Impedance Audiometry in Children (학동기 아동의 Impedance Audiometry에 대한 연구)

  • Since Metz had employed the impedance audiometry in 1946, scholars have carried out many investigation. Brook, Jeger, Cooper reported and evaluated the clinical studies of impedance audiometry and its screening test. Recently, in Korea, a studies of impedance audiometry have been reported. We analysed 100 children aged between 7-16 years old who visited OPD of E.N.T. department with the complaints of the nasal obstruction and hard of hearing from Nov. 77-Feb. 79. Through the use of otoscope and impedance audiometry, we evaluated the types of tympanogram, static compliance and the acoustic reflex. This paper is dealing with the statistical study of impedance audiometric result and its literature.

  • Since the clinical application of Impedance measurment by Metz in 1946. Many scholars carried out investigations. Brooks reported on the clinical studies of Impedance on 1053 children, Jerger evaluated on 398 children less than 6 years old, and Cooper studied on the abbreviated screening technic for school children. In Korea, Seoul National University Medical School reported on the experimental studies of Impedance with Madsen acoustic bridge (20~70) on 1976, and we reported on the clinical evaluation on 1023 school children aged between 7~14 with Teledyne Impedance bridge on 1976. This paper is dealt with the studies of Impedance bridge as 110 of children less than 6 years.

  • Harford has pointed out that the application of impedance audiometry in the Eustachian tube function is of special value. As you know, with excessive negative pressure in the middle ear cavity with intact eardrum tympanograms reveal indirect evidence of Eustachian tube insufficiency. With normal Eustachian tube function eardrum should be pushed laterally by Valsalva maneuver, resulting in a temporary decrease in the compliance of the system, indicated by a swing of the balance meter needle. Therefore the authors measured the swing of the balance meter needle by the Valsalva maneuver in persons with intact eardrums, intact middle ear cavities, and intact Eustachian tube function. The results are as follows; The swing of the eardrum by Valsalva maneuver is 1.0 to 4.0 (mean $1.6{\pm}0.06$) and we conclude that the persons under 1.0 of the swing is suggestive of some disfunctions of the Eustachian tube.

It is now widely used in clinical audiology and it plays an important role in otology. Recently there was the literature stated on normal value of various test c Impedance. This paper is dealing with the clinical evaluation of type c in comparison with following subjects as; 1. Comparison with the otoscopic finding. 2. Correlation with the pure tone audiometry. 3. Correlation with the stapedial reflex. 4. Correlation between pathologic negative pressure range and middle ear fluid.

The Effect of Workplace Noise on the Hearing Threshold of Dental Technicians

  • This study was conducted to investigate hypoacusis due to workplace noise among dental technicians. Pure tone audiometry, impedance audiometry and heart rate variability were measured for 60 dental technicians and office workers in D city from July to November 2013, and a survey on the effects of noise was conducted. SPSS 19.0 was used for statistical analysis. In the result for threshold of pure tone audiometry, the average hearing threshold of left ears in the air conduction (AC) test showed a statistically significant decrease (P=0.019) among dental technicians, who are frequently exposed to workplace noise. The hearing threshold in the AC test for each frequency was significantly different between the two groups at 125 Hz (P=0.012) in right ears and at 1 kHz (P=0.022), 2 kHz (P=0.040), and 8 kHz (P=0.018) in left ears. Dental technicians who had worked for 16 years or longer had a significantly higher incidence of hypoacusis in the right ear (P=0.030) and in the left ear (P=0.010). In impedance audiometry showed a tympanometry result of type A in both the dental technician group and the office worker group.

  • There are varying opinions about theories that the middle ear of Neonates is filled with mucous or other Material that interference with normal mobility of the middle ear system. DeDMon reported on the acoustic reflex as a tool for Neonatal screening, and Keith investigated the Middle ear function of Neonates during first few hours of life with an Electroacoustic impedance bridge. There are no reports on Neonstes in Impedance Audiometry in Korea. We tested 100 Neonates less than 24 hours old and this paper is dealt with its study related with tympanogram, static compliance and acoustic reflex.

  • 만성 상악동염은 동점막의 복잡한 변화를 일으키고, 만성증으로 지속되는 비분비물 및 비점막의 염증이 이관에 파급되어 중이에 영향을 미치며 그 치유경과를 늦추게 한다. 저자는 만성 상악동염에서 동점막의 비후정도가 이관 및 중이에 미치는 영향을 알아 보고자, 1986년 1월부터 3월까지 이화여자대학교 의과대학 부속병원 이비인후과에 비폐색 및 비루를 주소로하여 내원한 환자중 만성 상악동염으로 진단된 16세 이상의 성인 환자를 대상으로 검사하였다. 동점막 비후정도에 따라 비정상군을 경도(2~3mm), 중등도(4mm이상), 고도(상악동 전체혼탁) 3군으로나누어 각각 40예를 조사군으로 하고 동점막의 두께가 0~1mm인 경우를 정상대조군으로 하여 40예를 GSI 28 AUTO TYMP를 사용하여 screening audiometry, tympanometry, acoustic reflex threshold (ipsilateral and contralateral)를 시행하여 다음과 같은 결과를 얻었다. 1) 남녀의 비는 1 : 2.6이고 21세에서 30세군이 53예(33.0%)로 가장 많았다. 2) 증상발현기간은 6개월 이내가 42.5%로 가장 많았다. 3) Tympanogram의 분포에서 비정상적 type으로 나오는 비율이 정상군과 비정상군중에서 경도군이 각각 22.5%이었고 중등도군이 30.0%, 고도군이 50.0% 순이었다. 4) 등골근반사역치는 정상군과 비정상군 사이에 차이가 없었다. 5) 등골근무반사율 역시 정상군과 비정상군 사이에 차이가 없었다.

  • 삼출성 중이염은 소아연령군의 이비인후과질환중 유병율이 가장 높은 질환의 하나로 1867년 Politzer가 최초로 기술하고 치료법을 소개한 이후 그 병인 및 치료법이 다양하게 연구 개발되어 왔으며 1950년대 및 1960년대 electroacoustic impedance unit의 발전에 따라 진단법에 많은 변혁이 이루어졌다. 연자들은 1981년 1월1일 부터 1986년 12월31일까지 삼출성 중이염으로 중이내 통기관유치술 및 고막천자를 시행한 15세 이하의 290명의 환자를 대상으로 tympanogram의 양상 및 술전 기골도차를 조사하여 중이내 삼출액의 유무 및 점성도의 예측도에 대하여 살펴 보았다. Tympanometric peak pressure (T. P. P.) 가 -50mm$H_2$O 미만이며 compliance가 0.4 c.c.미만인 영역을 B영역으로. T.P.P.가 -50mm$H_2$O 미만이며 compliance가 0.4 c.c. 이상인 영역을 C영역으로 정하였을 때 중이내 삼출액 유무의 예측도는 B영역이 87.5%. C영역은 81.48%였으며 삼출액이 장액성일 경우의 예측도는 B영역이 24.51%, C영역이 22.72%이었고, 삼출액이 점액성일 경우의 예측도는 B영역이 70.39%, C영역이 54.54%이었다. 연령을 15세 - 7세군과 6세이하의 군으로 세분하였을 경우에도 비슷한 결과를 얻었다.

Experimental and clinical studies with impedance audiometry; the increase in air volume in the middle ear air system and the pneumatization of human temporal bones (측두골의 함기도와 중이강의 용적이 고막 임피던스에 미치는 영향에 관한 연구)

  • The vibratory energy introduced into the external ear canal is changed by the mechanical factors of eardrum itself, the motility of ossicles, and the air cushion of tympanic cavity and the like. This study was designed to investigate the volume of middle ear cavity and mastoid air cell system as a factor of determining the accoustic impedance of middle ear system. The author studied how the increase in air volume of middle ear cavity effects on the acoustic impedance of middle ear system with dogs' ears and researched the correlation between the degree of pneumatization of temporal bones and the acoustic impedance of middle ear system by comparing the radiological findings of pneumatization (Law's and Towne's projection) with the acoustic impedance measurements with Madsen ZO 70. The result is as follows: 1 The tympanometric findings in control state revealed the curves of type A, and did not change in its configuration by the increase in the air volume of dogs middle ear system. 2. The static compliance of middle ear revealed a distinct and linear increase in proportion to the increase in air volume of middle ear system; the rate of increase was $0.05{\pm}0.02$ cc of static compliance per cc of air volume. 3. Authenticated in the above result and the tendency to increase in static compliance in proportion to the increase in the degree of pneumatization of temporal bones, there was significant regression equation between the degree of pneumatization of temporal bones (x variable) and the static compliance of middle ear system; $y=0.19x{\pm}0.16{\pm}0.05$ It is suggested that the difference in volume of middle ear system plays an important role in the change of the static compliance of middle ear, and the author concludes that the measurement of static compliance of middle ear has clinical value as diagnostic means of evaluating the degree of pneumatization of temporal bones along with some radiological examination.

  • Among the various audiometric test batteries, pure tone and speech audiometry was carried as routine. During the past 20 years, there have been works on speech word list for Korean language. As a preliminary work for standardization of Korean speech word list, the authors are intended to study on the factors of test methods with these proposal speech word lists. Now, authors has studied as followings methods and materials and the results were obtained as followings. A. Materials and Methods a. Word List 1. Paik Chun Kee (Paik) 2. Soh Jin Myoung (Soh) 3. Cynn Kyu Shik (Cynn) 4. Lee Jong Dam (Lee) 5. Hahm Tae Young (Hahm) b. Test Method 1. ascending 2. descending c. Test Material 1. live voice 2. Recorded male voice 3. Recorded female voice B. Results 1. Score of ascending method was better result than descending method. 2. Speech reception threshold and discrimination score were better in recorded female voice, live voice and recorded male voice in that order. 3. Speech reception thresholds among tested word lists were Hahm, Lee, Soh, Cynn and Paik in that order. 4. Discrimination score was highest with Cynn and Lee, Soh, Hahm, Paik in that order.
