Design and Implementation of Electronic Medical Record System Based on HL7-CDA for the Exchange of Clinical Information (임상 정보교환을 위한 HL7-CDA 기반의 전자의무기록 시스템의 설계 및 구현)

  • For the sharing and exchange of information between medical clinics, the clinical document has to be built on a standardized protocol such as a HL7-CDA. But it is difficult to exchange information between medical clinics because clinical document such as electronic medical record that include text and image, have different structure of document and type of expression. In this paper, we propose the electronic medical record system based on HL7-CDA that can share and exchange clinical information between medical institute. For this purpose, we have to design the schema of the clinical document architecture after we select the essential items of medical record and define templates. The proposed system can minimize integrating process and save parsing time when clinical information exchange and refer, by converting electronic medical record to base64 encoding scheme and integrate it in a XML document.

  • This paper proposes a design and realization for the semantic search system. The proposed model includes three Architecture Layers of a Semantic Search System ; (they are conceptually named as) the Knowledge Acquisition, the Knowledge Representation and the Knowledge Utilization. Each of these three Layers are designed to interactively work together, so as to maximize the users' information needs. The Knowledge Acquisition Layer includes index and storage of Semantic Metadata from various source of web contents(eg : text, image, multimedia and so on). The Knowledge Representation Layer includes the ontology schema and instance, through the process of semantic search by ontology based query expansion. Finally, the Knowledge Utilization Layer includes the users to search query intuitively, and get its results without the users'knowledge of semantic web language or ontology. So far as the design and the realization of the semantic search site is concerned, the proposedsemantic search system will offer useful implications to the researchers and practitioners so as to improve the research level to the commercial use.

The Semantic Structure and Grammaticalization of the French Preposition de (프랑스어 전치사 de의 의미 구조와 문법화)

  • This study is about the French preposition de. This preposition is considered to have no meaning in the French language, and to have only the grammatical function in its use. This study begins with doubts about such views regarding this preposition. We organize and classify general descriptions of the uses of this preposition. The uses are classified as 'expressing starting point,' 'giving characteristics' and 'performing grammatical functions' and this data is analyzed in terms of the associated cognitive grammar. In the case of expressing the starting point, it is assumed that the trajector moves from the starting point, to the destination point, in the domain of space and in the domain of time. Therefore, it is noted in this case that the preposition de has a dynamic concept. On the other hand, it is significant to understand that the preposition of the 'giving characteristics' defines an outline of a particular class by giving the class's characteristics. The preposition de has a static concept in this case. For the preposition de which introduces the infinitive, all of these dynamic and static concepts are faded and extremely grammaticalized.

Searching for a New Path to Research on Basic Theory of Korean Medicine: Metaphorical Understanding of Korean Medicine Theories and Terminologies (한의학 기초이론 연구와 한의학 이론, 용어의 은유적 이해)

  • This paper examines whether the conceptual metaphor theory, which has been recently treated as important research topic in the field of cognitive linguistics, can be a new method that can promote the modernization of basic Korean medicine (KM) theory. In addition, the significance and potential of this study are reviewed by looking at Chinese research cases that applied this theory to Traditional Chinese Medicine theories and terminologies. The results are summarized as follows. From the viewpoint of metaphoric cognition, KM is a medicine that attempts to understand the human body (microcosm) through nature (great universe) by metaphorically projecting human experiences of nature on the human body based on the thought of correspondence between nature and human (天人相應). The language system of KM is based on a metaphor that extends our experience of nature to the human body, and an abundance of metaphors can be seen throughout the language of KM. Understanding and interpreting KM theories and terminologies from a metaphorical point of view allow us to understand the nature of KM theoretical key terms more deeply than now. And this understanding can help define and describe KM theoretical key terms and promote the modernization of KM theory research. In addition, various image schema that plays an important role in the metaphorical expansion of physical experience can be used for modeling KM theory. Research of KM theories and terminologies from a metaphorical point of view can serve as a bridge between traditional KM theory and modernization research, opening a new path to the modernization of basic KM theory in difficult situations.

  • In the medical field where artificial intelligence technology is introduced, research related to clinical decision support system(CDSS) in relation to diagnosis and prediction is actively being conducted. In particular, medical imaging-based disease diagnosis area applied AI technologies at various products. However, medical imaging data consists of inconsistent data, and it is a reality that it takes considerable time to prepare and use it for research. This paper describes a one-stop AI learning platform for converting to medical image standard R_CDM(Radiology Common Data Model) and supporting AI algorithm development research based on the dataset. To this, the focus is on linking with the existing CDM(common data model) and model the system, including the schema of the medical imaging standard model and report information for multi-center research based on DICOM(Digital Imaging and Communications in Medicine) tag information. And also, we show the execution results based on generated datasets through the AI learning platform. As a proposed platform, it is expected to be used for various image-based artificial intelligence researches.

Automatic Generation of DB Images for Testing Enterprise Systems (전사적 응용시스템 테스트를 위한 DB이미지 생성에 관한 연구)

  • In general, testing DB applications is much more difficult than testing other types of software. The fact that the DB states as much as the input data influence and determine the procedures and results of program testing is one of the decisive reasons for the difficulties. In order to create and maintain proper DB states for testing, it not only takes a lot of time and efforts, but also requires extensive IT expertise and business knowledge. Despite the difficulties, there are not enough research and tools for the needed help. This article reports the result of research on automatic creation and maintenance of DB states for testing DB applications. As its core, this investigation develops an automation tool which collects relevant information from a variety of sources such as log, schema, tables and messages, combines collected information intelligently, and creates pre- and post-Images of database tables proper for application tests. The proposed procedures and tool are expected to be greatly helpful for overcoming inefficiencies and difficulties in not just unit and integration tests but including regression tests. Practically, the tool and procedures proposed in this research allows developers to improve their productivity by reducing time and effort required for creating and maintaining appropriate DB sates, and enhances the quality of DB applications since they are conducive to a wider variety of test cases and support regression tests. Academically, this research deepens our understanding and introduces new approach to testing enterprise systems by analyzing patterns of SQL usages and defining a grammar to express and process the patterns.

Radiation Absorbed Dose Calculation Using Planar Images after Ho-166-CHICO Therapy (Ho-166-CHICO 치료 후 평면 영상을 이용한 방사선 흡수선량의 계산)

  • Ho-l66 was produced by neutron reaction in a reactor at the Korea Atomic Energy Institute (Taejon, Korea). Ho-l66 emits a high energy beta particles with a maximum energy of 1.85 MeV and small proportion of gamma rays (80 keV). Therefore, the radiation absorbed dose estimation could be based on the in-vivo quantification of the activity in tumors from the gamma camera images. Approximately 1 mCi of Ho-l66 in solution was mixed into the flood phantom and planar scintigraphic images were acquired with and without patient interposed between the phantom and scintillation camera. Transmission factor over an area of interest was calculated from the ratio of counts in selected regions of the two images described above. A dual-head gamma camera(Multispect2, Siemens, Hoffman Estates, IL, USA) equipped with medium energy collimators was utilized for imaging(80 keV${\pm}$10%). Fifty-nine year old female patient with hepatoma was enrolled into the therapeutic protocol after the informed consent obtained. Thirty millicuries(110MBq) of Ho-166-CHICO was injected into the right hepatic arterial branch supplying hepatoma. When the injection was completed, anterior and posterior scintigraphic views of the chest and pelvic regions were obtained for 3 successive days. Regions of interest (ROIs) were drawn over the organs in both the anterior and posterior views. The activity in those ROIs was estimated from geometric mean, calibration factor and transmission factors. Absorbed dose was calculated using the Marinelli formula and Medical Internal Radiation Dose (MIRD) schema. Tumor dose of the patient treated with 1110 MBq(30 mCi) Ho-l66 was calculated to be 179.7 Gy. Dose distribution to normal liver, spleen, lung and bone was 9.1, 10.3, 3.9, 5.0 % of the tumor dose respectively. In conclusion, tumor dose and absorbed dose to surrounding structures were calculated by daily external imaging after the Ho-l66 therapy for hepatoma. In order to limit the thresholding dose to each surrounding organ, absorbed dose calculation provides useful information.
