Prospective Clinical Trial of Survival Rate for Two Different Implant Surfaces Using the Osstem(R) SS II Non-submerged Implant System in Partially Edentulous Patients

  • Objective : This study sought to investigate the clinical survival rate of two implants with different surfaces: resorbable blasting media (RBM)-treated and calcium metaphosphate (CMP)-coated implant. Study design : SSII non-submerged implants (Osstem, Seoul, Korea) were placed in a total of 48 patients with mean age of 38.8. At least 31 patients in the experimental group had a CMP-coated implant, and 1 patient in the control group received a, RBM surface implant. The evaluation period was between April 2006 and December 2007. Radiographs, periotest, clinical periodontal examination, and prosthetic adjustment and occlusion were used. Results : The survival rate of the experimental and control groups after 1 year was 97.2% and 100%, respectively. The Wald confidence interval reported for the experimental group was not inferior to the control group. Conclusion : No significant differences were found between the RBM and CMP groups. The observed data suggest that CMP-coated methods can provide favorable clinical results for the functioning and healing of dental implants.

7-mm-long dental implants: retrospective clinical outcomes in medically compromised patients

  • Objectives: Dental implants shorter than 8 mm, called short dental implants (SDIs), have been considered to have a lower success rate than standard length implants. But recent studies have shown that SDIs have a comparable success rate, and implant diameter was more important for implant survival than implant length. Also, SDIs have many advantages, such as no need for sinus lifting or vertical bone grafting, which may limit use in medically compromised patients. Materials and Methods: In this study, 33 patients with 47 implants 7-mm long were examined over the last four years. All patients had special medical history and were categorized into 3 groups: systemic disorders, such as diabetes mellitus (controlled or uncontrolled), mental disability, and uncontrolled hypertension; oral cancer ablation with reconstruction, with or without radiotherapy; diverse osteomyelitis, such as osteoradionecrosis and bisphosphonate-related osteonecrosis of the jaw. Most of these patients have insufficient residual bone quality due to mandible atrophy or sinus pneumatization. Results: The implant diameters were 4.0 (n=38), 4.5 (n=8), and 5.0 mm (n=1). Among the 47 implants placed, 2 implants failed before the last followup. The survival rate of 7-mm SDIs was 95.74% from stage I surgery to the last follow-up. Survival rates did not differ according to implant diameter. The mean marginal bone loss (MBL) at 3 months, 1 and 2 years was significantly higher than at implant installation, and the MBL at 1 year was also significantly higher than at 3 months. MBL at 1 and 2 years did not differ significantly. Conclusion: Within the limitations of the present study, the results indicate that SDIs provide a reliable treatment, especially for medically compromised patients, to avoid sinus lifting or vertical bone grafting. Further, long-term follow-up is needed.

Clinical and radiographic evaluations of implants as surveyed crowns for Class I removable partial dentures: A retrospective study

  • PURPOSE. The purpose of this study was to evaluate survival rates and marginal bone loss (MBL) of implants in IC-RPDs. MATERIALS AND METHODS. Seventy implants were placed and used as surveyed crowns in 30 RPDs. The survival rates and MBL around implants based on multiple variables, e.g., position, sex, age, opposing dentitions, splinting, type of used retainer, and first year bone loss, were analyzed. Patient reported outcome measures (PROMs) regarding functional/esthetic improvement after IC-RPD treatment, and complications were also inspected. RESULTS. The 100% implant survival rates were observed, and 60 of those implants showed MBL levels less than 1.5 mm. No significant differences in MBL of implants were observed between implant positions (maxilla vs. mandible; P = .341) and type of used retainers (P = .630). The implant MBL of greater than 0.5 mm at 1 year showed significantly higher MBL after that (P < .001). Splinted implant surveyed crowns showed lower MBL in the maxilla (splinted vs. non-splinted; P = .037). There were significant esthetic/functional improvements observed after treatment, but there were no significant differences in esthetic results based on implant position (maxilla vs. mandible). Implants in mandible showed significantly greater improvement in function than implants in the maxilla (P = .002). Prosthetic complication of IC-RPD was not observed frequently. However, 2 abutment teeth among 60 were failed. The bone loss of abutment teeth was lower than MBL of implants in IC-RPDs (P = .001). CONCLUSION. Class I RPD connected to residual teeth and strategically positioned implants as surveyed crowns can be a viable treatment modality.

임프란트 지지 국소의치의 임상적 연구 (Clinical study of implant supported removable partial dentures)

  • OBJECTIVE The purpose of clinical study was to evaluate the survival of dental implants used in restoring patients with implant supported removable partial dentures (ISRPDs) of different configurations. MATERIALS AND METHODS The sample consisted of 20 consecutively treated partially edentulous patients, who, between 2003 and 2008, had a total of 84implants placed in different arch sites and who were treated with ISRPDs. The mean age was 57 years. Mean follow up time from delivery of ISRPDs was 3 years 2 months (range, 1 to 6 years). Osseointegration failure, postoperative complication of dental prosthesis, and the success rate of ISRPDs were retrospectively evaluated using clinical and radiographic examination. RESULT The overall implant survival rate was 100%. During follow up, the one clasp of removable partial denture was broken in 1 year11 months after prosthesis delivery. All patients were satisfied with their prosthesis. CONCLUSIONS Implant supported removable partial dentures could serve as favorable prognosis. Careful patient selection, with an appropriate maintenance and recall system, is recommended to obtain satisfactory results.

보철전단계의 치과 임프란트 실패 (PREPROSTHETIC STAGE DENTAL IMPLANT FAILURE)

  • Since the introduction of the concept of osseointegration, the success rate of dental implant has increased dramatically. So, the uses of dental implant in the treatment of partially or fully edentulous patients have played an important role in dental rehabilitation. Regardless of high success rate of dental implant, some amounts of fixtures cannot help failing. We can classify dental implant failure according to timing, causative factor, etc. This study is focused on dental implant fixture failure, occurring during preprosthetic stage. There are various reasons that cause implant failure on this periods, such as improper patient selection, poor bone quality, and periimplantitis, etc. We investigate the survival rate of 1058 fixtures, which installed in 306 patients in our clinic from January 1997 to December 1999, according to type, sex, location, fixture length and width, using Kaplan-Meier product-limit method and to compare each other with log-rank test. Overall survival rate was 96.80%, and 33 implants failed over the preprosthetic stage. Our survey data identified posterior location of mandible as being associated with implant failure(P<0.05).

Cranioplasty Using Autologous Bone versus Porous Polyethylene versus Custom-Made Titanium Mesh : A Retrospective Review of 108 Patients

  • Objective : The purpose of this study was to compare the cosmetic outcome and complications after cranioplasty (CP) due to three different implant materials, and analyze the mean implant survival and cumulative survival rate based on these results. Methods : We reviewed 108 patients retrospectively who underwent CP between January 2014 and November 2016. Autologous bone (AB; 45 patients) and synthetic materials with porous polyethylene (PP; 32 patients) and custom-made 3-dimensional printed titanium mesh (CT; 31 patients) were used as implants. Results : Regardless of implanted materials, more than 89.8% of the CP patients were satisfied with the cosmetic outcome. No statistically significant difference was observed among the three groups. The overall postoperative complication rates of each group were 31.1% in the AB group, 15.6% in the PP group and 3.2% in the CT group. The CT group showed lower complication rates compared with AB and PP groups (${\chi}^2$-test : AB vs. PP, p=0.34; AB vs. CT, p=0.00; PP vs. CT, p=0.03). The AB and PP groups demonstrated a higher post-CP infection rate (11.1% and 6.3%) than the CT group (3.2%). However, no significant difference in the incidence of post-CP infection was observed among the three groups. The PP and CT groups demonstrated a higher mean implant survival time and cumulative survival rate than the AB group at the last follow-up (p<0.05). Conclusion : In comparison with AB and PP, cranioplasty with CT shows benefits in terms of lower post-CP complication, less intraoperative bleeding loss, shorter operation time and in-hospital stay. The PP and CT groups showed higher implant survival time and cumulative survival rate compared with the AB group.

7년 이상 기능한 임플란트의 변연골 흡수와 생존율에 영향을 주는 요인 (Factors associated with the survival rate and the marginal bone loss of dental implant over 7-years loading)

  • 목적: 본 연구는 7년 이상 기능한 임플란트의 생존율과 평균 변연골 수준에 영향을 미치는 요인을 분석하고자 하였다. 연구 재료 및 방법: 92명의 환자에서 178개의 임플란트를 대상으로 하였다. 임상적 및 방사선학적 검사를 통해 임플란트 관련 요인(임플란트 직경, 임플란트 길이, 상부 보철물 고정 유무), 환자 관련 요인(성별, 흡연, 치태지수, 유지 치주치료 순응도) 및 수술 관련 요인(술자 숙련도, 골재생술 시행 유무)을 조사하였다. 구내 표준 방사선 촬영 이 후 각 임플란트의 근심 및 원심 변연골 수준은 이미지 분석 소프트웨어 프로그램을 사용하여 측정하였다. 결과: 임플란트의 생존율은 94.94%였고, 평균 변연골 흡수는 $0.89{\pm}1.05mm$였다. 임플란트 길이와 치태지수는 임플란트 생존율과 통계적으로 유의하였다(P < 0.05). 흡연과 골재생술 시행 유무는 변연골 흡수와 통계적으로 유의하였다(P < 0.05). 결론: 본 연구에서 7년 이상 기능한 임플란트는 양호한 생존율과 변연골 수준을 보였다. 임플란트의 장기적인 유지를 위해서는 임플란트의 길이, 치태조절에 유의하며 변연골 수준의 유지를 위해서는 골재생술의 신중한 적용, 흡연의 조절이 필요하다.

좁은 직경 임플란트 3년간의 생존율에 관한 후향적 연구 (A 3 year prospective study of survival rate of narrow diameter implants)

  • 목적: 3.6 mm 이하 좁은 직경 임플란트의 일정 기간 생존율을 초기고정 값과 방사선학적 계측을 통해 분석해보고자 하였다. 연구 재료 및 방법: 24명의 환자에 식립된 직경 3.6 mm 이하, 길이 7 mm 이상의 38개의 임플란트를 조사하였다. 식립된 임플란트의 platform 직경, body 직경, 길이, 식립 부위 등을 확인하였고 식립 시 골유도재생술의 동반 시행 여부, 완성된 보철의 형태를 확인하였다. 초기고정 값은 임플란트 식립 직후 측정한 implant stability quotient (ISQ) 값을 확인하였고, 방사선 사진을 통해 식립된 임플란트의 근 원심 marginal bone level (MBL)을 측정하였다. 결과: 전체 임플란트의 생존율은 92.11% 이었고 ISQ값은 평균 66.26으로 나타났다. 생존한 임플란트의 MBL 변화는 평균 $0.14{\pm}0.31mm$를 나타내었다. Platform 직경이 body 직경보다 큰 임플란트 중 실패한 임플란트는 없었다. 결론: 좁은 직경의 임플란트는 좁은 직경의 치조제에 안정적인 치료 결과를 나타낸다 사료된다.

짧은 임플란트의 생존율과 변연골 흡수량에 관한 임상적 연구 (Clinical Study on the Survival Rate and Marginal Bone Resorption of Short Implants)

  • 짧은 임플란트는 상악동이나 하치조신경 등의 해부학적 구조물이 있거나 심한 치조골 흡수로 인해 제한적인 치조제 높이를 가지는 부위에서 사용되고 있다. 본 연구는 길이 10 mm 이하의 임플란트에서 임플란트의 길이, 직경, 식립 부위, 골이식술 유무, 보철물의 연결고정 유무가 임플란트의 생존율과 변연골 흡수에 미치는 영향을 알아보고자 하였다. 원광대학교 치과병원 임플란트센터에서 길이 10 mm 이하의 임플란트를 식립한 137명의 환자, 227개 임플란트를 대상으로 진료 기록부를 통해 임플란트의 길이, 직경, 식립 위치, 골이식 유무, 보철물의 연결고정 유무를 조사하였다. 변연골 흡수량은 Emago advanced v5.6(Oral diagnostic systems, Amsterdam, The Netherlands) 프로그램을 이용하여 측정하였다. 총 227개의 임플란트 중 8개가 실패하여, 전체 짧은 임플란트의 생존율은 96.5 %로 나타났다. 골이식 부위와 상악에 식립된 경우 더 높은 실패율을 보이는 경향이 있었으며, 임플란트의 길이와 직경은 변연골 흡수량에 영향을 미치지 않았다. 실패 요인을 조사하였을 때, 상악의 불량한 골질과 골이식 유무가 임플란트의 더 높은 실패율에 영향을 미쳤다. 10 mm 이하 임플란트에서 길이, 직경, 식립 부위, 골이식술과 보철물의 연결 고정은 임플란트 생존율과 변연골 흡수량에 영향을 끼치지 않았다.
