Location of Back-shu Points of the Bladder Meridian in the Lumbar Region through Patient Measurement

  • Background: The purpose of this study was to compare the first branch of the bladder meridian (FBBM) as determined by the proportional bone measurement method (PBMM), to the line formed by the erector spinae muscle group, and to establish an academic basis for selection of acupuncture points and needling. Methods: Sixty participants were divided into 3 groups based on body mass index (BMI) and into 2 groups based on waist/height ratios. The distance from the midline of the spine to the first branch of the bladder meridian with PBMM (DFBBM), and the distance from the midline of the spine to the most elevated fleshy region of the erector spinae (DMEFR), at the same level as the inferior border of the spinous processes of L1-L5, were measured. The DFBBM and the 5 DMEFRs were then analyzed according to BMI and the waist/height ratio. Results: DFBBM was statistically different from DMEFR in all back-shu points in the lumbar region. DFBBM was not significantly different from DMEFR in the groups with a high BMI or waist/height ratio. However, there was a statistical difference in the groups with a low or moderate BMI or low waist/height ratio. Conclusion: Since the location of the most elevated fleshy region of the erector spinae does not coincide with the location of the FBBM, the selection of back-shu points in the lumbar region must be performed precisely by PBMM.

A STUDY ON THE CEPHALOMETRIC SIMILARITY BETWEEN PARENTS AND OFFSPRING IN CLEFT LIP WITH OR WITHOUT PALATE (순ㆍ구개열 환자의 두부규격방사선사진상을 이용한 친자간의 유사성에 관한 연구)

  • The purpose of this study was to determine whether any similarity existed in craniofacial morphology between parents and offspring in cleft lip with or without cleft palate. Thirty three measurements of the various regions of cranium and face were obtained from lateral cephalometric radiograms in 28 families comprising 28 fathers, 28 mothers and 28 cleft patients. The measurements of cleft patients were compared with those of their fathers, mothers and midparents. The obtained results were as follows: 1. There were similar measurements between the cleft patients and their fathers; rama1 height(Ar-Go), mandibular angle(∠MP-RP). 2. There were similar measurements between the cleft patients and their mothers; cranial base angle(∠NSBa), relation of maxilla to the cranial base(∠SNA), relation of maxilla to the cranial base(soft tissue:∠BaN'Sn), angle of inferior border of mandible(∠SNL-MP) and convexity of nose apex(soft tissue:∠N'PmPog'). 3. There were similar measurements between the cleft patients and their midparents; ramal height (Ar-Go), cranial base angle( ∠NSBa), relation of maxilla to the cranial base(soft tissue: ∠BaN'Sn), Y axis angle(∠NSGn) and mandibular angle(∠MP-RP). 4. There was no similar measurements between the cleft patients and their fathers and mothers simultaneously.

Hair Diameter Variation in Different Vertical Regions of the Occipital Safe Donor Area

  • Background Little is known concerning hair diameter variation within the safe donor area for hair transplantation surgery. Thicker or thinner hair may be needed, depending on the recipient area, hairline design, and the purpose of surgery. Methods Twenty-seven patients (7 men and 20 women; mean age, 28 years; range, 20-47 years) were included in this study. The midoccipital point was used as the reference point on the horizontal plane at the upper border of the helical rim. The target area width was 15 cm (7.5 cm to the right and left of the reference point) and the height was 8 cm (2 cm above and 6 cm below the reference point). The study area was divided horizontally into 3 5-cm sections (A, B, C) and vertically into 4 2-cm sections (1-4), creating a total of 12 zones. Ten anagen hairs were randomly obtained from each zone and their diameters were measured. Results Hair diameter in the 4 vertical sections varied significantly, gradually decreasing from sections 1 (superior) to 4 (inferior) in all 3 horizontal sections (A, B, and C). Conclusions Our results suggest that sections 1 and 2 of the occipital safe donor area would be useful for obtaining thicker hair, such as in procedures to treat male- and female-pattern hair loss, whereas hair from zones 3 and 4 could be useful for transplantation surgery requiring thinner hair, such as eyebrows, eyelashes, and female hairline correction. Our results may be clinically valuable for planning hair transplant surgery and choosing the optimal donor region.

Effect of Gastric Filling on the Images of Hepatic Scintigraphy (위의 충만유무에 의한 간신티그램영상의 변화)

  • To observe the effect of gastric filling on the images of hepatic scintigraphy, we performed hepatic scintigraphy using $^{99m}Tc-tin$ colloid in twenty patients both in the fasting state and after meal, and made the visual comparison of both images. We found that gastric filling could cause some of the following changes in individual patients. 1) In the anterior view, the distance between the liver and the spleen increased and the interlobar notch of the inferior hepatic border was obliterated. The spleen was displaced downwards, its radioactivity in the upper medial part was reduced, and its upper pole was laterally displaced. 2) In the posterior view, the left lobe of the liver was separated from the spleen and the distance between the liver and the spleen increased. 3) In the right lateral view, the notch separating the right and the left lobes of the liver was well demarcated. 4) In the left lateral view, the spleen was separated from the liver and the distance between the liver and the spleen increased. The shape of the spleen was also changed. In case of the follow-up study, above changes should make one consider the possibility of the effect of gastric filling.

An Empirical Analysis on the Effect of Naepo New Town on the Local Economy of Neighboring Areas (내포신도시 조성에 따른 지역경제 영향에 대한 실증분석)

  • A new town attracts a large population in the short period due to its excellent residential conditions, and forms a local economy. However, if the new town is constructed in an inferior area, it would cause a negative effect to neighboring areas by absorbing population and firms from them. Nevertheless, the relocation of Chungnam provincial office from Daejeon, a metropolitan area to Naepo belongs to the key project of constructing Naepo. If the officers and their family members move to Naepo following the relocation and form a new consumer group of a local economy, the construction of Naepo would cause a positive effect to its neighboring areas. This paper empirically analyzes the effect of Naepo on the local economy through the change in differences of the new openings of restaurants between areas (defined either by donut-shaped area with 500-meter width or by Eup, Myun, and Dong) inside and those outside of 10km from the border of Naepo. The results from regression equation estimation where difference-in-differences framework is applied imply that the construction of Naepo new town caused a positive economic effect to its neighboring areas.

Study on the position of the posterior superior alveolar artery in relation to the performance of the maxillary sinus bone graft procedure in a Korean population

  • Objectives: This study sought to investigate the positioning of the posterior superior alveolar artery in relation to the performance of the maxillary sinus bone graft procedure in a Korean population. Materials and Methods: We identified the position of the posterior superior alveolar artery relative to 93 maxillary sinuses in 58 patients and determined the distance from the inferior border of the artery in the premolar and molar areas to the alveolar ridge and sinus floor. Results: The mean distance from the alveolar ridge to the posterior superior alveolar artery in the dentate group ($20.62{\pm}3.05mm$ in the premolar region, $17.50{\pm}2.84mm$ in the molar region) was greater than as compared to the edentulous group ($18.83{\pm}2.79mm$ in the premolar region, $15.50{\pm}1.64mm$ in the molar region), and this difference was statistically significant (P<0.05). In contrast, there was no statistically significant difference (P>0.05) between the mean distance from the sinus floor to the posterior superior alveolar artery in the dentate group ($8.21{\pm}2.79mm$ in the premolar region, $7.52{\pm}2.07mm$ in the molar region) or in the edentulous group ($7.75{\pm}3.31mm$ in the premolar region, $7.97{\pm}2.31mm$ in the molar region). Conclusion: Prior to surgery, it is important to evaluate the position of the posterior superior maxillary artery by using computed tomography scans. The premolar area is safer than the molar area for performing the maxillary sinus bone graft without bleeding.

A CASE REPORT OF BRANCHIAL CLEFT CYST IN THE NECK (경부에 발생한 새열낭 (branchial cleft cyst)의 증례보고)

  • Branchial cleft cyst is the most common lateral neck cyst; the vast majority are of the second branchial cleft origin. This presumably reflects the greater depth and longer persistence of the second cleft, compared with the first, third, and fourth clefts. We experienced a 49-year-old male whose chief complaint was a abnormal mass of the left parotid gland area and neck. As a result of careful analysis of clinical, radiological, and histopathological findings, we diagnosed it as a second branchial cleft cyst in the neck and obtained results as follows: 1. In clinical examination, there was a 10×15㎝ sized, fluctuant painful mass in the left neck and parotid area. 2. In radiographic examination, a low echogenic mass with internal cystic change in the inferior parotid gland area was noted sonographically. Computed tomograph showed a 3×4㎝ sized, well-defined cystic mass with heterogenous solid component in the anterior border of sternocleidomastoid muscle. MRI revealed 5×6㎝ sized, well-marginated multi separated mass in the same area. 3. In histopathological examination, lining of cyst was stratified squamous epithelium with typical lymph node pattern and inflammatory cell infiltration.

Radiologic study of mandibular foramen of mandibular prognathism by three-dimensional computed tomography (3차원 전산화단층영상을 이용한 턱나옴증 환자의 하악공의 방사선학적 연구)

  • Purpose : This study is aimed to evaluate the position of mandibular foramen of mandibula prognathism patients using 3-dimensional CT images in order to reduce the chance of an anesthetic failure of the mandibular nerve and to prevent the damage to the inferior alveolar nerve during the orthognathic surgery. Materials and Methods : The control group consist of 30 patients with class I occlusion. The experimental group consist of 44 patients with class III malocclusion. Three-dimensional computed tomography was used to evaluate the position of the mandibular foramina. Results : The distance between mandibular plane and mandibular foramen, class I was 25.385 mm, class III was 23.628 mm. About the distance between occlusal plane and mandibular foramen, class I was 1.478 mm, class III was 5.144 mm. The distance between posterior border plan of mandibular ramus and mandibular foramen had not statistically significant. About the distance between sagittal plane of mandible and mandibular foramen did not also showed statistically significant. Conclusion : The result of this study could help the clinicians to apprehend more accurate anatomical locations of the foramina on the mandible with various facial skeletal types. thereby to perform more accurate block anesthesia of the mandibular nerve and osteotomy with minimal nerve damage. In addition, this study could provide fundamental data for any related researches about the location of the mandibular foramina for other purposes.

A COMPUTERIZED TOMOGRAPHIC STUDY ON THE STRUCTURE OF THE MANDIBULAR RAMUS (전산화단층사진을 이용한 하악지구조분석)

  • This study was conducted for the purpose of suggestion of the new technique of sagittal split ramus osteotomy pararell to the true sagittal plane. This pararellism is the important concept of the sagittal split ramus osteotomy to reduce the condylar sagging including mandibular hypomobility, temporomandibular disorder, occlusal relapse and other complications. We used 26 adult dry manibles(52 rami), and obtained the computed tomographs through the sagittal, horizontal and coronal sections. The results were obtained as follows. 1. On sagittal section, mean area of S1 was $8.63{\pm}2.10cm^2$, S2 was $8.93{\pm}1.94cm^2$, S3 was $9.49{\pm}2.15cm^2$, S6 was $10.72{\pm}2.22cm^2$. The wider area of sagittal section, the more lateral section, But, no significant differency between the areas of the sagittal sections(P>0.05). 2. On horizontal section, The distance between the inferior alveolar canal and the lateral cortical plate of the mandibular ramus were $6.73{\pm}1.24mm$ minum, $7.70{\pm}1.44mm$ maximum. 3. On coronal section, Outer mandibular angle were $4.84{\pm}2.37^{\circ}$ right side, $4.93{\pm}2.12^{\circ}$ left side. 4. The design of the ideal true sagittal split ramus osteotomy is that posterior border of osteotomy must be limited vertically, at the right posterior point of lingula mandibularis and anterior of osteotomy must be extended to mandibular body, anteroinferiorly.

  • The Ameloblastoma is a true neoplasm of enamel organ-type tissue histopathologically. Ameloblastoma is recognized as a benign tumor, but its clinical behavior is locally invasive. Therefore treatment of choice is surgical resection rather than conservative enucleation. This is a case report about En-bloc excision of granular ameloblastoma arising from dentigerous cyst in the right mandibular angle area of 50-year male patient. He visited local clinic with chief complaint of facial swelling on the right mandibular angle area. Surgical removal was performed by cyst enucleation and En-bloc excision with preservation of inferior border of mandible. After 1 year, iliac bone graft was done and hyperbaric oxygen therapy was applied. We obtained a good result of bony regeneration without any sign of recurrence.
