구강 작열감 증후군에서 클로나제팜의 국소적 적용 (Topical Application of Clonazepam to Burning Mouth Syndrome)

  • 구강 작열감 증후군은 혀나 구강점막에 객관적인 이상징후(abnormal sign)를 보이지 않으면서 혀 및 구강점막의 지속적인 통증을 보이는 만성 질환이다. 연관된 것으로 추정되는 요인이 다양하고 환자마다 통증 양상이 다소 다르게 나타날 수 있어 적절한 진단 및 효과적인 치료를 제공하기 어려운 경우가 많다. 혀나 구강점막의 작열감은 알러지, 캔디다감염, 부기능습관, 타액선 기능저하 등과 같은 국소요인과 당뇨병, 갑상선기능저하증, 영양결핍 등과 같은 전신적 요인, 그리고 우울증, 걱정, 암공포증 등과 같은 심인성 요인과 연관되어 발생할 수 있다. 그러므로, 임상가들은 작열감을 유발할 수 있는 원인들에 대하여 숙지하고 있어야 하며, 혈액검사 등의 관련 검사를 통한 적절한 평가를 정확하고 세심하게 시행하여 효과적인 치료를 제공할 수 있어야 한다. 이러한 구강 작열감 증후군의 치료에는 약물요법, 인지행동요법, 심리치료 등 다양한 치료들이 시도되어지고 있다. 현재 구강작열감 증후군의 치료에 이용되고 있는 약물에는 클로나제팜(clonazepam), 가바펜틴(gabapentin), 삼환성항우울제(amitriptyline), 알파리포산(alpha-lipoic acid), 캡사이신(capsaicin) 등이 보고되고 있으며, 이 중 클로나제팜을 국소적으로 적용하는 것이 전신 투여하는 다른 약물에 비해 부작용이 적으면서 효과도 상당히 좋은 것으로 보고되고 있다. 이에 구강 작열감 증후군 환자에게 클로나제팜을 국소적으로 적용한 증례를 통해 그 효과를 확인하고자 하였다.

여성 근로자의 영양섭취 및 건강상태 조사 : 사무직과 납 사업장 근로자의 비교 (Nutritional and Health Status of Women Workers by Working Fields)

  • The objective of this study was to investigate the nutritional and health status of women industrial workers by working fields. One hundred forty eight (105 lead and 43 office) workers were recruited from March 2005 to October 2005. Information on age, education, smoking and drinking status were collected using questionnaire and nutrient intake and diet quality of workers were assessed by average of two-day 24 hr recall method. Biochemical indexes including blood lead level (PbB), indexes for iron status, serum calcium (Ca) and serum lipid profiles were analyzed from fasting venous blood or serum. Results showed that education level of lead workers was lower than that of office workers (p<0.05), but nutrient intake levels were not significantly different by working fields. Overall nutritional status of the subject were good except for calcium, vitamin $B_2$, C and folic acid intakes. PbB of lead workers were significantly higher than that of office workers while mean corpuscular hemoglobin concentration (MCHC) and serum Ca levels were significantly lower in lead workers. MCHC was positively correlated with zinc intake (r=0.166) and serum Ca was positively correlated with vitamin C intake (r=0.179). This study confirms that lead workers need extra care to keep their health and nutritional management especially for the nutrients known to interact with lead. Tailored nutrition education for workers at specific working fields needs to be more focused for the improvement of health status of industrial workers.

송이균(松茸菌) (Tricholoma matsutake)의 배양환경(培養環境)에 대한 증식반응(增殖反應)에 관한 연구(硏究) (A Study on Multiplication Response of "Tricholoma matsutake" (Pine Mushroom) Conidio to Cultural Media Environment)

  • 송이(松茸)는 적송(赤松)의 세근(細根)에 활물기생(活物寄生)한다고 하나 그 본질(本質)이 아직 구명(究明)되지 못하고 있음으로 인공재배(人工栽培) 확립(確立)이 모색(模索)되고 있는 중이다. 본논문(本論文)에서는 일차적(一次的)으로 송이(松茸)에서 순수분리(純粹分離)한 균(菌)을 배양(培養)하여 생리적(生理的) 특성(特性)을 구명(究明)하고 인공원기(人工原基)를 만들어 인공적(人工的)인 송이재배(松茸栽培)의 본질(本質)을 구명(究明)하여 송이재배(松茸栽培) 방법(方法)을 돕고자 송이균(松茸菌)의 생리특성(生理特性)에 대(對)하여 조사한 결과(結果)는 다음과 같다. 1) 각종(各種) 배양기상(培養基上)에서의 송이균계(松茸菌系)의 발육(發育)은 봉밀(蜂蜜)+송이발생토양전즙(松茸發生土壤煎汁)+적송수근추출물(赤松鬚根抽出物)+건조효모(乾燥酵母)+$KH_2PO_4$+Inositol+엽산(葉酸)+Biotin의 배지(培地)가 가장 우수(優秀)하였다. 2) 송이포자(松茸胞子)의 발아(發芽) 및 균계배양(菌系培養)에서 최적온도(最適溫度)는 $24^{\circ}C$, 최적(最適) pH는 4.5였다. 3) 송이균계배양(松茸菌系培養)에 있어서 송이자실체(松茸字實體)의 조직(組織)에서 분리(分離)한 것과 포자(胞子)에서 분리(分離)한 것은 그 발율(發育)에 차이(差異)가 없었다, 4) Hamada 배지(培地)에 각종(各種) 미량중금속염류(微量重金屬鹽類) $ZnSO_4$, $MnSO_4$, $MgSO_4$, $CaCl_2$ 구연산철(枸櫞酸鐵) 등(等)의 첨가(添加)는 별효과(別效果)가 없었다. 5) 적송수근추출물(赤松鬚根抽出物) 첨가(添加)는 균계발육(菌系發育)에 확실(確實)한 효과(效果)가 있었다. 적송(赤松)에는 송이균계(松茸菌系)의 생장(生長)을 촉진(促進)하는 인자(因子)가 존재(存在)한다는 것을 알 수 있었다. 6) 송이균계(松茸菌系) 배양용(培養用) 인공배양기(人工培養基)의 C원(源)으로서 glucose보다 봉밀(蜂蜜)의 효과(效果)가 크다. 7) Fairy Ring이 가장 많이 존재(存在)하는 미생물(微生物)인 Mortierlla spp의 배양여액(培養濾液)을 송이균계(松茸菌系) 인공배양기(人工培養基)에 첨가(添加)한 것이 대조치(對照値)에 비(比)하여 균계(菌系)의 발육(發育)이 양호)良好)하였다. 8) 송이인공원기(松茸人工原基)의 배지(培地)에 적송수근추출물(赤松鬚根抽出物) lnsitol Biotin 엽산(葉酸)의 첨가(添加)는 효과(效果)가 크다. 균계(菌系)의 만연(蔓延)된 후(後) 온도(溫度)를 내려서 $19^{\circ}C$로 유지(維持)할 때 primordium의 형성(形成)을 확인(確認)하였다.

합성배지(合成培地)에서 불로초(不老草)가 생산(生産)하는 섬유소(纖維素) 분해효소(分解酵素)에 관한 연구(硏究) (Studies on the Cellulolytic Enzymes Produced by Ganoderma lucidum in Synthetic Media)

  • 합성(合成) 배지(培地)에서 Ganoderma lucidum CAFM 9065의 섬유소(纖維素) 분해(分解) 효소(酵素) 생산(生産)과 균사(菌絲) 생육(生育)에 관한 배양(培養) 조건(條件)을 검토(檢討)한 결과(結果)를 요약(要約)하면 다음과 같다. 탄소원(炭素源) 중에서 avicelase와 CMCase 생산(生産)은 Na-CMC를, ${\beta}-glucosidase$ 생산(生産)은 cellobiose를 첨가(添加)하였을 때 가장 좋았으며, 균사(菌絲) 생육(生育)은 soluble starch와 cellobiose가 양호(良好)하였다. 효소(酵素) 생산(生産)은 Na-CMC 1.0%에서 가장 양호(良好)하고 균사(菌絲) 생육(生育)은 Na-CMC의 농도(濃度) 증가(增加)에 따라 현저히 증가(增加)되었다. Glucose는 농도(濃度)의 증가(增加)에 따라 효소(酵素)의 생산(生産)이 감소(減少)되었으나 균사(菌絲) 생육(生育)은 증가(增加)되었다. 효소(酵素) 생산(生産)이 양호(良好)한 질소원(窒素源)은 peptone이었고 효소(酵素) 생산(生産)에 적당(適當)한 peptone의 농도(濃度)는 0.2% 이었으며, 균사(菌絲) 생육(生育)은 peptone 농도(濃度)의 증가(增加)에 따라 점차 증가(增加)되었다. 효소(酵素) 생산(生産)의 $KH_2PO_4$$MgSO_4{\cdot}7H_2O$의 최적(最適) 농도(濃度)는 각각 0.3%, 0.02 %이었고, 균사(菌絲) 생육(生育)은 0.2%, 0.02%에서 가장 좋았다. folic acid는 저농도(低濃度)(0.03 mg/l)로 효소(酵素) 생산(生産)을 증가(增加)시켰고, 균사(菌絲) 생육(生育)은 inositol에서 양호(良好)하였다. 효소(酵素) 생산(生産)과 균사(菌絲) 생육(生育)의 최적(最適) 온도(溫度)는 $30^{\circ}C$, avicelase와 ${\beta}-glucosidase$의 최적(最適) pH는 5.0, CMCase는 pH 5.5 이었으며, 균사(菌絲) 생육(生育)은 pH 5.0에서 양호(良好)하였다. 배양(培養) 기간(期間)은 avicelase가 8일(日), CMCase가 10일(日), ${\beta}-glucosidase$는 16 일간(日間) 배양(培養)하였을 때 최대치(最大値)를 보였고, 균사(菌絲) 생육(生育)은 12일(日) 배양(培養)했을 때 가장 양호(良好)하였다.

영양교육이 알코올중독자의 영양지식, 식습관 및 영양섭취상태에 미치는 효과 (Effects of nutrition education on nutrition-related knowledge, dietary habits, and nutrient intakes of alcoholic patients)

  • 본 연구는 알코올중독자의 영양지식과 식습관 및 영양섭취상태의 특성을 알아보고, 영양교육이 이들 특성에 미치는 효과를 알아보고자 수행되었다. 알코올중독으로 병원에 입원한 37명 환자를 연구대상자로 하였으며, 이들의 영양지식과 식습관은 설문지를 이용하여 조사하였고, 영양교육은 80분짜리 5차시 분량의 프로그램을 제작해 실시하였다. 본 연구대상자는 40대 중반의 남성이었으며, 정상 체위를 지녔고, 병원에 입원하기 전 음주 빈도가 높고, 음주량이 많으며, 소주를 선호하는 음주습관을 지니고 있었다. 이들은 저 학력, 중 하류층 직업, 독거 비율이 높은 가족형태 등 사회경제적 수준이 열악한 편이었고, 자신의 건상상태에 대한 인식은 낮은 경향이었으며, 간 질환을 비롯한 질병 보유율이 높아, 삶의 질이 취약한 편이었다. 영양교육 실시 전 이들의 영양지식은 1점 만점에 0.52점으로 비교적 낮은 수준이었고, 식습관은 3점 만점에 1.70점으로 바람직하지 않은 경향이었다. 영양섭취상태도 에너지와 식이섬유를 비롯해 본 연구에서 조사한 8종의 비타민 중에 비타민 C, 티아민, 리보플라빈 및 엽산이 영양섭취기준을 충족하지 못했고, 6종의 무기질 중에서는 칼슘과 칼륨은 영양섭취기준에 미치지 못한 반면에 나트륨 섭취는 과다한 문제점을 보였다. 그러나 영양교육을 받은 후 본 연구대상자의 영양지식이 0.81점으로 유의하게 향상되었고, 식습관은 2.13점으로 유의성 있게 개선되었으며, 영양섭취상태도 다음과 같이 유의적으로 향상되었다. 즉 에너지, 탄수화물, 식이섬유, 비타민 E를 제외한 비타민 A, 비타민 C, 비타민 $B_6$, 티아민, 리보플라빈, 엽산 및 니아신, 칼슘, 인, 칼륨, 철분, 아연 모두 섭취가 증가했다. 반면에 나트륨 섭취는 감소했으나 여전히 목표섭취량의 256%에 달하였지만, 감소했으며, 이러한 결과 Na/K 섭취 비율이 저하되었다. 이와 같은 본 연구결과는, 비록 연구대상자가 광주광역시에 소재한 G병원에 입원 중인 소수의 성인남자 환자였다는 제한점은 있으나, 알코올중독 입원환자의 경우 영양지식 정도가 낮고, 식습관이 좋지 않으며, 영양섭취상태가 불량한 편이지만 이들에게 비교적 단기간의 영양교육을 실시함으로써 이러한 문제점들을 긍정적으로 변화시킬 수 있다는 점을 확인해 주었다. 그러므로 알코올중독전문병원 등 알코올중독자의 재활을 돕는 현장에서 영양교육의 필요성을 인식하고 영양교육을 적극 실시하여야 할 것으로 사료된다.

상업용 김치와 담금 배추김치의 발효에 따른 품질특성 변화 (Change of Quality Characteristics of Commercial and Prepared Kimchi Depending on Fermentation)

  • Purpose: The purpose of this study was to evaluate the folate contents and quality characteristics of commercial and fermented Kimchi and compare the correlation between folic acid and quality characteristics. Methods: The contents of total folate and quality characteristics were evaluated in 10 kinds of industrial and local Kimchi and nine kinds of other commonly consumed Kimchi. Changes in folate content and quality characteristics of Kimchi during 14 days of fermentation at $15^{\circ}C$ were compared. Results: Chungnam (L4) Kimchi had the lowest pH of 4.62, and acidity (0.57%) and salinity (3.26%) were highest compared to other areas. The content of total folate of D company (A) was the highest at $73.66{\mu}g/100g$. The content of total folate was significantly higher than those of Sesame leaf Kimchi (O9) and Young leafy radish Kimchi (O5) containing 65.77 and $62.82{\mu}g/100g$, respectively. The pH of fermented Kimchi decreased from 6.09 to 4.11 (p<0.05), and the acidity significantly increased (p<0.001) to 0.32-0.66%. Reducing sugar content decreased rapidly by 4 days and then slowly decreased (p<0.001). Total bacteria and lactic acid bacteria counts significantly increased (p<0.05) with fermentation period. Content of total folate was highest at $69.82{\mu}g/100g$ and $68.16{\mu}g/100g$ on days 0 and 2 of fermentation, after which it decreased to 77.6% at $15.61{\mu}g/100g$ on day 10 of fermentation. Conclusion: As a result, there was no definite trend regarding folate content in commercial Kimchi and other Kimchi. Young leafy radish, Chives, and Welsh onion Kimchi, which are rich in green leaves, are excellent folate source foods. The highest folate content of fermented Kimchi was identified on day 2 of fermentation. Therefore, it is recommended to use raw or immature Kimchi when using Kimchi as a folate source food.

한국 청소년의 거주지역별 및 체격별 식사의 질 평가지수의 비교 (Comparison of Indices for Diet Quality Evaluation of Korean Adolescents by Residence Area and Body Size)

  • The purpose of this study was to assess several indices of diet quality based on nutrient, food and food group intake of Korean adolescents based on several indices on diet quality according to residence area and body size. Using the data from the 1998 National Health and Nutrition Survey, twenty-four-hour-dietary recalls of a total of 1,110 Korean adolescents aged 13 - 19 years (male 543, female 567) were analyzed for nutrient adequacy ratio (NAR) , index of nutritional quality (INQ) , the number of foods (Dietary Variety Score, DVS) and food group consumed (Dietary Diversity Score, DDS) . In doing that, it was attempted to apply only the minimum amount of solid foods of Kant's without inclusion of liquid foods because of the very limited variety in Korean foods. Based on weight length index, 13.1% of the subjects were categorized as obese, 14.2%, overweight, 44.4%, normal and 28.3%, underweight. Only vitamin B2 intake was higher in the obese group than in the underweight group. There was no meaningful difference in energy, protein and fat intakes according to the grade of the body size. In terms of residence area, intake of fat, niacin, vitamin $B_6$ and folic acid were lower in the rural areas than in the metropolitan city. Only vitamin E intake was higher in the rural areas. Mean value of NARs (MAR) and INQs (mINQ) was also higher in the metropolitan city than in the rural areas, but there was no significant difference of these two values according to body size of the subjects. Mean DVS was 21.02 for total subjects, and has no difference between male and female and between metropolitan city and other medium-small city. But, the rural areas showed the lowest DVS of 19.05. Mean DDS in which five is a maximum score was 3.3 with no significant difference by sex and by residence area in male subjects. However, in female subjects, DDS in the rural areas was the lowest. According to body size of the subjects, there was no meaningful difference in both scores of DVS and DDS. In conclusion, most indices of nutrient intake and food and food group intake were not significantly different by body size of the subjects, while most indices were significantly different by residence areas: higher in the metropolitan city than in the rural area.

서울 거주 20대 여성의 아침식사 섭취 빈도에 따른 식습관 및 영양소 섭취상태 조사 (Dietary Habits and Nutritional Status of Young Women according to Breakfast Frequency in Seoul)

  • Objectives: Although breakfast is important to nutrition balance, prevention of overeating, and weight control, people in their 20s (males: 55.1%, females: 49.9%) were reported to have the highest rate of skipping breakfast in 2016 Korea Health Statistics. This study aims to examine dietary habits and nutrient intake depending on breakfast frequency among young women in Seoul. Methods: The subjects were 655 young women in Seoul from August to October 2016, and the survey was performed by using a questionnaire that included general characteristics, dietary habits, and eating behavior. Body composition was determined by bioelectric impedance analysis. Nutritional status was examined by the 24-hour recall method. Results: The participants were classified by breakfast intake frequency; '${\geq}5times/week$ (n=160)', '1-4 times/week (n=327)', and 'breakfast skipping (n=168)'. The 'breakfast skipping' group had lower frequency and regularity of meals. In addition, the 'breakfast skipping' group had a higher frequency of eating-out and late-night meals. There was no difference in total calories between the 'breakfast skipping' group and other groups, but the 'breakfast skipping' group had significantly low carbohydrate and fiber intakes. The participants showed lower intakes of calories, fiber, vitamin A, vitamin C, niacin, folic acid, calcium, potassium, and zinc in comparison with recommended intakes. Especially, the 'breakfast skipping' group had significantly lower fiber, vitamin A, vitamin C, calcium, potassium levels compared to the '${\geq}5times/week$' group. For Mean Adequacy Ratio (MAR), the 'breakfast skipping' group recorded a ratio of 0.60, which was lower than those of other groups. Index of Nutritional Quality (INQ) including fiber, vitamin C, calcium and phosphorus were significantly lower in the breakfast skipper group, compared to the breakfast eater group. Conclusions: The 'breakfast skipping' group showed low regularity of meals and a high frequency of eating-out and late-night meals. The breakfast regular eater group showed high intake of micronutrients and quality of meals was high in general. Skipping breakfast could lower nutrient intake and quality of meals, which requires attention.

인천지역 장기요양시설과 주간보호시설 여성치매노인의 인지기능 및 영양섭취 비교 (Comparison of the Cognitive Function and Nutrient Intakes of Elderly Women with Dementia in a Long-Term Care Facility and a Day-Time Care Facility in Incheon)

  • In this study we compared the cognitive function (Mini-Mental State Examination for Dementia Screening: MMSE-DS) and nutrient intake of elderly women with dementia in a long-term care facility (EW-LCF) and a day-time care facility (EW-DCF). This survey was conducted from July 2015 to May 2017 on 73 elderly women with dementia (47 women in LCF and 26 women in DCF) in the city of Incheon. The data obtained from interviews with the subjects and caregivers were analyzed using the SPSS 20.0. The total score of the MMSE-DS and intakes of most nutrients in the EW-LCF were significantly lower compared to the EW-DCF (p<0.05). The nutrient adequacy ratios (NAR) of protein, vitamin A, C, $B_6$, $B_{12}$, thiamin, riboflavin, niacin, folic acid, calcium and phosphorus in the EW-LCF were significantly lower compared to the EW-DCF (p<0.05). However, the indexes of nutritional quality (INQ) of vitamin C, $B_6$, thiamine, niacin, calcium, magnesium, and iron in the EW-LCF were significantly higher compared to the EW-DCF (p<0.05). As a result, the subjects showed an unbalanced and insufficient nutrition intake status regardless of the type of care facility. Most nutrient intakes of the EW-LCF with lower cognitive function were very significantly lower compared to the EW-DCF (p<0.001). Therefore, it is necessary to assess the cognitive function and nutritional status regularly at care facilities for the elderly women suffering with dementia and to provide specialized individual nutritional management.

모유 경험이 미취학 아동의 신체발달, 손목 골밀도 및 영양섭취에 미치는 영향 (The Effect of Breast Feeding on Growth Development, Bone Mineral Density of Carpus, and Nutrient Intakes in Preschool Children)

  • The purpose of this study was to examine the long term effects of breast feeding on growth, bone development and nutrient intakes in preschool children. Subjects were 62 preschool children. Anthropometric characteristics and bone mineral density of carpus were measured using DEXA. The questionnaire was composed of health status, life style, dietary behaviors, and dietary intakes and was completed by the children's mothers. The average age of the boys was 62.4 months and that of the girls was 62.1 months. Average birth height and weight of the subjects was 50.9 cm and 3.4 kg for boys and 50.3 cm and 3.3 kg for girls, respectively. The average height, weight, $\%$ body fat, and obesity index were 111.7 cm, 19.6 kg, $15.0\%$, $-2.1\%$ in boys and 109.4 em, 18.7 kg, $17.5\%$, $0.2\%$ in girls, respectively. Forty children were fed colostrum, 21 were fed breast milk, 29 were fed formula, and 12 were fed mixed milk. There was no significant difference in growth status between children who were fed colostrum and those who were not. Children who had mixed feedings were significantly taller than those who did not (p <0.001), and children who had colostrum had significantly higher bone mineral density than those who did not ($0.25 \pm 0.04 g/ cm^2$ vs. $0.23 \pm 0.04 g/cm^2$, p < 0.05). However, the different feeding methods did not show any difference in bone mineral density. Except vitamin $B_6$, folic acid and vitamin E, vitamin, mineral and calorie intakes did not meet the Korean RDAs. Calcium intake was especially lower than recommendations by as much as $62\%$ and $70\%$ in boys and girls, respectively. There was no significant difference in nutrient intakes between children who had colostrum and those who did not. However, children who were formula consumed significantly more animal fat than those who did not (p < 0.05). Neonatal feeding and breast or formula feeding seems to associate with height, bone mineral density and animal fat intakes in preschool children, based on the results of this study. A longitudinal study is needed to clarify this relationship.
