Assessing the risk of recurrence of porcine epidemic diarrhea virus in affected farms on Jeju Island, South Korea

  • Background: Porcine epidemic diarrhea virus (PEDV) is a swine enteropathogenic coronavirus that has devastated the swine industry in South Korea over the last 30 years. The lack of an effective method to control the endemics has led to a surge in PEDV recurrences in affected farms throughout the country. Objectives: In the first step toward establishing systematic monitoring of and active control measures over the swine populations, we constructed an assessment model that evaluates the status of (1) biosecurity, (2) herd immunity, and (3) virus circulation in each of the PEDV-infected farms. Methods: A total of 13 farrow-to-finish pig farms with a history of acute PEDV infection on Jeju Island were chosen for this study. The potential risk of the recurrence in these farms was estimated through on-site data collection and laboratory examination. Results: Overall, the data indicated that a considerable number of the PEDV-infected farms had lax biosecurity, achieved incomplete protective immunity in the sows despite multi-dose vaccination, and served as incubators of the circulating virus; thus, they face an increased risk of recurrent outbreaks. Intriguingly, our results suggest that after an outbreak, a farm requires proactive tasks, including reinforcing biosecurity, conducting serological and virus monitoring to check the sows' immunity and to identify the animals exposed to PEDV, and improving the vaccination scheme and disinfection practices if needed. Conclusions: The present study highlights the significance of coordinated PEDV management in infected farms to reduce the risk of recurrence and further contribute towards the national eradication of PEDV.

A comparative study of the prevalence of Helicobacter pylori in the oral biofilms of a group of dental and non-dental undergraduates from Sri Lanka

  • Dental health care workers (DHCW) are at a risk of occupational exposure to Helicobacter pylori from the aerosolized oral biofilms and saliva of patients. We designed this study to investigate the prevalence of H. pylori in the oral biofilms of a group of dental and non-dental undergraduates from Sri Lanka. After obtaining informed consent, oral biofilms were collected from 38 dental undergraduates (19 males and 19 females) undergoing clinical training and 33 non-dental undergraduates (14 males and 19 females). The participants were in the age range of 22-27 years and had healthy periodontium. Total DNA from the oral biofilms were extracted, and H. pylori DNA was detected using polymerase chain reaction (PCR) amplification of 16S rRNA gene of H. pylori using JW22-JW23 primers, and the results were confirmed using PCR amplification of H. pylori-urease specific HPU1-HPU2 primers. Out of 71 participants, 11 (28.95%) dental and 3 (9.09%) non-dental undergraduates had H. pylori in their oral biofilms indicating an overall prevalence rate of 19.72% (14/71). Thus, the prevalence of H. pylori in oral biofilms was significantly higher in dental undergraduates than in non-dental undergraduates (p < 0.05). An odds ratio of 4.07 indicated that dental undergraduates were four times more likely to harbor H. pylori in their oral biofilms than non-dental undergraduates. Foregoing data support the fact that there may be greater occupational risk of exposure to H. pylori for dental undergraduates during clinical training than that for non-dental undergraduates, warranting meticulous infection control practices during clinical dentistry.

DLDW: Deep Learning and Dynamic Weighing-based Method for Predicting COVID-19 Cases in Saudi Arabia

  • Multiple waves of COVID-19 highlighted one crucial aspect of this pandemic worldwide that factors affecting the spread of COVID-19 infection are evolving based on various regional and local practices and events. The introduction of vaccines since early 2021 is expected to significantly control and reduce the cases. However, virus mutations and its new variant has challenged these expectations. Several countries, which contained the COVID-19 pandemic successfully in the first wave, failed to repeat the same in the second and third waves. This work focuses on COVID-19 pandemic control and management in Saudi Arabia. This work aims to predict new cases using deep learning using various important factors. The proposed method is called Deep Learning and Dynamic Weighing-based (DLDW) COVID-19 cases prediction method. Special consideration has been given to the evolving factors that are responsible for recent surges in the pandemic. For this purpose, two weights are assigned to data instance which are based on feature importance and dynamic weight-based time. Older data is given fewer weights and vice-versa. Feature selection identifies the factors affecting the rate of new cases evolved over the period. The DLDW method produced 80.39% prediction accuracy, 6.54%, 9.15%, and 7.19% higher than the three other classifiers, Deep learning (DL), Random Forest (RF), and Gradient Boosting Machine (GBM). Further in Saudi Arabia, our study implicitly concluded that lockdowns, vaccination, and self-aware restricted mobility of residents are effective tools in controlling and managing the COVID-19 pandemic.

콩 재배방법이 미이라병과 자주무늬병의 발생 및 식물생육에 미치는 영향 (Effect of Cultural Practices on the Occurrence of Pod and Stem Blight and Purple Blotch, and on Soybean Growth)

  • 조숙성 콩품종을 이용하는 풋콩 재배에서는 콩 미이라병과 자주무늬병에 의한 꼬투리 품질 저하와 종자콩의 확보문제가 큰 장애가 되고 있다. 콩 미이라병은 병든 식물의 잔사물이 중요한 제1차 전염원이며, 식물체에 감염하여도 살아 있는 조직에서는 균사의 생육이 극히 제한적이고, 잠재감염 상태로 포자형성이 거의 없으므로 이러한 병원균의 특성을 이용하는 재배적 방제의 가능성을 검토하였다. 점적관수는 분무관수에 비하여 무피복 재배에서는 종자감염율을 현저히 감소시켰으나 피복재배에서는 큰차이가 없었다. 피복재배는 콩 미이라병 28.0%, 자주무늬병 29.3%의 방제가를 보였으나 베노밀 방제가에는 미치지 못하였다. 식물체 잔사물 줄기의 78%는 7월초에 병포자가 성숙되어 제1차 전염원으로 작용하며, 당년의 식물체에서 탈락한 노화 떡잎과 염병에서도 7월 중순부터 9월에 걸쳐 충분한 제2차 전염원이 형성되었다 결론적으로, 비닐피복과 점적관수는 콩 수량성에 영향을 미치지 않고 어느 정도의 콩 미이라병과 자주무늬병 방제효과가 있으며, 제2차 전염원을 제거하는 포장위생관리의 병행은 방제효과를 더욱 제고시킬 것으로 보였다.

키위 궤양병 효율적 관리를 위한 매뉴얼 (A Proposed Manual for the Efficient Management of Kiwifruit Bacterial Canker in Korea)

  • 키위 궤양병균인 Pseudomonas syringae pv. actinidiae는 최근 전세계에서 심각하게 경제적 손실을 초래하고 있다. 궤양병균은 우리나라에서 그린키위와 골드키위 품종들을 각각 1988년과 2006년부터 침해해왔다. 최근에는 오염된 수입꽃가루에 의해 유입된 궤양병균 biovar 3 (Psa3)가 주변 키위 재배 농가로 2차감염에 의해 급속하게 확산되어 골드키위와 레드키위 품종들에 대발생하여 피해를 주고 있다. 이 총설에서는 지난 30년간 수행한 연구 업적과 현장 경험 그리고 세계적인 주요 연구 산물들을 기초로 하여 궤양병 발생 회피, 경종적 방제, 궤양병균의 전파 차단, 조기 진단, 전염원 제거, 침입 차단, 약제 방제, 나무 치료 등 다양한 키위 궤양병 관리방법들을 요약하여 장차 키위나무를 건강하게 재배할 수 있도록 농가에서 실용적으로 사용할 수 있는 매뉴얼을 제시하고자 한다.

의료기관 종사자의 손 위생 수행 분석 (Analysis of Hand Hygiene Practices of Health Care Personnels)

  • 의료기관 종사자의 손 위생 수행은 의료관련 감염의 전파를 예방하는 가장 효과적인 방법이다. 본 연구는 직종별 손 위생 수행 측정과 분석을 위해 세계보건기구(WHO)의 손 위생 수행 평가도구를 이용한 직접관찰방법으로 2010년 10월 25일부터 12월 31일까지 수행되었으며, SPSS 21.0으로 기술분석, 교차분석, 다중로지스틱분석을 하였다. 총 8,644 건의 손 위생 상황이 관찰되었고, 손 위생 수행률은 94.1%였으며 의사가 가장 낮았다(83.3%; OR:0.209, 95%CI:0.174-0.252). 직종별 수행률(p<0.001), WHO에서 권고하는 5개 상황(WHO's 5Moments)별 수행률은(p<0.001) 유의한 차이를 보였으며, 간호사(p=0.003)와 의료기사직(p<0.001)이었다. 본 연구결과 손 위생 수행률 향상을 위한 직종별 특성화된 손 위생 전략과 WHO's 5Moments에 따라 차별화된 전략 개발이 필요할 것으로 사료된다.

Phenotypic and Genotypic Detection of Metallo-β-Lactamase Producing Pseudomonas aeruginosa

  • This study was undertaken to evaluate phenotypic and genotypic methods for detection of Metallo-Beta-Lactamases (MBLs) among nosocomial Pseudomonas aeruginosa. Of the 50 P. aeruginosa isolates from clinical specimens, 20 were evaluated for carbapenem resistance and screened for MBL by double-disk synergy test and combined-disk test. Nineteen strains (95%) were found to be MBL producers among the 20 P. aeruginosa. MBL positives were further confirmed by Polymerase Chain Reaction (PCR). For the IMP and VIM types of MBLs, PCR analysis was performed on 19 of the 20, and 10 were positive for VIM MBL type. This study reports the validation of a simple and accurate MBL detection method that can be easily incorporated into the daily routine of a clinical laboratory. Early detection of MBL-carrying organisms, including those with susceptibility to carbapenems, is of paramount clinical importance, as it allows rapid initiation of strict infection control practices as well as therapeutic guidance for confirmed infection.Key Words : Hepatitis A virus (HAV), Anti-HAV, Hospital workers, Prevalence, Vaccination

일부 치위생과 학생들의 신종인플루엔자 인식도 및 예방태도 (Dental hygiene students' knowledge, attitude in an influenza A (H1N1))

  • Objectives : In order to identify the awareness of influenza A (H1N1) having currently high frequency and risk as an infectious disease, to find problems and to reflect them on curriculum improvements from students before/after clinical practices. Methods : The data was collected from 279 dental hygiene students of 1st and 2nd years at G health college university from December 6th 2010 to December 10th 2010. The questionnaire were consisted of awareness of influenza A (H1N1), preventive attitude, sociodemographic characteristics. Results : 1. 1st year was 51.6%, the case having dental experience was 51.2%, in the infection control training experience 'had' was 46.6%. In the route acquiring the information, the mass media was 70.6%, in obtained information, personal hygiene was 82.1%. In the impact on human body, 'great impact' was 58.1%. In terms of the most need for response and preparedness, vaccination was the highest, 67.4%. People who experienced influenza A (H1N1) were 10.7%. 2. Awareness of influenza A (H1N1) was 0.71 points, and treatment and spreading mechanism was 0.78 points, prevention was 0.63 points, causes and definition was 0.53 points. 3. In the attitude for infection prevention of influenza A (H1N1), 'washing hands before practice' was the highest, 0.99 points and 'wear the mask only in case of contact with patient within 1~2 meters upon occurrence of no aerosol' was the lowest, 0.72 points. 4. Awareness of influenza A (H1N1) according to sociodemographic characteristics showed the significant differences upon the impacts on systemic health (p<0.05). Preventive attitude didn't show a significant difference in grade, clinical experience, experience in infection control training, acquiring rmation routes, the possibility for occurrence, impact on systemic health, the most need for prevention, experience in influenza A (H1N1) (p>0.05). 5. The significantly correlated between awareness of influenza A (H1N1) and preventive attitude(p<0.01). Conclusions : Information and preventive attitude for influenza A (H1N1) as well as systematic training programs to identify actual affecting factors and to improve the practice are needed. Also government's institutional support is needed.

Rapid Detection of Vancomycin-resistant Enterococci (VRE) in Clinical Samples from University Hospital

  • Outbreaks of vancomycin-resistant enterococci (VRE) are being reported more frequently in many countries. While seven glycopeptide resistance genotypes have been described in Enterococci, vanA and vanB are the most common resistance genotypes. The aim of this study was to detect antibiotic susceptibilities of 23 Enterococcus faecium strains, which caused an outbreak in a University hospital by a disk diffusion test to investigate the presence of the species specific gene, and the resistant genotypes, vanA and vanB by duplex PCR. PCR for vanA and vanB was performed on 23 enterococci. Twenty three were identified as E. faecium and were tested positive for the vanA genotype. This study will report on the validation of a simple and accurate VRE detection method that can be easily incorporated into the daily routine of a clinical laboratory. Early detection of VRE strains, including those with susceptibility to vancomycin, is of paramount clinical importance as it allows rapid initiation of strict infection control practices, as well as the therapeutic guidance for confirmed infections. The PCR method developed in the present study is simple and reliable for the rapid characterization of VRE.

응급구조학과 대학생들의 B형간염에 대한 지식 및 태도 (The knowledge and attitudes towards hepatitis B among paramedic students)

  • Purpose: The purpose of this study was to investigate the knowledge and attitudes towards hepatitis B among paramedic students. Methods: A self-reported questionnaire was completed by 395 paramedic students in Daejeon and Chungnam from May 10 to June 10, 2016. The questionnaire consisted of general characteristics of the subjects (9 items), knowledge of hepatitis B (20 items), and attitudes towards hepatitis B (7 items). Data were analyzed by t test, ANOVA, post hoc Duncan test, and Pearson's correlation analysis using SPSS v. 23.0. Results: The knowledge of hepatitis B was 10.52(${\pm}5.18$) out of 20 points and was significantly correlated with grade, vaccination, or antibody-positive irerspective of education. The positive attitude toward hepatitis B was 3.15(${\pm}.43$) and there was a significant correlation with family history irrespective of education. There was a statistically positive correlation between the knowledge and attitudes towards hepatitis B (r=.104, p=.039). Conclusion: Precise knowledge delivery and full understanding of hepatitis B should be incorporated into the education of paramedic students. Active infection control by emergency medical technicians should be maintained by hepatitis B prevention practices.
