Relationship between the Biological Lead Exposure Indices and Air lead Concentrations measured by Personal Air Samplers (개인용 공기포집기를 이용한 기중 연농도와 생물학적 연 폭로지표와의 관련성)

  • This study was carried out to evaluate the relationship between the biological lead exposure indices and air lead concentrations measured by personal air samplers. The 72 occupationally lead exposed workers were observed and the bioiogical lead Exposure indices chosen for this study were blood lead(PbB), urine lead(PbU), zinc protoporphyrin in whole blood(ZPP), $\delta$-aminolevulinic acid in urine(ALAU), $\delta$-aminolevulinic acid dehydratase activity (ALAD), coproporphyrin in urine(CPU) and hemoglobin(Hb). The workers were divided into four groups by air lead concentrations: Group I; under $0.05mg/m^3$, Group II; $0.05-0.10mg/m^3$, Group III; $0.10-0.15mg/m^3$ and Group IV; and over $0.15mg/m^3$. For evaluation the relationship between the biological lead exposure indices and air lead concentrations was used as correlation coefficients. The results obtained were as follows: 1. In Group I, II, III and IV, the mean value of PbB were $25.45{\pm}1.84{\mu}g/dl,\;27.87{\pm}3.53{\mu}g/dl,\;31.21{\pm}1.76{\mu}g/dl\;and\;47.02{\pm}13.96{\mu}g/dl$. Between Group IV and other groups showed statistically significant difference(p<0.05). 2. There was an increasing tendency of PbB, PbU, ALAU and ZPP according to the increase the mean air lead concentration, while ALAD has decreasing tendency. CPU and Hb did not show any constant tendency. 3. Correlation coefficients between PbB, PbU, ZPP, ALAU, ALAD, CPU, Hb and air lead concentration were 0.95, 0.83, 0.89, 0.72, -0.83, 0.51 and -0.45 respectively, and regression coefficient between PbB(Y) and PbA(X) was Y=126.8746X+16.9996(p<0.01).

  • Changes in the physicochemical characteristics and trans acid of cottonseed oil under the condition of selective hydrogenation, temperature$210^{\circ}C,\;H_2\;pressure\;0.3\;kg/cm^2$ Ni catalyst amount 0.12% (in oil), agitation speed 280 rpm, were investigated. The saturated fatty acid such as palmitic acid and stearic acid did not show any difference, while linoleic acid($50.03%{\rightarrow}9.38%$) were transformed to oleic acid ($20.65%{\rightarrow}60.35%$) during hydrogenation. In linoleic acid isomers, cc form were reduced significantly, but ct, tc, tt form showed little change, respectively. In oleic acid isomer, t form increased markedly, whereas there was no significant difference in c form. Meanwhile, melting point(MP) and solid fat content (SFC) were linearly increased, but iodine value(IV) linearly decreased as hydrogenation proceeded. From these results, linear regression equations were obtained as follows. MP & IV : Y= 1.59-2.36X(r=-0.96, p<0.05), SFC($at\;20^{\circ}C$) & MP : Y=2.81+2.01X(r=0.96, p<0.05), SFC($at\;20^{\circ}C$) & IV : Y=9.40-5.16X(r=-0.99, p<0.01), SFC($at\;20^{\circ}C$) & 18 : 1t : Y=6.25+8.48X(r=0.97, p<0.05)

  • Heller recognized the children exhibiting developmental regression after normal period of development and proposed the term 'dementia' infanilis for the condition and reported it 1908. But the foolowing studies were insufficient, the diagnostic concept was not definded, and various names such as dementia infantilis, Heller's syndrome, disintegrative psychosis and childhood disintegrative disorder have been used for that condition. Recently the term childhood disintegrative disorder fir that condition was proposed as independent disease entity by DSM-IV and ICD-10, and the interset in that condition is increasing now. But because of insufficient previous studies on that condition, the definite concept, the characteristics, the relationship with autism or other similar conditions, the treatment and prognosis of that condition are not clear by this time, therefore we have not seen the official report on the condition in our country. These authors experienced a case considered as childhood disintegrative disorder and report it with the review of literatures.

Equity in the Delivery of Health care in the Republic of Korea (의료이용의 형평성에 관한 실증적 연구 -공.교 의료보험 피부양자를 대상으로-)

  • This study is an empirical analysis on the equity in the delivery of heatlh care under the Korean Medical Insurance Corporation System. The purposes of this study are to find out effects of income on the health care utiliztion and measure the income-related inequity in the distribution of health care. This study was carried out based on the fact that the health insurance program has been organized to achieve the equity objective, "equal treatment for equal needs". Of 41, 828 insured persons who had been diagnosed in the 1993 Health Screening Test and utilifzation data from 1, January 1993 through 31, December 1993 were derived from the Benefit Managment File. Inequity was measured by means of I) share approach, ii) standardization concentration curve approach, iii) inequity index, iv) test for inequity. The major findings were as follows : 1. The expenditure shares of the top two quintile groups exceeded their morbidity shares, whereas the opposite was true of the bottom three quintile groups, Which showed a positive HI$_{LG}$ inequity index, suggesting the presence of some inequity favoring the rich group. 2. Compared with other residential areas, the rural area showed the highest positive HI$_{LG}$ irrespective of need indicatior applied. 3. Standardized expenditure concentration indices adjusted by age, gender and need structure were also found to be positive, and therefore still indicated that there has been inequity favoring the rich after the standardization. 4. The Loglikelihood Ratio (LR) test for the statistical significance of income-related inequity of medical care utilization was carried out using the logistic regression model. The resulting loglikelihood ratio test statistic value was 176, which did exceed the 0.5 percent critical value of the chi-square distribution with 28 degrees of freedom, which is 50.993. Therefore, the null hypothesis of no income-related inequity of medical care utilization was rejected at the 99.5 percent confidence level. 5. The Regression based F-test has been carried out for analyzing the income-related inequity of medical expenditure in terms of age, gender, morbidity indicators as explanary variables. The hypothesis of the absence of income-relate inequity was rejected for all need indicators at the 95% confidence level.nce level.

Risk factors for short term thyroid dysfunction after hematopoietic stem cell transplantation in children

  • Purpose: The purpose of this study was to evaluate short-term thyroid dysfunction and related risk factors in pediatric patients who underwent hematopoietic stem cell transplantation (HSCT) during childhood. Methods: We studied 166 patients (100 boys and 66 girls) who underwent HSCT at the Catholic HSCT Center from January 2004 through December 2009. The mean age at HSCT was $10.0{\pm}4.8$ years. Thyroid function of the patients was tested before and during 3 months of HSCT. Results: Out of 166 patients, 165 (99.4%) underwent allotransplantation. Acute graft-versus-host disease (GVHD, grades II to IV) developed in 76 patients. Conditioning regimens before HSCT include total body irradiation (n=57), busulfan (n=80), and reduced intensity (n=29). Forty-five (27.1%) had thyroid dysfunction during 3 months after HSCT (29 euthyroid sick syndrome [ESS], 6 subclinical hyperthyroidism, 4 subclinical hypothyroidism, 3 hypothyroxinemia, 2 overt hyperthyroidism, and 1 high $T_4$ syndrome). In a univariate logistic regression analysis, age at HSCT (P=0.002) and acute GVHD (P=0.009) had statistically significant relationships with thyroid dysfunction during 3 months after HSCT. Also, in a univariate logistic regression analysis, ESS (P=0.014) showed a strong statistically significant association with mortality. Conclusion: In our study 27.1% patients experienced thyroid dysfunction during 3 months after HSCT. Increase in age and acute GVHD may be risk factors for thyroid dysfunction during 3 months after HSCT. There was a significant association between ESS and mortality.

A Study on the Measurement of the Normal Tracheal Length in Korea adults (한국성인의 기관 길이 측정에 관한 연구)

  • The trachea is defined as the airway from the inferior border of the cricoid cartilage to the top of the carinal spur. This paper would confirm the normal tracheal length of Korean adults through the actual measurement using the fiberoptic bronchoscopy. The subjects of this study were 25 patients, 13 males and 12 females between the age of 20 to 69 without abnormality on the neck, trachea, mediastinum and lung pharenchyme on the preoperative chest X-ray, who received the operations from the period of July to September, 1994. For those patients who had heart diseases, the cardiothoracic ratio was below 50%. The measurement was performed on the patients with endotracheal intubation under the general anesthesia in supine and neutral position. The tracheal length was calculated by the difference between the length from the tip of the endotracheal tube [E-tube to carina and to the needle which was inserted into the E-tube at the lower border of the palpated cricoid cartilage, by inserting the broncoscopy through the E-tube. The result was as follow : 1 The measured tracheal length for men was 11.8 0.2 cm[mean standard deviation and women was 10.5 0.3 cm, and that was longer than this [p<0.01 . The average was 11.2 1.0 cm and the standard error was 0.20 cm. 2 According to the correlation between the tracheal length to weight, height[Ht , age, and body surface area[BSA respectively, the Ht [p=0.003 , age [p=0.055 , and the BSA[p=0.017 were significant, while weight was not [p=0.314 . 3 From the regression analysis of the tracheal length[T.L. to the Ht, Age, and the BSA which were significant, the following equation was derived.i Ht : T.L.= -1.29 + 0.076 x Ht [P=0.003 ii Age: T.L.= 10.04 + 0.028 x Age [P=0.055 iii BSA : T.L.= 5.60 + 3.48 x BSA [P=0.017 iv In multi-regression : T.L. = -4.15 + 0.034 x Age + 0.085 x Ht [P=0.0002]

  • The present study was undertaken to establish the standard body weight by height in Korean adults by using the actually measured heights and weights of a total of 5,496 insured persons who were examined medically at the Honam Medical Room of Dong Bang Life Insurance Company, Ltd. from January, 1983 to January, 1986. The results were as follows: 1. The linear regression equations to establish the standard body weight of Korean adults were as follows: In male, for $18{\sim}19$ age group, $y=7.272{\times}10^{-6}{\times}x^3+23.560$ for $20{\sim}29$ age group, $y=8.187{\times}10^{-6}{\times}x^3+22.031$ for $30{\sim}39$ age group, $y=8.627{\times}10^{-6}{\times}x^3+23.169$ for $40{\sim}49$ age group, $y=9.561{\times}10^{-6}{\times}x^3+20.994$ and for $50{\sim}59$ age group, $y=8.604{\times}10^{-6}{\times}x^3+23.801$ In female, for $18{\sim}19$ age group, $y=8.252{\times}10^{-6}{\times}x^3+18.920$ for $20{\sim}29$ age group, $y=7.715{\times}10^{-6}{\times}x^3+22.409$ for $30{\sim}39$ age group, $y=8.808{\times}10^{-6}{\times}x^3+21.439$ for $40{\sim}49$ age group, $y=9.691{\times}10^{-6}{\times}x^3+21.940$ and for $50{\sim}59$ age group, $y=12.500{\times}10^{-6}{\times}x^3+11.031$ 2. The standard age, height, and weight tables by author were presented with the aid of linear regression equations. 3. The values of standard body weight by height established by author reveal to be a little higher than those of other Korean reports through all age groups of both sexes, and reveal to be considerably similar, compared with those of the reports in Japan for fourth and sixth decade of female group.

Tracking of blood pressure during childhood (아동혈압의 지속성에 관한 시계열 분석)

  • The purpose of this study is to find the tracking of blood pressure in primary school-age children. A follow-up study was conducted from 1986 to 1990 on 330 first grade children attending primary schools in Kangwha County, Kyungki-Do. Basically we employed a linear regression model with random coefficients to figure out the relation between blood pressure changes and initial blood pressure. We obtained the following results ; 1. The mean blood pressures were increased grade went up in both sexs and were generally higher in female than male except for the systolic blood pressure at first grade. The size of difference was about 0.8 mmHg in mean systolic blood pressure and 1.5 mmHg in mean diastolic blood pressure. 2. The average annual increasing rates of systolic blood pressure were 2.5 mmHg in male and 3.1 mmHg in female respectively. For the diastolic blood pressure IV the average annual increasing rates were observed to be 3.0 mmHg in male and 2.9 mmHg in female respectively. Increasing rate of systolic blood pressure was significantly higher in female than male. 3. The adjusted regression coefficient of systolic blood pressure change on initial value was -0.11 in male and -0.13 in female and that coefficient of diastolic blood pressure change on initial value was -0.01 in male and -0.11 in female. This result shows that children with higher initial blood pressure do not pick up their blood pressure faster than others with lower initial blood pressure. There is no evidence of tracking of blood pressure in children. It is essential to find the earliest age having the tracking of blood pressure and we leave it for the further study.

Mediating Effect of Anxiety and Moderating Effect of Religion on the Relationship between Severity of Depressive Symptom and Quality of Life and Disability (우울증상 심각도와 삶의 질, 기능손상간의 관계에 대한 불안의 매개효과 및 종교의 조절효과)

  • Objective : This study aimed to examine impact of anxiety and demographic factors on relationship between severity of depressive symptom and quality of life and disability. Methods : One hundred ninety five patients who met DSM-IV-TR criteria for depressive disorder were enrolled. It includes "Hamilton Rating Scale for Depression (HRSD)", "State-Trait Anxiety Inventory-State (STAI-S)", "General Health Questionnaire/Quality of Life-12 (GHQ/QL-12)", and "Sheehan Disability Scale (SDS)". Correlation analysis was used to see the correlations of each variable. Hierarchical regression analysis was used to see mediating effect of anxiety in the relationship. Sobel test was used to verify mediating effect. Multiple regression analysis was used to see moderating effect of demographic factors in the relationship. Results : There was partial mediating effect of anxiety on the relationship between severity of depressive symptoms and decreased quality of life (z=-11.68, p<.001)/increased disability (z=10.42, p<.001). Only religion was found to be moderating effect on the relationship between depressive symptoms and decreased quality of life. Conclusion : Rapid relief of anxiety along with depressive symptom had important implications for the treatment of patients with depression.

A Comparison of the Clinical Features between Panic Disorder with and without Agoraphobia (광장 공포증 유무에 따른 공황장애의 임상 양상 차이)

  • Objectives:This study was performed to investigate the differences of the clinical feature between panic disorder with agoraphobic patients and panic disorder without agoraphobic patients. Methods:Two hundred nine patients meeting the criteria of DSM-IV panic disorder were recruited. One group was panic disorder with agoraphobia(n=78, 42 male(53.8%), mean age $37.6{\pm}9.9$ years), another was panic disorder without agoraphobia(n=131, 81 male(61.8%), mean age $40.5{\pm}10.3$ years). The numbers and frequency of panic symptoms were compared between two groups with t-test, and the logistic regression analysis were used for predicting panic disorder with agoraphobia. Results:The number of panic symptoms during panic attack was significantly higher in the group of panic disorder with agoraphobia than the group of panic disorder without agoraphobia(p<0.05).'Sweating','nausea or abdominal distress','fear of losing control of going crazy','chills or hot flushes'were more frequent in the group of panic disorder with agoraphobia(p<0.05). Among panic symptoms on logistic regression analysis,'sweating',' nausea or abdominal distress','fear of losing control or going crazy'turned out to correlate significantly with risk of development of agoraphobia in panic disorder. Conclusion:These results suggest that the frequency of some symptoms during panic attack may be a predictor of agoraphobia in patients with panic disorder.
