한국인의 상악중절치 위치 및 길이에 관한 연구 (A Study on the Position and Length of the Maxillary Central Incisor in Koreans)

  • The arrangement of anterior artificial teeth is an important factor on the esthetics in the construction of complete dentures and prostheses. During construction of the complete denture the position of maxillary central incisor plays an important role in the arrangements of artificial teeth. The proper position of maxillary central incisor provides natural appearance of anterior teeth and the anterior reference point of occlusal plane. Many methods have been utilized to eatablish the position of the maxillary central incisor in complete denture prosthodontics. However, there has not been provided for the reference for Korean yet. This study was aimed to extablish a guide for the position of the maxillary central inciosr in Koreans. The horizontal and vertical distance between the maxillary central incisor and incisive papilla in Koreans were measured. 1. The mean value of horizontal distance from the maxillary central incisor to the incisive papilla had significant sexual difference, The average distance was 12.648mm in male and 11.385mm in female. 2. The horizontal distance of the incisive papilla had sexual difference, the average distance was 6.182mm in male and 5.622mm in female. 3. The vertical distance from the maxillary central incisor to the incisive papilla had not sexual difference, the average distance was 7.21mm. 4. The crown length of the maxillary central inciosrs from the distal interdental papilla had not sexual difference, the right central incisor was 6.40mm(mean) and the left central inciosr was 6.43mm(mean). 5. The clinical crown length of the maxillary central incisors had not sexual difference, the right central incisor 10.35mm(mean) and the left central incisor was 10.43mm(mean). 6. The shapes of the incisive papilla were pear(68.4%), oval(16%), triangular(6%), irregular(5%), rectangular(4%), and inverted pear(1%) shape.

분광측정기를 이용한 자연 치아의 색조 비교 분석 (Shade Comparative Analysis of Natural Tooth using Spectrophotometric Methods)

  • 본 연구는 상악 전치부 색조 선택에 객관적인 기준을 설정하고자 색체계 Shade-Eye NCC를 이용하여 CIE $L^*$, $a^*$, $b^*$값으로 측정하여 이 값들을 비교 분석하고 그 의미를 고찰하고자 실시되었다. 치과를 내원하는 환자에게 연구 의도를 설명해 주고 동의를 얻어 111명(남자 50명, 여자 61명)의 3개 치아, 즉 상악 중절치, 상악 측절치, 상악 견치 총 333개의 치아를 측색하였다. 절단부와 치경부의 색차를 비교한 결과 상악견치의 ${\Delta}E^*$값이 $5.81({\pm}2.98)$으로 가장 낮았으며, 상악 측절치 $6.51({\pm}3.23)$, 상악 중절치의 $7.51({\pm}3.04)$ 순으로 점차 그 색차가 커지는 것을 알 수 있었다. 중절치와 측절치, 견치 모두 여성이 남성에 비해 명도($L^*$)가 높았으며, 황색채도($b^*$)는 중절치에서 남성이 여성보다 약간 높게 나타났다(p<0.05). 연령은 중절치의 명도($L^*$),적색채도($a^*$), 황색채도($b^*$), 측절치와 견치의 명도($L^*$), 황색채도($b^*$)에 유의미한 영향을 미치는 것으로 나타났다(p<0.05). 흡연은 자연치의 색조에 유의미한 영향을 미치지 못하였으며, 음주는 중절치의 명도($L^*$)를 감소시키며, 측절치의 적색채도($a^*$)를 감소시키는 경향을 나타냈다(p<0.05). 만성질환은 중절치와 측절치의 명도($L^*$)를 감소시키는 경향을 나타냈다(p<0.05).

  • 진단용 셋업 모형 제작시 전치의 치축을 조절하기 위하여 incisor inclination indicator란 장치가 소개되었다. 이것은 상하악 전치의 후방 견인시 토오크 조절을 위하여 사용된다. 본 논문에서는 incisor inclination indicator를 사용하여 전치부 설측 브라켓에 적절한 토오크를 처방하는 방법을 묘사하고 이렇게 적절한 토오크가 처방된 전치부 설측 브라켓으로 치료한 양악 전돌증 환자의 치료결과를 평가한다. 후방 견인시 상하악 전치는 토오크가 적절하게 조절되었다. 설측교정치료에 있어서 incisor inclination indicator는 기공 과정에서 부가적으로 사용되어 전치부의 후방 견인시 효과적으로 토오크 조절을 할 수 있는 것으로 생각된다.

  • The data about dental caries obtained from 1438 preschool children (male 797, female 641) in Seoul aged from 3 to 6 were analyzed by their sexes, ages, jaws and teeth surfaces. The results were as follows. 1. dmf rate : 92.63% 2. dmft index : 6.06 dmft rate 30.81 % dmfs index : 11.82 dmfs rate 12.02 % 3. d,m & f rate d rate: 79.45% m rate: 8.15% f rate : 12.40 % 4. dft index : 5.94 5. 디지틀 카메라를 이용한 치아색상 측정 (NEW EVALUATION TECHNIQUE IN TEETH COLOR USING DIGITAL CAMERA)

  • In addition to their function, there is growing interest in esthetics of restorations for teeth from fracture and loss. Because the perception of color depends on a lot of factors such as light source, object, observer and so on. It is difficult to make an objective expression. Therefore the using of mechanical equipment is recommended to exclude these variables and observer's vias. This study was carried out to evaluate the teeth color with a newly developed digital camera and to suggest a way to solve its flaws. After photo-taking and storaging the images of normal teeth into a computer, color spaces were examined with an image analysis system, Adobe Photoshop 3.04 and a statistical significance was determined between groups in term of teeth positions and ages. Results were as follow ; 1. Central incisor looked darker than lateral incisor(p<0.05), but lateral incisor and canine showed a similar lightness. 2. Central incisor showed more greenish color than lateral incisor and canine(p<0.05). 3. Canine showed higher yellowish value than central and lateral incisors(p<0.05). 4. Central incisor and canine showed no difference with aging. But in lateral incisor, teeth in fifth decade were lighter than those of third(p<0.05) and teeth in third decade were more reddish and yellowish than those of fourth decade(p<0.05).

한국인(韓國人) 영구치(永久齒) 맹출시기(萌出時期)에 관(關)한 통계학적(統計學的) 연구(硏究) (STATISTIC STUDY ON ERUPTION TIME OF PERMANENT TEETH IN KOREA)

  • The author has sought to determine the time and the sequence of permanent teeth eruption in Korean children. The study group consisted of 15,671 healthy children (male 8,015 ; female 7,656) aged 5-13 years old who lived in Seoul or Cheongju. The results were as follows : 1. The eruption times of permanent teeth were 0.45 years earlier in females than in males. 2. The ages corresponding to $ER_{50}$ of permanent teeth were as follows : In Maxilla 1) central incisor was 7.37 yrs 2) lateral incisor was 8.50 yrs 3) canine was 10.83 yrs 4) 1st premolar was 10.30 yrs 5) 2nd premolar was 11.09 yrs 6) 1st molar was 6.49 yrs 7) 2nd molar was 12.79 yrs In Mandible 1) central incisor was 6.40 yrs 2) lateral incisor was 7.41 yrs 3) canine was 10.18 yrs 4) 1st premolar was 10.26 yrs 5) 2nd premolar was 11.15 yrs 6) 1st molar was 6.32 yrs 7) 2nd molar was 12.05 yrs 3. The eruption sequence of permanent teeth by Z-test was as follow: In Male 1st : Mandibular 1st molar, and Mandibular central incisor 2nd : Maxillary 1st molar 3rd : Maxillary central incisor 4th : Mandibular lateral incisor 5th : Maxillary lateral incisor 6th : Mandibular canine, Maxillary and Mandibular 1st premolar 7th : Maxillary canine 8th : Maxillary and Mandibular 2nd premolar 9th : Mandibular 2nd molar 10th : Maxillary 2nd molar In Female 1st : Mandibular 1st molar, and Mandibular central incisor 2nd : Maxillary 1st molar 3rd : Mandibular lateral incisor, Maxillary central incisor 4th : Maxillary lateral incisor 5th : Mandibular canine, Maxillary and Mandibular 1st premolar 6th : Maxillary canine 7th : Maxillary and Mandibular 2nd premolar 8th : Mandibular 2nd molar 9th : Maxillary 2nd molar 4. The corresponding permanent teeth in the mandible generally erupted earlier than the corresponding permanent teeth in the maxilla by an average of 0.73 years, but the mean eruption time of mandibular 1st premolars was almost the same as those of maxillary 1st premolars, and the mean eruption time of mandibular 2nd premolars was 0.06 years later than those of maxillary 2nd premolars. 5. There is no significant difference between left and right arch in the eruption time and sequence. 6. Generally, the ages of permanent teeth eruption tended to be earlier than those of Dr. Cha's data from 1963.

한국인 성인의 상악 전치부 치조골 형태에 관한 방사선학적 연구 (Radiographic evaluation of alveolar bone profile of maxillary anterior teeth in Korean young adult)

  • In order to achieve a satisfactory esthetic result of periodontal surgery or implant in maxillary anterior area, periodontists must be aware of normal alveolar bone anatomy. The purpose of this study was to evaluate the relationship of alveolar bone morphology to tooth shape and form. 78(mean age : 25 yrs) periodontally healthy volunteers participated in this study. Two maxillary central incisor and one lateral incisor were selected to study. With minimal local anesthesia, gutta-percha cone inserted to labial gingival sulcus of selected teeth just after bone sounding with periodontal probe. Metal ball (4mm diameter) attached to palatal fossa of central incisor. Then, periapical radiograph was taken according to long cone paralleling technique. After film scan, labial alveolar bone profile reproduced along interproximal bone and apical ends of gutta-percha cones on computer screen. By utilizing computer program, the distance from height of interproximal bone to the labial bone crest in central incisor-central incisor and central incisor-lateral incisor area was measured and converted to real distance by using vertical length of metal ball on film. After measuring crown length & width of central incisor, the 10 individuals ranked lowest GW/L ratio (crown width/length ratio) and the 10 ranked highest were selected as having a long-narrow(group N), or a short-wide(group W) form of the central incisors. Means of the distance from height of interproximal bone to the labial bone crest of group N, W were calculated and compared by means of independent t-test. The results were as follows: 1. Mean distance from the height of the interproximal bone to the labial bone crest was $3.5{\pm}0.7mm$ between two central incisor, and $2.8{\pm}0.6mm$ between central and lateral incisor. 2. Mean GW/L ratio of group N was 0.57, and group W was 0.8. Mean distance from the height of the interproximal bone to the labial bone crest of group N was higher than group W in both measured area(measurements of group N, W were $3.9{\pm0.2mm$ and $3.5{\pm}0.2mm$ between two central incisor, $3.0{\pm}0.2mm$ and $2.8{\pm}0.2mm$ between central and lateral incisor), but there were no statistically significant differences when the groups were compared. Within the limits of the present study, there was a tendency that subjects with long-narrow teeth have more scalloped alveolar bone profile than subjects with short-wide teeth in upper anterior area, but no statistically significant differences were found.

A COMPARATIVE STUDY OF THE ANGLES BETWEEN CROWN AXIS AND ROOT AXIS IN MESIODISTAL DIRECTION BY USING ORTHOPANTOMOGRAM

  • Orthopantomogram is commonly used to evaluate root parallelism. "Good parallelism" between roots is widely accepted as one of the guidelines of a successful orthodontic treatment. In case there was a large angle between crown axis and root axis, and if we valued only the position of crown in establishing occlusal relationship without considering of the situation of root, the problem of root arrangement between adjacent teeth would be occurred. The estimate of root parallelism in mesiodistal direction before and after orthodontic treatment must be emphasized. The intent of this study was to determine the clinical importance and correlation of the angle between crown axis and root axis. Orthopantomograms of 105 orthodontic patients being treated in Yonsei university were used in this study. Twenty-eight teeth in both maxilla and mandible were selected and analyzed quantitively to evaluate the angle between crown axis and root axis, and obtain the correlationship among the individual teeth. The results are as follows: 1. Among the teeth presenting normal distribution, the maxillary right canine showed the largest mean value( $5.73{\pm}4.42^{\circ}$), which was composed of the crown-root angles, and the mandibular left lateral incisor showed the smallest mean value( $0.60{\pm}3.76^{\circ}$). 2. The crown-root angles of the maxillary incisors and the first molars, and the mandibular central incisors and the first molars didn't show normal distribution and the ranges of these angles were dispersed. 3. Significant differences were present between the crown axis and the root axis except for lower first premolars. (p<0.05) 4. No significant difference was present for the crown-root angle between right and left side, (p<0.05) 5. No significant difference was present for the crown-root angle between male and female except for lower left first premolar. (p<0.05) 6. In the upper right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine. In the upper left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor. In the lower right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, first molar and second molar. In the lower left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine, first molar and second molar. 상악 측절치 과잉치의 특징 분석 (Analysis of the Characteristics of Maxillary Supernumerary Lateral Incisor)

  • 이 연구의 목적은 상악 측절치 과잉치의 해부학적인 특징과 정상 측절치와의 관계에 대해 정량적 분석을 시행하는 것에 있다. 43명의 44개의 상악 측절치 과잉치를 대상으로 전산화 단층 방사선사진을 이용하여 측절치 과잉치의 위치, 형태, 방향 및 측절치 과잉치와 인접한 영구전치와의 관계를 관찰하였다. 또한 측절치 과잉치와 인접한 측절치 및 반대측 측절치의 크기를 비교하기 위해 각각의 치관 근원심 폭경을 계측하였고, 치아성숙도를 비교하기 위해 Nolla stage를 이용하여 각 치아의 치근 발육 정도를 평가하였다. 측절치 과잉치는 90.9%의 비율로 정위 방향으로 존재하였고, 84.1%가 보충형의 형태로 존재하였다. 측절치 과잉치는 인접한 측절치나 반대측의 측절치에 비해 치관의 크기가 통계적으로 유의하게 작았고(p < .0001), 치근 발육 단계에서는 통계적 유의미한 차이를 보이지 않았다. 결론적으로, 상악 측절치 과잉치는 정상측절치와 매우 유사하지만 치관 크기에 있어서 차이를 보인다. 상악 측절치 과잉치가 존재하는 경우, 교정적 혹은 심미적인 문제를 줄이기 위해 측절치와 과잉치를 정확하게 구분하여 치료하는 것이 중요하다.
