내당능장애 가능성이 있는 중년 남성의 신체적 특성, 영양소 섭취상태 및 혈중 지질농도 양상 (The Nutritional Status of Middle Aged Korean Men Exhibiting Impaired Glucose Tolerance and Their Blood Lipid Profile)

  • The objective of this study was to investigate nutritional status of middle aged Korean men exhibiting impaired glucose tolerance (IGT) and identify the risk factors related to IGT Data were collected from 163 men with a fasting blood glucose level from 115 to 139mg/dl(high blood glucose group: HBG) and 170 men with a normal fasting blood glucose level(control) aged from 40 to 59 years in both groups. Weight, body mass index(BMI) and percent body fat were significantly higher in high blood glucose(HBG) group than those of control group. Age, weight, BMI, percent body fat were positively related to blood glucose. There were no differences in exercise, smoking and family history of diabetes between two groups. Frequency of fat eating and overeating of HBG were higher than those of control group but frequency of sweet snacks intake of HBG was lower than that of control group. There was no difference in daily total energy intake in two groups. Total and supper energy intakes were positively associated with blood glucose. Percent energy intake of alcohol was significantly higher in HBG group and positively related to blood glucose, however there were no difference in daily intake of nutrients in two groups. Alcohol intake was positively related to BMI, but after adjusting BMI, there was no correlation between alcohol intake and blood glucose. Serum total cholesterol and triglyceride were significantly higher in HBG group than those of control group. Serum total cholesterol i,nd triglyceride were positively related to blood glucose and high density lipoprotein cholesterol was negatively associated with blood glucose. After adjusting BMI, serum triglyceride was positively related to blood glucose. In conclusion, weight, BMI, percent body fat and blood total cholesterol, low density lipoprotein cholesterol and triglyceride levels were positively related to blood glucose level of middle aged Korean men exhibiting impaired glucose tolerance. Their eating habits exhibited higher frequency of overeating, fast eating, high energy intakes of supper. (Korean J Nutrition 33(1) : 59-67, 2000)

성인의 공복혈당 수치에 따른 치주건강의 관련성 (Relationship between Impaired Fasting Glucose and Periodontal Health among Adults)

  • 이 연구는 국가자료인 2009년도 국민건강영양조사 자료를 이용하여 19세 이상의 성인의 혈당조절 수치에 따른 치주상태의 관련성을 평가하고자 하였다. 분석 시 공복 시 혈당이 100 mg/dl 미만이면 정상군, 100-125 mg/dl인 경우 공복혈당장애군, 126 mg/dl 이상일 경우 당뇨군으로 분류하여 치주상태를 평가하였다. 연구대상자 7,062명 중 치주질환을 가진 대상자는 정상군 27%, 공복혈당장애군 41.2%, 당뇨군 43.6%로 나타났다. 혼란변수을 보정한 결과 공복혈당장애군과 당뇨군은 정상군에 비해 치주질환 위험비가 각각 1.35, 1.27으로 나타났다(p<0.05). 이상의 결론을 종합하였을 때 당뇨를 가진 사람 뿐만 아니라 공복혈당 수치가 높으면 치주질환을 야기하는 위험인자로 작용할 수 있다는 사실을 인지하고 정기적인 치과 방문을 통해 구강건강을 유지해야 할 것이다.

  • Plasma glucose levels before and after oral glucose administration have been compared in a group of 76 thyrotoxic subjects and a group of 8 normal control subjects in order to study the effect of glucose loading in thyrotoxicosis. Following were the results: 1. The mean fasting plasma glucose level was elevated in the thyrotoxic group(95.5mg%) compared to normal control group (88mg%). 2. The peak of glucose tolerance curve is at 30 minutes after glucose administration in both groups, but its mean value was 44mg% higher in thyrotoxic group than in control group. 3. The plasma glucose levels returned towards the fasting level in the later stage of the test more rapidly in thyrotoxic group than in control group. 4. 69.6% of oral glucose tolerance tests were impaired in the thyrotoxic group, and the occurance of abnormal glucose tolerance could be related to the degree of thyrotoxicity, sex and age. 5. The mechanisms of the impaired glucose tolerance in thyrotoxicosis are thought to be related to an increased rate of glucose absorption from gastrointestinal tract, abnormal liver function with decreased hepatic glycogenesis, increased glucose oxidation, decreased pancreatic release of insulin, and genetic relationship between diabetes and thyrotoxicosis.

공복혈당장애 및 당뇨병과 치주질환의 관련성 (Relationship among impaired fasting glucose and diabetes and periodontal disease)

  • 본 연구에서는 공복혈당장애 및 당뇨병과 치주질환의 관련성을 알아보고자 하였다. 본 연구는 국민건강영양조사 제5기(2010년, 2012년) 원시자료를 이용하였으며, 만 19세 이상 성인 총 10,856명을 최종 연구대상자로 선정하였다. 치주질환 유병여부를 종속변수로 한 로지스틱 회귀분석에서는 모든 변수가 보정된 상태에서 성별, 연령, 교육수준, 흡연 및 당뇨병상태가 통계적으로 유의하게 관련이 있는 것으로 나타났다(p<0.001). 본 연구결과 당뇨병의 전단계인 공복혈당장애가 치주질환의 확실한 위험요인으로 보이지는 않지만, 전당뇨병 환자는 당뇨병을 예방하거나 지연시키는 노력을 하지 않으면, 10년 내에 당뇨병으로 발전될 가능성이 높으므로 전당뇨병 단계에서부터 치주질환의 지속적인 검사와 관리가 필요하다고 할 수 있다.

내당성장애군에서의 당뇨병 발생률과 그 위험요소에 관한 연구 (A Study on the Incidence of Diabetes and Its Risk Factors in the Impaired Glucose Tolerence)

  • This is to study the incidence of Diabetes and its risk factors in the impaired glucose tolerence group. 1084 subjects who had been examined 3 times for regular check-up in one human dock center under the university hospital were studied between $1986{\sim}1993$. The results are as follows; Prevalence rate of diabetes was 3.56%, and that of impaired glucose tolerence was 30.90% as of initial examination. The rate of diabetes was increasing with the age where 1.2% at age group $30{\sim}39$, 3.21% at age group $40{\sim}49$, 5.84% at age group $50{\sim}59$, 14.28% at age group 60 and over. And also the rate of impaired glucose tolerance was increasing with the age where 21.29% at age group $30{\sim}39$, 31.42% at age group $40{\sim}49$, 38.91% at age group $50{\sim}59$, 33.33% at age group 60 and over Of the examinees who were initially examined, total incidence rate of diabetes who developed to obvious diabetes at the 3rd follow-up examination from the initially impaired glucose tolerence group was 4.63% and it was 11.3 times higher than from the normal group (0.41%). After controlling for the effects of both age and obesity, the risk of subsequent diabetes for subjects with impaired glucose tolerance remained significantly higher than for normal subjects (Relative Risk, 10.48). Test for trends for developing to diabetes by the increasing level of fasting blood sugar and 1hr blood sugar at the initial examination were statistically significant in either normal and impaired glucose tolerence group. To determine the risk factor for developing to diabetes, logistic regression test was applied. Only fasting blood sugar and 1hr blood sugar were predictors for developing diabetes from the impaired glucose tolerence group at initial examination.

Relationship between job stress and impaired fasting glucose in male steel industry workers: a cross-sectional study

  • Background: The purpose of this study is to investigate the relationship between job stress and impaired fasting glycemia (IFG) of male workers in a manufacturing industry. Methods: Data were collected from 5,886 male workers in a manufacturing industry who participated in the medical examination from June 19 to August 14, 2020 through self-reported questionnaires. The general characteristics of the subjects, shift work, high blood pressure, dyslipidemia, and job stress were included. Job stress was measured using the Korean Occupational Stress Scale (KOSS) consisting of 8 items and 43 questions. Multivariable logistic regression analysis was used to investigate the IFG association with job stress. Results: Among the various factors that can cause job stress, only high job demand was associated with a risk of IFG (odds ratio, 1.43; 95% confidence interval, 1.13-1.82) especially in non-shift worker. For all other factors, no statistically significant results were obtained. Conclusions: In this study of male workers engaged in the Korean steel manufacturing industry, the 'job demand' item among job stress of non-shift worker was related to IFG.

혈당 수준에 따른 영양섭취 및 음식섭취 상태 (Nutrients and Dish Intake by Fasting Blood Glucose Level)

  • The nutrient intake and association between dish group intake and blood glucose and serum lipid level (TG, cholesterol, LDL and HDL) was analyzed among 3 groups: 452 subjects in normal blood glucose group (NG: fasting blood glucose < 100 mg/dL and 2 hours postprandial blood glucose < 140 mg/dL), 258 subjects in impaired fasting glucose group (IFG: fasting blood glucose 100~125 mg/dL and 2 hours postprandial blood glucose ${\geq}$ 140 mg/dL) and 101 subjects in diabetic group (DG: fasting blood glucose $\geq$ 126 mg/dL and 2 hours postprandial blood glucose ${\geq}$ 140 mg/dL). The data were obtained from the 2005 National Health and Nutrition Survey of Korea. The 811 subjects were adults aged 40~64 without dietary treatment. In nutrients intake, IFG was the highest and DG the lowest in both quantity and quality. DG, especially, had the lowest intake in carbohydrates, fiber, proteins, Ca, P, K, vitamins B1 and C, and consumed the highest amount of alcohol. In macronutrients distribution ratio, the DG diet showed a lower energy intake from carbohydrates but higher from fat than the NG diet, while IFG showed a higher energy intake from carbohydrates and lower intake from fat in supper out of 3 meals and snacks. IFG preferred salt-fermented foods and DG preferred soups, braised foods and kimchi compared to other groups. NG preferred multi-grain cooked rice and both IFG and DG preferred plain white cooked rice. Regarding the association between dish group intake and blood glucose, cooked rice, soups, salt-fermented foods and kimchi were significantly related to blood glucose. In blood lipids, steamed-foods, beverages and fruits were inversely related to the risk of developing type 2 diabetes, whereas cooked rice, stews, saltfer-mented foods, seasoned-fermented foods and seasoned vegetables were directly proportional to the risk of developing type 2 diabetes and related diseases. Therefore, it is beneficial to avoid rich, salty and fatty foods and heavy alcohol consumption for controlling blood glucose and blood lipids, while steamed foods, foods rich in fiber (like multigrain rice) as a staple, and fruits and teas are recommended for preventing or managing type 2 diabetes risks.

당뇨와 지역사회치주지수의 관련성: 제5기 국민건강영양조사 (The association between diabetes mellitus and community periodontal index: The 5th Korean National Health and Nutrition examination survey)

  • Objectives: The purpose of the study was to examine the association between diabetes mellitus and community periodontal index in Korean adults. Methods: The study populations were recruited by the Fifth Korea National Health and Nutrition Examination Survey. Study subjects were 10,411 who were examined oral examination, blood test, and aged over 19 years. Using multiple logistic regression analyses, the variables were adjusted for gender, age, household income, family history of diabetes, body mess index, smoking habit, and frequency of tooth brushing. Periodontal tissue examination of the subjects was performed and scored by Community periodontal index(CPI). Using probe, six teeth were examined for hemorrhage, plaque, and pocket depth and classified into $CPI_0$, $CPI_1$, $CPI_2$, $CPI_3$ and $CPI_4$. Healthy periodontal groups($CPI_{0-2}$) and periodontal disease groups($CPI_{3-4}$) were divided by the periodontal disease status. The definition of diabetes mellitus(DM) was decided by the diagnosis by the doctors and fasting blood sugar level. Those who were diagnosed as DM were included in DM group. The DM variables included normal blood sugar level, increased fasting blood sugar level, and DM blood sugar level. The DM variables were compared to periodontal disease blood sugar level and analyzed. Results: The periodontitis prevalence rate was 23.2%. Those who had diabetes mellitus accounted for 5.5% of the subjects. Those who had impaired fasting glucose accounted for 17.7% and 7.9% of subjects were diabetes mellitus by blood test. In the confirmed diabetes group by doctor, the periodontitis prevalence rate was significantly higher than the non-diabetic group. Diabetic group by blood test had the highest prevalence rate of periodontitis than those who had impaired fasting glucose group or normal group. After adjusting for gender, age, household income, family history of diabetes, body mass index, smoking habit, and frequency of tooth brushing, the risk of periodontitis in diagnosed diabetes mellitus was 1.57 times(95% CI; 1.27-1.94) higher than the normal group. In impaired fasting glucose group and diabetes mellitus group by blood test, the risk of periodontitis was 1.11 times(95% CI; 0.95-1.30) and 1.45 times(95% CI; 1.45-2.12) higher, respectively. Conclusions: There was a significant relationship between diabetes mellitus and periodontitis in Korean adults. These results suggest that diabetes mellitus is a risk factor for periodontitis.

금식 여부에 따른 혈당치 분포와 당뇨병 선별을 위한 혈당조절지표의 평가 (The Assessment of Blood Glucose Distribution according to the Fasting State and Glycemic Control Indicators for Diabetes Screening)

  • 본 연구는 금식 여부에 따른 혈당치 분포 차이를 분석하였고 공복 혈당치와 혈당조절지표인 당화혈색소, 프록토사민, 1,5-AG의 상관성을 분석하여 전당뇨병 선별에 필요한 추가 정보를 제공하고자 하였다. 단국대학교병원에 내원한 707명의 외래 환자를 금식 군과 비금식 군으로 나누어 혈당 검사를 시행한 후 각 군의 혈당치 평균을 산출하고 성별, 연령별 및 임상과 별 결과치 분포를 분석을 하였다. 또한 건강검진을 목적으로 내원한 금식 상태의 153명에 대해 혈당치와 당화혈색소, 프록토사민, 1,5-AG을 측정하여 혈당치와 혈당조절지표의 상관성을 평가하였다. 비금식 군의 혈당 평균치는 111.9 mg/dL, 금식 군은 103.6 mg/dL로서 유의한 차이가 있었고 (p<0.05) 평균치 차이는 여자가 4.8 mg/dL로서 남자의 12.2 mg/dL보다 적었다. 연령대에 따른 중앙값의 차이는 비금식 군에서만 유의한 차이가 있었고(Kruskal-Wallis test, p<0.01) 금식 군에서는 유의하지 않았다. 공복 시 포도당 장애 선별 구간에서 혈당 농도와 상관성이 높은 검사 종목은 1,5-AG로 평가되었다. 저자들은 금식 여부에 따른 혈당치 분포의 차이를 외래 환자를 대상으로 분석하였으며, 건강검진 대상자의 결과치 분석을 통해 1,5-AG가 프록토사민, 당화혈색소보다 공복 혈당치와 유의한 상관성이 있는 것으로 평가하였다. 당뇨병 선별 시 혈당 측정과 함께 단기 혈당 조절능력을 반영하는 지표가 활용될 수 있을 것으로 사료된다.

만성 이첨판 폐쇄부전증 개에서 인슐린 분비기능 감소 (Decreased Insulin Secretion in Dogs with Chronic Mitral Valve Insufficiency)

  • 최근 사람연구에서는 심부전과 관련하여 발생하는 인슐린 저항성이나 공복혈당 이상과 같은 혈당 대사 이상 중요한 예후인자로 받아들여지고 있다. 그러나 심부전이 있는 개에서 이와 관련된 연구는 매우 드물다. 따라서 본 연구는 혈당대사이상이 이첨판 폐쇄 부전증이 있는 개에서도 나타날 것이라고 가정하였다. 총 113마리의 보호자가 있는 개를 대상으로 혈중 insulin, glucagon, fructosamine, glucose 를 측정하였으며, 인슐린 저항성은 homeostatic model assessment (HOMA) score를 이용하였다. 실험결과 혈중 인슐린 농도는 심장병이 심해짐에 따라서 유의성 있게 감소함을 확인하였다. 반면, fructosamine, HOMA score, and 공복 혈당은 심부전의 심각도와 어떠한 상관성도 보이지 않았다. 인슐린 농도, fructosamine, HOMA 의 경우 body condition scores (BCS)와 양의 유의적 상관관계를 보였으나, 혈당의 경우 그러지 않았다. 심부전과 BCS와의 음의 유의적 상관관계 또한 확인 되었다. 본 연구를 바탕으로 자연적으로 발생한 이첨판 폐쇄부전증에 따른 심부전 환자에서 심장 병이 심해짐에 따라서 인슐린 분비 기능이 감소함을 확인하였다.
