Convergence Study in Development of Severity Adjustment Method for Death with Acute Myocardial Infarction Patients using Machine Learning (머신러닝을 이용한 급성심근경색증 환자의 퇴원 시 사망 중증도 보정 방법 개발에 대한 융복합 연구)

  • This study was conducted to develop a customized severity-adjustment method and to evaluate their validity for acute myocardial infarction(AMI) patients to complement the limitations of the existing severity-adjustment method for comorbidities. For this purpose, the subjects of KCD-7 code I20.0 ~ I20.9, which is the main diagnosis of acute myocardial infarction were extracted using the Korean National Hospital Discharge In-depth Injury survey data from 2006 to 2015. Three tools were used for severity-adjustment method of comorbidities : CCI (charlson comorbidity index), ECI (Elixhauser comorbidity index) and the newly proposed CCS (Clinical Classification Software). The results showed that CCS was the best tool for the severity correction, and that support vector machine model was the most predictable. Therefore, we propose the use of the customized method of severity correction and machine learning techniques from this study for the future research on severity adjustment such as assessment of results of medical service.

Development of Mortality Model of Severity-Adjustment Method of AMI Patients (급성심근경색증 환자 중증도 보정 사망 모형 개발)

  • The study was done to provide basic data of medical quality evaluation after developing the comorbidity disease mortality measurement modeled on the severity-adjustment method of AMI. This study analyzed 699,701 cases of Hospital Discharge Injury Data of 2005 and 2008, provided by the Korea Centers for Disease Control and Prevention. We used logistic regression to compare the risk-adjustment model of the Charlson Comorbidity Index with the predictability and compatibility of our severity score model that is newly developed for calibration. The models severity method included age, sex, hospitalization path, PCI presence, CABG, and 12 variables of the comorbidity disease. Predictability of the newly developed severity models, which has statistical C level of 0.796(95%CI=0.771-0.821) is higher than Charlson Comorbidity Index. This proves that there are differences of mortality, prevalence rate by method of mortality model calibration. In the future, this study outcome should be utilized more to achieve an improvement of medical quality evaluation, and also models will be developed that are considered for clinical significance and statistical compatibility.

A Correlational Study of Obesity Stress, Self-esteem and Adaptation to School Life regarding to the Obesity Index of Upper Elementary School Students (초등학교 고학년 아동의 비만도에 따른 비만 스트레스, 자아존중감 및 학교생활적응과의 관계)

  • Purpose: The purpose of this study is to analyze the correlation between obesity stress, self-esteem, and adaptation to school life according to body mass index of elementary school students in higher grades and to provide basic data for the development of health education programs. Methods: A convenience sampling method was used to select four elementary schools. Self-reporting questionnaires were distributed to the subjects to collect data, which were then analyzed using the SPSS 18.0 program. Results: The higher the obesity index, the more obesity stress the students experienced. Second, the lower the level of obesity, the higher both self-esteem and the level of adjustment to school life were. There was a meaningful correlation between obesity stress, self-esteem, and adjustment to school life. Conclusion: The study results are expected to be useful in serving as a basic material for the development of programs that can improve students' adjustment to school life and self-esteem while reducing their stress caused by obesity.

  • Recently, with the advent of applications using locations of moving objects, it becomes crucial to develop efficient index schemes for spatio-temporal databases. The $TPR^*$-tree is most popularly accepted as an index structure for processing future-time queries. In the $TPR^*$-tree, the future locations of moving objects are predicted based on the CBR(Conservative Bounding Rectangle). Since the areas predicted from CBRs tend to grow rapidly over time, CBRs thus enlarged lead to serious performance degradation in query processing. Against the problem, we propose a new method to adjust CBRs to be tight, thereby improving the performance of query processing. Our method examines whether the adjustment of a CBR is necessary when accessing a leaf node for processing a user query. Thus, it does not incur extra disk I/Os in this examination. Also, in order to make a correct decision, we devise a cost model that considers both the I/O overhead for the CBR adjustment and the performance gain in the future-time owing to the CBR adjustment. With the cost model, we can prevent unusual expansions of BRs even when updates on nodes are infrequent and also avoid unnecessary execution of the CBR adjustment. For performance evaluation, we conducted a variety of experiments. The results show that our method improves the performance of the original $TPR^*$-tree significantly.

  • The construction of a facility is an act whereby infrastructure such as roads and railways and other facilities such as housing and office spaces are constructed, and due to the characteristics of the relevant industry where such construction is carried out in a custom order method, it requires a long period of time mostly. While the construction is carried out over a long period of time, fluctuations in the price of raw materials required for the construction occur. Therefore, the cost of the construction also fluctuates inevitably. The fluctuation of the construction cost meshes closely with the profit of a constructor and the efficiency of government spending. Therefore, it is necessary to maintain the construction cost at a reasonable level by adjusting the construction cost rationally in order to secure the required construction quality and spend the budget efficiently. The buffer system for solving such problem is the contract amount adjustment system by fluctuations in prices (Escalation/De-Escalation). In this study, the method combining the escalation method based on the item adjustment method was suggested for an efficient contract amount adjustment method. In addition, it is proposed that there be an investigation and codification of matters not applied to government acts and regulations on contracts related to the adjustment of fluctuations in prices legally.

  • Although the Korean construction industry has risen rapidly, the quality of the buildings is not met with the industry standard compared to the potential of the technology development and the cost of maintenance has been increased. The user's request for quality have been increased, but in the current building construction, the contractors only follow the specifications of design, material and technology, and their main concern is whether they are in right track with the specification or not. Thus, the necessity of developing technology has been ignored and sufficient opportunities for the development have not been provided as well. In the developed countries, in contrast, the quality assurance and the payment regulation system has been utilized for the active response to the changes of users' request. As a result of this, the maintenance cost has been decreased and the development of construction technology has been improved. Therefore the pay adjustment system is needed to create the improvement of the quality of the buildings by the contractors' earnest building construction. In order to introduce the pay adjustment system suitable to the Korean construction industry, this study presented the logical method of adjusting the construction cost and making payment with the pay adjustment index that is created based on the result of the performance test, after making a set of the indicators of the building performance standard and applied them to the targeted building.

  • The author devised a simple technique for duplicating occlusal contour of natural teeth on the prepared abutments using hard stone index obtained from the study cast of natural teeth. Throughout this author devised method, the following advantages are studied. Firstly the dentist can obtain an ideal occlusal wax patterns which have arrived at the physiologic state through a long period of adjustment to various habits and functional movements. Secondly the construction procedure is simple, and thirdly it is easy to adjust wax pattern of nomal natural teeth which duplication is required.

Severity Measurement Methods and Comparing Hospital Death Rates for Coronary Artery Bypass Graft Surgery (관상동맥우회술의 중증도 측정과 병원 사망률 비교에 관한 연구)

  • Objective : Health insurers and policy makers are increasingly examining the hospital mortality rate as an indicator of hospital quality and performance. To be meaningful, a risk-adjustment of the death rates must be implemented. This study reviewed 5 severity measurement methods and applied them to the same data set to determine whether judgments regarding the severity-adjusted hospital mortality rates were sensitive to the specific severity measure. Methods : The medical records of 584 patients who underwent coronary artery bypass graft surgery in 6 general hospitals during 1996 and 1997 were reviewed by trained nurses. The MedisGroups, Disease Staging, Computerized Severity Index, APACHE III and KDRG were used to quantify severity of the patients. The predictive probability of death was calculated for each patient in the sample from a multivariate logistic regression model including the severity score, age and sex to evaluate the hospitals' performance, the ratio of the observed number of deaths to the expected number for each hospital was calculated. Results : The overall in-hospital mortality rate was 7.0%, ranging from 2.7% to 15.7% depending on the particular hospital. After the severity adjustment, the mortality rates for each hospital showed little difference according to the severity measure. The 5 severity measurement methods varied in their statistical performance. All had a higher c statistic and $R^2$ than the model containing only age and sex. There was a little difference in the relative hospital performance evaluation by the severity measure. Conclusion : These results suggest that judgments regarding a hospital's performance based on severity adjusted mortality can be sensitive to the severity measurement method. Although the 5 severity measures regarding hospital performance concurred, more often than would be expected by chance, the assessment of an individual hospital mortality rates varied by the different severity measurement method used.

  • Data quality and climate forecasting performance deteriorates because of long climate data contaminated by non-climatic factors such as the station relocation or new instrument replacement. For a trusted climate forecast, it is necessary to implement data quality control and test inhomogeneous data. Before the inhomogeneity test, a reference series was created by $d$ index to measure the temperature series relationship between the candidate and surrounding stations. In this study, a inhomogeneity test to each season and climatological station was performed on the daily mean temperatures, daily minimum temperatures and daily maximum temperatures. After comparing two inhomogeneity tests, the traditional and the adjusted SNHT method, we found the adjusted SNHT method was slightly superior to the traditional one.

  • The business market of architecture has got a system that controls a deposit according to the price function. This system is written on a law of contract about countries. So the main body of construction has to make a reasonable contract. This study is written about a rate of numerical index on controling a deposit. We tried to fine problems and solutions of labor expenses, instrument costs and material costs which is so big and changable on the construction market Labor expenses are expressed according to the rate of construction scale between direct and indirect cost that applies ability of works. Instrument costs are expressed according to an output method of a unit price annually and a weight allowance of local instrument conditions and use frequence. The last material costs expressed according to a local weight allowance make a decision of the material cost index. They applies locally relative index more than absolute one on what uses the price rate of producers and importations. This solutions are not enough to apply to the real market, so it needs to exam and to be on the market after a feasibility study.
