Anticariogenic activity of piceatannol isolated from Callistemon citrinus fruit against Streptococcus mutans

  • Dental caries is the destruction of the enamel of teeth by Streptoccus mutans. S. mutans has been isolated from human dental plaque and is associated with the initial development of enamel lesions. We have studied the antibacterial action of the fruit of Callistemon citrin us against a cariogenic bacterium, S. mutans. From the fruit of C. citrinus, piceatannol (3,3',4',5-tetrahydroxystilbene) was isolated by repeated column chromatography with $SiO_2$ and Sephadex LH-20. Its structure was elucidated by instrumental analysis using 1D-NMR, 2D-NMR and EI-MS. This compound was isolated from the fruit of C. citrin us for the first time. The anticarcinogenic activity of this compound was determined by using agar well-diffusion method and minimal inhibition concentration (MIC).

THE EFFECT OF ACID CONCENTRATION AND pH OF LACTATE BUFFER SOLUTION ON THE PROGRESS OF ARTIFICIAL CARIES LESION IN HUMAN TOOTH ENAMEL (유산완충액을 이용한 인공치아우식의 형성에 미치는 산의 농도와 pH에 관한 연구)

  • Dental caries is considered to be caused by demineralization by organic acid produced by microorganism. But the formation of subsurface lesion in initial caries make it diffcult to explain by simple demineralization. This study is carried out on the basis of thermodynamic concept proposed by Margolis and Moreno. The purpose of this study is to evaluate the effects of acid concentration and pH of lactate buffer system on the artificial caries lesion progress. 160 teeth without any crack, defect or opaque enamel were used and coated with nail varnish except the window ($2{\times}3$ mm). Under the constant degree of saturation(D.S.). The teeth were divided into 8 groups according to acid concentration(10mM, 25mM, 50mM, 100mM) and pH(4.3, 5.0, 6.0). Each group was immersed in buffer solution for 3, 6, 9, 18 days under controlled temperature($25^{\circ}C$). After cutting through the window and grinding, the specimens, 100-150 um in thickness, were imbibed in water or air and examined using polarilizing microscope. The depth of the surface and subsurface surface lesion were measured. 1. In the constant pH and D. S. value, the subsurface lesion progresses more rapidly as the concentration of lactic acid increases. (0.01, 0.025, 0.05, 0.1) 2. In the constant acid concentration and DS value, the subsurface lesion progresses more slowly as the pH increases. (4.3, 5.0, 5.5, 6.0) 3. The width of surface lesion seems to be constant independant of pH and acid concentration.

DEVELOPMENT OF HIGH SENSITIVE MODEL OF CARIES ACTIVITY TEST FOR EARLY DIAGNOSIS OF DENTAL CARIES (치아우식증의 조기진단을 위한 고감도 우식활성검사 모형개발)

  • The purpose of this study is to develop the system which convert the optical difference of teeth texture between intact enamel and incipient caries lesion into shade difference by laser fluorescence and to develop new and simple caries activity test using laser fluorescence. The experimental design of this study consists of three parts. In first part, a new method for the in vitro assessment of changes in initial enamel caries lesion of Bovine teeth using laser fluorescence is tested. In second part, in vivo assessment undertaken. Number of teeth which showed incipient carious lesion on buccal surface examined by laser fluorescence was compared with the caries activity test of $Cariescreen^{(R)}$ test and other oral environmental test of dDfFtT. In third part, new caries activity test measured by laser fluorescence was developed on the basis of above results and evaluated the sensitivity, specificity, and diagnostic power. Optical density measured by laser fluorescence was increased as increasing the depth of incipient carious lesion and showed high correlation$(\gamma=0.7015)$ with lesion depth. Optical density showed direct proportion to lesion depth. Linear equation was obtained between the optical density and the lesion depth by regression analysis. The result of caries activity test with laser fluorescence showed high correlation with those of $Cariescreen^{(R)}$ test and dDfFtT examination. Caries activity test with laser fluorescence showed 48% of sensitivity, 52% of specificity, and 45% of diagnostic power on the basis of dDfFtT examination, and also showed 48% of sensitivity, 51% of specificity, and 36% of diagnostic power on the basis of $Cariescreen^{(R)}$ test. In regard above result, caries activity test with laser fluorescence considered to be reliable for caries activity test compared with other oral environmental test. and it was also considered to be practical because it would be simple, inexpensive, and time saving method.

THE EVALUATION OF DENTAL TREATMENT RECORDS IN CHARITY DENTAL CLINIC FOR THE DISABLED PEOPLE (장애인 무료 치과 진료소에서의 진료 기록 평가)

  • Most of disabled people face hardness in caring their general oral hygiene by themselves. So that, they are once involved in dental caries or periodontal disease, they have much bigger chance of aggressive progress. Therefore preventive dentistry is more important to disabled one than non-disabled. They need to be checked with routine periodic dental examination and by that, oral disease must be found at initial stage. We selected 37 patients from newly visited 237 patients who had dental treatment at the free dental clinic, Gangnam district, Seoul, between 2000 and 2001. This study is a comparative evaluation of first 2 years' treatment records of selected 37 patients (male 28, female 9) with that of their last 2 years. 24 of 37 have mental retardation, 9 have autistic disorder, 2 have auditory disorder, 1 has brain disorder and 1 has crippled disorder. Their dental treatment records categorized by 4 level; score 4 stands for endodontic treatment, score 3 stands for general conservative treatment, score 2 stands for preventive treatment, such as sealant and score 1 stands for routine check. In first 2 years of treatment, average score was 2.85 which score means approximately general conservative treatment. And that of last 2 years was 1.44, which means routine check. In early time of their visit, they will receive the treatment due to their chief complaint. And after that early time, they can have a routine check so that they can be treated before the disease get worse. The patient's oral health can remain decent only by a simple treatment.

ANTICARIOGENIC EFFECT OF FLUORIDE VARNISHES AND CHLORHEXIDINE VARNISHES (불소 바니쉬와 클로르헥시딘 바니쉬의 항우식 효과)

  • Dental caries which is one of the most common chronic disease complexly developed by the action of oral bacteria, diet, and host factor. Various prevention program enhance resistance of demineralization and reduce the acidogenecity of oral bacteria have been introduced, representative material is fluoride and chlorhexidine. The purpose of the study was to evaluate and compare effectiveness of fluoride varnish and chlorhexidine varnish in vivo. Bovine tooth specimens were implanted in the lower space maintainers and applied with fluoride varnish and chlorhexidine varnish. After seven days in oral environment, metal mesh was covered to make similar condition of plaque accumulation and induce caries. All specimens were analysed by EPMA to evaluate quantitative change of Ca, P and by polarized microscope to identify histological changes. The results were as follows : After initial artificial caries induction in the mouth, there were remarkable enamel caries lesion in the control group under polarized light microscopy. The highest amount of mineral decrease were showed in control group. No statistically significant mineral decrease were showed in fluoride varnish group, while chlorhexidine varnish group showed only significant decrease of P(P<0.05). In conclusion both fluoride varnish and chlorhexidine varnish seemed to be effective for protecting enamel surface from caries activity, although fluoride varnish has more anticariogenic effect than chlorhexidine varnish.

Elemental analysis of the fluoride varnish effects on root caries initiation (불소 바니쉬 도포 후 초기 치근 우식 발현에 대한 정량원소분석)

  • Objectives: The usage of fluoride varnish for a moderate to low caries-risk group has not been well validated. This study aimed to evaluate the preventive and therapeutic efficacies of fluoride varnish on the initiated root caries. Materials and Methods: Ten premolars were sectioned into quarters, further divided into two windows, one of which was painted with Fluor Protector (1,000 ppm fluoride, Ivoclar Vivadent). An initial lesion with a well-preserved surface layer was produced by pH cycling. Scanned line analysis using energy dispersive spectrometry determined the weight percentages of Ca and P in the demineralized layer. Scanning Electron microscopy and confocal laser scanning microscopy (CLSM) evaluated the varnish-applied root surfaces. Results: The mean lesion depth (SD) was 12.3 (2.6) ${\mu}m$ (single cycling) and 19.6 (3.8) ${\mu}m$ (double cycling). Double cycling extended the lesion depth, but induced no more mineral loss than single cycling (p < 0.05). The mean weight percentages of Ca and P between groups with and without varnish were not significantly different (p < 0.05). A CLSM showed varnish remained within 15 ${\mu}m$ of the surface layer. Conclusions: When a mild acid challenge initiated root tissue demineralization, the application of low-concentration fluoride varnish did not influence the lesion depth or the mineral composition of the subsurface lesion.

In vitro Measurement of the Mechanical Properties of Resin Infiltrant: Viscosity, Penetration Pattern, Surface Roughness, Microhardness (초기 법랑질 우식 병소에 대한 Resin infiltrant의 점도, 침투 양상, 표면 조도 및 미세경도)

  • In this study, we observed the penetration of the resin infiltrant into natural initial caries by SEM and CLSM, evaluated the pH and viscosity of resin infiltrant and several adhesives(All-bond 2, Scotchbond Multi-purpose, Clearfil SE bond), and measured the surface roughness and the microhardness on permanent teeth before and after applied resin infiltrant into initial enamel caries. The viscosity(Pa.s) was observed for resin infiltrant($0.173{\pm}0.012$), All-bond 2($0.754{\pm}0.030$), Scotchbond Multi-purpose($1.46{\pm}0.137$), and Clearfil SE bond($1.28{\pm}0.125$). The pH was measured for resin infiltrant (4.7), All-bond 2(7.5), Scotchbond Multi-purpose(8.2), and Clearfil SE bond(4.67). An application procedure of resin infiltrant increased the surface roughness and the microhardness insignificantly(p>0.05), when compared to non-applied specimens.

The effect of different fluoride application methods on the remineralization of initial carious lesions

  • Objectives: The purpose of this study was to assess the effect of single and combined applications of fluoride on the amount of fluoride release, and the remineralization and physical properties of enamel. Materials and Methods: Each of four fluoride varnish and gel products (Fluor Protector, FP, Ivoclar Vivadent; Tooth Mousse Plus, TM, GC; 60 Second Gel, A, Germiphene; CavityShield, CS, 3M ESPE) and two fluoride solutions (2% sodium fluoride, N; 8% tin(ii) fluoride, S) were applied on bovine teeth using single and combined methods (10 per group), and then the amount of fluoride release was measured for 4 wk. The electron probe microanalysis and the Vickers microhardness measurements were conducted to assess the effect of fluoride application on the surface properties of bovine teeth. Results: The amount of fluoride release was higher in combined applications than in single application (p < 0.05). Microhardness values were higher after combined applications of N with FP, TM, and CS than single application of them, and these values were also higher after combined applications of S than single application of A (p < 0.05). Ca and P values were higher in combined applications of N with TM and CS than single application of them (p < 0.05). They were also increased after combined applications of the S with A than after single application (p < 0.05). Conclusions: Combined applications of fluoride could be used as a basis to design more effective methods of fluoride application to provide enhanced remineralization.

Efficacy of buccal piroxicam infiltration and inferior alveolar nerve block in patients with irreversible pulpitis: a prospective, double-blind, randomized clinical trial

  • Objectives: This randomized clinical trial aimed to assess the effectiveness of buccal infiltration with piroxicam on the anesthetic efficacy of inferior alveolar nerve block (IANB) with buccal infiltration in irreversible pulpitis, with pain assessed using the Heft-Parker visual analogue scale (HP-VAS). Materials and Methods: This study included 56 patients with irreversible pulpitis in mandibular molars, randomly distributed between 2 groups (n = 28). After evaluating the initial pain score with the HP-VAS, each patient received IANB followed by buccal infiltration of 2% lignocaine with adrenaline (1:80,000). Five minutes later, the patients in groups 1 and 2 were given buccal infiltration with 40 mg/2 mL of piroxicam or normal saline, respectively. An access opening procedure (AOP) was performed 15 minutes post-IANB once the individual showed signs of lip numbness as well as 2 negative responses to electric pulp testing. The HP-VAS was used to grade the patient's pain during caries removal (CR), AOP, and working length measurement (WLM). Successful anesthesia was identified either by the absence of pain or slight pain through CR, AOP, and WLM, with no requirement of a further anesthetic dose. A statistical analysis was done using the Shapiro-Wilk and Mann-Whitney U tests. Results: The piroxicam group presented a significantly lower (p < 0.05) mean pain score than the saline group during AOP. Conclusions: Buccal infiltration with piroxicam enhanced the efficacy of anesthesia with IANB and buccal infiltration with lignocaine in patients with irreversible pulpitis.

THE EFFECT OF XYLITOL AND CARBOHYDRATES ON STREPTOCOCCUS (자일리톨과 탄수화물의 Streptococcus mutans에 대한 효과)

  • Xylitol is a 5-carbons carbohydrate, which can be replaced with sucrose for preventing caries. The replication of Streptococcus mutans and its formation of artificial plaque were studied in the media containing xylitol. The combined effect of xylitol and other carbohydrates on Streptococcus mutans was also studied. The replication of Streptococcus mutans was inhibited according to the increased concentration of xylitol. Streptococcus mutans replicated at the initial stage of incubation in the media containing glucose, fructose or lactose, while replicating from the beginning of incubation in the media containing fructose as combining with xylitol. The formation of artificial plaque by Streptococcus mutans was significantly reduced in the media containing with xylitol and fructose. These results indicated that the replication of Streptococcus mutans was inhibited according to the increased concentration of xylitol, and the formation of artificial plaque by Streptococcus mutans was significantly inhibited in the media containing xylitol and fructose.
