Stress of the Pediatric Patient′s Parent in the Emergency Department (응급의료센터에 내원한 환아부모의 스트레스)

  • This study was performed to identify the level of stress recognized by the parents as a support system for their infant patients who were hospitalized in an emergency department, thus to provide the resulting data as the basic material of care intervention for the families of infant patients. This study subjected the parents with infant patients who were hospitalized in emergency department of C University Hospital in Daejeon, and the data was collected from questionnaires for them, dating from Mar. 20, to Jun. 24, 2001 This study used the tool(30 questions) which was originally developed by Ji, Dong-ok (1992) for measuring the stress of families of infant patients in emergency room, and then was modified and complemented by this researcher suitably for infant patients. Collected data was statistically analyzed with frequency, percentage, mean, standard deviation, t-test, ANOVA, by using SPSS WIN10.0 program, and the results were as follows: 1. The stress of parents with infant patients hospitalized in emergency department averaged 3.31 on the basis of 5-point measure, which means that they felt stress beyond means. 2. The factor with high average out of stress factors the parents of infant patients recognized included the followings: 'about the pain of infant patients due to examination(3.91±1.00)'; 'about rare opportunity to be in contact with physician(3.78±1.09)'; 'delay in emergent treatment required for infant patients(3.75±1.31)'; 'delay in the general treatment of infant patients(3.72±1.32)'. Factors of the stress level includes the followings: 'care and medical treatment'(3.46±.72); 'body and diseases'(3.41±.97)'; 'lack of information and supportable resources (3.25±.77)'; and so on. 3. For the stress level according to general characteristics, there were statistically significant differences in the result of the condition of infant patients, medical treatment(P<.01), religion, the procedure of treatment(P<.05). With little preceding studies for infant patients hospitalized in emergency department, this study looked through the level of stress recognized by the parents of infant patients. Based on the comprehension on the parents with infant patients and the knowledge on stress factors recognized by the parents, it is expected that we can seek the methods of care intervention such as explanation of care and treatment procedures, unit policies, continuous interest and emotional supports as well as the provision of information to understand the responses of parents and reduce accompanied stresses.

The Effect of Fathers' Kangaroo Care Experience of Preterm Babies on Paternal Attachment (미숙아 아버지의 캥거루 케어 경험이 부성 애착에 미치는 영향)

  • Purpose: This study was intended to standardize the Kangaroo care protocol for fathers, and to determine the effect of fathers' Kangaroo care experience on paternal attachment. Methods: The data was collected from February to April, 2013. The study subjects were 34 fathers (17 experimental group subjects, 17 control group subjects) of premature babies, bornatatertiaryhospitalinSeoul, who agreed to participate . The standardized Kangaroo care protocol, which consisted of at least three 60-minutes sessions during the hospitalization period in a neonatal intensive care unit, was carried out with the experimental group. The data was analyzed by a $x^2$-test and Mann-Whitney U test. Results: The results were as follows: 1) There were no between-group differences in the general characteristics of babies and their fathers. 2) The Kangaroo care fathers showed higher scores of paternal attachment than the control group (Z=-3.657, p=0.008). Conclusion: Fathers who attended the Kangaroo care sessions showed stronger paternal attachment than those who did not. Therefore, use of a Kangaroo care program for fathers of premature babies at neonatal intensive care units is recommended.

The Educational Needs of the Mothers of LBW infant and Normal Neonate (저출생체중아 어머니와 정상신생아 어머니의 간호교육요구도 조사)

  • This study was designed to compare the educational needs of the mothers of LBW infant and normal neonate for development of the educational program. The subjects for this study were 37 mothers of LBW and 60 mothers of normal neonate at 3 general hospitals in Seoul and Inchon. The data were collected during the period from July to November, 1996. The Educational Need for Infant Care was measuerd by questionnaire that has developed by researchers. The data were analized by descriptive statistics, non-paired t-test, ANOVA, ANCOVA using SPSS PC/sup +/. The results were as fellow : 1) The educational needs of the mothers of LBW infant were higher than the mothers of normal neonate(F=14.50, P=.000). 2) There were significant associations between the educational needs of the mothers of LBW infant and delivery history(nullipara. or multipara. ) and caregiver of postpartum(t:-2.08, P=.045 ; F=3.94, P=.0282). 3) There were significant associations between the educational needs of the mothers of normal neonate and numbers of children and caregiver of postpartum(F=5.53, P=.0064 ; F=3.22, P=.0480) .4) The educational need by content was signs, symptomes and management of disorders (i. g. cyanosis, seizure, fever, vomiting etc.) were higher than general care of baby(i. g. feeding, diaper change etc.) at two groups. In conclusion, when the nurses teaching the method of infant care to mother, there sholud be in consideration of delivery history and caregiver of postpartum. Also, its educational contents must be Included of signs, symptomes and management of disorders.

Influence of Empowerment and Job Satisfaction of Private Day Care Center Teachers on Infant's Right Respect Care (민간어린이집 교사의 임파워먼트와 직무만족도가 영유아 권리존중보육에 미치는 영향)

  • Objective: The purpose of this study is to provide practical data to effectively implement day care that respects the rights of infants in day care centers by investigating the relationship between private day care center teachers' empowerment, job satisfaction, infant's right respect care, and examining the influence of empowerment and job satisfaction of private day care center teachers on Infant's right respect care. Methods: The 228 day care teachers who work in private daycare centers located in Seoul and Gyeonggi province were the investigated subjects. For the analysis of questionnaire, frequency, percentage, reliability, t-test, ANOVA, correlation, and regression were used as SPSSWIN 20.0 program. Results: First, the difference of empowerment according to the individual variables of teachers was different to career, monthly salary, marital status, and turnover intentions. The difference of job satisfaction according to personal variables showed difference to age, career, marital status, type of work organization, and number of teachers and classes. There was a difference in infant's right respect care to age and marital status. Second, in the relationship between empowerment and job satisfaction and between empowerment and infant's right respect care, there was a positive correlation. The relationship between job satisfaction and infant's right respect care showed there is a positive correlation between infant's right respect care and all other factors except for the pay and promotion factors of job satisfaction. Third, Expert extension, autonomy, fellow relationship and job self have a positive influence on infant's right respect care. Conclusion/Implications: This study suggests that specific efforts should be made at the daycare center to raise the level of expertise and autonomy of private daycare teachers and to give satisfaction to their fellow relationship and their job self factors to effectively implement infant's right respect care.

A Study on the Development of an Education Curriculum for Maternal-Child Health Center Managers (산후조리간호 관리자 교육과정 개발 연구)

  • Purpose: The purpose of this study was to develop a specialized education curriculum aimed at helping nurses prepare for running and managing a Maternal-Child Health Center (Postpartum Care Center). Method: This study was conducted by an academy and industry joint research group consisting of professors of Nursing, and nurses actually running a Postpartum Care Center. The group compiled job descriptions of nursing through document research, interviews and observation during site visits, surveys, and seminars. They then performed a feasibility study and developed the final curriculum. Result: The education curriculum is a 32-week(2semester) program compromised of a theory part (12 credits, 180 hours) covering maternal and infant care and business start-up and field practice (3 credits, 45 hours). Courses in the theory part include an antenatal care, overview and details of maternal care, starting a business and its management. Of these courses, the overview of a maternal care course was developed with web-based contents. Field practice is designed to give students opportunities to visit Postpartum Care Centers, observe the care provided, and get hands-on experience. Conclusion: The specialized education curriculum is a 32-week course comprised of 12 credits on theory of antenatal care, overview and details of maternal care, infant care, starting and operating a business and 3 credits of field practice.

The Study on the Effects of Parent Role Education Program for infants' health promotion - Focusing on the twelve-months results - (영아의 건강증진을 위한 부모역할교육 프로그램의 효과에 관한 연구 -생후 12개월의 결과를 중심으로-)

  • This longitudinal quasi-experimental research was conducted to develop parent role education program and to evaluate the effect of this program for mother-infant interaction, childrearing environment and infant development. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37weeks, and their healthy mothers. The sample consisted of eighteen mother-infant dyads for intervention group and sixteen dyads for control group. Data were collected from March 15th in 1999 to Jun 20th in 2000. The intervention group received programmed education consisted of discharge education, telephone counselling, and home visiting care. But control group were collected data without programmed education. In this study, the Nursing Child Assessment Teaching Scale(NCATS) devised by Barnard was used to determine the mother-infant interaction, HOME was used to determine the childrearing environment, and Griffiths mental development scale was used to determine the infant development. The data were analyzed using SPSS Win using chi-square test, t-test, and repeated measure ANOVA. This study was focused on the results of twelve months time point. Summaries of the results were as follows: 1. There was no significant difference in mother-infant interaction(NCATS) between intervention group and control group. But both of two groups showed significantly higher in interaction score at twelve months than at six months in the subscales of social-emotional growth fostering, and responsiveness to caregiver. 2. There was no significant difference in childrearing environment(HOME) between two groups at twelve months. But when each subscale of HOME was examined, intervention group showed higher scores in the dimensions of maternal involvement with child(p=.001), and maternal emotional-verbal responsivity(p=.048). 3. There was no significant difference in GQ of the Griffiths mental development scale between two groups, although significant difference was found in performance subscale. 4. Infant development at twelve months showed significant correlation with mother-infant interaction and childrearing environment at six months, although mother-infant interaction and childrearing environment at twelve months did not show significant correlations with infant development at twelve months. 5. Developmental scores at six months showed significant correlations with variety in daily stimulation, and mother's emotional, verbal responsivity, whereas developmental scores at twelve months showed significant correlations with acceptance of child behavior at six months, and appropriate play material at twelve months. In conclusion, the maternal education program for primipara showed long term effect in some categories in organizing the childrearing environment, and fostering the infant development. We suggest further study and implications of parent role education program for high risk parents such as parents in low economic status or with premature babies.

A Survey on the Experience of Accident of Infant, Toddler and Preschooler, and the Education about Emergency Care for Parents (영 유아 및 학령전 아동의 안전사고 및 어머니의 응급처치 정보 접촉 경험에 관한 조사 연구)

  • The purpose of this study was to investigate the experiences of accident and visiting emergency center, and the experiences of education about emergency care for parents. The subjects of this study were 117 mothers whose children are attending 9 different nursery schools or kindergartens. The instrument used in this study was developed by researchers and validated by nursing professor, first aid doctor and the president of nursery school, and consisted of 'most important thing in child rearing' ,'experiences of accident and visiting emergency center', 'type of trauma' and 'need for parent education program' the result of this study were as followed ; 1.There were 70.09% of the experience of visiting emergency center among the subjects. The cause of visiting emergency center were high fever and convulsion(42.86%), and trauma by accident(28.04%) 2. The most common type of the indoor accident were fall down and slipping(76.92%), the common type of trauma were contusion (76.92%), abrasion(47.01%), laceration(29.06%), dislocation or fracture(12.82%), burn(10.26%), piercing(8.55%). 3.47.01% of subjects had the experience of education about emergency care for parents. They were educated by child rearing journals (32.72%), small books which were published by hospitals or community health center (25.45%), mass media(21.8%), parent seminar (12.72%), nursery school or kindergarten (5.45%). In conclusion, the need of emergency care for children was strongly found. Therefore, on the basis of above results, the parent education program which fits their cognitive level and their health care needs So, it must be directed on the further study of parents' knowledge about emergency care for children. To enhance the effectiveness of program and accomplish the children's health promotion, advanced instructional media and demonstration must be included.

  • The purpose of this study was to investigate how environment-friendly activities through nature affects attitude of children. 5-6 years old children were targeted for study from March to December, 2014. Rapid changes of modern society made increase of female workers and their participation rates in economic activity which results children to play more time with artificial toys and media. There are 3 stages to investigate effects. $1^{st}$ stage is to know about woods by visiting woods and experience environment-friendly activity. $2^{nd}$ stage is to experience woods with 5 senses not only in real woods but also in classroom. Lastly $3^{rd}$ stage is to make art work with natural object and make woods in classroom. Changes of hildren's attitude and view toward to the nature were recorded and analyzed by anecdotes perpetual inventory and environment-friendly attitude examination. By analysis of infant gives you the opportunity to encounter nature, often in conjunction with ongoing enjoys nature-friendly program in the classroom to play with toys, rather than a complete natural objects gradually formalized when presenting a natural, concentrated than the previous game this time is enhanced and creativity through nature through the promotion doeeojim and attitude to nature is also eco-friendly activities byeonhwadoem program showed that the impact on the attitude of the infant.

Parent's Needs for Center-based Child Care Health Program (보육시설 영유아의 건강관리를 위한 특수보육 요구조사 I - 부모의 요구를 중심으로 -)

  • Purpose: To investigate the actual conditions of health care management in child day care centers and parents needs for a Center-based Child Care Health Program. Methods: Study design was cross-sectional descriptive study. 937 parents from 33 day care centers in Seoul completed a structured questionnaire. Results: 73.0% of the parents experienced sick events with their child at least once or more every 1-3months. These events resulted in 35.2% of the parents having to leave work early or to be absent from their jobs. 30.4% were asked to drive a sick child from the child day care centers to their home. Most parents (76.7%) agreed on the need for a Center-based Child Care Health Program (CCCHP), and expected general health & illness management, and growth & development evaluation from the CCCHP. Conclusion: These results show that there is a great demand for Center-based Child Care Health Program. They suggest that there is an urgent need to develop CCCHPs to promote children's health and to support parents. Also the results of this study can be utilized as a basis for the establishment of a childcare policy in Korea.

Knowledge and Attitudes towards Infant Oral Health, and Related Health Behavior among Parents (영유아 구강건강에 대한 부모의 지식, 태도 및 실천)

  • Purpose: The purpose of this study was to examine levels of knowledge, attitude towards infant oral health, and related health behavior of parents. Methods: The sample consisted of 380 parents who were providing care for a child ranging in age from 11 to 16 months. Results: Participants obtained information on infant oral health via the internet, infant care books, and friends. While showing the correct attitude towards infant oral health, the parents were ill-informed regarding infant oral health and engaged in low levels of appropriate health behavior. There were statistically significant differences in infant oral health knowledge according to parents age, education, income, and age of infant. Attitude towards infant oral health was significantly different according to parents education and income. Also, there were significant differences in health behavior practice for infant oral health according to parents age, income, caregiver, and infant age. A significant positive relationship was found between knowledge and attitude to infant oral health. Conclusion: The results of this study indicate that for maintenance and improvement of infant oral health, the parents and caregivers of infants should be provided with information and specific methods for infant oral health care.
