신경가동성에 대한 신경생물학과 신경생역학적 이해 (Neurobiology and Neurobiomechanics for Neural Mobilization)

  • Nervous system is clinically important, and involved in most disorders directly or indirectly. It could be injury and be a source of symptoms. Injury of central or peripheral nervous system injury may affect that mechanism and interrupt normal function. An understanding of the concepts of axonal transport is important for physical therapist who treat injury of nerves. Three connective tissue layers are the endoneurium, perineurium, epineurium. Each has its own special structural characteristics and functional properties. The blood supply to the nervous system is well equipped in all dynamic and static postures with intrinsic and extrinsic vasculation. After nerve injury, alternations in the ionic compression or pressures within this environment may interfere with blood flow and, consequently conduction and the flow of axoplasm. The cytoskeleton are not static. On the contrary, elements of the cytoskeleton are dynamically regulated and are very likely in continual motion. It permits neural mobility. There are different axonal transport systems within a single axon, of which two main flows have been identified : First, anterograde transport system, Secondly, retrograde transport system. The nervous system adapts lengthening in two basic ways. The one is that the development of tension or increased pressure within the tissues, increased intradural pressure. The other is movements that are gross movement and movement occurring intraneurally between the connective tissues and the neural tissues. In this article, we emphasize the biologic aspects of nervous system that influenced by therapeutic approaches. Although identified scientific information in basic science is utilized at clinic, we would attain the more therapeutic effects and develop the physical therapy science.

울산지역 근로자의 보철물과 임플란트의 기대수준에 대한 인식 (Perception on Expectation of Prosthetics and Implant of Workers in Ulsan)

  • 본 연구는 울산시에 소재하는 H기업 예비군 동원훈련에 참여한 307명을 대상으로 보철물(틀니, 브릿지)과 임플란트 기대 수준에 대한 인식을 조사하고, 환자교육 및 상담에 기초자료로 활용하고자 한다. 보철물 기대수준의 신뢰도 cronbach'${\alpha}$는 .971, 임플란트 기대수준의 신뢰도 cronbach'${\alpha}$는 .967로 높게 나타났고 자료의 분석은 IBM SPSS Statistics. v. 19.0 프로그램을 이용하였으며, 유의수준은 0.05로 고려하였다. 연구결과는 첫째, 보철물 기대수준은 평균 3.98점으로 나타났다. 둘째, 임플란트 기대수준은 평균 3.74점으로 나타났다. 셋째, 주관적 건강상태에 따른 임플란트 기대수준은 평균 3.74점으로 나타났고 통계적으로 유의하였다(p=0.003). 넷째, 구강위생관리 관심정도에 따른 임플란트 기대수준의 차이는 평균 3.74점으로 나타났고 통계적으로 유의하였다(0<0.001). 다섯째, 보철물 기대수준과 임플란트 기대수준은 0.392로 강한 양의 상관관계를 나타났다. 위의 결과로 보철과 임플란트에 대한 정확한 정보전달이 필요하다.

The Impact of Educational Status on the Postoperative Perception of Pain

  • Background: Postoperative (PO) pain interferes with the recovery and mobilization of the surgical patients. The impact of the educational status has not been studied adequately up to now. Methods: This prospective study involved 400 consecutive general surgery patients. Various factors known to be associated with the perception of pain including the educational status were recorded as was the preoperative and postoperative pain and the analgesia requirements for the $1^{st}$ PO week. Based on the educational status, we classified the patients in 3 groups and we compared these groups for the main outcomes: i.e. PO pain and PO analgesia. Results: There were 145 patients of lower education (junior school), 150 patients of high education (high school) and 101 of higher education (university). Patients of lower education were found to experience more pain than patients of higher education in all postoperative days (from the $2^{nd}$ to the $6^{th}$). No difference was identified in the type and quantity of the analgesia used. The subgroup analysis showed that patients with depression and young patients (< 40 years) had the maximum effect. Conclusions: The educational status may be a significant predictor of postoperative pain due to various reasons, including the poor understanding of the preoperative information, the level of anxiety and depression caused by that and the suboptimal request and use of analgesia. Younger patients (< 40), and patients with subclinical depression are mostly affected while there is no impact on patients over 60 years old.

쌀겨발효추출물의 항혈전효과 (Anti-thrombotic activity of fermented rice bran extract with several oriental plants in vitro and in vivo)

  • Although the effects of the rice bran have recently been investigated, there is no information regarding platelet physiology available. However, it is well known that fermented natural plants have a beneficial effect on cardiovascular diseases. Therefore, this study was conducted to investigate whether fermented rice bran extract (FRBE) with several plants (Artemisia princeps, Angelica Gigantis Radix, Cnidium officinale, and Camellia sinensis) affected agonist-induced platelet aggregation, and if so, what the underlying mechanism of its activity was. We performed several experiments, including in vitro platelet aggregation, intracellular calcium concentration and adenosine triphosphate release. In addition, the activation of integrin ${\alpha}_{II}b{\beta}3$ was determined using fibrinogen binding. Thrombus formation was also evaluated in vivo using an arterio-venous shunt model. The FRBE inhibited collagen-induced platelet aggregation in a concentration-dependent manner. FRBE significantly and dose dependently attenuated thrombus formation using rat arterio-venous shunt. FRBE suppressed the intracellular calcium mobilization in collagen-stimulated platelets. We also found that FRBE inhibited extracellular stimuli-responsive kinase 1/2, p38-mitogen-activated protein kinases and c-Jun N-terminal kinase phosphorylation. These results suggested that FRBE inhibited collagen-induced platelet aggregation, which was mediated by modulation of downstream signaling molecules. In conclusion, FRBE could be developed as a functional food against aberrant platelet activation-related cardiovascular diseases.

섭식훈련이 연하치료를 받는 뇌졸중 환자의 삼킴기능에 미치는 영향 (The Effects of Eating Training on Patients with Swallowing Disorder Caused by Stroke to Their Swallowing Functions)

  • Purpose : This study was conducted for the purpose of finding out of effects Eating training on patients with swallowing disorder caused by stroke to their swallowing functions. Method : 29 subjects were selected and the divided into two groups. Group 1 is consist of 14 patients under eating training group and Group 2 is consist of 15 patients each group has five times per week. 30 minuted per time covering 12-week period. Selected patients can swallow them selves without aspiration and basically eating trainings involve Thermal Tactile Stimulation(TTS), Shaker exercise, tongue movements, laryngeal mobilization exercise. MASA(The Mann Assessment of Swallowing Ability)and VFSS(Videofluoroscopic Swallowing Study) was carried out to find out effects of training. And We use FDS(Functional Dysphagia Scale) for VFSS to more objective score. Result : Through the results of this study was increase in Eating performance skill actual eating training group than the practiceless group. Therefore, the actual eating training is to improve the swallowing function and It will be useful in clinical intervention for stroke patients. Conclusion : Through the results of this study was increase in Eating performance skill actual eating training group than the practice less group. Therefore, the actual eating training is to improve the swallowing function and It will be useful in clinical intervention for stroke patients.

깊은목굽힘근 강화운동과 등뼈 관절가동운동이 뇌졸중 환자의 전방머리자세와 목통증에 미치는 영향 (The Effect of Deep Neck Flexor Exercise and Thoracic Mobilization Exercise on Forward Head Posture and Neck Pain in Chronic Stroke)

  • 본 연구는 깊은목굽힘근운동과 등뼈 관절가동운동이 뇌졸중 환자의 통증, 전방머리자세에 미치는 영향을 알아보고자 한다. 연구대상자는 36명은 사전측정 후 무작위로 깊은목굽힘근군(DNFE), 등뼈 관절가동운동군(TROM), 대조군으로 각각 12명씩 배정되었다. 중재 전후에 통증(VAS), 두개척추각도(CVA), 두개회전각도(CRA), 전방머리내밈자세(FHP), 등뼈관절가동범위(TROM)를 측정하였다. 그 결과. DNFE 군과 TROM 군은 중재 전후 VAS, CVA, CRA, FHP, TROM에서 유의한 차이를 보였으며(p<.05), 두 군은 대조군에 비해 VAS, CVA, CRA, FHP, TROM에서 보였다(p<.05). 결론적으로 깊은목굽힘근운동과 등뼈 가동범위운동이 뇌졸중 환자의 전방머리자세와 목통증 회복에 효과적이었다. 그러므로 임상에서 뇌졸중 환자 중재프로그램에 깊은목굽힘근운동과 등뼈 가동범위운동을 적극적으로 활용되기를 바란다.

충남에서 발생한 중증응급환자의 응급의료기관 일차 방문, 전원 및 재전원 현황 (Primary visit, transfer, and re-transfer to emergency department in patients with severe emergency diseases in Chungnam)

  • Objective: Regionalization is one of the principal subjects for the advancement of rural emergency medical service systems in South Korea. This study shows the characteristics of interhospital transfer and status of the incidence of three major emergency disorders (acute myocardial infarction [AMI], acute stroke, and severe trauma) in one local province. Methods: A retrospective study was conducted for patients with three major emergency disorders who visited emergency medical facilities in one local province from January 2013 to December 2015, on the basis of the National Emergency Department Information System (NEDIS) data. Results: The incidence of three major emergency disorders had increased annually. Patients with each of these disorders tended to choose distinguishing methods of visiting emergency medical facilities. AMI patients tended to visited emergency medical facilities using private cars or on foot, while severe trauma patients usually visited by 119 ambulance, and acute stroke patients used 119 ambulance and private car in similar amounts. Overall, 65% of AMI patients were treated in intraregional medical facilities, but about 70% of acute stoke and severe trauma patients were transferred outside of the region. Conclusion: Because each of these disorders has an individual characteristic, it is difficult to expect a solution for the problems associated with emergency disorders just by assuring the availability of medical resources. Based on regionalization, a policy to provide the optimal treatment for those emergency disorders should be developed by planning public medical service systems based on the individual characteristics of emergency disorders, the standardized transfer plans of emergency patients and the assurance for mobilization and sharing of confined medical resources.

러시아-우크라이나 전쟁에서의 사이버공격 사례 분석을 통한 한국의 대응 방안에 관한 연구 (A Study on Korea's Countermeasures Through the Analysis of Cyberattack Cases in the Russia-Ukraine War)

  • 러시아-우크라이나 전쟁이 군사적 무력 충돌과 함께 사이버공격이 진행되고 있다. 이번 전쟁과 관련하여 러시아가 한국을 비우호 국가로 지정함에 따라 한국에 대한 사이버공격의 위험성도 고조된 만큼 대응 방안 마련이 시급한 상황이다. 이에 따라 이번 전쟁에서 나타난 사이버공격 사례(19건)를 유형별로 영향을 분석하고, 자원 동원, 기술 진보 등 5가지 관점에서 고찰하여 특징과 시사점을 도출하였다. 이를 통해 한국 정부의 대응 방안으로, 가치공유 국가와의 다자협력 강화, 사이버공격 역량확보와 방어체계 강화, 해외 보안업체와의 연계 방안 마련 등 총 7가지를 제시하였다. 연구 결과는 한국 정부의 사이버안보 정책 수립에 활용될 수 있을 것이다.

3D 프린터로 제작된 도구를 이용한 연부조직가동술(IASTM) 적용방법에 따른 근활성도 비교 (Comparison of Muscular Activity in Method with the Application of 3D Printer Maded Instrument Assistant Soft Tissue Mobilization(IASTM) Tools)

  • 본 연구는 3D 프린터로 제작 도구를 이용하여 근육의 주행방향에 대해 강찰법 방향의 차이로 인한 근활성도 차이를 확인하고자 한다. 대상자는 건강한 20대 성인 20명으로 진행하였고, IASTM을 근육 주행방향의 수직 강찰법 중재(수직군)와 근육 주행방향의 수평 강찰법 중재(수평군)를 적용하였으며 대상자를 각 10명씩 배치하였다. 모든 대상자는 중재 전과 후의 근활성도를 독립표본 t-검정과 대응표본 t-검정을 사용하여 측정, 비교하였다. 본 연구의 결과는 수평군에서 근활성도가 감소하였으나 수직군에서 근활성도는 유의한 증가가 나타났다. 이는 IASTM의 적용에 방향성의 차이가 의미가 있음을 알 수 있다.

만성 봉우리 밑 충돌증후군을 위한 물리치료적 접근법 : 편심성 훈련과 일반적 운동의 효과 비교 (Physiotherapy Approaches for Chronic Subacromial Impingement Syndrome : Comparison of Effects of Eccentric Training and General Exercise)

  • Purpose : This study was to verify the effect of eccentric training and suggest a specific application method by comparing the effects of acromohumeral distance (AHD), supraspinatus tendon thicknees (STT), pain intensity and functional performance after MWM and eccentric training (MWM-ET) or general exercise (MWM-GE) in chronic subacromial impingement syndrome (SAIS) patients. Methods : A total of 55 participants were randomly assigned to each group, and according to the intervention method, "MWM-ET group (n=28)" vs. "MWM-GE group (n=27)" was divided into two groups. AHD, STT, pain intensity, and functional performance were measured before intervention, and both groups were re-measured 3 times a week after 6 weeks of intervention in the same way. Results : The AHD was significantly increased in MWM-ET group compared to MWM-GE group. No significant difference was observed between the groups in the STT, but Pain intensity was significantly lower in MWM-ET group than in MWM-GE group, and functional performance was significantly increased in MWM-ET group compared to MWM-GE group. Conclusion : As a result of MWM-ET intervention that further increases AHD compared to MGE, it can be clinically presented as a more effective intervention method for faster recovery from injury due to pain reduction and smooth return to daily life due to improved functional performance.
