생쥐의 유선발달에 있어서 Elvax 40P Implant를 이용한 서방형 Adenosine의 역할 (Effect of Sustained-Release of Adenosine by Elvax 40P Implant on Mouse Mammary Development)

  • To examine local response of adenosie(purine nucleoside) on the developing mammary gland, Elvax 40P implants containing adenosine were surgically implanted into mammary fat pad of the five week old female ICR mice. Inguinal(the 4th) mammary glands of anesthetized mice were exposed andplaced the implants for 12 days. One gland was treated with an adenosine implant, while the contralateral gland received a blank implant as control. For whole-mount preparations, glands were stained with alum carmine, and for histological observation, micro-selected mammary glands were stained with hematoxylin and eosin Y. Implantation with Elvax 40P did not affect on the damage of neighboring mammary tissue. Adenosie 25 or 250$\mu\textrm{g}$ per slow-release implant stimulated local mammary end bud formation of ovariectomized mice such as end bud size and numbers of end bud per gland in a dose dependent manner(P<0.05), and lower concentration of adenosie(2.5 or 25$\mu\textrm{g}$/implant) increased numbers of end bud(P<0.05) and end bud size(P<0.1) of intact mice. Adenosine treatment and intact ovarian function had moderate interation effects on the stimulation of end bud formation at 2.5$\mu\textrm{g}$ adenosine/implant(P<0.1). In histological observation, adenosine implants increased numbers of mammary epithelial type of cells at mammary duct in the presence or absece ofovary. These results indicate that adenosine should be one of regulators in mouse mammary ductal growth.

  • The choice of suitable hip implant is one of important factors in the total hip arthroplasty (THA). In clinical view point, an improper adaptation of hip implant might induce abnormal stress distribution to the bone, which can shorten the lifespan of replaced hip implant. Currently, interest in the custom-designed hip implants has increased as studies reveal the significance of geometric shape of patient's femur in modeling and designing the implants. In this study, we have developed custom-designed hip implant models with various sizes, and analyzed the stress distribution in the bone and bone cement using the Finite Element Method. It was found that minimizing the gap between implant stem and femoral cavity is crucial to minimize the stress concentration in the bone.

  • Statement of problem: Standard type of ITI solid implant model in the 6.2mm thick jaw bone was axisymmetrically modelled for finite element stress analyses. Purpose: Primary objective was to investigate the influences or the characteristic design configuration of the ITI solid implant model on the bone stress with the course of osseointegration process at the bone/implant interfaces. To simulate the characteristics of the osseointegration process, five different stages of the bone/implant interface model were implemented. As load conditions, vertical load of 50N was taken into consideration. Bone at the cervical region of implant was the areas of concern where the higher level of stress were likely to take place. Results: The results indicated that rather slightly different stress level could be obtained as a function of the osseointegration conditions. Conclusion: Under vertical load, the lower level of stress was observed at the cervical cortical bone in the initial and final stages of osseointegration. Relatively higher stress level, however, was observed during the transitional stages where the osseointegration at the cancellous bone interface were yet to fully develop.

Evaluation of reliability of zirconia materials to be used in implant-retained restoration on the atrophic bone of the posterior maxilla: A finite element study

  • PURPOSE. Zirconia materials have been used for implant-retained restorations, but the stress distribution of zirconia is not entirely clear. The aim of this study is to evaluate the stress distribution and risky areas caused by the different design of zirconia restorations on the atrophic bone of the posterior maxilla. MATERIALS AND METHODS. An edentulous D4-type bone model was prepared from radiography of an atrophic posterior maxilla. Monolithic zirconia and zirconia-fused porcelain implant-retained restorations were designed as splinted or non-splinted. 300-N occlusal forces were applied obliquely. Stress analyses were performed using a 3D FEA program. RESULTS. According to stress analysis, the bone between the 1) molar implant and the 2) premolar in the non-splinted monolithic zirconia restoration model was stated as the riskiest area. Similarly, the maximum von Mises stress value was detected on the bone of the non-splinted monolithic zirconia models. CONCLUSION. Splinting of implant-retained restorations can be more critical for monolithic zirconia than zirconia fused to porcelain for the longevity of the bone.

교정력이 골유착성 티타니움 임프란트의 초기 고정에 미치는 영향에 관한 실험적 연구 (The effect of early loading on the direct bone-to-implant surface contact of the orthodontic osseointegrated titanium implant)

  • 본 연구는 경희대학교 치과대학 교정과와 Biosstech$^{\circledR)$이 개발한 교정용 골유착성 티타니움 임프란트를 이용하여 임프란트 매식 후 가해지는 교정력이 임프란트의 골유착에 미치는 영향과 임프란트 제거 후 손상된 치조골의 치유를 알아보기 위해 시행되었다. acid-etched type, sand-blasted type, sand-blasted & acid-etched type, 3종류의 교정용 임프란트 24개를 2마리 실험 동물의 구강 내에 식립하였다. dog1의 상하악 좌측에 식립한 6개 임프란트에는 식립 후 교정력을 주지 않았고, 우측에 식립한 6개 임프란트에는 식립 즉시 200-300gm의 교정력을 가하였다. dog2에서는 임프란트 식립후 4주의 치유기간을 기다린 후, 골 내에 고정되어 있는 모든 임프란트에 4주 동안 200-300gm의 교정력을 가하였다. 4주, 8주의 관찰 기간이 지난 후 실험 동물을 각각 희생시켜 임프란트와 주위 조직을 포함하는 비탈회 표본을 제작하여 광학 현미경하에서 검경하였다. 이 실험을 통해 다음과 같은 결과를 얻었다. 1. 골내 고정에 성공한 임프란트들의 식립 4주 후 조직 소견에서, 식립 즉시 교정력을 가한 임프란트와 교정력을 가하지 않은 임프란트 사이에 조직학적 차이가 없었다. 두 군 모두에서 미성숙 소주골이 임프란트 식립시 손상된 부위로 생성되고 있었으며 부분적인 골유착이 보였다. 2. 임프란트 식립 부위의 골손상이 완전히 치유되기 전에 가해진 교정력은 골유착의 진행을 방해하지 않았다. 4주의 치유기간 후 4주간 교정력을 가한 임프란트의 조직소견에서 골조직과 임프란트 계면 사이의 접촉은 더 넓은 부위로 진행되었고 나사골을 채우고 있는 신생골은 기저골의 구조와 형태를 닮은 층판골로 성숙되어 있었다. 3. 식립 후 임프란트 주위 연조직 염증이 심했던 임프란트는 대부분 탈락되거나 동요가 있었으며, 실험 동물 희생시 동요가 있었던 임프란트의 조직 소견에서 임프란트 주위는 연조직으로 둘러 쌓여 골조직과 분리되어 있었다. 4. 골내 고정에 실패한 임프란트의 식립 부위에서는 임프란트 제거 2주 후 염증 소견없이 왕성한 골형성을 나타내는 조직학적 소견이 관찰되었다.

치과용 임플란트 구조설계 (1): 구조해석 비교연구 (Structural Design of a Dental Implant (I): Comparative Structural Analysis)

  • 본 논문은 두 편으로 구성된 치과용 임플란트 구조설계에 대한 논문 중 첫 번째 논문으로 먼저 치과용 임플란트의 종류 및 특징에 대하여 조사 분석을 수행하였다. 이를 통하여 현재 사용 중인 대표적인 치과용 임플란트들의 장단점들을 비교분석하였다. 그 결과를 토대로 새로운 임플란트 구조모델을 제안하였다. 특히 제안된 새로운 구조형상의 임플란트에 대하여 기존의 대표적인 임플란트들과 유한요소해석 기법을 이용한 구조해석 비교연구를 수행하였다. 구조해석 비교연구를 수행한 결과 본 논문에서 제안한 치과용 임플란트의 구조적 성능의 우수성이 확인되었다. 본 연구에서 구조해석 비교연구를 수행한 치과용 임플란트 제품은 3i 임플란트 제품과 Sargon 임플란트 제품이다. 이들은 모두 수입 제품들인데 임플란트 종류 및 특징 조사 결과 현재 임상으로 사용 중인 임플란트들은 대부분 골내식립형인데 이들도 그들 중 하나임을 알 수 있었다.

The cumulative survival rate of sandblasted, large-grit, acid-etched dental implants: a retrospective analysis

  • Purpose: This retrospective study aimed to assess the long-term cumulative survival rate of titanium, sandblasted, large-grit, acid-etched implants over a 10-year follow-up period and investigate the factors affecting the survival rate and change in marginal bone loss (MBL). Methods: The study included 400 patients who underwent dental implant placement at the Department of Periodontology of Seoul National University Dental Hospital (SNUDH) between 2005 and 2015. Panoramic radiographic images and dental records of patients were collected and examined using Kaplan-Meier analysis, Cox proportional hazards regression analysis, and multiple regression analysis to determine the survival rates and identify any factors related to implant failure and MBL. Results: A total of 782 implants were placed with a follow-up period ranging from 0 to 16 years (mean: 8.21±3.75 years). Overall, 25 implants were lost, resulting in a cumulative survival rate of 96.8%. Comparisons of the research variables regarding cumulative survival rate mostly yielded insignificant results. The mean mesial and distal MBLs were 1.85±2.31 mm and 1.59±2.03 mm, respectively. Factors influencing these values included age, diabetes mellitus (DM), jaw location, implant diameter, bone augmentation surgery, and prosthetic unit. Conclusions: This study found that the implant survival rates at SNUDH fell within the acceptable published criteria. The patients' sex, age, DM status, implant location, implant design, implant size, surgical type, bone augmentation, and prosthetic unit had no discernible influence on long-term implant survival. Sandblasted, large-grit, acid-etched implants might offer advantages in terms of implant longevity and consistent clinical outcomes.

임플랜트 나사선 경사각이 치조골 응력 분포에 미치는 영향 (THREE-DIMENTIONAL FINITE ELEMENT ANALYSIS OF STRESS DISTRIBUTION FOR DIFFERENT IMPLANT THREAD SLOPE)

  • Statement of problem: The screws of dental implant, having various thread types, can be categorized into different classes by their geometrical form, and each type transmits dissimilar amount and form of stress to alveolar bone. Purpose: The purpose of this study was to find an inclination angle of the screw thread that is favorable in distributing the stresses to alveolar bone. Material and methods: In this study, We used three dimensional finite element analysis with modeling having three types of thread inclination angles and fixed pitch-0.8 mm (single thread type with $3.8^{\circ}$ inclination, double thread type with $7.7^{\circ}$ inclination, triple thread type with $11.5^{\circ}$ inclination). Results: The results obtained from this study were as follows; 1. When the number of thread increased, the amount of Von-Mises stress was reduced since the generated stress was effectively distributed. 2. Since the maximum principal stress affects on the alveolar bone can influence deeply on the longevity of the implants when comparing the magnitude of the maximum principal stress double thread had least amount of stress. This shows that the double thread screw gave best result. Conclusion: In conclusion, double, and triple thread screws were found to be more effective on distribution of the stress than the single thread screws. But, increasing in the thread inclination angle such as triple thread screw relate on the magnitude of the maximum principal stress affecting on the alveolar bone can become problematic. Thus, effective combination of thread number and thread inclination angle can help prolonging the longevity of implant.

개방형과 폐쇄형 임플랜트 매식후 주위골 형성에 관한 실험적 연구 (A STUDY ON THE BONE FORMATION OF OPEN TYPE AND CLOSED TYPE IMPLANTS)

  • A two-stage procedure is ideal for getting a successful osseointegration. But if a one-stage procedure can achieve a similar osseointegration, the one-stage procedure has several advantages. The purpose of this study was to observe the initial bone formation and bone remodeling of open type (nonsubmerged) and closed type (submerged) titanium implants. Eight ITI hollow-screws and eight Branemark fixtures were divided into two groups (submerged and nonsubmerged) and were installed on the lower jaws of four mongrel dogs. The animals were sacrificed three months later and bone sections with implants were processed for light microscopic and fluorescent microscopic observation. The results were as follows : 1 There was no significant difference in bone-to-implant contact between submerged and nonsubmerged implants. 2. Smooth surface titanium implants showed more bone-to-implant contact than that of titanium plasma coated implants histologically. 3. Under fluorescent microscopy, the active bone remodeling and new bone formation were observed in the interface zone. 4. Under fluorescent microscopy, submerged and nonsubmerged implants had no difference in bone remodeling pattern, and intramembranous bone formation was more prominent. 5. The connective tissue fibers orienting perpendicularly toward implant surface were oberved in the neck of implants.

자연감 있는 임플란트 보철물 (Naturalization of Implant Prostheses)

  • 임플란트 술식을 이용하여 소실된 구강조직을 수복하는 방법은 보철적 수복 술식중 중요한 부분을 차지하고 있다. 구강수복의 목적은 손상된 구강조직의 기능성, 심미, 발음 등을 회복하는 데 있다. 최근 환자, 치과의사, 기공사들의 심미성에 대한 요구가 증가하는 추세이므로 임플란트 보철수복의 자연미에 대한 관심 또한 고조되고 있다. 최종 보철물의 심미적인 결과에는 형태, 색조, 비율, 잇몸의 형태 등등의 많은 요소가 관여한다. 자연감과 생동감이 있는 인공보철물을 환자에게 제공해 주기 위하여 주의 깊은 진단 및 치료계획과 이를 뒷받침하는 기공사의 역할이 필수적이다. 자연감 있게 보이는 임플란트 보철물을 제공하기 위하여 재료, 잇몸의 재현, 형태 및 발음, 보철물의 design 등에 관하여 screw type, cement type, 그리고 overdentuure 등의 몇 가지 증례를 통하여 살펴보고자 한다.
