Measurement of Incident-reporting Rate for Developing a Leading Indicator of Safety Culture (안전문화 선행지표 개발을 위한 사건보고율 측정)

  • Various leading indicators of safety culture have been advocated for proactive actions as lagging indicators have limitations in reflecting the attitudes and behaviors due to their reactivity and low sensitivity. This study proposes a model of incident-reporting culture (IRC) and determines the influence of the components on incident-reporting rate (IRR) in order to develop proactive indicators of safety culture. A questionnaire survey was administered to 614 workers at a chemical company in Korea, and the internal psychological aspects were explored by using perceptions, attitude, and backgrounds. The relationship between these factors and IRR was quantitatively confirmed. The workers are more reluctant to report injury than property damage, the perception of severity is the most influencing factor, and most property damages are reported regardless of worker's willingness. These features should be prioritized when improving IRC, and the criteria of IRC need to be aligned with safety culture.

  • Domestic plant construction sites have a tendency to become bigger, more various and complicated. Moreover frequency of industrial incident is increasing due to short project period, coexistence work condition and using heavy construction equipment. Installing safety facilities and safety training contribute to prevent incident. But mostly incident happens due to workers' unsafe action and wrong work method. Technically, it is very difficult for us to closely manage workers preventing incident. Therefore, the role of foreman who ordinarily works together with his workers is considered more important. Specific execution plan was drawn through industrial incident statistics from Ministry of Labor and questionnaire survey to interest parties(supervisor, safety staff and foreman). If foreman has raised sense of belonging and responsibility by conducting practical safety training for his workers, manages job site and receives proper compensation for these roles, it will be expected that it fully influences incident decrease.

Brain Metastases from Cholangiocarcinoma: a First Case Series in Thailand

  • Background: Brain metastasis from cholangiocarcinoma (CCA) is a rare but fatal event. To the best of our knowledge, only few cases have been reported. Herein, we report the incident rate and a first case series of brain metastases from CCA. Methods: Between January 2006 and December 2010 5,164 patients were treated at Srinagarind hospital, Khon Kaen University; of those, 8 patients developed brain metastasis. Here we reviewed clinical data and survival times. Results: The incident rate of brain metastases from CCA was 0.15%. The median age of the patients was 60 years. Tumor subtypes were intrahepatic in 6 and hilar in 2 patients. All suffered from symptoms related to brain metastasis. Three patients were treated with whole-brain radiation therapy (WBRT), one of whom also underwent surgery. The median survival after the diagnosis of brain metastasis was 9.5 weeks (1-28 weeks). The longest survival observed in a patient in RPA class I with two brain lesions and received WBRT. Conclusion: This is a first case series of brain metastases from CCA with the incident rate of 0.15%. It is rare and associated with short survival time.

Analysis of Accelerated Soft Error Rate for Characteristic Parameters on Static RAM (정적 RAM 특성 요소에 의한 소프트 에러율의 해석)

  • This paper presents an ASER (Accelerated Soft Error Rate) integral model. The model is based on the facts that the generated EHP/s(electron hole pairs) are diminished after some residual range of the incident alpha particle, where residual range is a function of the incident angle and the capping layer thickness over the semiconductor junction. The ASER is influenced by the flux of the alpha particles, the junction area ratio, the alpha particle incident angle when the critical charge is same as the collected charge, and the sizes of the alpha source and the chip. The model was examined with 8M static RAM samples. The measured ASER data showed good agreement with the calculated values using the model. The ASER decreased exponentially with respect to the operational voltage. As the capping layer thickness increases up to $16{\mu}m$, the ASER increases, and after that thickness, the ASER decreases. The ASER increased as the depth of BNW increased from $0{\mu}m\;to\;4{\mu}m$. and then saturated. The ASER decreased as the node capacitance increased from 2fF to 5fF.

Effects of Bias Voltage and Ion-incident Angle on the Etching of Photoresist in a High-density CHF3 Plasma (고밀도 CHF3 플라즈마에서 바이어스 전압과 이온의 입사각이 Photoresist의 식각에 미치는 영향)

  • The etch rates of photoresist (PR) and the etch selectivity of $SiO_2$ to PR in a high density $CHF_3$ plasma were investigated at different ion-incident angles and bias voltages. A Faraday cage was employed for the accurate control of ion-incident angles. The ion energy was controlled by changing bias voltages. The etch rate of $SiO_2$ continuously decreased with ion-incident angles but the etch rate of PR remained constant up to the middle angle region and decreased afterwards. The etch rates of $SiO_2$ normalized to those at $0^{\circ}$ incident angle changed with the ion-incident angle following a cosine(${\theta}$) curve. On the other hand, the normalized etch rates of the PR changed showing a drastic over-cosine shape in the middle angle region. The etch selectivity of $SiO_2$ to PR decreased with an increase in the ion-incident angle because the etch yields of PR were enhanced by physical sputtering in the middle angle region compared to the case of $SiO_2$ etching. The etch selectivity of $SiO_2$ to PR decreased with an increase in the bias voltage at nearly all ion-incident angles.

  • Incident detection rate and false alarm rate are the key measures tot estimating the performance of automatic incident detection algorithms. It is, however inherently very difficult to improve the two measures simultaneously. The main purpose of this study is to present some methods for solving the problem. For this, an incident detection algorithm has been designed in this study. The algorithm is consisted of two functions, one for detecting incident and another for detecting congestion. A logic for distinguishing non-recurrent congestion from recurrent congestion was employed in the algorithm. The new algorithm basically requires speed, flow, and occupancy data for defining incident situation, but the algorithm is able to perform this task without one of the three parameters. The performance of the algorithm has been evaluated by using the field data collected from Interstate Highway 880 in Bay Area, California. The empirical analysis results are very promising and thus, the algorithm proposed may be very useful for the analysts. This paper presents some empirical test results for the performance of California incident detection algorithm, only for the reference purpose.

  • A high rate deposition sputtering process of magnesium oxide thin film in oxide mode has been developed using a 20 kW unipolar pulsed power supply. The powersupply was operated at a maximum constant voltage of 500 V and a constant current of 40 A. The pulse repetition rate and the duty were changed in the ranges of 10 ${\sim}$ 50 kHz and 10 ${\sim}$ 60 %, respectively. The deposition rate increased with increasing incident power to the target. Maximum incident power to the magnesium target was obtained by the control of frequency, duty and current. The deposition rate of a moving state was 9 nm m/min at the average power of 1.5 kW. This technique is proposed to apply high through-put sputtering system for plasma display panel.

  • Research of this paper focused on developing and demonstrating of algorithm with the figures of difference between historical traffic pattern data and real-time traffic data to decide on what the incident is. The aim of this dissertation is to develop incident detection algorithm which can be understood and modified easier to operate. To establish traffic pattern of this algorithm, weighted moving average method was applied. The basis of this method was traffic volume and speed of the same day and time at the same location based on 30-second raw data. The model was completed by a serious of steps of process-screening process of error data, decision of the traffic condition, comparison with pattern data, decision of incident circumstances, continuity test. A variety of parameter value was applied to select reasonable parameter. Results of application of the algorithm came out with figures of average detection rate 94.7 percent, 0.8 percent rate of misinformation and the average detection time 1.6 minutes. With these following results, the detection rate turned out to be superior compared with result of existing model. Applying the concept of traffic patterns was useful to gain excellent results of this study. Also, this study is significant in terms of making algorithm which theorized the decision process of actual operators.

Application and Evaluation of the Pilot Program for the Education Nurse System in a Medical Institution (교육전담간호사 제도 시범사업 적용에 따른 일 의료기관에서의 성과)

  • Purpose: This study was conducted to evaluate the performance following the application of the pilot program for the education nurse system of the Ministry of Health and Welfare in a medical institution. Methods: This study was employed a non-homogeneous comparative group design by comparing new nurses who entered the medical institution after the pilot project from December 2019 to April 2020 with new nurses who entered before the pilot project during the same period. Satisfaction, academic achievement, job adaptation, personal turnover within one year, and patient safety incident rate were investigated as performance indicators. Results: After the pilot project, the overall satisfaction among new nurses, preceptors, and fellow nurses increased, but there were no significant changes in academic achievement and job adaptation in new nurses. The personal turnover rate decreased from 15.6% to 9.1%, and the patient safety incident rate also decreased from 26.3% to 15.7%. Also, the preceptor overtime also decreased from 3.67 to 0.66 hours. Conclusion: The performance of the pilot project for the education nurse system was related to improvements in satisfaction, turnover rate of new nurses, patient safety incident rate, and preceptor overtime. Above all long-term monitoring of each performance indicator is necessary through the continuation of the education nurse system of the Ministry of Health and Welfare.
