Clinical features and prognostic factors in drowning children: a regional experience

  • Purpose: This study aimed to evaluate the clinical features of children who have survived a water submersion incident, and to identify risk factors for prognosis. Methods: We retrospectively reviewed the medical records of patients who experienced submersion between January 2005 and December 2014. The patients were classified into 2 groups, according to complications, and prognostic factors were evaluated. Results: During the study period, 29 children experienced submersion (20 boys and 9 girls; mean age, $83.8{\pm}46.4$ months). Submersion occurred most commonly in the summer, with the peak incidence in August. The most frequent Szpilman clinical score was grade 5 (13 patients; 44.8%), followed by grade 6 (7 patients; 24.1%), and grades 1 or 2 (3 patients; 10.3%). Five children (17.2%) in the poor prognosis group died or had hypoxic ischemic encephalopathy, and the overall mortality rate was 6.9%. Poor prognosis after submersion was associated with lower consciousness levels (P=0.003), higher Szpilman scores (P=0.007), greater need for intubation and mechanical ventilator support (P=0.001), and longer duration of oxygen therapy (P=0.015). Poor prognosis was also associated with lower bicarbonate levels (P=0.038), as well as higher sodium, aspartate transaminase (AST), and alanine transaminase (ALT) levels (P=0.034, P=0.006, and P=0.005, respectively). Szpilman clinical scores were positively correlated with consciousness levels (r=0.489, P=0.002) and serum liver enzyme levels (AST and ALT; r=0.521, P=0.004). Conclusion: We characterized the prognostic factors associated with submersion outcomes, using the Szpilman clinical score, which is comparable to consciousness level for predicting mortality.

Development of a Real Time Video Image Processing System for Vehicle Tracking (실시간 영상처리를 이용한 개별차량 추적시스템 개발)

  • Video image processing systems(VIPS) offer numerous benefits to transportation models and applications, due to their ability to monitor traffic in real time. VIPS based on wide-area detection, i.e., multi-lane surveillance algorithm provide traffic parameters with single camera such as flow and velocity, as well as occupancy and density. However, most current commercial VIPS utilize a tripwire detection algorithm that examines image intensity changes in the detection regions to indicate vehicle presence and passage, i.e., they do not identify individual vehicles as unique targets. If VIPS are developed to track individual vehicles and thus trace vehicle trajectories, many existing transportation models will benefit from more detailed information of individual vehicles. Furthermore, additional information obtained from the vehicle trajectories will improve incident detection by identifying lane change maneuvers and acceleration/deceleration patterns. The objective of this research was to relate traffic safety to VIPS tracking and this paper has developed a computer vision system of monitoring individual vehicle trajectories based on image processing, and offer the detailed information, for example, volumes, speed, and occupancy rate as well as traffic information via tripwire image detectors. Also the developed system has been verified by comparing with commercial VIP detectors.

Neural-network-based Driver Drowsiness Detection System Using Linear Predictive Coding Coefficients and Electroencephalographic Changes (선형예측계수와 뇌파의 변화를 이용한 신경회로망 기반 운전자의 졸음 감지 시스템)

  • One of the main reasons for serious road accidents is driving while drowsy. For this reason, drowsiness detection and warning system for drivers has recently become a very important issue. Monitoring physiological signals provides the possibility of detecting features of drowsiness and fatigue of drivers. One of the effective signals is to measure electroencephalogram (EEG) signals and electrooculogram (EOG) signals. The aim of this study is to extract drowsiness-related features from a set of EEG signals and to classify the features into three states: alertness, drowsiness, sleepiness. This paper proposes a neural-network-based drowsiness detection system using Linear Predictive Coding (LPC) coefficients as feature vectors and Multi-Layer Perceptron (MLP) as a classifier. Samples of EEG data from each predefined state were used to train the MLP program by using the proposed feature extraction algorithms. The trained MLP program was tested on unclassified EEG data and subsequently reviewed according to manual classification. The classification rate of the proposed system is over 96.5% for only very small number of samples (250ms, 64 samples). Therefore, it can be applied to real driving incident situation that can occur for a split second.

Computer simulation for the effects of inserting the textured ZnO and buffer layer in the rear side of ZnO/nip-SiC: H/metal type amorphous silicon solar cells (Zno/nip-SiC:H/금속기판 구조 비정질 실리콘 태양전지의 후면 ZnO 및 완충층 삽입 효과에 대한 컴퓨터 수치해석)

  • In the structure of ZnO/nip-SiC: H/metal substrate amorphous silicon (a-Si:H) solar cells, the effects of inserting a rear textured ZnO in the p-SiC:H/metal interface and a graded bandgap buffer layer in the i/p-SiC:H have been analysed by computer simulation. The incident light was taken to have an intensity of $100mW/cm^2$(AM-1). The thickness of the a-Si:H n, ${\delta}$-doped a-SiC:H p, and buffer layers was assumed to be $200{\AA},\;66{\AA}$, and $80{\AA}$, respectively. The scattering coefficients of the front and back ZnO were taken to be 0.2 and 0.7, respectively. Inserting the rear buffer layer significantly increases the open circuit voltage($V_{oc}$) due to reduction of the i/p interface recombination rate. The use of textured ZnO markedly improves collection efficiency in the long wavelengths( above ${\sim}550nm$ ) by back scattering and light confinement effects, resulting in dramatic enhancement of the short circuit current density($J_{sc}$). By using the rear buffer and textured ZnO, the i-layer thickness of the ceil for obtaining the maximum efficiency becomes thinner(${\sim}2500{\AA}$). From these results, it is concluded that the use of textured ZnO and buffer layer at the backside of the ceil is very effective for enhancing the conversion efficiency and reducing the degradation of a-Si:H pin-type solar cells.

A study on the pedicatric accident (응급실 내원 아동에 대한 분석)

  • Children's accident is a largely preventable public health problem. Little is known. however, about population-based incident and outcome of pediatric accident. From 1997.9 through 1998,8. admission data from emergency center in I city were collected. 1418 patient from 0 through 13 years of age were selected. All children with unintensional accidental problems were identified through coded sheet which categorizes epidemiologic characteristics. The specific purposes of this study are analysis about the characteristics of pediatric accidents. And it aims to produce the basic data necessary for accident prevention policy development. The results of this study were as follows; 1. The number of male children$(62.6\%)$ were higher than female children$(37.4\%)$ 2. The age group from 1 to 3 years represents the highest proportion$(45.4\%)$ of every accidents except on traffic accident. 3. The highest proportion of accident were as follows occured during the June-August$(34\%)$, Sunday$(22.6\%)$, and 17-21 p.m. $(37.2\%)$ 4. The main causes of accident include general trauma$(70.9\%)$, environmental accident$(l6.8\%)$. and traffic accident$(l2.1\%)$, 5. Preschool age group represents more than half$(65.4\%)$ of traffic accident. 6. environmental injury includes burns $(46.6\%)$, foreign body$(43.6\%)$, exposure to poisonous materials$(6.3\%)$. and bite(3.3) This results could be used to develope prevention programs and assist in accident prevention system development. And also these data substantiate that accident prevention program decrease safety-related injury rate in preschool age group must be concentrated on enhancing access to a system to have a significant effect. Furthermore, it is necessary for accident prevention. So several suggestions are described here: 1. Development of parent's educational program for accident prevention and safety education should be done actively. 2. Home safety surveillance system should be initiated. 3. The initiation of children's accident report system could be contribute the analysis and the reduction of accident.

  • In general, dose rates for a monoenergetic gamma emitter uniformly distributed in an infinite cloud have been calulated by using the monoenergetic point-isotorpic source kernel technique. The most serious limitation on use of the kernel technique is subjected to the fact that it estimates the dose only at the surface of body. As a result, an alternative method is presented in which estimates of dose rate for immersion in a radioactive cloud are resulted from the scattered photon spectra incident on the surface of body. The results are in excellent agreement with other's. Work is currently in progress to apply these results to immersion dose problems associated with absorbed dose distribution in the MIRD phatom.

An Experimental Study on the Characteristics of Flux Density Distributions in the Focal Region of a Solar Concentrator (태양열 집광기의 초점 지역에 형성된 플럭스 밀도 분포의 특성)

  • This experimental study represents the results of an analysis on the characteristics of flux density distributions in the focal region of solar concentrator. The characteristics of flux density distributions are investigated to optimally design and position a cavity receiver. This deemed very useful to find and correct various errors associated with a dish concentrator. We estimated the flux density distribution on the target placed along with focal lengths from the dish vertex to experimentally determine the focal length. It is observed that the actual focal point exists when the focal length is 2.17 m. We also evaluated the position of flux centroid, and it was found that there were errors within 2 cm from the target center. The total integrated power of 2467 W was measured under focal flux distributions, which corresponds to the intercept rate of 85.8%. As a result of the percent power within radius, approximately 90% of the incident radiation is intercepted by about 0.06 m radius.

Current Status of Patient Safety Regulations, Guidelines and Support Mechanisms in Korean Hospitals

  • Purpose: This study was conducted to investigate patient safety regulations and guidelines in order to understand their current status, and to examine support measures to improve patient safety in Korean hospitals. Methods: The participants were the safety officers from hospitals with 200 or more beds and 112 hospitals responded to the online survey. The questions covered patient safety regulations, the performance level of patient safety activities, patient safety incident reporting systems, the dedicated professional, training, support mechanisms, and expectations of reporting systems. Results: Among preventative measures, fall prevention and hand hygiene were reported to be most widely practiced (92% and 91%, respectively). Time-out for invasive procedures showed a relatively low practice rate at 70%. Among patient care activities, transfusion, surgery and sedation, medication, and infection management were performed by 84, 74, 93 and 93% of the hospitals, respectively. Patient safety activities included patient safety committee, patient safety cooperation between decision-making bodies, patient safety workshops, seminars, lectures, and training for employees. Conclusion: Patient safety regulations and guidelines have not yet been sufficiently prepared, and a public institution such as a certification authority is of crucial importance to enforce these guidelines.

Relationship Between Life Style and Obesity Indices in Adults Using Data from Health Examination (건강검진자료에 의한 일반 성인의 생활습관과 비민지표와의 관련성)

  • The purpose of this study was to obtain the relationship between life styles and obesity indices, and to investigate the influence of life styles on obesity indices among adults who examined health checkup in an university hospital. The subjects for this study were 4,112 adults who underwent medical examinations at a health center of a university hospital in Daejeon city from Jan 2012 to Dec 2013. We surveyed the life styles and obesity indices of study subjects from self-recorded questionnaires and medical examination charts after confirmed IRB of the hospital. As a results, the levels of obesity indices of subjects was closely correlated with increasing age, and life styles as alcohol drinking, smoking, regular exercise, overeating, meat consumption. The influence of life styles on obesity indices was incident in female than in male. And the main risk factors were regular exercise factor in obesity indices in both sex.

On the primary productivity in the southern sea of korea (한국남해역(韓國南海域)의 일차생산력(一次生産力))

  • Southern sea of Korea was investigated for primary productivity during four scientific cruises of Korea Ocean research and Development Institute. Frontal structure appeared to be an important physical characteristic in enhancing the phytoplankton production in the study area. Relatively high productivity was occurred near the front between Tsushima Warm Current Water and Coastal Waters of China continent in March 1990 and in November 1989, and near the front between Tsushima Warm Current Water and Korean coastal Water in April 1989. In August 1988 high productive zone was limited to the tidal front off the southwestern coast of Korea. Nutrient supply related to the frontal structure might play a dominant role in increasing the primary productivity but mechanisms of nutrient enrichment are not clear. Average column productivity showed its maximum in April 1989 (1727 mgC/m$^2$/day). In the costal Waters of the china Continent incident light may be an important factor in regulating the regulating the phytoplankton production because of low light penetration rate resulting from high turbidity.
