부분 설절제술을 받은 환자에서의 연마면 인상 및 Direct Metal Laser Sintering 을 이용한 총의치 제작 증례 (Complete denture making in a patient of partial glossectomy using polished surface impression taking and direct metal laser sintering method: A case report)

  • 성공적인 총의치를 위해서는 유지, 안정, 지지 세가지 요소가 필수적으로 요구된다. 또한 심한 치조제의 흡수나 수술에 의한 반흔으로 인해 하악 설측변연 형성이 어려운 총의치 제작시에는 무교두 치아배열, 연마면 인상, 무게 보강 내부금속 삽입, 의치 접착 크림의 사용 등의 다양한 고려사항이 필요하다. 본 환자의 경우 심한 치주염으로 심하게 골흡수가 된 무치악 치조제를 가졌고, 또한 설암으로 인한 설절제술로 인해 연조직의 문제를 가진 환자이다. 부가적인 유지와 안정을 얻기 위해 연마면 인상과 내부 보강 금속구조물의 삽입 그리고 최소 가압 인상 등의 방법을 적용하여 보다 나은 임상적 결과를 나타내었다. 또한 원하는 3차원적 형태를 정확하게 부여하고 제작시 복잡한 기공과정을 줄인 금속 구조물을 제작하기 위해 최소 가압 인상 채득과 Direct metal laser sintering이 사용되었다.

첨단 제조기술 기반으로 제작된 치과용 모형의 정확도에 관한 연구 (Accuracy of dental model based on the state-of-the-art manufacturing technique)

  • 본 연구의 목적은 전통적인 인상채득방식으로 제작된 석고모형과 구강 디지털 인상법을 이용하여 제작된 폴리우레탄 모형의 각 계측지점을 비교함으로써 첨단 제조기술로 제작된 모형의 정확성을 검증하는 것이다. 유치악 아크릴릭 구강모형을 선정하여 기존에 사용하던 방식의 석고모형과 최신경향의 방식으로 모형을 각 10개씩 제작하였다. 계측지점은 총 6곳을 지정한 후, 2인의 검사자가 2회로 나누어 버니어 캘리퍼스를 이용하여 측정하였다. 제작방법에 따른 모형의 각 계측치의 평균, 표준편차, 계측치 차이의 평균 등의 기술 통계량으로 제시하였고, 정확성 검정을 위해 윌콕슨 부호순위 검정을 시행하였으며, 검사자 내, 검사자 간, 제작방법 간의 신뢰성 분석은 급내 상관계수 분석과 측정오차를 구하였다. 실험결과 신뢰성은 급내 상관계수의 범위는 0.76에서 0.99의 값이 나타나 높은 신뢰도를 보였으며, 모형 제작방법에 따른 정확성은 모든 계측지점 간의 거리를 비교하였을 때 폴리우레탄 모형이 작게 계측되었으며 0.09~0.20mm의 범위로 선행연구의 결과에 의해 임상적으로 오차범위를 허용할 수 있는 것으로 나타났다. 구강 디지털 인상법의 사용으로 제작된 모형은 임상적으로 적절한 것으로 판단되지만, 향후 보다 정밀하고 사용하기 편리한 치과용 모형을 제작하기 위한 연구들이 추가적으로 이루어져야 할 것이며, 다양한 임상 데이터를 활용한 평가가 이루어져야 할 것으로 사료된다.

  • 중증 근무력증을 가지고 있는 환자의 경우 구강 내 근육 및 신경계가 적절히 조정되지 않아 의치 제작 및 유지가 어렵다. 또한 과도하게 확장되거나 두꺼운 의치는 근육에 자극을 주어 근무력 증상을 유발할 수 있으므로 이를 고려하여 의치 제작을 해야 한다. 본 증례에서는 개구 제한을 가진 중증 근무력증을 가진 환자에서 폐구 인상법을 이용하여 의치를 제작하였다. 폐구인상법을 이용하여 환자가 가지고 있는 저작압 및 근육의 움직임을 재현하고 이를 통해 구강 기능 운동시 구강 내 근육에 자극이 적고 적절한 유지와 안정을 갖는 임플란트 유지 피개의치를 제작하였다.

고려시대 목조건축물의 상록하단(上綠下丹) 단청기법 수용 (The Adaptation of Sangrokhadan Technique on the Color Painting of Wooden Buildings in the Goryeo Dynasty)

  • The color that painted on the ground of Dancheong becomes Gachil(basecoat)-Dancheong and underpainting of Moro-Dancheong or Geum-Dancheong. So, the color of underpainting is the most important element that determines impression of the building. Thus, the architecture after using "Sangrokhadan" has different characters from what it had been. In the existing perception toward the background color of Dancheong, it was considered the characters of Korean Dancheong so-called "Sangrokhadan" that paint vertical elements like columns red and upper part of the columns green. But this study examined the color of Dancheong according to the era and region before and after Goryeo Dynasty era, then it reveals that Sangrokhadan technique was applied from the 14th century in the late Goryeo Dynasty. One of the Goryeo architecture, Geungnakjeon Hall of Bongjeongsa Temple is thought to be a previous style that is not applied "Sangrokhadan" technique because old elements are painted red pigment.

Comparison of the accuracy of digitally fabricated polyurethane model and conventional gypsum model

  • PURPOSE. The accuracy of a gypsum model (GM), which was taken using a conventional silicone impression technique, was compared with that of a polyurethane model (PM), which was taken using an iTero$^{TM}$ digital impression system. MATERIALS AND METHODS. The maxillary first molar artificial tooth was selected as the reference tooth. The GMs were fabricated through a silicone impression of a reference tooth, and PMs were fabricated by a digital impression (n=9, in each group). The reference tooth and experimental models were scanned using a 3 shape convince$^{TM}$ scan system. Each GM and PM image was superimposed on the registered reference model (RM) and 2D images were obtained. The discrepancies of the points registered on the superimposed images were measured and defined as GM-RM group and PM-RM group. Statistical analysis was performed using a Student's T-test (${\alpha}=0.05$). RESULTS. A comparison of the absolute value of the discrepancy revealed a significant difference between the two groups only at the occlusal surface. The GM group showed a smaller mean discrepancy than the PM group. Significant differences in the GM-RM group and PM-RM group were observed in the margins (point a and f), mesial mid-axial wall (point b) and occlusal surfaces (point c and d). CONCLUSION. Under the conditions examined, the digitally fabricated polyurethane model showed a tendency for a reduced size in the margin than the reference tooth. The conventional gypsum model showed a smaller discrepancy on the occlusal surface than the polyurethane model.

A Study on the Production of Fashion Pictorials using Depaysement Technique and Narrative Structure

  • Fashion pictorials are recognized as another form of communication method beyond the concept of simply providing information or being artistic photography. Therefore, it is believed that there is necessity to know methods of making a strong impression on people's conscience by effective usage of various presentation techniques that can be used in fashion pictorials. This research studied characteristics of closed form and open form of Wolfflin's precept of forms presented in scene construction. Depaysement technique was classified into three classes which are alien transportation of a part, deformation and ambivalence of an image through literature review of depaysement presentation technique. After studying the concept and characteristics of narrative applicable as the open form, Annie Leibovitz's "Alice's Adventures in Wonderland" published in an actual fashion magazine was analyzed as an empirical study based on the theoretical research. Through this study, potential of applying depaysement technique as a presentation technique of closed form and narrative as a presentation technique of open form on fashion pictorials was identified. Factors that infuse creativity and achieve uniformity in producing fashion pictorials were shown through analysis of actual fashion pictorials.

Accuracy of intraoral scans of edentulous jaws with different generations of intraoral scanners compared to laboratory scans

  • PURPOSE. Purpose of this in vitro study was to determine the accuracy of different intraoral scans versus laboratory scans of impressions and casts for the digitization of an edentulous maxilla. MATERIALS AND METHODS. A PEEK model of an edentulous maxilla, featuring four hemispheres on the alveolar ridges in region 13, 17, 23 and 27, was industrially digitized to obtain a reference dataset (REF). Intraoral scans using Cerec Primescan AC (PRI) and Cerec AC Omnicam (OMN), as well as conventional impressions (scannable polyvinyl siloxane) were carried out (n = 25). Conventional impressions (E5I) and referring plaster casts were scanned with the inEOS X5 (E5M). All datasets were exported in STL and analyzed (Geomagic Qualify). Linear and angular differences were evaluated by virtually constructed measurement points in the centers of the hemispheres (P13, P17, P23, P27) and lines between the points (P17-P13, P17-P23, P17-P27). Kolmogorov-Smirnov test and Shapiro-Wilk test were performed to test for normal distribution, Kruskal-Wallis-H test, and Mann-Whitney-U test to detect significant differences in trueness, followed by 2-sample Kolmogorov-Smirnov test to detect significant differences in precision (P < .008). RESULTS. Group PRI showed the highest trueness in linear and angular parameters (P < .001), while group E5I showed the highest precision (P < .001). CONCLUSION. Intraoral scan data obtained using Primescan showed the highest trueness while the indirect digitization of impressions showed the highest precision. To enhance the workflow, indirect digitization of the impression itself appears to be a reasonable technique, as it combines fast access to the digital workflow with the possibility of functional impression of mucosal areas.

심한 하악골 흡수와 혈관기형을 보이는 무치악 환자에서 폐구인상법과 단일평면교합법을 이용한 총의치 수복 증례 (Complete denture rehabilitation in the edentulous patient with severe mandibular bone resorption and vascular malformation using closed mouth impression and monoplane occlusion: A case report)

  • 본 환자는 혈관기형으로 인한 구순과 혀, 전정 등 연조직의 비대와 팽윤감, 심한 하악골 흡수로 인해 일반적인 총의치를 사용할 수 없었다. 최선의 치료 방법은 임플란트 피개의치이나 경제적 제약 때문에 선택할 수 없었다. 하악 총의치로 수복을 하기 어려운 증례에서 음압을 형성하는 BPS 흡착식 원리를 응용한 폐구인상법과 고딕아치 묘기법을 이용한 중심위 악간관계기록 및 비해부학적치아를 이용한 단일평면교합법으로 총의치를 제작하여 그 결과가 양호하였다. 치료 전 환자와의 충분한 대화를 통해 요구사항을 확인함으로써 환자가 원하는 심미성을 보유하면서 기능적으로 양호한 총의치 제작이 가능하였다.

Effect of scanning strategies on the accuracy of digital intraoral scanners: a meta-analysis of in vitro studies

  • PURPOSE. This study aimed to investigate whether the accuracy of intraoral scanners is influenced by different scanning strategies in an in vitro setting, through a systematic review and meta-analysis. MATERIALS AND METHODS. This review was conducted in accordance with the PRISMA 2020 standard. The following PICOS approach was used: population, tooth impressions; intervention, the use of intraoral scanners with scanning strategies different from the manufacturer's instructions; control, the use of intraoral scanners following the manufacturers' requirements; outcome, accuracy of intraoral scanners; type of studies, in vitro. A comprehensive literature search was conducted across various databases including Embase, SciELO, PubMed, Scopus, and Web of Science. The inclusion criteria were based on in vitro studies that reported the accuracy of digital impressions using intraoral scanners. Analysis was performed using Review Manager software (version 5.3.5; Cochrane Collaboration, Copenhagen, Denmark). Global comparisons were made using a standardized mean difference based on random-effect models, with a significance level of α = 0.05. RESULTS. The meta-analysis included 15 articles. Digital impression accuracy significantly improved under dry conditions (P < 0.001). Moreover, trueness and precision were enhanced when artificial landmarks were used (P ≤ 0.02) and when an S-shaped pattern was followed (P ≤ 0.01). However, the type of light used did not have a significant impact on the accuracy of the digital intraoral scanners (P ≥ 0.16). CONCLUSION. The accuracy of digital intraoral scanners can be enhanced by employing scanning processes using artificial landmarks and digital impressions under dry conditions.

Accuracy of casts produced from conventional and digital workflows: A qualitative and quantitative analyses

  • PURPOSE. Comparing the accuracy of casts produced from digital workflow to that of casts produced from conventional techniques. MATERIALS AND METHODS. Whole arch alginate (ALG) and polyvinyl siloxane (PVS) impressions were taken with stock trays and custom trays, respectively. The ALG impressions were poured with type III dental stone, while the PVS impressions were poured with type IV dental stone. For the digital workflow, IOS impressions were taken and physical casts were produced by 3D printing. In addition, 3D printed casts were produced from images obtained from a laboratory scanner (LS). For each technique, a total of 10 casts were produced. The accuracies of the whole arch and separated teeth were virtually quantified. RESULTS. Whole arch cast accuracy was more superior for PVS followed by LS, ALG, and IOS. The PVS and ALG groups were inferior in the areas more susceptible to impression material distortion, such as fossae and undercut regions. The LS casts appeared to have generalized errors of minor magnitude influencing primarily the posterior teeth. The IOS casts were considerably more affected at the posterior region. On the contrary, the IOS and LS casts were more superior for single tooth accuracy followed by PVS and ALG. CONCLUSION. For whole arch accuracy, casts produced from IOS were inferior to those produced from PVS and ALG. The inferior outcome of IOS appears to be related to the span of scanning. For single tooth accuracy, IOS showed superior accuracy compared to conventional impressions.
