A Case Report of Takotsubo Cardiomyopathy During Breast Augmentation (유방확대술 중 발생한 Takotsubo 심근병 1례)

  • Purpose: Takotsubo cardiomyopathy is a relatively uncommon type of stress-induced cardiomyopathy characterized by transient left ventricular regional wall motion abnormalities. Emotional and physical stresses play a key role in this type of cardiomyopathy in postmenopausal women. The current hypothesis is that the syndrome represents a form of catecholamine surge due to stress or epinephrine-mediated acute myocardial stunning. Methods: A 44-year-old woman had suffered premature ventricular contraction following a cardiogenic shock during a breast augmentation surgery under enflurane anesthesia and tumescent solution infiltration. She was treated with cardiopulmonary resuscitation at a local clinic. Then she was brought to the Emergency Department of the authors' hospital. Results: The woman's echocardiogram showed an ejection fraction of 20~25% with associated basal hyperkinesis and left ventricular apical ballooning. The patient was admitted to the ICU and required inotropic support for two weeks. The patient's condition dramatically improved, and her ejection fraction returned to 70%. Conclusion: It is believed that there were multiple triggering factors of the onset of Takotsubo cardiomyopathy in the woman's social and family history, including infiltration of a large volume of the tumescent solution and VPCs induced by enflurane anesthesia without premedication. The importance of careful history-taking, careful pre-operative consultation on psychological suffering especially for breast surgery, premedication before surgery, patient reassurance, and post-operative psychosocial and emotional assistance was again seen in this case.

Influence of Gibangganbang on Ethanol-Induced Fatty Change and Regeneration Ability of Liver (지방간방(脂肪肝方)이 Ethanol로 유도(誘導)된 지방간(脂肪肝) 및 간재생능(肝再生能)에 미치는 영향(影響))

  • This study was to investigate the effects of Gibangganbang on the liver injury induced dimethylnitrosamine, CCl4, ethanol and partial hepatectomy. Hydroxyproline, hepatic fibrosis, hepatic inflammatory cell infiltration, fatty value, lipidoperoxide levels, glutathione levels, mitotic index, contents of protein in the serum and liver tissues were measured and observed. The results obtained were as follows. 1. The increasing level of hydroxyproline volume induced by dimethylnitrosamine in mice was decreased by the oral administration of Gibangganbang. 2. The degree of hitological fibrosis and hepatic inflammatory cell infiltration induced by $CCl_4$ was decreased by the oral administration of Gibangganbang. 3. The degree of fatty value and the increasing level of lipidoperoxide in liver tissues was decreased by the oral administration of Gibangganbang. 4. The level of glutathione in liver tissues was increasing by the oral administration of Gibangganbang. 5. The increasing level of microsomal lipidoperoxide in vitro assay was decreasing by the oral administration of Gibangganbang. 6. The increasing level of the mitotic index, weight of liver, contents of protein, RNA and DNA synthesis of the liver tissues after partial hepatectomy was activated by the oral administration of Gibangganbang. These results suggest that Gibangganbang not only inhibits liver cirrhotic change and ethanol-induced fatty change but also activates antioxidant enzymes and regeneration ability ocf liver.

  • For receiving water quality protection a control systems of urban drainage for CSOs reduction is needed. Examples in combined sewer systems include downstream storage facilities that detain runoff during periods of high flow and allow the detained water to be conveyed by an interceptor sewer to a centralized treatment plant during periods of low flow. The design of such facilities as storm-water detention storage is highly dependant on the temporal variability of storage capacity available as well as the infiltration capacity of soil and recovery of depression storage. For the continuous long-term analysis of urban drainage system this study used analytical probabilistic model based on derived probability distribution theory. As an alternative to the modeling of urban drainage system for planning or screening level analysis of runoff control alternatives, this model has evolved that offers much ease and flexibility in terms of computation while considering long-term meteorology. This study presented rainfall and runoff characteristics of the subject area using analytical probabilistic model. Runoff characteristics manifested the unique characteristics of the subject area with the infiltration capacity of soil and recovery of depression storage and was examined appropriately by sensitivity analysis. This study presented the average annual CSOs, number of CSOs and event mean CSOs for the decision of storage volume.

Neuroprotective Effects of Ginkgo biloba extract, GBB, in the Transient Ischemic Rat Model

  • In the present study, we investigated the neuroprotective effects of standardized Ginkgo biloba extract (GBB) (total terpene trilactones, 13 ${\pm}$ 3%; biflavone, 4.5 ${\pm}$ 1.5%; flavonol glycoside, < 8%; proanthocyanidine, under detection limit) on ischemia-reperfusion-induced brain injury in the rats. Ischemia was induced by the intraluminal occlusion of the right middle cerebral artery for 2 h and reperfusion was continued for 22 h. GBB was orally administered, promptly prior to reperfusion and 2 h after. Total infarction volume in the ipsilateral hemispheres of ischemia-reperfusion rats were significantly reduced by treatment with GBB in a dose-dependent manner (P<0.05). The therapeutic time window of GBB was 3 h in this ischemia-reperfusion rat model. Furthermore, GBB also significantly inhibited increased neutrophil infiltration of ischemic brain tissue, as estimated by myeloperoxidase activity. These findings suggest that GBB plays a crucial protective role in ischemia-induced brain injury, in part, via inhibition of neutrophil infiltration, and suggest that this GBB could serve as a neuroprotective agent following transient focal ischemic brain injury.

Decay Rate of the Nitrogen Dioxide in Indoor Residence Using Mass Balance Model (물질수지 모델을 이용한 주택 실내의 이산화질소 감소율)

  • The purpose of this study was to determine nitrogen dioxide(NO$_2$) decay rate by reaction between NO$_2$ and interior materials in Korean residence. The results of this research could be helpful to choose the interior construction materials and to study on reduction of indoor air pollutants. The results of this research are as follows; For 30 residences in Seoul and Incheon from October 2000 to march 2001, the mean of infiltration rate was 0.70$\pm$0.44 ACH, and single-detached houses (7 houses) and apartments (19 houses) were 0.97$\pm$0.55 ACH and 0.61$\pm$0.34 ACH, respectively. The $CO_2$ decay followed approximately first-order process ($R^2$=0.97$\pm$0.02). There existed a statistic significance in filtration rate between houses built in 1980’s and built in 1990’s by t-test (p<0.02). Mean of NO$_2$ decay rates in 26 residence3s except 4 residences was 0.94$\pm$0.49hr$^{-1}$ , and also 0.86$\pm$0.49hr$^{-1}$ , 0.97$\pm$0.50hr$^{-1}$ in single-detached houses and apartments, respectively. Mean NO$_2$ decay rates in houses built in 1980’s were 0.78$\pm$0.37hr$^{-1}$ , 1.33$\pm$1.03hr$^{-1}$ , respectively. Nothing were showed statistical significance among indoor temperature, indoor humidity, and NO$_2$ decay rate. However, NO$_2$ decay rates had a tendency to increase by increase of temperature and humidity. Average volume/surface of participated houses was 0.55$\pm$0.07m and mean NO$_2$ deposition velocity was calculated as 1.46$\pm$0.59msec$^{-1}$ .

  • Shallow failures occur frequently in both engineered and natural slopes in expansive soils. Rainfall infiltration is the most predominant triggering factor that contributes to slope failures in both expansive soils and clayey soils. However, slope failures in expansive soils have some distinct characteristics in comparison to slopes in conventional clayey soils. They typically undergo shallow failures with gentle sliding retrogression characteristics. The shallow sliding mass near the slope surface is typically in a state of unsaturated condition and will exhibit significant volume changes with increasing water content during rainfall periods. Many other properties or characteristics change such as the shear strength, matric suction including stress distribution change with respect to depth and time. All these parameters have a significant contribution to the expansive soil slopes instability and are difficult to take into consideration in slope stability analysis using traditional slope stability analysis methods based on principles of saturated soil mechanics. In this paper, commercial software VADOSE/W that can account for climatic factors is used to predict variation of matric suction with respect to time for an expansive soil cut slope in China, which is reported in the literature. The variation of factor of safety with respect to time for this slope is computed using SLOPE/W by taking account of shear strength reduction associated with loss of matric suction extending state-of-the art understanding of the mechanics of unsaturated soils.

Influences of heating processes on properties and microstructure of porous CeO2 beads as a surrogate for nuclear fuels fabricated by a microfluidic sol-gel process

  • The control of microstructure is critical for the porous fuel particles used for infiltrating actinide nuclides. This study concerns the effect of heating processes on properties and microstructure of the fuel particles. The uniform gel precursor beads were synthesized by a microfluidic sol-gel process and then the porous $CeO_2$ microspheres, as a surrogate for the ceramic nuclear fuel particles, were obtained by heating treatment of the gel precursors. The fabricated $CeO_2$ microspheres have a narrow size distribution and good sphericity due to the feature of microfluidics. The effects of heating processes parameters, such as heating mode and peak temperatures on the properties of microspheres were studied in detail. An optimized heating mode and the peak temperature of $650^{\circ}C$ were selected to produce porous $CeO_2$ microspheres. The optimized heating mode can avoid the appearance of broken or crack microspheres in the heating process, and as-prepared porous microspheres were of suitable pore size distribution and pore volume for loading minor actinide (MA) solution by an infiltration method that is used for fabrication of MA-bearing nuclear fuel beads. After the infiltration process, $1000^{\circ}C$ was selected as the final temperature to improve the compressive strength of microspheres.

Effects of Dangguijakyaksan on the Development of Experimentally-induced Endometriosis and Related Cytokines in Rats (당귀작약산(當歸芍藥散)이 실험적으로 유발된 흰쥐의 자궁내막증의 발달 및 관련 Cytokine에 미치는 영향)

  • Purpose: This study was performed to investigate the effects of Dangguijakyaksan on the development of experimentally-induced endometriosis in rats. Methods: Endometriosis was induced in rats by autotransplanting uterine tissue to the peritoneum and divided them into three groups: (1) sham-operated group(n=8). (2) surgically induced endometriosis and untreated control group(n=8). (3) surgically induced endometriosis and Dangguijakyaksan treated group. Dangguijakyaksan was orally administrated for 15 days after operation. Then we measured the body weight. the volume of endometriotic implants. the weight of uterus and ovary. and analysed the concentration of cytokines (MCP-1, TNF-$\alpha$, IL-1$\beta$, IL-6) in peritoneal fluids. Histopathology, immunohistochemistry for COX-2 and VEGF. and histochemistry for mast cell in transplanted uterine tissue were also performed. Results: - The volume($mm^3$) of endometriotic implants in Dangguijakyaksan treated group was significantly decreased compared with control group. - The concentration(pg/ml) of MCP-1, TNF-$\alpha$ and IL-l$\alpha$ in peritoneal fluids in Dangguijakyaksan treated group was significantly decreased compared with control group. - Histopathologically, proliferation of endometriotic epithelia. infiltration of inflammatory cells and angiogenesis in transplanted uterine tissue of Dangguijakyaksan treated group were weakly observed than those of control group. - The percentage of positive epithelial layers for COX-2 in Dangguijakyaksan treated group was significantly decreased compared with control group. - The VEGF expression of endometriotic epithelia, neovascular endothelia and stromal cells in transplanted uterine tissue of Dangguijakyaksan treated group were weakly observed than those of control group. - The number of mast cells in transplanted uterine tissue in Dangguijakyaksan treated group was significantly decreased compared with control group. Conclusion: On the basis of these results. we concluded that Dangguijakyaksan has inhibiting effects on the development of transplanted uterine tissue. And these effects may be related with decreased production of MCP-1, TNF-$\alpha$ and IL-1$\beta$, and decreased expression of COX-2 and VEGF, and inhibition of mast cell infiltration by administration of Dangguijakyaksan.

Effects of Bojungikgi-Tang on the Development of Experimentally-induced Endometriosis in the Rats (보중익기탕(補中益氣湯)이 실험적으로 유발된 흰쥐의 자궁내막증(子宮內膜症) 발달에 미치는 영향)

  • Purpose: This study was performed to investigate the effects of Bojungikgi-Tang on the development of experimentally-induced endometriosis in rats. Histopathology, immunohistochemistry for COX-2 and VEGF, and histochemistry for mast cell in transplanted uterine tissue were also performed. Results: 1. The volume($mm^3$) of endometriotic implants in Bojungikgi-Tang treated group(129${\pm}$39) was significantly decreased(p<0.05) compared with control group(222${\pm}$109). 2. The concentration(pg/$m{\ell}$) of MCP-1 in peritoneal fluids in Bojungikgi-Tang treated group(1012${\pm}$248) was significantly decreased(p<0.05) compared with control group(1413${\pm}$346). 3. The concentration(pg/$m{\ell}$ of TNF-${\alpha}$ in peritoneal fluids in Bojungikgi-Tang treated group(887${\pm}$196) was significantly decreased(p<0.05) compared with control group(1126${\pm}$140). 4. Histopathologically, proliferation of endometriotic epithelia, infiltration of inflammatory cells and angiogenesis in transplanted uterine tissue of Bojungikgi-Tang treated group were weakly observed than those of control group. 5. The percentage of positive epithelial layers for COX-2 in Bojungikgi-Tang treated group(56${\pm}$17) was significantly decreased(p<0.05) compared with control group(75${\pm}$16). 6. The VEGF expression of endometriotic epithelia, neovascular endothelia and stromal cells in transplanted uterine tissue of Bojungikgi-Tang treated group were weakly observed than those of control group. Conclusion: On the basis of these results, we concluded that Bojungikgi-Tang has inhibiting effects on the development of transplanted uterine tissue. And these effects may be related with decreased production of MCP-1, TNF-${\alpha}$, and decreased expression of COX-2 and VEGF infiltration by administration of Bojungikgi-Tang.

  • The purpose of this study was to evaluate histologically the effect of LiF-maleic acid added calcium aluminate(LM-CA) bone cement & CA-PMMA composite bone cement on the healing of calvarial defect in Sprague-Dawley rats. The critical size defects were surgically produced in the calvarial bone using the 8mm trephine bur. The rats were divided in three groups : In the control group, nothing was applied into the defect of each rat. LM-CA bone cement was implanted in the experimental group 1 and CA-PMMA composite bone cement was implanted in the experimental group 2. Rats were sacrificed at 2, 8 weeks after surgical procedure. The specimens were examined by histologic analysis, especially about the bone-cement interface and the response of surrounding tissue. The results are as follows; 1. In the control group, inflammatory infiltration was observed at 2 weeks. At 8 weeks, periosteum and duramater were continuously joined together in the defect area. But the center of defect area was filled up with the loose connective tissue. 2. In the experimental group 1, the bonding between implanted bone cement and the existing bone was seen, which more increased in 8 weeks than 2 weeks. Inflammatory infiltration and the dispersion of implanted bone cement particles were seen in both 2 weeks and 8 weeks. 3. In the experimental group 2, implanted bone itself had a dimensional stability and no bonding between implanted bone cement and the existing bone was seen in both 2 weeks and 8 weeks. Implanted bone cement was encapsulated by fibrous connective tissue. In addition, inflammatory infiltration was seen around implanted bone cement. On the basis of these results, when LM-CA bone cement or CA-PMMA composite bone cement was implanted in rat calvarial defect, LM-CA bone cement can be used as a bioactive bone graft material due to ability of bonding to the existing bone and CA-PMMA can be used as a graft material for augmentation of bone-volume due to dimensional stability.
