실패한 이중관 국소의치에서 하이브리드 텔레스코픽 이중관 국소의치를 이용한 임상증례 (A clinical report of hybrid telescopic double crown denture with friction pin in a failed double crown denture case)

  • 가철성 국소의치 치료 시 유지력을 얻는 방법에 따라 많은 연구와 임상적인 적용이 이루어 지고 있다. 그중 한 종류인 이중관 의치는 독일과 스웨덴과 같은 유럽의 나라에서 널리 사용되어지고 있다. 텔레스코픽 이중관 국소의치는 구강 위생관리의 편의성, 지대치로 교합력의 수직적인 전달, 그리고 지대치들 간의 2차적인 고정과 같은 장점들이 있어 클라스프 유지형 가철성 국소의치에 비해 임상적으로 더 나은 결과를 보여줄 수 있다. 본 증례에서 처음에 환자는 자연치를 이용하여 상악에 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관 의치를 제작하였습니다. 7년 뒤 환자의 불량한 구강위생관리 습관과 정기검진의 부재로 지대치들에 치주염이 발생하였고 5개중 4개의 자연치 지대치를 발치하게 되었다. 남은 1개의 자연치 지대치의 내관을 그대로 사용하면서 3개의 추가적인 임플란트를 식립하였다. 대합치인 하악이 고정성 보철임에도 불구하고 전략적인 임플란트 식립으로 인해 환자는 새롭게 제작된 상악 치아-임플란트 연합 이중관 의치에 적응하였고 만족하였다.

완전 디지털 시스템을 이용한 상악동 거상술 및 구치부 임플란트 고정성 보철 수복 증례 (Sinus floor elevation and implant-supported fixed dental prosthesis in the posterior area, with full-digital system: a case report)

  • 진단, 수술, 인상, 보철물 제작 등 임플란트 치료 전 과정에서 디지털 기술이 활용되고 있다. 본 증례에서는 디지털 수술 가이드를 이용하여 합병증 없이 상악동 거상술을 시행하고, 계획된 위치에 임플란트를 식립하였다. 골유착을 위한 치유 기간 이후, CAD-CAM(Computer-aided design/Computer-aided Manufacturing)으로 맞춤형 지대주 및 임시 보철물을 제작하여 장착하고, 환자의 적응도와 교합을 평가하였다. 임시 보철물상에서 교합 변화가 관찰되어, 광중합형 컴포지트 레진으로 수리하였다. 최종 보철물 제작 시, 지대주의 수직 침하, 임시 보철물의 형태와 적응된 교합 관계를 반영하기 위해, 이중 스캔과 지대주 수준의 디지털 인상을 채득하였다. 치은 압배 없이 치은 연하 변연을 인기하기 위해, CAD-CAM 소프트웨어상에서 라이브러리화된 지대주 데이터를 중첩하고, 지르코니아 최종보철물 제작하여 장착하였다. 구치부 임플란트 수복 시, 디지털 시스템을 이용하여 전통적인 방법에서 겪는 어려움을 줄이고, 수술부터 보철물 제작까지 효율적인 치료 과정과 안정적이고 예지성 있는 결과를 얻어 보고하는 바이다.

Kimplant에 관한 3차원 유한요소 분석적 연구 (Three Dimensional Finite Element Analysis of Kimplant)

  • In this study, the biomechanical characteristics of Kimplant were compared with that of Branemark implant by using three dimensional finite element analysis. Two finite element models were fabricated by inserting each implant into the bone model. The bone model was designed to have 18mm height, 13mm width and 15mm length. The size of each implant was planned to have 4mm width and 10mm length. A 200N force was applied on the center of abutment top in three directions - vertical, horizontal and oblique. After analyzing the stresses of fixture and surrounding bone, following results were obtained. 1. There was similar stress distribution between the two models. 2. The magnitude of maximum principal stress on the implant was similar between the two models but the location of maximum principal stress on the implant was different. 3. The magnitude and location of maximum principal stress on the surrounding bone was similar between the two models.

Clinical and scanning electron microscopic analysis of fractured dental implants: a retrospective clinical analysis

  • Many longitudinal studies have reported the successful osseointegration of dental implants, with survival rates approaching 90-95%. However, implants regarded as a "success" may have also failed to undergo osseointegration. A variety of complications and failures have been observed, including implant fracture - a rare and delayed biomechanical complication with serious clinical outcomes. Given the increasing popularity of dental implants, an increase in the number of failures due to late fractures is expected. This study sought to determine the rate of implant fractures and factors associated with its development. This retrospective evaluation analyzed implants placed at Wonkwang Dental Hospital (from 1996 to the present). In our study we found that the frequency of dental implant fractures was very low (0.23%, 8 implant fractures out of 3,500 implants placed). All observed fractures were associated with hybrid-surface threaded implants (with diameter of 4.0 or 3.75 mm). Prosthetic or abutment screw loosening preceded implant fracture in a majority of these cases.

Clinical outcomes of rigid and non-rigid telescopic double-crown-retained removable dental prostheses: An analytical review

  • PURPOSE. The objective of this literature review was to analyze the cumulative survival rates (CSRs) of rigid and non-rigid double-crown-retained removable dental prostheses. MATERIALS AND METHODS. Screening of the literature published from January 1995 to December 2019 was performed by using electronic data base (Pubmed) and manual search. The CSRs of rigid and non-rigid double crown removable dental prostheses were investigated. RESULTS. A total of 403 articles were reviewed and 56 relevant articles of them were selected. Subsequently, 25 articles were included for data extraction. These articles were classified according to rigid and non-rigid type double crowns and further subdivided into teeth, implants, and teeth-implant combination types. The CSRs of rigid type double crown ranged from 68.9% to 95.1% of 5 to 10 years in tooth abutments, 94.02% to 100% over a 3-year mean observation periods in implant abutments, and 81.8% to 97.6% in tooth-implant combination. Non-rigid type double crowns had various CSR ranges from 34% to 94% maximum during 10 years observation in teeth abutment. The CSRs of non-rigid type had over 98% in implant abutments, and ranged from 85% to 100% in tooth-implant combination. CONCLUSION. The CSRs of double crowns varies according to types. With accurate evaluation of the remaining teeth and plan of the strategic implant placement, it could be successful treatment alternatives for partially or completely edentulous patients.

연조직 이식술과 넓은 computer aided design/computer aided mamufacturing 지대주를 이용한 임플란트 고정성 보철물의 출현윤곽 재현 (Regeneration of emergence profile with soft tissue graft and wide computer aided design/computer aided mamufacturing abutments: a clinical report)

  • 최근 구치부 부분 무치악 환자의 임플란트 보철 수복은 기능적뿐만 아니라 심미적으로도 환자의 만족을 이끌어내야 하는 과제를 안고 있다. 이렇게 환자의 요구가 높아지면서 상실된 자연치아와 유사한 형태의 임플란트 보철을 사용한 구강회복이 보철수복의 중요한 이슈가 되고 있다. 이를 위해서는 기성 지대주 보다는 더 넓은 customized CAD/CAM 지대주를 사용함으로써 자연스러운 출현윤곽을 재현할 수 있다. 넓은 지대주를 지지해주기 위해서는 충분한 폭과 높이를 가지는 각화치은이 존재해야 하는데, 만약 긴 치아상실기간이나 골이식술 등으로 인해 임플란트 주위에 각화치은이 부족한 경우에는 연조직 증대술을 시행하는 것이 추천된다. 이러한 과정을 통해 심미적이고 기능적인 임플란트 보철 제작이 가능하며 구강위생관리를 용이하게 하고 향후 임플란트 주위염의 발생 가능성을 감소시킨다. 본 증례는 구치부 부분 무치악 환자에서 골이식술, 상악동 이식술, 임플란트 식립술 및 연조직 이식술을 사용하여 부족한 치조골과 각화치은 및 협측전정을 회복하였으며, customized CAD/CAM 지대주와 지르코니아 보철물을 이용하여 수복한 증례로 주기적인 내원 및 검사를 통해 기능적, 심미적으로 만족할만한 결과를 얻었기에 이 사례를 보고하고자 한다.

Influence of the implant-abutment connection design and diameter on the screw joint stability

  • PURPOSE. This study was conducted to evaluate the influence of the implant-abutment connection design and diameter on the screw joint stability. MATERIALS AND METHODS. Regular and wide-diameter implant systems with three different joint connection designs: an external butt joint, a one-stage internal cone, and a two-stage internal cone were divided into seven groups (n=5, in each group). The initial removal torque values of the abutment screw were measured with a digital torque gauge. The postload removal torque values were measured after 100,000 cycles of a 150 N and a 10 Hz cyclic load had been applied. Subsequently, the rates of the initial and postload removal torque losses were calculated to evaluate the effect of the joint connection design and diameter on the screw joint stability. Each group was compared using Kruskal-Wallis test and Mann-Whitney U test as post-hoc test (${\alpha}$=0.05). RESULTS. The postload removal torque value was high in the following order with regard to magnitude: two-stage internal cone, one-stage internal cone, and external butt joint systems. In the regular-diameter group, the external butt joint and one-stage internal cone systems showed lower postload removal torque loss rates than the two-stage internal cone system. In the wide-diameter group, the external butt joint system showed a lower loss rate than the one-stage internal cone and two-stage internal cone systems. In the two-stage internal cone system, the wide-diameter group showed a significantly lower loss rate than the regular-diameter group (P<.05). CONCLUSION. The results of this study showed that the external butt joint was more advantageous than the internal cone in terms of the postload removal torque loss. For the difference in the implant diameter, a wide diameter was more advantageous in terms of the torque loss rate.

임플란트 보철물의 합착에 대한 문헌고찰 (A literature review on cementation of implant prosthesis)

  • 시멘트유지형 임플란트 보철물은 교합과 심미적인 측면에서 상당한 장점을 가지지만 시멘트잔사가 남을 경우 임플란트주위염을 유발하는데 대부분 후기 증상으로 발현되며 특히 제거의 어려움과 시멘트잔사로 인한 임플란트주위염은 임플란트의 예후를 위협하는 요인이 될 수 있다. 그러므로 사용목적에 맞는 유지력을 가지는 시멘트를 선택하는 것이 필요하다. 일반적으로 레진시멘트, 폴리카복실레이트시멘트, RMGI 시멘트가 GI 시멘트나 ZPC보다 높은 유지력을 가지며 임시합착제는 열순환 후에는 유지력이 낮아진다. 시멘트 외에도 지대주의 높이와 경사도, 지대주의 수 및 분포도 유지력에 영향을 줄 수 있다. 무엇보다 목적에 맞는 시멘트를 선택하고 생역학적인 원칙을 준수한 보철물을 설계하고 시멘트잔사를 줄이는 방법을 이용하여 합착한 후 방사선사진으로 잔사를 확인하여 완벽하게 제거해야 시멘트유지형 보철의 성공을 얻을 수 있을 것이다.

나사형 임플란트 고정체의 길이, 직경, 플랫폼 형태에 따른 임플란트와 주위조직의 응력분포 (Influence of diameter, length, and platform shape of implant fixture on the stress distribution in and around the screw type implant)

  • Seven finite element models were constructed in mandible having single screw-type implant fixture connected to the premolar superstructure, in order to evaluate how the length, diameter and platform shape of a screw-type fixture influence the stress in the supporting tissue around fixtures. Each finite element model was varied in terms of length, diameter, and platform shape of the fixture. In each model, 250N of vertical load was placed on the central pit of an occlusal plane and 250N of oblique load placed on the buccal cusp. The stress distribution in the supporting tissue and the other components was analysed using 2-dimensional finite element analysis and the maximum von Mises stress in each reference area was compared. Under lateral loading, the stress was larger at the abutment/fixture interface, and in the crestal bone, compared to the stress pattern under vertical loading. The amount of stress at the superstructure was similar regardless of the length, diameter and platform shape of a fixture. Around the longer fixture, the stress was decreased at the bone crest and subjacent cancellous bone and increased in the cancellous bone area apical to the fixture. Around the wider fixture, the stress was decreased at the abutment/fixture interface, and the bone crest and increased in the cancellous bone area apical to the fixture. Around the fixture having wider platform, less stress was produced at the abutment/fixture interface and the upper part of the cortical bone, compared to the fixture having standard platform. In conclusion, the stress distribution of the supporting tissue was affected by length, diameter, and platform shape of a fixture, and the fixture which was larger in diameter and length could reduce the stress in the supporting tissues at the bone-fixture interface and bone crest area.

임플란트를 이용한 국소의치 수복 (Implants in conjunction with removable partial denture)

  • The benefits of implant supported overdenture are readily apparent for the fully edentulous patients and have been well documented, however, there is deficiency of the studies regarding the combination of implants with removable partial dentures for partially edentulous patients. The purpose of this article is to review the literature concerning implants with removable partial dentures and evaluate the evidence for this clinical approach. Through many clinical case reports and studies we have searched from a broad variety of journals, we present the six considerations needed to contemplate respecting implants with removable partial denture in partially edentulous patients. First, the connection between abutment tooth and removable partial denture has to be rigid and the link between implant and removable partial denture should be hinged. Second, a mesial rest acts better in the point of force distribution for distal extension removable partial denture and splinting between implants is also a favorable choice. Third, T bar has an advantage for implants which are used as abutments in distal extension removable partial denture. Forth, as we all known functional impression is better way to reproduce movement for distal extension removable partial denture. Fifth, indirect retainer and guiding plane on the proximal surfaces of terminal abutment teeth are important in preventing denture base lifting. Sixth, implants in conjunction with removable partial denture is superior in the esthetic and phonetic as well as cost-effective point of view. We also suggest that which place we should install implants for force distribution and which diameter and length of implants should be used. in this review article, we recommend to locate the implant near of the abutment tooth for esthetics or near of first molar position for good stress distribution. The diameter and length of implant also influence to stress distribution. When we compare to conservative partial denture, patients go for removable partial denture using implants due to convenience, better support and retention according to several studies. But it is true that we need to study more on this subject and collect long term follow up cases before we discuss on it. So it is enough to bring this subject into the surface of prosthetic treatment by this article.
