A Retrospective Statistical Study on Sedation Cases in Department of Pediatric Dentistry at Dankook University Dental Hospital for 5 Years (단국대학교 치과병원 소아치과에서 5년간 시행된 진정법에 대한 후향적 통계 연구)

  • Recently, sedation is routinely used in children who lacked cooperation in pediatric dental clinics. The purpose of this study is to analyze dental treatment cases in department of pediatric dentistry at Dankook University Dental Hospital associating with sedation during 5 years. From January 2011 to December 2015, information about dental treatment cases in department of pediatric dentistry was collected and reviewed. The information gathered included patient gender, age, sedation type, sedative agents, date of operation, and type of procedure. The proportion of treatment cases with conscious sedation for total treatment cases continued to decline until 2014. On the other hand, the incidence of general anesthesia increased gradually. The frequency of midazolam administration and nitrous oxide inhalation sedation increased recently. The use of conscious sedation was significantly decreased when the age of patient was 3 years or older. The frequency of general anesthesia was similar in all age groups. There was a tendency to perform treatment with deeper sedation when the patient was male than female, and when the pulp treatment was performed together with the restorative treatment than not performed. The frequency of re-treatment within 90 days after restoration treatment decreased as the depth of sedation increased.

CONVERTING FROM ORAL SEDATION TO INTRAVENOUS SEDATION USING TOPICAL ANESTHETICS ON SKIN AFTER ORAL SEDATION FAILURE (경구진정 실패 후 피부 도포마취제를 사용한 정주진정으로의 전환 치료)

  • The use of chloral hydrate and hydroxyzine for oral sedation is most effective in children aged less than 36 months and weighing less than 15 kg. Children who do not belong to this category may show frequent movements due to shallow sedation level, and it can lead to sedation failures. One of the solutions to such sedation failure is conversion to deeper sedation. But, it is not so much of an option, since inhalation anesthetics and devices are required. In this case, conversion from oral sedation to intravenous sedation was successfully achieved without causing injection pain while searching for an intravenous route, by using EMLA cream (Eutectic Mixture of Local Anesthesia). A patient aged 46 months and weighing 15 kg visited the Pediatric Department of Dankook University Dental Hospital. Treatment under TSD(Tell Show Do) was offered, but due to the parent's request, oral sedative measures were taken. Considering prompt converting from oral sedation to iv sedation in case the oral sedation fails, EMLA cream was apllied preemptively. Adequate sedation level could not be achieved after 90 minutes of oral administration, therefore, under the parent's consent, intravenous route was prepared after conscious sedation by $N_2O-O_2$. During treatment, $ETCO_2$, $SPO_2$ and heart rate was monitored every 5 minutes. The patient showed stable vital signs and did not show any movements. The whole procedure took two and a half hours in total, and the treatment was completed without any adverse effects.

THE NITROUS OXIDE CONCENTRATION IN BREATHING ZONE ACCORDING TO SCAVENGING METHODS DURING DEEP SEDATION OF PEDIATRIC DENTAL PATIENTS (소아치과 환자의 깊은 진정시 호기가스 제거 방법에 따른 호흡대역에서 Nitrous Oxide 농도 변화)

  • The concentration of nitrous oxide in dental environment has increased especially in pediatric department. In pediatric department frequently met the behavior disordered patients who need the deep sedation. As the deeply sedated patients could not respond well to verbal command, the amount of mouth breathing would be increased especially with mouth prop which backward transposition of mandible. Inhalation of low concentration of nitrous oxide for a long duration has caused various side effects such as spontaneous abortion and inhibition of methionine synthetase activity which is harmful to DNA synthesis. For evaluation of factors of mouth breathing during deep sedation. The author measured the concentration of nitrous oxide in breathing zone by the change of the scavenging methods. One is drain the gas through the tail part of reservoir bag of Jackson Ree's system naturally. Another is scavenge from tail portion of reservoir bag with negative pressure. Last one is scavenge from nasal mask with negative pressure. The nitrous oxide concentration in breathing zone was the lowest in nasal part drainage but high above the recommended concentration of NIOSH. The order of nitrous concentration in breathing zone was: natural drainage, tail part with negative pressure, nasal part with negative pressure. This would reflect the order of resistance of nasal airway and showed the amount of mouth breathing. From the above experiment, the resistance of nasal airway by the increment of gas flow in corrugating tube and reservoir bag would be one of the causative factors of mouth breathing in deeply sedated patients.

Sedation for Dental Treatment of Patients with Disabilities (장애인 환자의 치과치료를 위한 진정법)

  • Background: Dental disabilities mean the poor cooperation for dental treatment because of patient's inherent disability, severe fear and anxiety, and communication problem. Sedation and general anesthesia are usually used for behavioral control in dentally disabled patients. In particular, sedation (conscious and deep) can help them to tolerate the proper dental treatment effectively and safely. Methods: From March 2002 to September 2007, total 35 sedation were carried out in 33 patients (male : female = 20 : 13) with dental disabilities at Seoul National University Dental Hospital and Hanyang University Medical Center. Patients' dental charts and sedation records were retrospectively reviewed. Results: Tooth extraction (19 cases) was the most common dental treatment performed under intravenous sedation (30 cases). Occasionally, inhalation sedation using Sevoflurane 1-2% was adapted (5 cases). Deep sedation (28 cases) was carried out using midazolam 2-3 mg bolus injection and propofol infusion via TCI (4.2 ${\pm}$ 0.9 mg/kg/h), and conscious sedation (7 cases) was carried out using midazolam bolus onlywithout severe complications. The duration of dental treatment was 25.5 ${\pm}$ 12.3 min and that of sedation was 43.2 ${\pm}$ 9.7 min. Conclusion: Sedation for dentally disabledpatients should be selected for effective behavioral control in conjunction with general anesthesia, considering the duration and pain-evoking potentials of dental treatment, the type and severity of patients' disabilities, and the experience of dental anesthesiologists altogether.

Effects of Intraperitoneal Administration of Bupivacaine on Relief of Pain and Change of Behavior following Ovariohysterectomy in Cats (고양이에서 난소자궁적출술 후 복강 내 Bupivacaine 투여가 통증 감소와 행동 변화에 미치는 영향)

  • Perioperative pain relief is essential in veterinary practice. However, the cat is one of the most poorly understood species regarding pain control management. Ovariohysterectomy(OHE) produces considerable postoperative pain in cats. Practitioners are often reluctant to administer analgesics due to lack of familiarity with available drugs, concern about side effects, or frustration with the need for record keeping of controlled substances. The purpose of this study was to determine if intraperitoneal administration of bupivacaine can provide relief of pain following OHE in cats. Twelve healthy female cats were randomly divided into two groups. OHE was performed under general inhalation anesthesia. Just prior to complete closure of the linea alba, 6 cats in SAL group received 0.88 ml/kg 0.9% saline, 6 cats in BUP group received 4.4 mg/kg 0.75% bupivacaine diluted to an equivalent volume with saline in the intraperitoneal space. Cats were scored at 0, 1, 2, 4, 8, 12 and 24 hours post-extubation by one observer. Cats were evaluated using a visual analogue scale(VAS) and composite pain scale(CPS) that included physiologic variables. There were no significant differences in body weight, anesthesia time, surgery time, and incision length between the two groups. Cats in the BUP group had significantly(p<0.05) lower VAS-pain scores than cats in the SAL group at 4, 8, 12 hours after surgery. Cats in the BUP group had significantly lower CPS scores than cats in the SAL group at 8, 12 hours after surgery. No adverse side effects were observed. These results support that the intraperitoneal administration of bupivacaine following OHE can be used for the prevention of postoperative pain and pain-induced behavioral changes in cats.

Participation of $K_{ATP}$ Channels in the Antinociceptive Effect of Pregabalin in Rat Formalin Test

  • Background: Pregabalin is an anticonvulsant and analgesic agent that interacts selectively with the voltage-sensitive-$Ca^{2+}$-channel alpha-2-delta subunit. The aim of this study was to evaluate whether the analgesic action of intrathecal (IT) pregabalin is associated with KATP channels in the rat formalin test. Methods: IT PE-10 catheters were implanted in male Sprague-Dawley rats (250.300 g) under inhalation anesthesia using enflurane. Nociceptive behavior was defined as the number of hind paw flinches during 60 min after formalin injection. Ten min before formalin injection, IT drug treatments were divided into 3 groups: normal saline (NS) $20\;{\mu}l$ (CON group); pregabalin 0.3, 1, 3 and $10\;{\mu}g$ in NS $10\;{\mu}l$ (PGB group); glibenclamide $100\;{\mu}g$ in DMSO $5\;{\mu}l$ with pregabalin 0.3, 1, 3 and $10\;{\mu}g$ in NS $5\;{\mu}l$ (GBC group). All the drugs were flushed with NS $10\;{\mu}l$. Immunohistochemistry for the $K_{ATP}$ channel was done with a different set of rats divided into naive, NS and PGB groups. Results: IT pregabalin dose-dependently decreased the flinching number only in phase 2 of formalin test. The log dose response curve of the GBC group shifted to the right with respect to that of the PGB group. Immunohistochemistry for the $K_{ATP}$ channel expression on the spinal cord dorsal horn showed no difference among the groups 1 hr after the formalin test. Conclusions: The antinociceptive effect of pregabalin in the rat formalin test was associated with the activation of the $K_{ATP}$ channel. However, pregabalin did not induce $K_{ATP}$ channel expression in the spinal cord dorsal horn.

The Antinociceptive Effects of Epidural Tramadol with Bupivacaine in Beagle Dogs

  • This study investigated the antinociceptive effect of epidural tramadol with bupivacaine in 36 healthy Beagle dogs. The dogs were divided into 6 groups; 1) C (control), 2) B (0.5% bupivacaine 0.1 mL/kg), 3) BT0.5 (0.5% bupivacaine 0.1 mL/kg + tramadol 0.5 mg/kg), 4) BT1 (0.5% bupivacaine 0.1 mL/kg + tramadol 1 mg/kg), 5) BT2 (0.5% bupivacaine 0.1 mL/kg + tramadol 2 mg/kg), 6) BT3 (0.5% bupivacaine 0.1 mL/kg + tramadol 3 mg/kg). The epidural injection was performed under isoflurane inhalation, after then, nociceptive block and motor block scores were assessed with physiologic parameters (HR, RR, RT, MAP). BT groups showed significantly longer antinociceptive time than C and B, while motor block time of BT groups were not different from B except BT3. Durations of total nociceptive block of BT2 ($60.83{\pm}19.08min$) and BT3 ($74.17{\pm}8.61min$) were significantly longer than those of BT0.5 ($33.33{\pm}8.76min$) and BT1 ($37.50{\pm}19.43min$), but there was no significant difference between BT2 and BT3. Durations of total motor block in all groups were less than 20 minutes although that of BT3 was significantly longer than B. There were no significant differences in HR, RR, RT, MAP among groups. Consequently, epidural administration of tramadol (2 mg/kg) with 0.5% bupivacaine (0.1 mL/kg) can be used safely and effectively in dogs.

A Case of New Surgical Correction of Angular Limb Deformities Using One Screw Implant & Periosteal Transection in a Thoroughbred Foal (Thoroughbred 망아지에서 단일나사못 장착 및 골막박리를 이용한 지세교정술)

  • Angular limb deformities (ALD) are common in foals. A 30-days-old Thoroughbred foal was presented for the evaluation of severe ALD of the both forelimbs. On radiographic examination, both distal radiuses were diagnosed as valgus angular limb deformities. But the degree of deviation of right forelimb was so severe that we tried to correct one after the other. We tried new surgical correction method combination of one screw implant on medial aspect for growth retardation and periosteal transection on lateral aspect of the right forelimb. 40 days later, successfully corrected and then removed the screw. After the right forelimb correction, the periosteal transection on left forelimb was performed. We did the inhalation anesthesia using isoflurane. There were no complications such as fibrosis over the screw heads, and overcorrection that produces an opposing deformity identified. These results suggest that combination of one screw implant and periosteal transection technique is able to be a safe and effective method to correct severe ALD in the foal.

The Effect of Enflurane Anesthesia on The Liver in patient with posilive HBsAg and increased SGOT, SGPT (B형 간염 항원 양성 및 간효소치가 증가된 환자에 Enflurane 마취가 간에 미치는 영향)

  • Halothane is usually a safe and effective inhalation anesthetic agent but it rarely has damaged liver. The authors selected 11 patients who had HBsAg positive and increased SGOT, SGPT at Yeungnam university hospital. Their physical status was ASA class 1 and 2. They had no previous history of operation or liver disease. The liver function tests were performed before surgery, and on 3rd, 7th and 10th postoperative days. The result were as follows : 1) The values of SGOT and SGPT were gradually increased on 3rd postoperative day and markedly increased on the 7th and 10th postoperative day 2) Alkaline phosphatase, total protein, albumin, total bilirubin and direct bilirubin were not significantly changed.

Effect of Intravenous Administration of Tramadol on the Minimum Alveolar Concentration of Isoflurane in Dogs (개에서 트라마돌의 정맥투여가 아이소플루란의 최소폐포농도에 미치는 영향)

  • This study was aimed to evaluate the effects of tramadol hydrochloride on the minimum alveolar concentration of isoflurane ($MAC_{ISO}$) in dogs. Six healthy, female German shepherd dogs (aged 1-2 years) were used in this study. Anesthesia was induced by mask induction and maintained with isoflurane in oxygen. Mechanical ventilation maintained the end-tidal $CO_2$ partial pressure ($P_{ET}CO_2$) from 35 to 45 mmHg throughout the study. A baseline $MAC_{ISO}$ ($MAC_{ISO}B$) was determined starting 45 minutes after induction of anesthesia by clamping a pedal digit until gross purposeful movement was detected. After $MAC_{ISO}B$ determination, dogs received a tramadol loading dose of 3 mg/kg followed by a continuous rate infusion (CRI) of 2.6 mg/kg/h. The determination of $MAC_{ISO}$ after administration of tramadol ($MAC_{ISO}T$) began 20 min after the start of the CRI. Arterial blood pressure and heart rate were recorded continuously and arterial blood samples for blood gas analysis were collected at the end of the equilibration period. Mean ${\pm}$ SD values for the $MAC_{ISO}B$ and $MAC_{ISO}T$ were $1.33{\pm}0.04%$ and $1.23{\pm}0.04%$, respectively. The $MAC_{ISO}B$ decreased significantly by $7.5{\pm}0.2%$ (P < 0.05) after administration of tramadol. The mean heart rate and arterial blood pressure of six dogs were not changed significantly after tramadol administration. The blood gas levels remained constant during the study. In conclusion, tramadol could significantly reduce $MAC_{ISO}$ without depression of cardiorespiratory function. Thus, the use of tramadol on inhalation anesthesia with isoflurane in dogs can improve the stability of anesthesia and the quality of recovery.
