The Status of Dietary Supplements Intake in Korean Preschool Children: Data from the Korea National Health and Nutrition Examination Survey 2010-2012

  • Purpose: The use of dietary supplements (DS) has increased in most nations. We investigated the amount of DS intake in the Korean population by analyzing a national survey, to support the preparation of a national institutional strategy regarding DS intake and marketing. Methods: The data of the fifth Korea National Health and Nutrition Examination Survey (a year between 2010 and 2012) were investigated, analyzing the rate of DS intake, and the characteristics of the intake group and non-intake group in Korean preschool children. Results: The intake rate of DS was 49.0-54.2% (1,313,874-1,491,240) and 19.6-30.3% (250,603-421,922) in children from 1 to 6 years old and in those less than 1 year, respectively, from 2010 to 2012. The highest intake rate was observed in the age group of five. The mean age was significantly higher in the DS intake group than in the non-intake group. Intake of essential nutrients, minerals, and vitamins were also higher in the DS intake group. The level of family income was significantly associated with the intake rate (p<0.001). In children less than 1 year, probiotics accounted for the highest intake of DS. Conclusion: Korean preschool children have high consumption of DS. Therefore, problems may arise from the waste of money purchasing unnecessary DS, and from the overuse of DS in preschoolers who do not require DS intake. We hope these results can be used to produce an appropriate national institutional strategy regarding DS intake and marketing.

  • Background: This study was conducted to evaluate the factors affecting adherence in patients with hypertension and type 2 diabetes mellitus before and after a clinic based patient incentive program in Incheon. Methods: An observational follow-up study was done for 28,355 patients in one registered group and 245,598 patients in a non-registered group from March 16th 2009 to December 31th 2010 in Incheon. The registration, mandatory laboratory tests and number of clinic visits were collected by merging the Incheon Chronic Disease Management System data and the National Health Insurance Corporation (NHIC) data. As a measure of patient adherence, we used a variable of prescription days from the NHIC and defined above 80% of average prescription days as an appropriate patient adherence. Repeated measures analysis of variance and logistic regression were used to analyze the differences in patient adherence and factors affecting adherence. Results: The changes in prescription days for the registered group are larger than for the non-registered group. In the logistic regression model, including the variables with sex, age, income status and number of clinic visits, the registered group exhibited a higher Odds ratio in the patient adherence. Conclusion: This study revealed the association between registration and appropriate patient adherence in patients with hypertension or type 2 diabetes mellitus.

상용치료원 보유가 의료기관 종별 선택에 미치는 영향: 대형병원 환자집중현상 완화방안을 중심으로 (The Effect of Having Usual Source of Care on the Choice among Different Types of Medical Facilities)

  • Background: Concentration of patients to large hospitals is serious problem in Korea. The purpose of this paper is to propose appropriate policy direction to relieve concentration of patients to large hospitals. It is focused on evaluation of the possibility of family doctor system as a policy alternative to relieve concentration of patients to large hospital by empirically analyzing the effect of usual source of care (USC) on large hospitals medical care use. Methods: Korea Health Panel conducted 2009, 2012, 2013 by KIHASA (Korea Institute for Health and Social Affairs) and NHIS (National Health Insurance Service) was used for analysis. For dependent variables, first, the ratio of the amount of using large hospital to total amount of using medical care, and second, the amount of using large hospital are estimated. Independent variables are having an USC and type of USC. Panel analysis was done with above variables. Results: Main results are as follows. First, having an USC increases using large hospital. Second, having a domestic clinic type USC decreases using large hospital and ratio of using large hospital. Third, the effect of domestic clinic type USC is greater in older group, less income group, worse health status group, not having private insurance group, and having chronic disease group. Conclusion: These results show that family doctor program can be a policy alternative to relieve concentration of patients to large hospital. Nonetheless, primary care system in Korea is unsatisfied. It is recommended to reinforce primary care system and family doctor system to relieve concentration of patients to large hospitals.

  • It is the developing policy of fishing village of korea that the fishermen are not to leave their fishing village by the construction of abandant and rich fishing village in the social, cultural and economic side of view. The government has promoted the project to improve the fishing and agrarian village from 1992 year, but it is not so helpful to solve the problem of the island and fishing village. Therefore, it has to improve the structure of fishing village and to develope the vital and lively fishing village, Japan had enacted the special law for the promotion of out-island which was called the law for the promotion of Ri-do isalnd, and commenced to develop the Ri-do island. The law for the promotion of Ri-do island is limited to 10 years, but it was amended 10 times and Japanese government has invested as many as budget to Ri-do isalnd during 30 years. Korean government also had enacted and enforced the special law for the promotion of out-isalnd developement referring to Japanese system. However, it is neccessary at first to improve the system that the fishing village is able to develope the fishing port in order to make higher the efficiency of investment. Moverover, the earning differentials bewteen the city and the fishing village has to decrease by the developement of transport convenience and pavement of the road of the out-isalnd and remote fishing village. Otherwise, it is requested rapidily to set up the welfare policy for the aged people because the inhabitants of fishing village become to great age, and inhabitants could be at least diagnose one time every week in the good medical facilities. The ministry of marine and fisheries has to permit the sports fishing business by the small fishing vessel in order to increase the income of inhabitants of fishing village and put a lot of the fishing reefs which are constructed to 2-3 story into the sea. And then the sports fishing business will be flourish by the small fishing vessel.

지역아동센터교사들의 구강보건 지도현황 분석 (Analysis of the current status of oral health guidance in community children's centers)

  • Objectives : The purpose of this study was to examine the oral health programs for children of the low income classes in order to prevent oral health problems and to provide the effective oral health programs in community children's centers. Methods : The subjects were 464 teachers in community children's centers in Seoul, Incheon, Gyeonggi-do, Chungcheongbuk-do, and Jeollabuk-do. Results : There were differences between the regions(p<0.05) in oral health guidance among teachers in municipal and provincial community children's centers. The oral health education was considered as the most important thing in those who had more than 6 years career(71.8%), and followed by those having less than five years career(61.3%) and those having two years career(53.8%)(p<0.05). Although many children registered the program and newsletter was sent to the children's homes, but 70% of the children did not brush their teeth regularly(p<0.05). Regardless of region, gender, teaching career, and number of enrolled children, 90 percent or more people found that it necessary for community children's centers to collaborate with dental clinics. There were gender differences between the male and female(p<0.05). Male accounted for 42.4% response rate and female accounted for 36.4%. Approximately About 48.5% of male and 61.6% of female thought that the oral health program is the most important thing(p<0.05). Conclusions : The development of the customized oral health program is the most critical factor to the oral health behavior change in the children in the community children's centers and public dental clinics.

성인여성의 의복가치관과 영향변인에 관한 연구 (A Study on Clothing Value and Influencing Factors of Women)

  • 본 연구의 목적은 성인여성의 인구통계적 변인에 따른 신체만족도, 욕구, 의복가치관의 차이를 조사하고, 인구통계적 변인, 신체만족도, 욕구의 변인이 의복가치관에 미치는 영향을 규명하는 데 있다. 본 연구는 인구통계적 변인에 따른 신체만족도, 욕구, 의복가치관의 차이를 알아보기 위하여 one-way ANOVA분석과 $Scheff{\acute{e}}$검증을 하였으며, 인구통계적 변인, 신체만족도, 욕구의 변인이 의복가치관에 미치는 영향을 알아보기 위하여 다중회귀분석을 실시하였다. 조사대상자는 경남지역에 거주하는 20-50대 사이의 성인여성 436명이었으며, 연구결과를 요약하면 다음과 같다. 인구통계적 변인에 따른 신체만족도, 욕구, 의복가치관은 부분적으로 유의미한 차이를 보였고, 특히 미디어접촉도는 신체만족도, 욕구, 의복가치관에 미치는 가장 중요한 요인으로 드러났다. 그러나 수입과 직업유무는 의복가치관의 어떠한 하위변인에서도 차이를 나타내지 않았다. 또한 의복가치관은 인구통계적 변인, 신체만족도, 욕구가 부분적으로 영향을 미치는 변수임을 알 수 있었다.

The Effects of Demographic Factors on Fashion Orientation, Fashion Response, and Buying Criteria(paper no.1)

  • This study is designed to identify how Demographic Factors affect Fashion Orientation(value), Fashion Response, and Buying Criteria. A total of 355 usable data was collected from housewives in three metropolitan cities(Seoul, Daejeon, Sungnam) in Korea. Young housewives who have one child are a market segment whose buying power is recognized by both the retailers and the market. The housewives' fashion orientation consists of four categories : social orientation, practical orientation, political orientation, and aesthetic orientation. The housewives' fashion response is classified into three areas : self conscious, self esteem, and self monitoring. The criteria of buying children's wear consists of nine components. As a result, the key reason for buying children' wear was 'attractive design'. Research result showed that POLITICAL ORIENTATION(SE beta=.229, p<.001) was more effective than AESTHETICS ORIENTATION(SE beta=.203, p<.001), for enhancing SELF-CONSCIOUS RESPONSE and SELF-ESTEEM RESPONSE. Therefore, this study suggests that the key factor for understanding trend can be a human self concept, consciousness, values, and orientation. The housewives' fashion orientation is responsible for 18.7% of BRAND ROYALTY(F = 20.172, p<.001) from among nine buying criteria. More poignantly, POLITICAL ORIENTATION covered 66.9% of selection of BRAND ROYALTY, and it explained 34.6% of selection of DESIGN among nine buying criteria. Thus, it showed that POLITICAL ORIENTATION(SE beta=.331, p<.001) is more effective than SOCIAL ORIENTATION(SE beta=.146), for upgrading BRAND ROYALTY. In addition, it showed that POLITICAL ORIENTATION(SE beta=.238, p<.001) is more effective than AESTHETICS ORIENTATION(SE beta=.040) for upgrading DESIGN evaluation. Housewives' fashion orientation, and fashion response are differentiated by demographic factors, such as occupation, women's career, husband' job, income, and location related to social status.

전문대학 식품영양과의 교수실태에 관한 연구 (A Study on the Actual Status of Professors in Department of Food and Nutrition at Junior College)

  • This study was carried out to propose the way to give substantial education for the students of Departments of Food and Nutrition in Junior College. Questionnaires were gathered from the professors of the Departments of Food and Nutrition in Junior Colleges. The results of this study could be summarized as follows. 1. The professors and assistant teachers appointed in Department of Food and Nutrition in Junior Colleges were insufficient in number showing the sufficiency proportion of 35.8% and 61% respectively. 2. Part time lecturers took charge of lectures on major subject up to 27%. 3. The academic backgrounds of all professors were above a master's degree, and the number of professors who were In the doctor's course and who had got doctor's degree amounted to 31.5% and 27.9% respectively. 4. The number of professors who had majored in the course of Food and Nutrition at university or at graduate school was the most and that of Food Engineering ranked the next. 5. The number of professors having careers in the fields of food and nutrition before the appointment to professor amounted to 32%. 6. The number of professors having domestic training managed by the Education Ministry amounted to 67.8%, but those having foreign training amounted to only 8.3%. 7. The number of domestic scientific journals subscribed by one professor was on an average 2.8, but that of foreign was only 0.8. 8. Each professor associated on an average 2.7 kinds of academic society, and published 3.6 pieces of thesis in the last three years. 9. The professors were comparatively satisfied with their present occupations, but they were dissatisfied with such points as deficiency of educational postulates, insufficient income, low level in intellectual faculties of their students and lack of social understanding.

국공립병원 의사의 근무지속의사 관련 요인 (Factors Related to Job Retention of Physicians in Public Hospitals)

  • Background : Public hospitals suffer worsening shortage of physicians and face great pressure of recruiting doctors. This study is aim to identify the factors associated with retention of physicians who are working in public hospitals. Methods : We conducted a cross-sectional and self-administered questionnaire survey in July, 2011. A total of 333 physicians responded from the 31 public hospitals. We analyzed the difference of job retention across the variables among doctors stratified as salaried and public health doctors. We used chi-square test and multiple logistic regression analysis. Results : To the salaried doctor, longer work period(OR=2.04 in 3rd quartile), professional autonomy(OR=2.69), and positive attitude toward public health(OR=2.39) affect to the higher job retention whereas complain of low income(OR=0.33) and complain of poor clinical environment(OR=0.26) affects to the lower job retention. To the public health doctors, community connections such as hometown(OR=6.27), spouse factors(OR=3.49), and positive attitude toward public health(OR=3.19) affect to the higher job retention. But longer work period(OR=0.17 in 3rd quartile) affects to the lower job retention. Conclusions : Associated factors of job retention vary across physician's status. Professional autonomy has major impact on the job retention to the salaried doctor. And familial factors as well as community relationship have greatest impact to the public health doctor. Positive attitude toward public health is associated with the higher job retention to the both of salaried and public health doctors.

한국 청소년의 비타민.무기질 보충제 섭취 및 관련 요인에 관한 연구 (Prevalence of vitamin.mineral supplement use and its related factors among Korean adolescents)

  • This study is conducted in order to investigate the prevalence of vitamin mineral (VM) supplement use and its related factors among Korean adolescents. Using a cross-sectional study design, information was obtained by questionnaires from adolescents selected by stratified sampling in large, small and middle-size cities, as well as rural areas of the whole country, followed by convenience sampling. The total number of participants in this study was 1,407 adolescents ($15.2{\pm}1.6$ years) attending middle or high schools, chosen from various cities and rural areas in Korea. The prevalence of VM supplement use was 65.5%, and there was no significant difference between the genders. City dwellers took VM supplements more frequently than those adolescents from rural areas (p < 0.01). The VM supplement users tended to have higher family income, parents' academic background, and socioeconomic status, respectively (p < 0.001). The greater part of VM supplement users replied that they acquired their nutritional information from their parents, relatives and friends, while many nonusers replied that they did it from school classes (p < 0.05). Compared to nonusers, VM supplement users had greater 'self concern about health' (p < 0.05), 'parents' concern about child's health, growth, and nutritional intake' (p < 0.001), and 'parents' concern about child's grade point' (p < 0.01). However, there was no significant difference between users and nonusers when it came to 'self perception of body shape' or 'BMI'. In addition, users significantly had greater nutritional beliefs concerning VM supplements than nonusers (p < 0.001). These results showed that VM supplement use has been common among adolescents in Korea, and supplement use of subjects has been related with various factors. Among them, it was shown that especially parents great influence the use of VM supplements by adolescents. Thus, not only education for adolescents, but also that for parents is needed to help adolescents take VM supplements properly.
