The Clinical Study on the Effects of $Tapazole^{(R)}\;upon\;^{131}I$ Uptake in Hyperthyroidism (갑상선(甲狀腺) 기능항진증(機能亢進症)에서 $Tapazole^{(R)}$$^{131}I$섭취율(攝取率)에 미치는 영향(影響)에 관(關)한 임상적(臨床的) 고찰(考察))

  • The differences in the change of the uptake rate of radioactive iodine ($^{131}I$) in the thyroid gland after $Tapazole^{(R)}$ administration before $^{131}I$ treatment were analysed in 137 patients who were diagnosed as diffuse toxic goiter in the Radioisotope Clinic and Laboratory, Seoul National University Hospital since Jan. 1967 to July, 1969. The uptake rate of the therapeutic dose of $^{131}I$ was changed diffusely compared with that of the trace dose in the patients who had no $Tapazole^{(R)}$ administration before $^{131}I$ treatment. In those patients who had $Tapazole^{(R)}$ more than 20 mg/day for more than one week before $^{131}I$ treatment, the uptake rate was decreased significantly. When the patients discontinued the administration of $Tapazole^{(R)}$ 7 days prior to $^{131}I$ treatment, the uptake rate was increased in all cases.

  • In order to compare the therapeutic effect as well as side effects between antithyroid therapy and radioiodine therapy in hyperthyroidism, the author evaluated 111 cases of hyperthyroidism which were composed of 57 patients with antithyroid treatment, 23 patients with combined treatment comprising of antithyroid and radioactive iodine ($^{131}I$) and 31 patients with treatment of $^{131}I$ alone. The $^{131}I$ treatment was limited to older age, above 20 years of age, and not employed in patients with pregnancy and lactation. The patients treated with antithyroid belonged to relatively younger age, and also milder in symptoms and thyroid function tests. The remission rate of antithyroid therapy group was 97.8% within 16 months. Among them the early remission rate occured within 4 months was found to be 21.7%. The combined therapy group, having prescribed more severe cases, showed the remission rate to be 75% within 16 months. The remission rate of $^{131}I$ therapy group revealed 90.9% within 16 months. Among them the early remission rate taking place within 4 months disclosed 16.7%. The side effects of antithyroid drug, propylthiouracil, were noted as follows: itching (2.7%) skin rashes (1.3%), and adverse enlargement of goiter (10.7%). The side effects $^{131}I$ therapy were transient hypothyroidism(3.9%) permanent hypothyroidism (7.8%) and itching (2.0%).

Radioactive iodine analysis in environmental samples around nuclear facilities and sewage treatment plants

  • Many radionuclides exist in normal environment and artificial radionuclides also can be detected. The radionuclides ($^{131}I$) are widely used for labeling compounds and radiation therapy. In Korea, the radionuclide ($^{131}I$) is produced at the Radioisotope Production Facility (RIPF) at the Korea Atomic Energy Research Institute in Daejeon. The residents around the RIPF assume that $^{131}I$ detected in environmental samples is produced from RIPF. To ensure the safety of the residents, the radioactive concentration of $^{131}I$ near the RIPF was investigated by monitoring environmental samples along the Gap River. The selected geographical places are near the nuclear installation, another possible location for $^{131}I$ detection, and downstream of the Gap River. The first selected places are the "front gate of KAERI", and the "Donghwa bridge". The second selected place is the sewage treatment plant. Therefore, the Wonchon bridge is selected for the upstream of the plant and the sewage treatment plant is selected for the downstream of the plant. The last selected places are the downstream where the two paths converged, which is Yongshin bridge (in front of the cogeneration plant). In these places, environmental samples, including sediment, fish, surface water, and aquatic plants, were collected. In this study, the radioactive iodine ($^{131}I$) detection along the Gap River will be investigated.

A Study on the Radiation Dose of $^{131}I$ in the Thyroid Gland during the Treatment of Hyperthyroidism (갑상선 기능항진증의 $^{131}I$ 치료시 갑상선 조사량에 관한 연구)

  • 53 patients with hyperthyroidism have been analyzed with special reference to therapeutic response to radioactive iodine ($^{131}I$) treatment. Mean effective half-life, 24 hour uptake rate and radiation dose of $^{131}I$ in hyperthyroid patients included in this study were respectively. 1. Mean effective half-life of $^{131}I\;was\;4.7{\pm}1.5$ days in the tracer dose and $5.0{\pm}1.5$ days in the therapeutic dose. 2. Mean 24 hour uptake rate of $^{131}I\;was\;72.7{\pm}11.1%$ in the tracer dose and $73.4{\pm}12.3%$ in the theapeutic dose. 3. Mean radiation dose of $^{131}I\;was\;5,319{\pm}2,648$ RAD as predicted and $5,692{\pm}2,843$ RAD as actual. A single dose of radioactive iodine treatment was satisfactory in 34 patients (radioiodine sensitive) and multiple doses of radioactive iodine treatments were required in 19 patients (radioiodine resistant). A radioiodine resistant group of patients with hyperthyroidism was distinctively characteristic in the following aspects. 1. Mean thyroid weight calculated in the resistant group ($63.9{\pm}14.0gm$) was significantly (p<0.01) greater than that of the sensitive group ($46.6{\pm}13.3gm$). 2. Mean 24 hour uptake rate of the tracer dose in the resistant group ($67.3{\pm}10.7%$) was significantly (p<0.01) lower than that of the sensitive group ($75.7{\pm}10.5%$). 3. Mean 24 hour uptake rate of the therapeutic dose in the resistant group ($68.5{\pm}13.7%$) was significantly (p<0.05) lower than that of the sensitive group ($76.1{\pm}10.9%$). 4. Mean predicted radiation dose, of $^{131}I$ in the resistant group ($3,684{\pm}1,745$ RAD) was significantly (p<0.01) lower than that of the sensitive group ($6,232{\pm}2,683$ RAD). 5. Mean actual radiation dose of $^{131}I$ in the resistant group ($4,100{\pm}1,691$ RAD) was significantly (p<0.01) lower than that of the sensitive group ($6,582{\pm}3,024$ RAD). 6. No significant difference was detected in terms of effective half-life of $^{131}I$ among the groups (p>0.05). 7. The average mean % difference of effective half-life, uptake rate and radiation dose measured following the tracer and therapeutic dose of $^{131}I$ were not statistically significant (p>0.05). Therefore effective half-life, uptake rate and radiation dose of the therapeutic dose of $^{131}I$ were readily predictable following the tracer dose of $^{131}I$. 8. It is concluded that the possibility of resistance to radioactive iodine treatment may be anticipated in patients with thyroid gland large in size and compromised $^{131}I$ uptake rate.

Analysis of Diffuse Hepatic Visualization after Iodine-131 Treatment in Patients with Thyroid Carcinoma (갑상선암 환자에서 전절제술 후 I-131 치료에서 미만성 간침착 정도의 분석)

  • Objectives: Any uptake of I-131 after total thyroidectomy means the remant thyroid tissue or distant metastasis of the thyroid cancer. However diffuse hepatic uptake of I-131 without abnormal uptake was showen in many cases on I-131 whole body scan. The aim of this study was to classify the liver uptake after I-131 scan and to evaluate the analysis of this finding. Materials and Methods: Between 1982 and 1998, 104 patients(l4 males, 90 females) with normal liver function underwent I-131 scan after total thyroidectomy. Prospectively we reviewed the films of the whole body scan and analysed the correlations between results of radioiodine uptake, pathologic diagnosis, prognostic factors, lymphatic metastasis, and thyroid function test. Result: Diffuse hepatic uptake was found in 44 of 104(42%) patients. 10 of 39(26%) patients on I-131 100mCi, and 34 of 63(54%) on I-131 150mCi showed hepatic uptake. 52 of 104(50%) patients was locally invasive thyroid cancer. The rate of the hepatic uptake was no significant differences with the thyroid hormone levels(T3, Free T4) and thyroglobulin between uptake group and non-uptake group. Conclusion: The rate of I-131 uptake was high in high-dose radioiodine treatment group. However, we can not find any correlation among the thyroid functions, the extent of metastasis or the extent of local invasion. We need further study to find out the causes of the hepatic uptake of I-131 after total thyroidectomy, besides liver metabolism of I-131 attached thyroid hormones.

Effects of the Information Providing Program of $I^{131}Treatment$ on the Anxiety and Immune Response of Patients with Thyroid Cancer (I$^{131}$ 치료에 대한 정보제공 프로그램이 갑상선암 환자의 불안과 면역기능에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of the information providing program of $I^{131}$ treatment on the pre-treatment anxiety and immune response of patients with thyroid cancer. Method: The used design was a nonequivalent control group design with pretest and post-test. Subjects of this study were 54 patients who had received a total thyroidectomy. They were given a information providing program with a brochure before 4 weeks of the hospitalization for $I^{131}treatment$, with problem solving phone counseling before 2 weeks of the hospitalization. Collected data were analyzed using the SPSS WIN program. Result: Hypothesis one, "The anxiety score of an experimental group would be lower than the score of a control group" was supported(t = -2.12, p = .03). Hypothesis two, "The level of cortisol of an experimental group would be lower than the level of a control group" was supported(t = -3.19, p = .00). However, hypothesis three "The level of T, B and N-K cells of an experimental group would be higher than the level of a control group" was not supported. Conclusion: The information providing program of $I^{131}$ treatment effectively decreased the anxiety of patients with thyroid cancer.

  • The thyroid gland is an interesting endocrine organ where a spectrum of tumors with different behavior arise. At one end of spectrum there is differentiated thyroid carcinoma (DTC) with excellent prognosis, whereas at the other end of the spectrum is anaplastic thyroid cancer which has universally poor outcome. Radioiodine (I-131) therapy has been in use for the treatment of thyroid diseases since 1946. It was introduced by Seidlin et al. 1) Although the use of I-131 has been vouge for a long time, its use in therapy for well differentiated thyroid cancer is still controversial 2). This is because, thyroid cancers (TC) are generally slow growing tumors, with low mortality and normal spans of survival. To record recurrence and mortality, long term follow up studies over a period of two to three decades are needed to establish definite conclusions on the acceptable mode of treatment The incidence of the disease being very low a large number of cases needed to establish a meaningful statistical data is lacking as most published reports feat with small series. Here again in the problem encountered are the differing protocols for treatment with I-131, the indications for treatment which may include or exclude ablation of residual thyroid tissue, cervical nodal and distal metastases. The dosage of I-131 used for ablation of residual thyroid tissue and metastatic disease also vary. The most reliable conclusion regarding I-131 treatment are obtained from studies reported on a large series of patients followed over a period of 2 decades or more from a single institute with a more or less unchanged protocol of management.

A Discrepancy between $^{131}I$-Metaiodobenzylguanidine ($^{131}I$-MIBG) Scintigraphy and $^{18}F$-FDG PET/CT after $^{131}I$-MIBG Therapy in a Patient with Recurred Malignant Pheochromocytoma (재발한 악성 갈색세포종 환자에서 $^{131}I$-Metaiodobenzylguanidine ($^{131}I$-MIBG) 치료 후 $^{131}I$-MIBG와 $^{18}F$-FDG PET/CT 영상의 차이점)

  • A 59-year-old woman who was diagnosed with malignant pheochromocytoma underwent $^{18}F$-fluorodeoxyglucose positron emission tomography/computed tomography ($^{18}F$-FDG PET/CT). She had undergone left adrenalectomy for pheochromocytoma 4 years previously. Recent multiple metastatic pulmonary nodules were noted on the chest X-ray. After treatment with $^{131}I$-metaiodobenzylguanidine ($^{131}I$-MIBG) with 7.4 GBq, post-therapy $^{131}I$-MIBG scintigraphy depicted multiple distant metastases including lung, liver, abdominal para-aortic and mesenteric lymph nodes. $^{18}F$-FDG PET/CT also depicted multiple metastases in lung, liver, and abdominal para-aortic lymph nodes, but some lesions were not shown. In this case, $^{131}I$-MIBG scintigraphy found additional lesions in metastatic malignant pheochromocytoma.

Management of Recurrent Thyroid Carcinoma with Negative Diagnostic Radioiodine Whole-Body Scan (진단적 방사성옥소 전신스캔이 음성인 갑상선 재발암의 진료)

  • Serum thyroglobulin measurement and I-131 whole-body scintigraphy (WBS) are well-established methods for the detection of recurrence in the follow-up of patients with thyroid carcinoma. However, inconsistent results are observed frequently, and these two methods are not always able to detect recurrence. In some patients, serum thyroglobulin level is elevated but the WBS is negative, because the recurrent tumor is too small and below the sensitivity of the diagnostic scan, or there is a dissociation between thyroglobulin synthesis and the iodine frapping mechanism. In such cases, various nuclear imaging methods including Tl-201 Tc-99m-sestamibi, and F-18-FDG PET can be used besides anatomical imaging methods. Among them, FDG PET localizes recurrent lesions in WBS-negative thyroid carcinoma with high accuracy. Several studies have suggested that empirical high-dose I-131 therapy resulted in a high rate of visualization in post-therapy scans with evidence of subsequent improvement. An important question is when to operate on patients with recurrent tumor. We believe that surgical removal is the best means of treatment for patients with localized persistent tumor, despite the high-dose I-131 therapy. with tumor in thyroid remnant, and with isolated recurrence in the lymph node, lung or bone. In addition, we recommend palliative resection of locally unresectable mass with subsequent treatment with high-dose I-131 therapy. Before I-131 therapy, the evaluation of sodium-iodide symporter expression in thyroid carcinoma can predict iodine uptake. Retinoic acid is known to induce redifferentiation, and to enhance I-131 uptake in thyroid carcinoma. Retinoic acid therapy may represent an alternative approach before high-dose I-131 therapy.

Diagnosis and Treatment of the Recurrent Thyroid Cancer (갑상선 재발암의 진단과 치료에 대한 고찰)

  • Objectives: To demonstrate effective diagnostic method and proper management of recurrent thyroid cancer through to compare treatment and surveillance of $I^{131}$ scanning detected recurrence and clinically detected recurrence. Material and Methods: We retrospectively analyzed clinical information about 46 patients who has recurrent thyroid cancer of 298 patients who have been primarily operated due to thyroid cancer in PMC at the over 10 years between 1986 and 1995. We examine incidence of recurrence due to pathologic types, site of recurrence, disease free interval, detection method of recurrence, and also treatment and progression of recurrence. A patients in which the clinical examination was entirely negative and the $I^{131}$ scan demonstrated either a new area of $I^{131}$ uptake or an increased area of concentration, compared to the previous scan, was designated as a recurrence detected by $I^{131}$ scan only. Recurrences that were obviously by physical examination or chest x-ray, etc were considered clinically detected recurrence, regardless of the the results of the thyroid scan. Results: Mean of disease tree interval(DFI) is 36months. When mean DFI of $I^{131}$ scan detected recurrence is 28months, whereas mean DFI of clinically detected recurrence is 47months. In statiscal analysis, p-value is 0.043 as significantly. In progression of recurrent patient, NED is 28case, AWD is Sease, DOD is 13case. Among the 13case, scan detected recurrence is lease of 20 patients(5%), whereas clinically detected recurrence is l2case of 26 patient(46%). In statiscal analysis, p-value is 0.003 as significantly. Conclusion: Early detection of the recurrent thyroid cancer by $I^{131}$ scanning leads to good progress compare with detection by clinical examination.
