청소년기 특발성 척추측만증 환자의 임상적 균형 평가지수와 균형 수행 모니터 측정값의 상관성 연구 (The study of correlations between clinical balance scales and balance performance monitor parameters in patients with adolescent idiopathic scoliosis)

  • Purpose : The purpose of this study was to investigate the correlations between clinical balance scales and Balance Performance Monitor parameters in patient with adolescent idiopathic scoliosis (AIS). Methods : Twenty AIS subjects (age, $14.26{\pm}1.93yrs$; height, $160.56{\pm}7.98cm$; weight, $47.54{\pm}6.94kg$)were participated in this study. Postural sway(mean balance, sway angle, sway area, sway path, maximal sway velocity) were were evaluated by balance performance monitor. Measurements for clinical balance scales were Functional reach test (both side), the Lateral reach test (both side) and One leg standing test (both legs). Results : The results were as follows. There were positive strong correlation between major curve direction and left-right sway angle, sway path, maximal sway velocity. There were negative strong correlation between the functional reach and left-right sway angle, sway area, sway path, maximal sway velocity. And the lateral reach were also showed negative strong correlation parameters of balance performance monitor. One leg standing were negatively correlated with left-right sway angle, sway path, maximal sway velocity. Conclusion : The clinical balance scales will be useful tools for balance measurements, and basic tools for clinical setting for patient with AIS.

  • The purpose of this study was to analyze the effects of body shape variation of the clinical characteristics of patients who suffered from Adolescent idiopathic scoliosis. The data were collected from the spine clinics who were all thirty healthy make and female, from April, 1995 to May, 1998. The results of study were as follows ; 1. Total patients statistics mean age is 19years and 21 female, 9 male. 2. We divided 30 patients into four types(I, II, III, IV) by Evaluation form. As the result, Lots of patients included type IV. 3. Body type(IV) : Deformity style of Rt. shoulder & Lt. Pelvis forward style. 4. Body type IV showed a specific character in their life style. When the right shoulder forwarded and left pelvis is ant. tilting. The patients moved always their Lt. lower extremity first in Rt. lower extremity support the weight of body. And moved always their Rt. upper extremity in Lt. upper extremity support the weight of body.

Effects of Spine Mobilization on Cobb's Angle and Respiratory Function in Patients with Adolescent Idiopathic Scoliosis

  • Background: Incorrect postures of adolescents caused by the use of smart devices have been noted as a factor causing spinal diseases. Objectives: To examine the effect of joint mobilization and stretching on Cobb's angle and respiratory function in adolescent idiopathic scoliosis (AIS). Design: Cluster-randomized controlled trial. Methods: A total of 22 subjects with AIS were enrolled. They were allocated to two groups: the joint mobilization (n=11) and the stretching (n=11). All interventions were conducted for 30 minutes, three times a week for six weeks. Outcome measures were the Cobb's angle and respiratory function. The Cobb's angle and respiratory function measured using the X-ray and Micro-Quark. Results: Joint mobilization group showed significant differences in Cobb's angle and respiratory function, but stretching group showed significant differences Cobb's angle. The differences in peak expiratory flow (PEF) between the two groups were significant. Conclusion: This study proved that joint mobilization is a more effective intervention for AIS to improve Cobb's angle and respiratory function, when compared to stretching.

흉부 유연성 운동이 척추측만증 환자의 폐활량과 흉곽 확장에 미치는 영향 (The Effects of Thoracic Flexibility Exercise on Vital Capacity and Chest Expansion in Patients With Idiopathic Scoliosis)

  • The objectives of this study were to investigate the effects of thoracic flexibility exercise on chest function and mobility and to provide the information of physical therapy for patients with idiopathic scoliosis. Forty female subjects who were diagnosed with scoliosis participated in this study and were divided into the experimental and control groups. The experimental group consisted of 20 patients who were treated with thoracic flexibility exercise program during the admission (10 days) ad one month after discharge. The control group consisted of 20 patients who were not treated with thoracic flexibility exercise program. Vital capacity was measured using a respirometer. The chest expansion were calculated using differences of chest circumference between maximum inspiration and maximum expiration measured under armpits, at the junction between the sternum and xyphoid process, and at the waist. All subjects were measured two times: before the admission and at one month after discharge. Data were compared by groups using independent t-test, Vital capacity and chest expansion values (the armpit, chest and waist values) were significantly higher for the experimental group compared to those of the control group (p<.05). The findings of this study show that thoracic flexibility exercise program can lead to an increase in vital capacity and chest expansion and has a positive effects in relieving symptoms and restoring thoracic mobility.

The Role of Vitamin D in the Pathogenesis of Adolescent Idiopathic Scoliosis

  • Several theories have been proposed to explain the etiology of adolescent idiopathic scoliosis (AIS) until present. However, limited data are available regarding the impact of vitamin D insufficiency or deficiency on scoliosis. Previous studies have shown that vitamin D deficiency and insufficiency are prevalent in adolescents, including AIS patients. A series of studies conducted in Hong Kong have shown that as many as 30% of these patients have osteopenia. The 25-hydroxyvitamin D3 level has been found to positively correlate with bone mineral density (BMD) in healthy adolescents and negatively with Cobb angle in AIS patients; therefore, vitamin D deficiency is believed to play a role in AIS pathogenesis. This study attempts to review the relevant literature on AIS etiology to examine the association of vitamin D and various current theories. Our review suggested that vitamin D deficiency is associated with several current etiological theories of AIS. We postulate that vitamin D deficiency and/or insufficiency affects AIS development by its effect on the regulation of fibrosis, postural control, and BMD. Subclinical deficiency of vitamin K2, a fat-soluble vitamin, is also prevalent in adolescents; therefore, it is possible that the high prevalence of vitamin D deficiency is related to decreased fat intake. Further studies are required to elucidate the possible role of vitamin D in the pathogenesis and clinical management of AIS.

척추 측만증의 보존적 치료에 관한 최신 지견 - PubMed 검색을 위주로 2008년 이후 - (Current Trends in Research on Conservative Treatment of Scoliosis)

  • Objectives Scoliosis is that spine's arrangement was changed to be curved. Mostly scoliosis was found at childhood and recently rate of scoliosis is increasing. Causes of scoliosis were very various, but the patients are due to idiopathic reasons. Scoliosis was treated by conservative treatment or surgery treatment. If scoliosis is diagnosed early enough, it can be treated with conservative treatment. Methods We are searching PubMed by keywords and found 47 theses complied with requirements. Results Studies of conservative treatment of Scoliosis were active. Conservative treatments are considered before surgery treatment and there was a set guideline of conservative treatment. Also, there were several studies about effectiveness of conservative treatment and quality of life, mental stress. Studies about various kinds of conservative treatment were found as well. There were brief studies about introduction of new brace, effectiveness of several braces, and case studies. Conclusions Conservative treatment is very effective choice before surgery treatment at Scoliosis patient.

Usefulness of Simple Rod Rotation to Correct Curve of Adolescent Idiopathic Scoliosis

  • Objective : To correct apical vertebral rotation for adolescent idiopathic scoliosis (AIS), direct vertebral derotation (DVD) or simple rod rotation (SRR) might be considered. The aim of the present study is to introduce the surgical experiences of AIS by a Korean neurosurgeon and to evaluate the effectiveness of SRR for apical vertebral rotation. Methods : A total of 9 patients (1 male and 8 females) underwent scoliosis surgery by a neurosurgeon of our hospital. The Lenke classifications of the patients were 1 of 1B, 2 of 1C, 1 of 2A, 1 of 2C, 3 of 5C and 1 of 6C. Surgery was done by manner of simple rod rotation on the concave side and in situ coronal bending. Coronal Cobb's angles, vertebral rotation angles and SRS-22 were measured on a plain standing X-ray and CT before and after surgery. Results : The mean follow up period was 25.7 months (range : 5-52). The mean number of screw positioning level was nine (6-12). The mean age was 16.4 years (range : 13-25) at surgery. The mean Risser grade was $3.7{\pm}0.9$. The apical vertebral rotation measured from the CT scans was $25.8{\pm}8.5^{\circ}$ vs. $9.3{\pm}6.7^{\circ}$ (p<0.001) and the Coronal Cobb's angle was $53.7{\pm}10.4^{\circ}$ vs. $15.4{\pm}6.5^{\circ}$ (p<0.001) preoperatively and postoperative, respectively. The SRS-22 improved from 71.9 preoperatively to 90.3 postoperatively. There were no complications related with the operations. Conclusion : SRR with pedicle screw instrumentation could be corrected successfully by axial rotation without complications. SRR might serve as a good option to correct AIS deformed curves of AIS.

Short Segment Anterior Correction of Adolescent Idiopathic Scoliosis

  • Objective : The purpose of this study was to introduce our surgical experiences of scoliosis and to evaluate the effectiveness of anterior correction and fusion in adolescent idiopathic scoliosis (AIS). Methods : Between August 2004 and August 2007, four patients with AIS were treated with anterior segmental fusion and fixation at our hospital. Mean follow-up period was 9 (6-12) months. The average age was 14.0 (13-15) years. According to Lenke classification, three patients showed Lenke 1 curve and one patient with Lenke 5 curve. Single rod instrumentation was performed in one patient, dual rod instrumentation in one patient and combined rod instrumentation in two patients. Coronal Cobb measurements were performed on all curves in thoracic, thoracolumbar and, lumbar spine and the angle of hump was measured by a scoliometer pre- and postoperatively. Results : The average operative time was 394 minutes (255-525) with an average intraoperative blood loss of 1,225 ml (1,000-1,700). The mean period of hospital stay was 19.3 days and there was no complication related to the surgery. The mean Cobb angle was reduced from $43.3^{\circ}$ to $14.8^{\circ}$ (65.8% correction) postoperatively and the rib hump corrected less than $5^{\circ}$. All patients and their parents were satisfied with the deformity correction. Conclusion : Anterior spinal correction and fusion of AIS with Lenke 1 and 5 curve showed excellent deformity correction without any complications. In particular, we recommend anterior dual rod instrumentation because of mechanical stability, better control of kyphosis, and a higher fusion rate.

Occlusal deviations in adolescents with idiopathic and congenital scoliosis

  • Objective: This cross-sectional study aimed to investigate the characteristics of malocclusions in scoliotic patients through clinical examinations. Methods: Fifty-eight patients with idiopathic scoliosis (IS) and 48 patients with congenital scoliosis (CS) participated in the study. A randomly selected group of 152 orthopedically healthy children served as the control group. Standardized orthodontic and orthopedic examination protocols were used to record the occlusal patterns and type of scoliosis. Assessments were made by three experienced orthodontists and a spinal surgery team. The differences in the frequency distribution of occlusal patterns were evaluated by the chi-squared test. Results: In comparison with patients showing IS, patients with CS showed a higher incidence of Cobb angle ≥ 45° (p = 0.020) and included a higher proportion of patients receiving surgical treatments (p < 0.001). The distribution of the Angle Class II subgroup was significantly higher in the IS (p < 0.001) and CS (p = 0.031) groups than in the control group. In comparison with the healthy controls, the CS and IS groups showed significantly higher (p < 0.05) frequencies of asymmetric molar and asymmetric canine relationships, upper and lower middle line deviations, anterior deep overbite, unilateral posterior crossbite, and canted occlusal plane, with the frequencies being especially higher in CS patients and to a lesser extent in IS patients. Conclusions: Patients with scoliosis showed a high frequency of malocclusions, which were most obvious in patients with CS.
