSQI and ESS score between controls and OSA. OSA with insomnia had significantly increased total score (p<0.001) and decreased total sleep time (p=0.001) and sleep efficiency (p=0.001) on the PSQI compared to those without insomnia. OSA with EDS showed significantly increased PSQI score (p=0.022) and decreased total sleep time (p=0.018) on PSG compared to those without EDS. Neither PSQI nor ESS score had a correlation with respiratory variables such as AHI and oxygen saturation. Total sleep time had a significant effect on both insomnia and EDS in patients with OSA. Conclusion: Decreased total sleep time had important effects on subjective symptoms of OSA and comorbid insomnia. Therefore, restoration of decreased sleep time is important in the management of OSA.

불면을 호소하는 Lewy's body dementia 환자 치험 1례 (A Case Report of a Patient with Lewy's Body Dementia and Insomnia Treated with Korean Medicine)

  • The patient was a 69-year-old Korean male with Lewy's body dementia and insomnia. He was treated with acupuncture and a herbal medicine, Ukan-san. To evaluate insomnia, the patient's total sleeping time was recorded. The patient's sleeping time increased after the administration of the Korean medicine treatments. Treatment with Korean medicine may be effective in treating insomnia in cases of Lewy's body dementia.

간호대학생의 불면증, 수면의 질, 우울 및 일주기 리듬의 관계 (Correlation among Insomnia, Sleep Quality, Depression, and Circadian Rhythm in Nursing Baccalaureate Students)

  • Purpose: This study examined the correlation of insomnia, sleep quality, depression, and circadian rhythm in nursing students. Methods: A total of 213 subjects completed a questionnaire consisting of their general characteristics, Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), Center for Epidemiologic Studies Depression (CES-D), and Composite Scale of Morningness (CSM). The collected data were analyzed by descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficient using the SPSS 23.0 program. Results: The subjects' mean scores were ISI 7.18; PSQI 11.18; CES-D 16.00; and CSM 30.18. In insomnia, there were significant differences according to caffeine, perceived health status and major satisfaction. In sleep quality, there were significant differences according to perceived health status and major satisfaction. Significant differences in depression were observed according to gender, caffeine, subjective health status, major satisfaction, and circadian rhythm by drinking and exercise. A significant positive correlation was observed among ISI, PSQI, and CES-D. ISI and CES-D were negatively correlated. Conclusion: Tailored health care programs should be developed and applied to prevent and manage sleep-related and emotion-related problems in nursing students by considering the health status, major satisfaction, and gender.

  • Insomnia is one of the highest frequency neuropsychosis of neuropsychiatry patients and is primarily caused by major depressive disorder. This case study is about a remedy for a 33 year-old woman who has insomnia and intense depressed mood caused by the continuous family jar. Also, she had the feeling of squeezing her body, pouring cold water into her body, anorexia and feeling cold and hot turn and turn. I operated mindfullness with accupuncture, herb-medication and aroma theraphy. Especially, mindfullness and Gagamguibi-tang were effective for relaxation and the sound sleep in quality and quantity. According to this study, the traditional oriental medical remedy such as accupuncture, herb-medication is effective for the cure of insomnia and mindfullness fortifies the oriental medical remedy.

방문웃음요법이 취약계층 노인의 우울과 불면증에 미치는 효과 (Effects of Visiting Laughter Therapy on Depression and Insomnia among the Vulnerable Elderly)

  • Purpose: The purpose of this study is to investigate the effects of visiting laughter therapy on depression and insomnia in the vulnerable elderly. Methods: A quasi-experimental nonequivalent control group pretest-posttest design was used for this study. The participants were 87 elderly who were registered in the Tailored Visiting Health Program of public health centers. Data were collected from September to November 2010. The experimental group received visiting laughter therapy froma visiting nurse who had taken laughter training provided by laughter therapy experts. The experimental group received 10~15 min of laughter therapy once a week for 8 weeks. The instruments included Geriatric Depression Scale and Insomnia Severity Index to measure depression and sleep problems before and after the laughter therapy. Results: The results showed that visiting laughter therapy was effective in decreasing depression and insomnia among the vulnerable elderly. Conclusion: These findings indicate that laughter therapy may be an effective nursing intervention to improve depression and insomnia Further studies would be needed to identify the difference of effects according to time, interval, or period of visiting laughter therapy and to evaluate the lasting effect of visiting laughter therapy.

Comparison of nutrient intake by sleep status in selected adults in Mysore, India

  • Insomnia has become a major public health issue in recent times. Although quality of sleep is affected by environmental, psychophysiological, and pharmacological factors, diet and nutrient intake also contribute to sleep problems. This study investigated the association between nutrient intake and co-morbid symptoms associated with sleep status among selected adults. Subjects in this study included 87 men and women aged 21-45 years. Presence of insomnia was assessed using the Insomnia Screening Questionnaire, and dietary intake was measured over three consecutive days by dietary survey. Descriptive analysis, ANOVA, and Chi-Square tests were performed to compute and interpret the data. Approximately 60% of the participants were insomniacs. People with insomnia consumed significantly lesser quantities of nutrients as compared to normal sleepers. Differences in intakes of energy, carbohydrates, folic acid, and $B_{12}$ were highly significant (P < 0.002). Further, intakes of protein, fat, and thiamine were significantly different (P < 0.021) between insomniacs and normal sleepers. The nutrient intake pattern of the insomniacs with co-morbid symptoms was quite different from that of the normal sleepers. Based on these results, it is probable that there is an association between nutrition deficiency, co-morbid symptoms, and sleep status. More studies are required to confirm these results.

수면장애에 대한 한방통합프로그램의 적용 연구 (A Study on Applying a Korean Medical Integrative Program for Sleep Disorder)

  • Objectives : Korea's average life expectancy is getting longer and longer and the pace of change in society has increased every year as people experience stress more easily. Under this situation, chronic sleep disorder is increasing, but there is no specific program for the treatment of sleep disorder in Korean medicine. Therefore, we studied a Korean medical treatment program for sleep disorder to apply to outpatient clinic patients. Methods : For this study, we evaluated the Insomnia Severity Index (ISI), BDI, STAI, FSS on 29 insomnia patients who visited Kyunghee Korean medical hospital from July 2010 to May 2012. Herbal medicine, acupuncture, herbal acupuncture, moxibustion, sleep hygiene, and medical qigong were applied. We compared ISI scores before treatment and after 4 weeks of treatment. Results : ISI score was improved after Korean medical treatment for sleep disorder. Before the treatment ISI score was 18.1, which means clinical insomnia. After the Korean medical treatment, the score was 7.5, which means no clinically significant insomnia. Conclusions : The study showed a significant effect on the Korean medical treatment program for sleep disorder to apply to outpatient clinic patients. ISI score was improved and the patients were satisfied with the treatment, so it would be more helpful to apply in sleep clinics.

노인의 휴식.활동 일주기 생체리듬과 수면양상 (Rest-Activity Rhythm and Sleep pattern in the Elderly)

  • Purpose: The study compared the rest-activity rhythm and sleep pattern of elderly with young group. Methods: The subjects were 22 over than 65 years old and 23 under 65 years old. An actigraph, sleep diary, Pittsburgh Sleep Quality Index and Insomnia Severity Index scale were used as measurement tools for this study. The data were analyzed with $x^2$, Lamda test, t-test and correlation with SPSS 15.0 program. Results: The elderly had lower curve than the young group in rest-activity rhythm on each time zone. In particular, the elderly group had lower rest-activity rhythm curve of 8, 9, 14, 18, 19, 20, 21, 22 and 23 time zone than those of young group. Sleep pattern had statistical difference in the total sleep time, PSQI and insomnia. Total sleep time of elderly had lower score and PSQI and Insomnia had higher score than young group. Age had correlation with rest-activity rhythm, sleep efficiency, PSQI and insomnia. Conclusion: Rest-activity rhythm of the elderly showed an increase in activity in the early morning because of earlier get up than the young group and an decrease in activity in the afternoon because of taking a nap at this time. Elderly sleep was that total sleep time increased but sleep efficiency decreased and insomnia intensified. This sleep pattern was related to age and rest-activity rhythm.

중풍환자의 불면에 대한 산조인 단미(안심산)의 유효성 및 적응증 평가 (Effectiveness of Zizyphus seed (Ansim-san) for Insomnia in Stroke Patients)

  • Objectives : Zizyphus seed has long been used in hypnotics and sedatives in oriental medicine, and it is reported that the Zizyphus seed elicited a variety of pharmacological actions besides CNS depression. We did a clinical study on the effectiveness of Zizyphus seed (Ansim-san) for insomnia in stroke patients. Methods : To 41 stroke patients suffering from insomnia, we administered the extract of Zizyphus seed (Ansim-san). Outcome measures were subjective in nature and included a morning questionnaire, visual analog scales, and observation forms by study personnel. We also, to find the applicable symptom-complex of Zizyphus seed (Ansim-san), examined accompanying symptoms. Results : 1. Zizyphus seed extract use meant that sleep latency was shorter, total sleep time was longer, number of wakenings was lower, wake time during sleep was shorter, sleep quality was improved, condition upon awakening was improved, ability to concentrate was improved, and the patients fell asleep more easily (P<0.001). There were no differences in morning sleepiness. 2. Among accompanying symptoms, an oppressed feeling in the chest was significant (P=0.035) in the patients who improved sleep quality. Conclusions : The results of this clinical study suggest that in stroke patients, Zizyphus seed (Ansim-san) can be effective for insomnia, and significantly effective in patients who have an oppressed feeling in the chest.

불면증(不眠症)에 이침(耳鍼) 시술(施術)을 한 환자(患者) 20례(例) 임상고찰(臨床考察) (Clinical study on 20 cases of insomnia patients with auricular acupuncture therapy)

  • OBJECTIVE: This study is aimed to evaluate the clinical improvement of insomnia and patients' original disease after the therapy of auricular acupuncture. METHODS: A questionnaire regarding patients' sleeping behavior was executed to secure the objectivity. 20 patients were selected from 55 patients in Joong Hwa oriental medicine hospital. 20 patients who had undergone auricular acupuncture for insomnia for 6 days were asked before the first therapy to fill in a form regarding their sleeping behavior and retrospectively after 6 days a questionnaire regarding the success of therapy. Auricular acupuncture therapy consisted of 3 consultations at an interval of 2 days. RESULTS: 12 patients were good conditions, 5 were same conditions and 3 were rather poor conditions of 20 patients after auricular acupuncture therapy. CONCLUSIONS: 60% of object improved insomnia after auricular acupuncture therapy. Auricular acupuncture therapy can be applied and adapted individually, furthermore it is economical and small side effects.
