Modification and Test of Self-help Program for Patients having Fibromyalgia-Pilot Study- (섬유조직염환자용 자조관리과정의 수정 및 검증 -예비조사-)

  • The purposes of this study are 1) to modify a self-help program for patients having fibromyalgia, 2) to revise instruments to measure the self-efficacy and impact of fibromyalgia; and 3) to test the effects of the program in terms of self-efficacy, physical activities, and clinical symptoms. The subjects of this study were persons diagnosed as fibromyalgia at a university hospital. For the first two purposes, six patients were selected. Twenty three subjects for the third purpose were selected and assigned to experimental and control group on the base of their residence. Thirteen were assigned to the experimental group, while 10 to the control group. Instruments selected for revision were self-efficacy scale and fibromyalgia impact profile. On the basis of the text developed by Arthritis Foundation, a program for patients with fibromyalgia was tentatively developed. Instruments for measurement of self-efficacy and impact of fibromyalgia were revised to show high reliabilities. Results obtained by utilization of the program were as follows : 1) There were no increase of self-efficacy and physical activities. 2) There were no improvement of the number of tender points and pain. 3) There were improvement of depression, fatigue and sleep impairment. These findings indicate the needs for modification of the program with emphasis of exercise for relaxation of tender points, increases of enactive action and vicarious learning.

Impact of coronavirus disease 2019 on patients with chronic pain: multicenter study in Korea

  • Background: The coronavirus disease 2019 (COVID-19) pandemic has caused significant changes. This study aimed to investigate the impact of COVID-19 on patients with chronic pain. Methods: Patients with chronic pain from 23 university hospitals in South Korea participated in this study. The anonymous survey questionnaire consisted of 25 questions regarding the following: demographic data, diagnosis, hospital visit frequency, exercise duration, time outside, sleep duration, weight change, nervousness and anxiety, depression, interest or pleasure, fatigue, daily life difficulties, and self-harm thoughts. Depression severity was evaluated using the Patient Health Questionnaire-9 (PHQ-9). Logistic regression analysis was used to investigate the relationship between increased pain and patient factors. Results: A total of 914 patients completed the survey, 35.9% of whom had decreased their number of visits to the hospital, mostly due to COVID-19. The pain level of 200 patients has worsened since the COVID-19 outbreak, which was more prominent in complex regional pain syndrome (CRPS). Noticeable post-COVID-19 changes such as exercise duration, time spent outside, sleep patterns, mood, and weight affected patients with chronic pain. Depression severity was more significant in patients with CRPS. The total PHQ-9 average score of patients with CRPS was 15.5, corresponding to major depressive orders. The patients' decreased exercise duration, decreased sleep duration, and increased depression were significantly associated with increased pain. Conclusions: COVID-19 has caused several changes in patients with chronic pain. During the pandemic, decreased exercise and sleep duration and increased depression were associated with patients' increasing pain.

Impact of a Cryotherapy and Tai Chi Self-help Program on Women with Fibromyalgia Syndrome (섬유근통 자조관리 프로그램이 여성 섬유근통 증후군에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate the impact of a protocol of cryotherapy and Tai Chi on women with Fibrolmyalgia Syndrome. Methods: The study design was a pretest and posttest quasi-experimental, nonrandom assignment. The subjects were 48 outpatients with a diagnosis of fibromyalgia based on the American College of Rheumatology Standards. The setting was a University Medical Center in S city. The protocol was self administered ten times a week, for a two week period resulting in twenty treatments. Results: A protocol of cryotherapy and Tai Chi self-help program showed significant improvement in the fibromyalgia impact as measured by reports of physical impairment, feeing good, number of missed work days, ability to perform work, reports of pain, fatigue, rest, stiffness, anxiety and depression as reported by the comparison group (p<.001). No significant differences between the two regarding in reports of the tender points (p=.062). Conclusion: The results suggested that a protocol of cryotherapy and Tai Chi self-help program may be a promising multidisciplinary strategy for fibromyalgia patients.

The Solution of Peening Residual Stress by Angled Impact of Multi Elliptical Shot Ball Based on Finite Element Analysis (유한요소해석에 기초한 다중 타원구 숏볼의 경사충돌에 의해 생성된 피닝잔류응력해)

  • Shot peening is widely used to improve the fatigue life and strength of various mechanical parts and an accurate method is important for the prediction of the compressive residual stress caused by this process. A finite element (FE) model with an elliptical multi-shot is suggested for random-angled impacts. Solutions for compressive residual stress using this model and a normal random vertical-impact one with a spherical multi-shot are obtained and compared. The elliptical multi-shot experimental solution is closer to an X-ray diffraction (XRD) than the spherical one. The FE model's peening coverage also almost reaches the experimental one. The effectiveness of the model based on an elliptical shot ball is confirmed by these results and it can be used instead of previous FE models to evaluate the compressive residual stress produced on the surface of metal by shot peening in various industries.

  • The driving methods of car are front wheel drive, rear wheel drive and four wheel drive. At driving methods, constant velocity joint is the most important part at carrying out two functions for converting to the direction which the driver wants and transferring the power to wheels. At driving on the road, the impact can be applied to the parts transmitting power according to the state of road surface. In this study, each models of three constant velocity joints whose shaft length are different respectively were modelled with CATIA and the structural and fatigue analyses were carried out by using ANSYS. This study result is thought to be the useful material at designing the constant velocity joint with the durability against impact. And it is possible to be grafted onto the convergence technique at the design of constant velocity joint and show the esthetic sense.

  • In this paper, finite element analysis and real time monitoring experimental work using FBG sensor were carried out for analyzing structural integrity of a Type IV hydrogen pressure vessel under impact loading condition. By using finite element analysis with the ply based modeling technique, sensor insertion points and pressure condition were suggested. Tensile test with an angle ply specimen was conducted for getting the reliability of FBG sensor insertion method. After fabricating the vessel, total five times pressurization fatigue tests were conducted (Non-impact pressurization: 1, After impact pressurization: 4). Experimental results revealed that filling cycle time was gradually increased and filling gradient was decreased when the vessel experienced impact.

Analysis of Mechanical Property Changes of Polymer Eyeglass Frames by Thermal Impact (고분자 안경테의 온도에 의한 기계적 물성 변화 분석)

  • Purpose: To analyze thermal effect on mechanical properties of domestic commercial polymer-based eyewear frames. Methods: In this study, materials of cellulose acetate, polyamide, epoxy, and polyetherimide were exposed to high or low temperature and were mounted on universal test machine (TO-100-IC) for tensile strength test. Elastic behavior, Young's modulus, maximum displacement, and fatigue were tested with various temperature ($-25^{\circ}C$, $25^{\circ}C$, $60^{\circ}C$). Results: As a result, at room temperature, displacements of materials were changed with increasing impact load. At low temperature ($-25^{\circ}C$), maximum displacements of all specimens were decreased but young's modulus were increased. However, at high temperature, maximum displacements of all specimens were increased but young's modulus were decreased. Conclusions: Degree of displacements due to fatigue behavior was increased following direction of PEI, epoxy, polyamide, acetate. We concluded that commercial polymers used in eyewear frames physical properties were changed differently to exposed temperature.

Reliability of Maintained Hull Girders of Two Bulk Carrier Designs Subjected to Fatigue and Corrosion

  • The objective of the paper is to study the impact of changing the traditional hull design of bulk carriers by providing them with a double hull while keeping the same deadweight. It is demonstrated that by introducing the double hull the structural reliability is increased throughout the entire life and also the extend of the needed repair is reduced. The results are obtained with recently developed mathematical tools for the reliability assessment of ship hulls subjected to the existence of multiple cracks both in the stiffeners and in the plating and it models the crack growth process. The effect of corrosion is represented as time dependent. The long-term stress range acting on the elements is defined as a function of the local transverse pressure of the internal cargo and outside sea water combined with the stresses resulting from the longitudinal bending of the hull, which is a combined with the stresses resulting from the longitudinal bending of the hull, which is a combineation of horizontal and vertical bending moments. The effect of maintenance actions is modelled as a stochastic process. The results show that a different design of the midship section improves the structural safety and also the economy with respect to structural repair of bulk carriers.

Effect of a Prolonged-run-induced Fatigue on the Ground Reaction Force Components (오래 달리기로 인한 피로가 지면반력 성분에 미치는 영향)

  • The purpose of this study was to estimate the potential injury via analyzing ground reaction force components that were resulted from a prolonged-run-induced fatigue. For the present study, passive and active components of the vertical ground reaction force were determined from time and frequency domain. Shear components of GRF also were calculated from time and frequency domain. Twenty subjects with rear foot contact aged 20 to 30, no experience in injuries of the extremities, were requested to run on the instrumented tread-mill for 160 minutes at their preference running speed. GRF signals for 10 strides were collected at 5, 35, 65, 95, 125, and 155 minute during running. In conclusions, there were no significant difference in the magnitude of passive force, impact load rate, frequency of the passive and active components in vertical GRF between running times except the magnitude of active force (p<.05). The magnitude of active force was significantly decreased after 125 minute run. The magnitude of maximum peak and maximum frequency of the mediolateral GRF at heel strike and toe-off have not been changed with increasing running time. The time up to the maximum peak of the anteroposterior at heel-strike moment tend to decrease (p<.05), but the maximum peak and frequency of that at heel and toe-off moment didn't depend significantly on running time.

Study on Convergence Technique through Structural Analysis on the Axle of Railway Vehicle (철도 차량의 축에 대한 구조 해석을 통한 융합 기술연구)

  • As the axle at the vehicle of railway has the important role for safe running, the strength, and impact-proof, safety factor, stress and deformation must be considered. There are the simulation models of 1 and 2 in this study. These models are investigated by performing the convergence technique through the design, the structural and fatigue analyses with CATIA and ANSYS. As the maximum deformation and equivalent stress of model A are lower than those of model B, model A has more durability than model B. The durability to prevent the damage can be investigated by applying the result of this study into the part design of the vehicle of rail road. And it is possible to be grafted onto the convergence technique at design and show the esthetic sense.
