Pregnant Women's Labor Progress, Childbirth Outcome, and Childbirth Satisfaction according to the Presence or Absence of Labor Induction (유도분만 시행 여부에 따른 임산부의 분만진행과정, 분만결과, 분만만족도)

  • Purpose: To provide accurate information on induced labor and find strategies to enhance women's childbirth satisfaction. Methods: Participants were pregnant women expected to have normal vaginal delivery. A total of 113 women with induced labor and 61 women with spontaneous labor were surveyed. Data were collected using a questionnaire and electronic medical records. Results: The following variables related to labor progress showed significant differences between the induced labor group and the spontaneous labor group: length of the first stage of labor in primigravidas, use of analgesic, incidence of uterine hyperstimulation, incidence of fetal distress, and medical treatment for the expectant mother. Delivery type and the incidence of postpartum complications showed significant difference between the two groups. Induced labor women's childbirth satisfaction was mainly affected by the process of labor whereas spontaneous labor women's childbirth satisfaction was affected by the outcome of childbirth. Conclusion: Medical staff should have accurate information on the risk of induced labor and the benefits of a natural delivery. Moreover, medical staff should provide necessary information and environment for women to participate in the decision-making process.

Constitution of Prescription and Medicinal Effect & Adaptation Diseases of 'Bullsoosan(佛手散)' in Korean Medical Books (한국(韓國) 의서(醫書)에 보이는 불수산(佛手散)의 처방구성(處方構成)과 효능(效能)·주치(主治)에 대한 고찰)

  • Subjects : A literature research on the constitution and medicinal effect & adaptation diseases of "Bullsusan". "Bullsusan" is a herbal prescription composed of Angelicae Gigantis Radix(當歸) and Cnidii Rhizoma(川芎). Objectives : Through the researching on the records of "Bullsusan" in Korean Traditional Medical Books, gain the literature evidence for adaptation to these days child labor as a pre-labor keeping herbal medicine. And have detailed consideration on the constitution of prescription and medicinal effect & adaptation diseases of "Bullsusan". Methods : First, researched the records of "Bullsusan" in Korean Traditional Medical Books which were included at A Series of Korean Medicine(韓國醫學大系) and analysed component ratio, nickname, herbal manufacture and drug processing method, medicinal effect and adaptation diseases. Second, referred related Korean and Chinese researches that examined the medicinal effect and adaptation diseases of "Bullsusan" by scientific experimentation. Conclusions : We found total 46 records of "Bullsusan" from 20 kinds of Korean Traditional Medical Books included at A Series of Korean Medicine. Prescription component ratio of Angelicae Gigantis Radix and Cnidii Rhizoma were 3:2, 1:1, 2:1, 1:1. 3:2 had most 20 records and 1:1 had second 14 records. Especially 1:1 had a tendency of having nickname "Goonguitang", but not must had. First herbal manufacture was powder, it had 8 records. First drug processing method was decocting with water and alcohol, had 19 records. Medical Effects of "Bullsusan" can be induced to next 8, that were "remove get bad blood, give birth new blood", "easy labor by reducing fetal volume", "acceleration of labor", "test of fetal survival, elimination of dead embryo", "elimination of placenta", "revive", "allaying pain", "nourish the blood". From these medical effects, 9 adaptation diseases can be induced. That were "threatened abortion", "womb ache and vaginal bleeding by spontaneous abortion", "pre-labor keeping(prevention of hard labor)", "acceleration of labor", "hard labor", "missed abortion", "postnatal vaginal bleeding, dizziness, asthma, headache, womb ache", "postnatal mastoptosis and mastodynia", "first aid symptom like as dizziness, unconsciousness, stroke caused by excessive bleeding". The medical effect of "acceleration of labor" and "elimination of placenta" have been examined by modern clinical research. The effect of "remove get bad blood, give birth new blood", "allaying pain" and "nourish the blood" have been examined by modern experimental study. But overdosing on "Bullsusan" to pregnant mouse can cause natural abortion, so the proper dose of "Bullsusan" in pregnant period is very important.

Continuous Lumbar Epidural Analgesia for Labor and Vaginal Delivery in Epileptic Pregnant Women (간질이 있는 산모에서 간질의 예방 및 분만통 감소를 위한 경막외 진통의 경험 1예 -증례 보고-)

  • A 25-year-old epileptic female patient scheduled for vaginal delivery, was referred to the pain clinic for the relief of labor pain. She had been taking anticonvulsant drugs, but suffered from seizure attacks three or four times a month. We had induced continuous lumbar epidural analgesia successfully and she subsequently gave birth to a healthy infant. So we report that continuous lumbar epidural analgesia should be considered as a safe method for reduction of labor pain and prevention of seizure attacks in epileptic pregnant women.

Impact of Population Growth on Labor Force and Employment in Korea; Transition and Prospect (장기인구성장과 노동력 수급 전망)

  • Three stages of population growth during last forty years affected differently to the labor force and employment in Korea. The first impact of rapid population growth on the labor force occured after the end of World War II. Sudden growth of population due to repartriation and refugees directly increased the labor force. Deteriorating labor market conditions were caused not only by the explosive labor supply but also by the shortage of employment opportunities due to a lack of productive facilities. This severe excess supply of labor continued until the early 196Os. Population growth in the second stage which caused by high fertility during the post Korean War baby boom period induced an eventual increase in the labor supply with time lag of more than fifteen years. Younger persons born during baby boom period were flooded the labor market. Fortunately, job opportunities were expanded more rapidly than the labor force supply because high rates of economic growth and speedy industrialization were continued until the later half of 1970s. Unemployment, therefore, decreased dramatically during this period. The effect of third stage which is characterized as mitigated population growth due to birth control has appeared in the labor market since late 1970s. The growth rate of labor force has been going down and the proportion of younger workers was also been decreasing. From the early 1980s, furthermore, partial disharmony between supply of and demand for the younger workers is closing up. Less educated younger workers who works at low wage are lacking while more educated youngers who want to work at high wage are being excess, because a lot of younger prefer higher education rather than productive job. It is expected that the structural inharmony will be diversified in the future in Korea. The labor force will be changed to middleaged, highly educated and womenized till year 2000, and, after then, to old-aged. On the demand side, industries and jobs will transferred to be labor-saving and soft. These structural changes of labor supply and demand will not matching in time. Aggregate supply of labor force will be steadily increasing more rapidly than aggregate demand for labor until year 2000, and this trend will continue to the first one or two decades of the 2lth century because the persons born dufing the baby boom pariod are being eligible couples in recent. Therefore, conclusion is that appropriate manpower development policy as well as sustained birth control policy is necessary for harmonizing the structural unbalance and the disequilibrium between aggregate labor supply and demand in the future.

The Role of Emotional Labor Strategies Based on Job Demand- resource Theory (직무요구- 자원 이론에서의 감정노동전략의 역할)

  • This study investigated the effects of job demand-resource variables that appear in a special organizational environment such as a hospital on job burnout and job engagement using the job demands-resources model of Schaufeli & Bakker(2004). And also the study investigated to verify discriminatory mediating roles of emotional labor strategies in the process. The main finding of this study was as follows: Job demands-resources showed a positive relationship in the job burnout and job engagement, and job demands induced surface acting and job resources induced deep acting. However, deep acting showed a positive relationship to job engagement, and surface acting showed a negative relationship with job burnout unlike in previous study results, which was not significant. The results of verifying the meditation model through Boostrap showed that since the two indirect effects were not significant, the meditating role of emotional labor strategies was not verified in the effects of job demand-resource levels on job attitudes, including job burn out and job engagement.

A Study on the Induced Abortion Behavior of Currently Married Women in Korea (우리나라 유배우부인의 인공임신중절에 관한 연구)

  • An attempt had been made to obtain current information on induced abortion among currently married women aged 15 to 44. The source of data was 1985 National Fertility and Family Planning Survey conducted by Korea Institute for Population and Health in may 1985. 1. At the time of the survey, 53 percent of currently married women aged 15 to 44 had experienced induced abortion: 25.5 percent of the respondents who had experienced induced abortion only one time and another 27.6 percent more than two times. 2. The proportion of women who had experienced induced abortion seemed to increase according to the advance in age. It seemed that there was no significant difference in the experience rate of induced abortion by academic career, and there was inverse relationship between the experience rate of induced abortion and the age at first marriage. The experience rate if induced abortion by the number of living children was highest among those who had three children as 65.6 percent. 3. To analyze the effect of induced abortion on fertility, this study employed multiple regression analysis as a statistical technique. Instead of index representing fertility level the number of living children served as dependent variable. Independent variables used in analysis included age, age at first marriage, education level, ideal number of children, religion, frequency of induced abortion, total number of pregnancies and participation in labor force. Standardized partial regression coefficient of induced abortion was no less than -0.61. So, it can be concluded that induced abortion offered the great contribution on the birth control. A consistant health education and efficient management of family planning program would be essential for effectiveness of contraceptive practice.

Relationship Between Farm Land Structure and Machine Operation in Korea

  • The shortage of agricultural labour due to industrial growth has greatly induced the mechanization in Korean agriculture. However small and scattered land holdings have been the main constraints in the process of mechanization. This paper describes the interrelationships of farm land structure, machinery selection and machinery operation areas. The sandy silt loam irrigated paddy land having single crop a year was selected as a target areas for this study. Machine operation cost is greatly influenced by operation period, plot geometry and operation area. On the improved geometry plots, optimal machine size increases slowly with increase in operation area. Operable area increases due to increased effective machine capacity on better geometry plot. The difference between the effects of operation period and plot geometry is that in the former case, the cost reduction is caused by delay in increase of machine size, whereas in the latter case timeliness cost is reduced by increase ffective capacity. The effect of farmland consolidation is greater on small plots than that on big plots. Increasing wage rates have induced the adoption of more labor saving machinery. Bigger labor saving machines require enlargement of operation area and larger plots through improvement in farm land structure. Machine cost on poor plot geometry increases more rapidly than that on the good plot geometry and as operation area increases machine cost reduces significantly. It is concluded that the development of agricultural mechanization ion Korea will depend on the improvement in farm land structure and enlargement of operation area.

Anti-inflammatory effect of remifentanil in lipopolysaccharide-stimulated amniotic epithelial cells

  • Background: Sometimes general anesthesia is required for dental surgery in pregnant women. Facial bone fractures or neck abscess should be treated immediately. Dental surgery, however, creates a stressful situation that can cause inflammation. Inflammatory responses are a well-known major cause of preterm labor and preterm birth. Here we demonstrate the effects of remifentanil on the factors related to preterm labor and its mechanism of action on amniotic-derived epithelial cells (WISH cells). Methods: WISH cells were exposed to lipopolysaccharide (LPS) for 24 h and co-treated with various concentrations of remifentanil. MTT assays were performed to measure cell viability. To explain the effects of remifentanil on the factors related to inflammation in WISH cells, activation of nuclear factor kappa B ($NF-{\kappa}B$) and p38 and the expression of interleukin $(IL)-1{\beta}$, tumor necrosis factor $(TNF)-{\alpha}$, cyclooxygenase (COX)2, and prostaglandin E $(PGE)_2$ were quantified using western blotting and RT-PCR, respectively. Results: Remifentanil did not affect WISH cell viability. In western blot analysis, co-treatment with remifentanil resulted in decreased phosphorylation of $NF-{\kappa}B$, and expression of COX2 and $PGE_2$ in LPS-induced inflammation, but the results were statistically significant only at low concentrations. Reduction of $IL-1{\beta}$ and $TNF-{\alpha}$ expression was also observed with RT-PCR. Conclusion: Co-treatment with remifentanil does not affect the viability of WISH cells, but reduces the expression of the factors related to inflammation, which can induce uterine contraction and preterm labor. These findings provide evidence that remifentanil may inhibit uterine contraction and preterm labor in clinical settings.

Propofol protects against lipopolysaccharide-induced inflammatory response in human amnion-derived WISH cells

  • Background: Nonobstetric surgery is sometimes required during pregnancy, and neck abscess or facial bone fracture surgery cannot be postponed in pregnant women. However, dental surgery can be stressful and can cause inflammation, and the inflammatory response is a well-known major cause of preterm labor. Propofol is an intravenous anesthetic commonly used for general anesthesia and sedation. Studies investigating the effect of propofol on human amnion are rare. The current study investigated the effects of propofol on lipopolysaccharide (LPS)-induced inflammatory responses in human amnion-derived WISH cells. Methods: WISH cells were exposed to LPS for 24 h and co-treated with various concentrations of propofol (0.01-1 ㎍/ml). Cell viability was measured using the MTT assay. Nitric oxide (NO) production was analyzed using a microassay based on the Griess reaction. The protein expression of cyclooxygenase-2 (COX-2), prostaglandin E2 (PGE 2), p38, and phospho-p38 was analyzed using western blotting. Results: Propofol did not affect the viability and NO production of WISH cells. Co-treatment with LPS and propofol reduced COX-2 and PGE2 protein expression and inhibited p38 phosphorylation in WISH cells. Conclusion: Propofol does not affect the viability of WISH cells and inhibits LPS-induced expression of inflammatory factors. The inhibitory effect of propofol on inflammatory factor expression is likely mediated by the inhibition of p38 activation.

Relaxant and anti-inflammatory effect of two thalidomide analogs as PDE-4 inhibitors in pregnant rat uterus

  • The aim of this study was to evaluate the relaxant and anti-inflammatory effects of two thalidomide analogs as phosphodiesterase-4 (PDE-4) inhibitors in pregnant rat uterus. Uteri from Wistar female rats were isolated at 19 day of pregnancy. Uterine samples were used in functional studies to evaluate the inhibitory effects of the thalidomide analogs, methyl 3-(4-nitrophthalimido)-3-(3,4- dimethoxyphenyl)-propanoate (4NO2PDPMe) and methyl 3-(4-aminophthalimido)- 3-(3,4-dimethoxyphenyl)-propanoate (4APDPMe), on prostaglandin-$F2{\alpha}$ ($PGF2{\alpha}$)-induced phasic, $K^+$-induced tonic, and $Ca^{2+}$-induced contractions. Accumulation of cAMP was quantified in uterine homogenates by ELISA. Anti-inflammatory effect was assessed by using ELISA for determination of the pro-inflammatory cytokines tumor necrosis factor-${\alpha}$ ($TNF{\alpha}$) and interleukin (IL)-$1{\beta}$, and anti-inflammatory IL-10, from uterine explants stimulated with lipopolysaccharide (LPS). Nifedipine, forskolin and rolipram were used as positive controls where required. Both thalidomide analogs induced a significant inhibition of the uterine contractions induced by the pharmaco- and electro-mechanic stimuli. Nifedipine and forskolin were more potent than the analogs to inhibit the uterine contractility, but these were more potent than rolipram, and 4APDPMe was equieffective to nifedipine. Thalidomide analogs increased uterine cAMP-levels in a concentration-dependent manner. The LPS-induced $TNF{\alpha}$ and $IL-1{\beta}$ uterine secretion was diminished in a concentration-dependent fashion by both analogs, whereas IL-10 secretion was increased significantly. The thalidomide analogs induced utero-relaxant and anti-inflammatory effects, which were associated with the increased cAMP levels as PDE-4 inhibitors in the pregnant rat uterus. Such properties place these thalidomide analogs as potentially safe and effective tocolytic agents in a field that urgently needs improved pharmacological treatments, as in cases of preterm labor.
