임상적용을 위한 전자선의 선량분포 특성에 대한 고찰 (A Consideration on the Characteristics of Electron Beam Dose Distributions for Clinical Applications)

  • High energy electron beams were to concentrically dose inside a tumor and more energy is a shape decreased of dose. Therefore, it is useful to radiation therapy of a tumor. Also high energy electron beams ionized into collision with a atom in structure material of tissue and it has big changes to dose distribution by multiple scattering. The study had to establish characteristic of electron beams from interaction of electron beams and materials. Experiment method was to measure dependence of electron beam central axis for depth dose curve, field flatness and symmetry and field size dependence. The results were able to evaluate data for a datum pint of electron beam. Also radiotherapy has to be considered for not only energy pencil of lines but characteristic, electron guide and isodose curves distribution.

Tube Voltage, DNA Double-Strand Breaks, and Image Quality in Coronary CT Angiography

  • Objective: To evaluate the effects of tube voltage on image quality in coronary CT angiography (CCTA), the estimated radiation dose, and DNA double-strand breaks (DSBs) in peripheral blood lymphocytes to optimize the use of CCTA in the era of low radiation doses. Materials and Methods: This study included 240 patients who were divided into 2 groups according to the DNA DSB analysis methods, i.e., immunofluorescence microscopy and flow cytometry. Each group was subdivided into 4 subgroups: those receiving CCTA only with different tube voltages of 120, 100, 80, or 70 kVp. Objective and subjective image quality was evaluated by analysis of variance. Radiation dosages were also recorded and compared. Results: There was no significant difference in demographic characteristics between the 2 groups and 4 subgroups in each group (all p > 0.05). As tube voltage decreased, both image quality and radiation dose decreased gradually and significantly. After CCTA, γ-H2AX foci and mean fluorescence intensity in the 120-, 100-, 80-, and 70-kVp groups increased by 0.14, 0.09, 0.07, and 0.06 foci per cell and 21.26, 9.13, 8.10, and 7.13 (all p < 0.05), respectively. The increase in the DNA DSB level in the 120-kVp group was higher than those in the other 3 groups (all p < 0.05), while there was no significant difference in the DSBs levels among these latter groups (all p > 0.05). Conclusion: The 100-kVp tube voltage may be optimal for CCTA when weighing DNA DSBs against the estimated radiation dose and image quality, with further reductions in tube voltage being unnecessary for CCTA.

Geant4-DICOM Interface-based Monte Carlo Simulation to Assess Dose Distributions inside the Human Body during X-Ray Irradiation

  • This study uses digital imaging and communications in medicine (DICOM) files acquired after CT scan to obtain the absorbed dose distribution inside the body by using the patient's actual anatomical data; uses geometry and tracking (Geant)4 as a way to obtain the accurate absorbed dose distribution inside the body. This method is easier to establish the radioprotection plan through estimating the absorbed dose distribution inside the body compared to the evaluation of absorbed dose using thermo-luminescence dosimeter (TLD) with inferior reliability and accuracy because many variables act on result values with respect to the evaluation of the patient's absorbed dose distribution in diagnostic imaging and the evaluation of absorbed dose using phantom; can contribute to improving reliability accuracy and reproducibility; it makes significance in that it can implement the actual patient's absorbed dose distribution, not just mere estimation using mathematical phantom or humanoid phantom. When comparing the absorbed dose in polymethly methacrylate (PMMA) phantom measured in metal oxide semiconductor field effect transistor (MOSFET) dosimeter for verification of Geant4 and the result of Geant4 simulation, there was $0.46{\pm}4.69%$ ($15{\times}15cm^2$), and $-0.75{\pm}5.19%$ ($20{\times}20cm^2$) difference according to the depth. This study, through the simulation by means of Geant4, suggests a new way to calculate the actual dose of radiation exposure of patients through DICOM interface.

가임기 여성의 유방촬영에서 Dose mode 적용의 유용성 분석 (The Usefulness Analysis of Applying Dose Mode in Mammography Women of Childbearing Age)

  • Purpose : Automatic exposure system(AOP mode) in DR Mammography of the STD and DOSE apply to women of childbearing age, the average glandular dose(AGD) and image quality by comparing was to demonstrate the usefulness of DOSE. Materials and Methods : Of the under 40 age 108 patients who visit to our hospital and examined STD and DOSE mammography from January 2008 to July 2013. AGD was obtained by DICOM header information provided by GE Senographe DS. STD and DOSE images were evaluated with obtained patients, Image J program was compared by calculating the SNR. Results : The average AGD of DOSE mode was 0.99mGy, and which decreased by 19% comparing to that of STD mode which was 1.18mGy. The two the average AGDs indicated statistically significant difference(p< .01). The average SNR of STD was 40.26, DOSE displayed, and to 39.68 in, there was no statistical significance. Results : The average AGD using DOSE mode which is one of DR mammography AOP modes decreased by comparing to that of STD mode showing no difference in image quality. The use of DOSE is considered to be useful.

영상유도방사선치료에 있어 영상선량 가이드라인 (Guideline for Imaging Dose on Image-Guided Radiation Therapy)

  • 방사선 치료 시 보다 정확한 환자자세 및 종양위치 확인을 위해 다양한 형태의 방사선영상장치들이 사용되면서 이에 따른 환자 피폭 관리의 필요성이 증대되고 있다. 진단영상의학 분야에서는 의료방사선 이용의 급격한 증가로 인해 이에 의한 2차 암발생률 증가에 대한 보고들이 사회적인 반향을 일으켰고, 투시촬영 및 CT 등에 의한 과다 피폭 사례가 밝혀지면서 이를 막기 위해 image gently, image wisely 캠페인이 수년전부터 미국을 중심으로 확산되고 있다. 반면에 방사선 종양학 분야에서는 방사선치료로 받는 선량에 비해 영상선량은 무시할 수준이어서 아직까지 이에 대한 관심이 상대적으로 작은 게 사실이다. 하지만 암의 조기 발견, 방사선치료 성적의 향상 등으로 환자의 기대수명이 증대되고 있고, 특히 소아의 경우 상대적으로 높은 방사선 민감도 및 기대 수명을 고려할 때 방사선장해방어를 위해 ALARA (As Low As Reasonably Achievable) 원칙에 입각하여 영상유도 방사선치료에 수반되는 영상선량의 적절한 관리가 필요하다고 사료된다. 하지만 영상유도기법으로 인해 방사선치료의 정확도를 높이고 고 선량이 피폭되는 치료범위를 더 작게 할 수도 있기 때문에 단순한 최소화가 아닌 최적화가 이루어져야 하겠다. 이러한 맥락에서 본 가이드라인에서는 (1) 영상유도기술 및 수반되는 영상선량에 대해 정리하고, (2) 영상유도 장비 및 이용실태에 대한 국내 현황을 파악, (3) 적절한 영상유도를 위한 최적화 방안들을 모색하여 권고안을 제시하고자 한다.

  • Radiation therapy is an important part of cancer treatment in which cancer patients are treated using high-energy radiation such as x-rays, gamma rays, electrons, protons, and neutrons. Currently, about half of all cancer patients receive radiation treatment during their whole cancer care process. The goal of radiation therapy is to deliver the necessary radiation dose to cancer cells while minimizing dose to surrounding normal tissues. Success of radiation therapy highly relies on how accurately 1) identifies the target and 2) aim radiation beam to the target. Both tasks are strongly dependent of imaging technology and many imaging modalities have been applied for radiation therapy such as CT (Computed Tomography), MRI (Magnetic Resonant Image), and PET (Positron Emission Tomogaphy). Recently, many researchers have given significant amount of effort to develop and improve imaging techniques for radiation therapy to enhance the overall quality of patient care. For example, advances in medical imaging technology have initiated the development of the state of the art radiation therapy techniques such as intensity modulated radiation therapy (IMRT), gated radiation therapy, tomotherapy, and image guided radiation therapy (IGRT). Capability of determining the local tumor volume and location of the tumor has been significantly improved by applying single or multi-modality imaging fur static or dynamic target. The use of multi-modality imaging provides a more reliable tumor volume, eventually leading to a better definitive local control. Image registration technique is essential to fuse two different image modalities and has been In significant improvement. Imaging equipments and their common applications that are in active use and/or under development in radiation therapy are reviewed.

수술실 내 C-arm 장치의 산란선 분포에 대한 연구 (The Study on Scattered Ray by C-arm in Operation Room)

  • A radiation imaging system used in a surgery room is mainly using C-arm which is purposed to fluoroscope. C-arm is often use to watch an operation's accuracy and progress, but not only being bombed to this first beam but also affected to this scattered beam, so now we are look for the way to reduce bombed amount of doctor, nurses and radiological technologists. We measured the exposure dose in $0^{\circ}$ spot according to the distance to find out frequency distribution of scattered ray in an operation room and found the spot which has the same exposure dose from $30^{\circ}$ distance of all directions and wrote isodose curve. We analyzed the data and found out the sudden reduction of scattered ray according to the long direction also found out that scattered ray was not related to the directions. Operators must recognize the reduction of exposure dose. Because reducing scattered ray from all directions in an operation room is really difficult. So every operators must use shelters to reduce the exposure dose and notice the safety.

The Relationship of Radiation Dose and Image Quality According to the Condition of Chest PA

  • The purpose of this study is to compare the measurement result of radiation dose by using standard thoracic phantom and ionization chamber to advice proposal in the shooting condition of chest PA projection at hospitals recently. And to understand the change between radiation dose and resolution in different conditions. The period this study was from August 2010 to September 2010 and the subjects of the study was 3 general hospitals, 4 personal hospitals and 1 laboratory at the college. Finally we study with 6 DR, 1 CR, and 4 F/S equipments. Most hospitals met advice proposal, but some of the hospitals exceed advice dose from the result of our study. We can lower radiation dose about 25% when kVp is lowered about 20% in DR equipment. And we can lower radiation dose about 50% when mAs is lowered about 35%. The image quality was similar to the original in the study. Most hospitals which exceed advice dose were personal hospitals. The reason why it happened is that radiation dose for chest PA projection at personal hospitals is higher than general hospitals and the personal hospitals' equipments are older than general hospitals' equipments. We guess that patients' radiation dose of chest PA projection can be lowered from the result.

심장 전기생리학 검사 시 조건 변화에 따른 환자 피폭 선량 평가 (Evaluation of Patient Exposure Dose during Cardiac Electrophysiology Study under Various Conditions)

  • This study used a adult absorption dose phantom (CIRS model 701-G, USA) made of human equivalent material and the vascular imaging equipment Allura Xper FD 20 (Philips, Netherlands). Optically stimulated luminescent dosimeters (OSLD) were inserted into the anatomical positions corresponding to each organ, and the exposure dose was measured. Dose area product (DAP) and air kerma (AK) measured by the dose meter in the equipment were compared. Continuous imaging was performed at two angles for a total of 20 minutes, with a frame per seconds of 3.75 and 7.5 fps and an FOV of 42 cm, 37 cm, and 31 cm, respectively, under the conditions of fluoflavor I, II, and III, each selected for 5 repetitions. This study was found that selecting a lower fps was the most effective way to reduce patient exposure dose, and adjusting the fluoflavor was a good alternative method for reducing patient exposure dose at high fps. Therefore the method of condition change with the greatest dose reduction effect is to set the minimum FPS and can reduce patient exposure dose according to geometric conditions and fluoflavor characteristics.

디지털 진단방사선촬영에서 부가여과판 사용에 따른 피폭선량 감소와 화질 개선 (Reduction of Patient Dose Exposure and Improvement of Image Quality by Use of Additional Filtration in Digital Radiology)

  • This study is mainly for the reduction of exposure dose by using a heavy elements filter(Gd) in the digital radiology. They contained heavy elements filter of Gd and X-ray beam hardening filters such as Al and Cu. According to the results of experimental evaluation, X-ray property was not changed with variety of kVp in the case of the Gd filter. The surface dose and absorption dose were increased in order of Cu and Al. The contrast of image showed the higher value in order of Cu, Al and Gd. While the use of Gd has increased the numerical value of the CR image, and grayscale has decreased noise value of the DR image.
