A Comparison of Medical Care Services by Type of Medical Care Facility -In cases of normal spontaneous vaginal delivery and acute appendicitis- (의료기관 종류별 진료내역 비교 -정상분만과 급성 충수염을 중심으로-)

  • To find out the differences in medical practice according to the environment of patient care and managerial situation of the medical care institutions, normal spontaneous vaginal delivery and acute appendicitis were selected, which is suitable for comparison because of their high comparability. A total of 473 cases of normal spontaneous vaginal delivery and 408 cases of acute appendicitis was sampled from the claims to Korea Medical Insurance Cooperations during January to June 1984. Complicated cases were excluded from population and sampling was restricted from 40 to 60 percentile for total charges by the type of medical care facility in order to rule out the influence originating from case mix. Important items representing type and quantity of medical care service were compared by type of facility. Major findings are as follows : 1. University hospital shows the highest in charges per case and decrease in order of general hospital, and clinic. 2. In case of normal spontaneous vaginal delivery, average length of stay shows statistically significant difference by type of facility. 3. Charge amount for each service item affected by practice pattern shows statistically significant difference mostly by type of facility. It is suggested that medical practice pattern is different by type of facility for medical services. 4. Difference in total medical expenditure by type of facility is affected more with charges for materials, consumables and drugs than with fee for service activity. 5. In administering drugs to patients, hospital and clinic show higher injection rate than university and general hospital. 6. Clinical Laboratory tests were common in order of uninalysis, hemoglobin, hematocrit, white blood cell count, urine microscopic examination in cases of normal spontaneous vaginal delivery; white blood cell count, urinalysis, hemoglobin, hematocrit, urine microscopic examination, white blood cell differential count, in cases of acute appendicitis. 7. The result for Laboratory test and Radiologic study shows extreme difference by type of facility. Test rate is lowest in clinic and increase hospital, general hospital, and university hospital in order, both in type and frequency.

Protective Effects and Mechanisms of Salvia Miltiorrhizae Radix Extract on Endotoxin Shock (패혈증 쇼크에서 단삼 추출물의 억제 효과와 그 기전에 관한 연구)

  • Objectives : This study was performed to investigate the protective effects and mechanisms of Salvia miltiorrhizae Radix extract (SME) on endotoxin shock.Methods : We used two models; LPS-induced sepsis model for in vivo model, and murine peritoneal macrophages responses for in vitro. SME was administrated orally to mice. After 1 hr, LPS was injected intraperitoneally. Survival rate was checked each time per 12 hr for 5 days. Mice were sacrificed 3 hr after LPS injection, then blood samples and organs were harvested. Cytokines secretion was measured by ELISA. Organs tissues were observed with microscope. Murine peritoneal macrophages were cultured for 1 hr either in a medium alone or in a medium that contained SME, as indicated. Then, the cells were treated with LPS for 24 hr. mRNA levels of cytokines were measured by real-time RT-PCR. Cytokine levels in the supernatants were measured by ELISA. The amount of nitrite was measured by using the Griess method to evaluate NO production. The cell lysates were analysed by Western blotting using antibodies for iNOS and β-actin was used as an internal control to monitor equal protein loading.Results : SME improverd the survival rate of mice model. SME inhibited the secretion of inflammatory cytokines and organs damages on Endotoxin Shock model. SME suppressed cytokine expression, cytokine secretion,NO production, iNOS expression in LPS-induced murine peritoneal macrophages.Conclusions : The results suggest that SME has protective effects on endotoxin shock through suppression of inflammatory cytokines, organ damages, NO production and so on.

Predictors of Anaphylactic Shock in Patients with Anaphylaxis after Exposure to Bee Venom (벌독에 의한 아나필락시스 쇼크 발생의 예측 인자)

  • Purpose: The purpose of this study is to analyze the clinical characteristics of anaphylaxis and anaphylactic shock caused by bee venom. Methods: We retrospectively collected the data of the patients who experienced anaphylaxis caused by natural bee sting or acupuncture using bee venom from January 1999 to December 2008. Seventy subjects were divided into the shock and non-shock groups. The clinical characteristics, sources of bee venom, treatments and outcomes were compared between the two groups. Results: The mean age of the subjects was $45.5{\pm}16.3$ years old and the number of males was 44 (62.9%). There were 25 patients in the shock group and 45 in the non-shock group. The age was older (p=0.001) and females (p=0.003) were more frequent in the shock group. Transportation to the hospital via ambulance was more frequent in the shock group (p<0.001). No difference was found in species of bee between the two groups. The cephalic area, including the face, was the most common area of bee venom in both groups. Anaphylaxis caused by bee sting commonly occurred between July and October. Cutaneous and respiratory symptoms were the most frequent symptoms related to anaphylaxis. Cardiovascular and neurologic symptoms were more frequent in the shock group. The amount of intravenously administered fluid and subcutaneous injection of epinephrine were much more in the shock group than that in the non-shock group. Conclusion: Older age was the factors related to anaphylactic shock caused by bee venom. Further validation is needed to evaluate the gender factor associated with shock.

Experimental In-Vivo Models Used in Fat Grafting Research for Volume Augmentation in Soft Tissue Reconstruction

  • As the popularity of fat grafting research increases, animal models are being used as the source of pre-clinical experimental information for discovery and to enhance techniques. To date, animal models used in this research have not been compared to provide a standardized model. We analyzed publications from 1968-2015 to compare published accounts of animal models in fat grafting research. Data collected included: species used, graft characteristics (donor tissue, recipient area, amount injected, injection technique), time of sacrifice and quantification methods. Mice were most commonly used (56% of studies), with the "athymic nude" strain utilized most frequently (44%). Autologous fat was the most common source of grafted tissue (52%). Subcutaneous dorsum was the most common recipient site (51%). On average, $0.80{\pm}0.60mL$ of fat was grafted. A single bolus technique was used in 57% of studies. Fat volume assessment was typically completed at the end of the study, occurring at less than 1 week to one year. Graft volume was quantified by weight (63%), usually in conjunction with another analysis. The results demonstrate the current heterogeneity of animal models in this research. We propose that the research community reach a consensus to allow better comparison of techniques and results. One example is the model used in our laboratory and others; this model is described in detail. Eventually, larger animal models may better translate to the human condition but, given increased financial costs and animal facility capability, should be explored when data obtained from small animal studies is exhausted or inconclusive.

A Performance Evaluation of the Highly Efficient Coagulation System for the Treatment of Overflows from Primary Clarifier in WWTP (강우시 하수처리장 일차침전지 월류수 처리를 위한 고효율응집시스템의 적용성 평가)

  • This study is to develop highly efficient coagulation system(HECS) that runs treatment with a short retention time to cover the overflow in the rain, which coagulation, mixing and settling are contacted in a single reactor and to estimate the applicability. Setting up 100ton/day-size pilot scale plant, the results of continuous operation in case of runoff, maintaining 20 minute-retention time at optimum chemical injection condition(Alum 100mg/L, Polymer 1.0mg/L) shows the highest removal efficiency(Turbidity 93.1%, TCODcr 80.6%, BOD 81.8%, SS 92.5%, TN 72.3% and T-P 87.3%). It was estimated that the large amount of cost for separate sewage system and the size of area for system instruction can be reduced if the HECS is applied for CSOs treatment because the HECS is so compact and quickly. When we see the results, HECS from this study could be able to treat the pollutant quickly within a short retention time only with coagulant and polymer, which could show high applicability.

The Effect of Hydrogen Added into In-let Air on Industrial Diesel Engine Performance (흡기중의 수소첨가가 산업용 디젤기관의 성능에 미치는 영향)

  • Diesel engines introduce only air into the cylinder, and the air is high lycompressed. Fuel is directly injected into the combustion chamber in high temperature and pressure. Therefore diesel engines have high thermal efficiency because of the high compression ratio, while having high level of particulate matter and nitrogen oxide emissions because of the direct fuel injection. Many technologies have been developed to reduce particulate matter and nitrogen oxide emissions from diesel engines. One of the technologies is hydrogen fuel introduced into the combustion chamber with diesel fuel. In this thesis tiny amount of hydrogen is supplied into the combustion chamber in order to enhance the combustion performance. The engine, in which hydrogen is introduced, is tested. There are 20 test conditions given as 5 torque values of 100%, 75%, 50%, 25%, 0%, and 4 engine speeds of 700rpm, 1000rpm, 1500rpm and 2000rpm for the two cases with or without hydrogen addition. Maximum torques and Idle torques at each engine speed are measured, then the torque values are divided into 4 levels with 25% increasing step. The result shows that the fuel consumption, smoke, CO are reduced while the NOx emission is slightly increased, and the hydrogen addition has not a great effect on the performance at low loads but a great effect at a maximum load.

  • In a CNG/diesel dual-fuel engine, CNG is used as the main fuel and a small amount of diesel is injected into the cylinder to provide ignition priming. In this study, a remodeling of the existing diesel engine into a CNG/diesel dual-fuel engine is proposed. In this engine, diesel is injected at a high pressure by common rail direct injection (CRDI) and CNG is injected at the intake port for premixing. The CNG/diesel dual-fuel engine had an equally satisfactory coordinate torque and power as the conventional diesel engine. Moreover, the CNG alternation rate is over 89% throughout the operating range of the CNG/diesel dual-fuel engine. PM emission by the dual-fuel engine is 94% lower than that by the diesel engine; however, NOx emission by the dual-fuel engine is higher than that by the diesel engine.

Arthroscopic Treatment for Meniscal Cyst (관절경을 이용한 반월상연골 낭종의 치료)

  • Recent studies suggest that most meniscal cysts can be treated surgically by arthroscopic management of the meniscal tear and arthroscopic cyst evacuation. But arthroscopic cyst decompression may sacrifice a substantial amount of meniscal tissue that is not torn in order to expose the "stalk" of the cyst. Nowadays, the trend is changing as preserving the involved meniscus to prevent from inevitable degenerative changes after meniscectomy. The purpose of this report is to describe a new surgical technique that minimizes loss of meniscal tissue in hopes of maximizing residual meniscal function. We experienced 10 patients with meniscal cysts that were consisted of four lateral cysts and six medial cysts. Menisci were torn in all cases. Arthroscopic partial meniscectomy and decompression of cysts were performed in 9 cases, and arthroscopic partial meniscectomy and open cystectomy in 1 case. The procedures were consisted of injection of the methylene blue into the cyst, partial meniscectomy of the meniscal tear until the dye was seen in orifice of the cyst, and decompression of cyst through cystic opening. This article serves to confirm the relationship between torn menisci and cysts, to re-evaluate the occurrence ratio of the meniscal cyst on the medial to lateral meniscus, and to assess the efficacy of arthroscopic partial meniscectomy and decompression of cyst as a potentially meniscal sparing procedure.

Antioxidative and Hepatoprotective Effects of Acer tegmentosum M. Extracts (산겨릅나무 추출물의 항산화 및 간 기능 보호효과)

  • This study was carried out to investigate antioxidative and hepatoprotective effects of Acer tegmentosum M. (ATM) extracts. Content of total polyphenol of EtOAc fraction was the highest amount among fractions. Electron donating abilities of all fractions were increased as concentrations of each fraction were increased. ATM BuOH fraction showed the highest SOD like activity at low concentration (<$250\;{\mu}g/mL$). To investigate the protective effect of ATM on hepatotoxicity, ATM BuOH fraction was administered to mice for 7 consecutive days, and then lipopolysaccharide (LPS) was injected at a dosage of 1 mg/kg. The LPS led to increase of aspartate aminotransferase (AST), alanine aminotransferase (ALT), and blood urea nitrogen (BUN). However, pretreatment with BuOH fraction prior to LPS injection significantly decreased plasma hepatospecific enzyme levels. Histological findings demonstrated that pretreatment with BuOH fraction resulted in an attenuation of LPS induced liver damages.

  • We report a case of an accidental extravasation of contrast material. A large-volume extravasation occurred in an adult during spiral contrast-enhanced CT. The amount of contrast material extravasated was 47 ml. The patient had a swelling of the dorsum right hand. The extravasation injury site was determined by CT scanning. The extavasation case was examined using five separate display techniques: axial, multi planar reformation (MPR), maximum intensity projection (MIP), volume rendering, and shaded-surfaced display (SSD). This paper introduces extravasation with the CT and the three-dimensional appearance.
