A 55-year-old woman who had received an implantable left ventricular assist device 3 months earlier presented with dyspnea and a low-flow alarm of the device. Computed tomography and log-file analysis of the device system suggested inflow cannula obstruction. Since the patient had cardiogenic shock due to pump failure, venoarterial extracorporeal membrane oxygenation (ECMO) was initiated. With ECMO, surgical exchange of the pump was considered. However, the obstruction spontaneously resolved without surgical intervention. It turned out that an obstructive thrombus was washed out by rebooting the pump. Moreover, the thrombus was embolized in the patient's left subclavian artery. The patient underwent heart transplantation 4 months after the pump obstruction accident and continued to do well.

A new type of motor-driven total artificial heart system with a moving-actuator mechanism has been developed. The prototype system consists of a brushless do motor inside of actuator, two polyurethane sacs, and peripheral devices with internal controller. In this research work, the moving-actuator type electromechanical total artificial heart was redesigned for a good anatomical fitting. For total implantation we are developing the internal controller with high reliability and stability, and automatic control algorithm in response to the physiological requirements of animal. A differential floating mass transducer has been developed in Korea for fully implantable middle ear hearing devices (F-IMEHDs). In particular, the performance of a differential floating mass transducer (DFMT) is very important among the parts of the F-IMEHDs because the mechanical vibration generated by DFMT is delivered to the inner ear directly. In this paper, the electrical model is proposed to analyze the DFMT vibration characteristic using the mechanical model of the DFMT. The electrical model enables the simple analysis of DFMT vibration characteristics using a computer program. The proposed electrical model is simulated through PSpice as changing the values of passive elements in the electrical model. To verify the proposed model, the DFMT has been implemented on the basis of the simulated results and the experiment for vibration measurement has been carried out. Through the comparison, it is verified that the proposed model is useful to analyze the vibration characteristics of the DFMT.
A flexible polyvinylidene fluoride (PVDF)/polydimethylsiloxane (PDMS) composite prototype with high piezoelectricity and force sensitivity was constructed, and its huge potential for applications such as biomechanical energy harvesting, self-powered health monitoring system, and pressure sensors was proved. The crystallization, piezoelectric, and electrical properties of the composites were characterized using an X-ray diffraction (XRD) experiment and customized experimental setups. The composite can sustain up to 100% strain, which is a huge improvement over monolithic PVDF fibers and other PVDF-based composites in the literature. The Young's modulus is 1.64 MPa, which is closely matched with the flexibility of the human skin, and shows the possibility for integrating PVDF/PDMS composites into wearable devices and implantable medical devices. The $300{\mu}m$ thick composite has a 14% volume fraction of PVDF fibers and produces high piezoelectricity with piezoelectric charge constants $d_{31}=19pC/N$ and $d_{33}=34pC/N$, and piezoelectric voltage constants $g_{31}=33.9mV/N$ and $g_{33}=61.2mV/N$. Under a 10 Hz actuation, the output voltage was measured at 190 mVpp, which is the largest output signal generated from a PVDF fiber-based prototype.
Background: Left ventricular assist devices (LVADs) are widely employed as a therapeutic option for end-stage heart failure. We evaluated the outcomes associated with centrifugal-flow LVAD implantation, comparing 2 device models: the Heartmate 3 (HM3) and the Heartware Ventricular Assist Device (HVAD). Methods: Data were collected from patients who underwent LVAD implantation between June 1, 2015 and December 31, 2022. We analyzed overall survival, first rehospitalization, and early, late, and LVAD-related complications. Results: In total, 74 patients underwent LVAD implantation, with 42 receiving the HM3 and 32 the HVAD. A mild Interagency Registry for Mechanically Assisted Circulatory Support score was more common among HM3 than HVAD recipients (p=0.006), and patients receiving the HM3 exhibited lower rates of preoperative ventilator use (p=0.010) and extracorporeal membrane oxygenation (p=0.039). The overall early mortality rate was 5.4% (4 of 74 patients), with no significant difference between groups. Regarding early right ventricular (RV) failure, HM3 implantation was associated with a lower rate (13 of 42 [31.0%]) than HVAD implantation (18 of 32 [56.2%], p=0.051). The median rehospitalization-free period was longer for HM3 recipients (16.9 months) than HVAD recipients (5.3 months, p=0.013). Furthermore, HM3 recipients displayed a lower incidence of late hemorrhagic stroke (p=0.016). In the multivariable analysis, preoperative use of continuous renal replacement therapy (odds ratio, 22.31; p=0.002) was the only significant predictor of postoperative RV failure. Conclusion: The LVAD models (HM3 and HVAD) demonstrated comparable overall survival rates. However, the HM3 was associated with a lower risk of late hemorrhagic stroke.
최근 첨단 과학의 발달과 함께 인체조직에 대한 적용도가 뛰어난 소재가 개발되어 초소형의 이식형 장치가 개선되어감에 따라 전력공급방법의 다양한 연구가 이루어져 유도코일을 이용하여 무선으로 전력을 전송하는 장치가 연구되어오고 있다. 이에 저자는 이론적으로 효율이 100%인 E급 전력증폭기를 사용하여 $2{\sim}30mm$의 공극거리에서 가장 이상적인 주파수를 1MHz로 설정하여 제작하였고, 직경 46mm의 송수신 코일을 이용하여 코일의 비정렬에 대한 전송율이 이격거리가 10mm일 때, 20% 감소되었다. PLL을 사용하여 주파수추적동조법으로 공극거리 15 mm 이내에서는 완만하게 20% 정도의 에너지 전송효율을 얻을 수 있었다. 또한 최적의 공진거리에서 50 mA 정도의 출력전류가 얻을 수 있다는 것은 초소형 전기 자극기와 같은 이식형 장치를 동작시키는데 구동 전력으로 가능하다는 것을 알 수 있었다.
For the biocompatibility test of implantable devices or for the sensitivity evaluation of biomedical sensors, it is required to understand the mechanism of the protein adsorption and the interaction between the adsorbed proteins and cells. In this study, the adsorption of proteins in a cell culture medium with fetal bovine serum onto an indium tin-oxide electrode was characterized by using linear sweep voltammetry and impedance spectroscopy. We immersed the fabricated ITO electrodes in the culture medium for 30, 60, or 90 min, and then measured the electrochemical properties of electrodes with 10 mM $Fe(CN){_6}^{3-/4-}$ and 0.1 M KCl electrolyte. With an increase of contacting time, the anodic peak current was decreased and the charge transfer resistance was increased. However, both parameters were recovered to the values before contact with the medium after the treatment of Trypsin/Ethylenediaminetetraacetic acid hydrolyzing proteins.
The research on microbial fuel cells (MFCs) needs various knowledge of different fields such as electrochemistry, microbiology, environmental engineering, and material engineering. Although electrochemically active bacteria are very diverse, the performance of MFCs is affected primarily by the structure of the reactor system. Thus, the development in the system architecture is critical to lower internal resistance and increase power generation for commercialization. This paper summarizes the principles of MFCs and demonstrates the infinite potential of MFCs in various applications including wastewater treatment, biosensors, biohydrogen production, remote power sources, implantable medical devices, etc.
This article highlights new opportunities of inorganic semiconductor materials for bio-implantable electronics, as a subset of 'transient' technology defined by an ability to physically dissolve, chemically degrade, or disintegrate in a controlled manner. Concepts of foundational materials for this area of technology with historical background start with the dissolution chemistry and reaction kinetics associated with hydrolysis of nanoscale silicon surface as a function of temperature and pH level. The following section covers biocompatibility of silicon, including related other semiconductor materials. Recent transient demonstrations of components and device levels for bioresorbable implantation enable the future direction of the transient electronics, as temporary implanters and other medical devices that provide important diagnosis and precisely personalized therapies. A final section outlines recent bioresorbable applications for sensing various biophysical parameters, monitoring electrophysiological activities, and delivering therapeutic signals in a programmed manner.
