The Comparison of Kupperman's Index and MENQOL between Women with Surgical Menopause and Women with Natural Menopause (수술적 폐경을 경험한 여성과 자연 폐경 된 여성의 Kupperman's Index, MENQOL 비교)

  • Purpose: To investigate the change of the climacteric syndrome with women with induced menopause by hysterectomy or ovariectomy compared with women with natural menopause. Methods: Menopausal women(aged 45-55) were selected at two oriental medicine hospitals, Sam-se Oriental Medicine Hospital and Daegu Hannny Oriental Medicine Hospital. They were asked about Kupperman's Index, MENQOL and a few additional questions. They were divided into two groups: women with induced menopause by hysterectomy or ovariectomy and women with natural menopause. To analyse the Kupperman's index and MENQOL, SPSS for Windows 12.0 and Wilcoxon Ranked Sum Test were used. Results: The total sums of Kupperman's index and MENQOL were significantly correlated with women with induced menopause and women with natural menopause and women with induced menopause had higher total sums. Both the renal and urinary tract disorder and the sexual disorder were significantly correlated with women with induced menopause and women with natural menopause. The gastrointestinal and psychosocial disorder of Kupperman's index and MENQOL were not significantly correlated with women with induced menopause and women with natural menopause. Conclusions: Women with induced menopause by hysterectomy or ovariectomy experienced climacteric syndrome more severely compared with women with natural menopause, specifically the renal and urinary tract disorder and sexual disorder.

  • Endometrial cancer (EC) in young women tends to be early-stage and low-grade; therefore, such cases have good prognoses. Fertility-sparing treatment with progestin is a potential alternative to definitive treatment (i.e., total hysterectomy, bilateral salpingo-oophorectomy, pelvic washing, and/or lymphadenectomy) for selected patients. However, no evidence-based consensus or guidelines yet exist, and this topic is subject to much debate. Generally, the ideal candidates for fertility-sparing treatment have been suggested to be young women with grade 1 endometrioid adenocarcinoma confined to the endometrium. Magnetic resonance imaging should be performed to rule out myometrial invasion and extrauterine disease before initiating fertility-sparing treatment. Although various fertility-sparing treatment methods exist, including the levonorgestrel-intrauterine system, metformin, gonadotropin-releasing hormone agonists, photodynamic therapy, and hysteroscopic resection, the most common method is high-dose oral progestin (medroxyprogesterone acetate at 500-600 mg daily or megestrol acetate at 160 mg daily). During treatment, re-evaluation of the endometrium with dilation and curettage at 3 months is recommended. Although no consensus exists regarding the ideal duration of maintenance treatment after achieving regression, it is reasonable to consider maintaining the progestin therapy until pregnancy with individualization. According to the literature, the ovarian stimulation drugs used for fertility treatments appear safe. Hysterectomy should be performed after childbearing, and hysterectomy without oophorectomy can also be considered for young women. The available evidence suggests that fertility-sparing treatment is effective and does not appear to worsen the prognosis. If an eligible patient strongly desires fertility despite the risk of recurrence, the clinician should consider fertility-sparing treatment with close follow-up.

Korean Medical Treatment of Sequela Following Chemotherapy for Recurrent Ovarian Cancer after Hysterectomy: a Case Report (재발성 난소암 환자의 화학적 항암요법 후유증 치험 1례)

  • Objectives: Most of the time after chemotherapy for ovarian cancer, the immune system is getting even worse with reduction of whole blood cell. The purpose of this study is to report the effect of korean medical treatment on Sequela including pancytopenia after chemotherapy for recurrent ovarian cancer after hysterectomy. Methods: The patient having treated with hospital management from June 8th in 2017 until July 5th in 2017, participated. Her chief complaints were fatigue, tinnitus, dizziness, decreased appetite due to glossodynia and constipation. We diagnosed her as pancytopenia based on her whole blood test result. We treated the patient with acupuncture, herbal medicine, cupping and moxibustion. The efficacy of treatment was evaluated with Numerical Rating Scale (NRS) and blood test. Results: During hospitalization period, the symptoms including fatigue, tinnitus, dizziness, decreased appetite, constipation and pancytopenia were reduced. After discharging, we identified that her blood levels are maintained and ovarian cancer no longer recurred. Conclusions: To recover from Sequela including pancytopenia following chemotherapy for recurrent ovarian cancer after hysterectomy, Korean medical treatment is considered to be effective.

Establishment of Hysterectomy for Gnotobiotic Pig Production (무균돼지 생산을 위한 자궁적출술 확립)

  • Gnotobiotic piglets were routinely produced by hysterectomy. In this study, 22 pregnant miniature pigs (111th to 113th day of gestation) were used for hysterectomy. Before surgery, 14 pigs were insensibilizated by Ketamine 50® plus CO2 gas and 8 pigs by a slaughter pistol. The high level of Ketamine 50® (0.09㎖/kg) was faster (146 vs 283 seconds) in surgery but the time taken for complete revival of one piglet was more prolonged (427 vs 64 seconds) than 0.03㎖/kg level. In hysterectomies with a slaughter pistol, surgery time was faster (470 vs 155 seconds) and the rate of alive piglets was higher (97.0 vs 83.8%) than in those with Ketamine 50®. There were no problems in the rate of alive newborn piglets even when sows were hysterectomized at 3 days prepartum.

A Clinical Study of Uterine Myoma Cases For Total Abdominal Hysterectomy(TAH) (자궁근종에 의한 전자궁적출술 적응증 환자의 보존적 한방치료 증례)

  • Purpose : The purpose of this study was to investigate the clinical application of Traditional Korean Medicine(TKM) treatment to uterine myoma for TAH. Methods : Two patients were diagonosed as uterine myoma for TAH in other Obstetrics & Gynecology hospital. They were treated by herbal medicine with each diagonosis in our hospital, and the first case was also treated by acupuncture, moxibustion and herbal acupuncture. Results : After TKM, the uterine myoma size of the first case was reduced($12.2^{\ast}7.3cm{\to}9^{\ast}7cm$) significantly. And all the symptom of uterine myoma, Gonadotropin-releasing hormone(Gn-RH)'s side effects were improved. The metrorrhagia of the second case was stopped, and was not repeated during six month. The second symptoms of metrorrhagia were improved. So, the two cases don't need to have a TAH. Conclusion : The results indicate that TKM can be effective for the management of uterine myoma for TAH without operation. Further study will be investigated in TKM for healing uterine myoma for TAH completely.

A Comparison of Butorphanol and Fentanyl Administered in Conjunction with Ketorojac in Intravenous Patient Controlled Analgesia after Total Abdominal Hysterectomy (복식 전자궁 절제술 후 통증 자가 조절 장치를 통하여 Ketorolac과 함께 투여한 Butorphanol과 Fentanyl의 비교)

  • Background: We compared butorphanol and fentanyl for opioids use in patient-controlled analgesia(PCA) with ketorolac to determine a suitable drug combination for postoperative pain control. Methods: Sixty patients were equally divided into 2 Groups. Group 1 (n=30) butorphanol 10 mg with ketorolac 180 mg; Group 2 (n=30) fentanyl 1 mg with ketorolac 180 mg, diluting 100 ml solutions intravenously via PCA pump after total abdominal hysterectomy under general anesthesia. Total infusion dosage of PCA drug, VAS pain scores, and side effects of both group were manitored. Results: Total infusion dosages were as follows: (Group 1) butorphanol 8.3 mg with ketorolac 149.7 mg; (Group 2) fentanyl $646.6\;{\mu}g$ with ketorolac 116.2 mg. The two groups showed similar pain scores auld side effects. Conclusions : Both butorphanol and fentanyl were effective for postoperative pain control using PCA pump, but butorphanol was more economical. The putative potency ratio of butorphanol to fantanyl was 12.8 : 1.

  • Objectives : The purpose of this study was to compare differences in bowel movement in women who had an abdominal hysterectomy and were treated for 10 minutes with San Yin Jiao (SP6) acupressure at three different time periods. Methods : The design was a nonequivalent control group non-synchronized post test only design. The participants included 123 women, 30 in experimental group 1 (post anesthesia 30 min), 22 in the experimental group 2 (post anesthesia 4 hours), 23 in experimental group 3 (Post anesthesia 24 hours), and 48 in a control group. Data were collected using a structured questionnaire consisting of general characteristics, self reported time when gas was first passed. Differences in the time when first gas was passed among four groups were analyzed using ANOVA. Results : There was no a statistical difference among the 4 groups for the time when gas was first passed. Conclusion : It is necessary to replicate the study with sonographic data for bowel movement.

Gestational Diabetes in a Yorkshire Terrier Dog (요크셔테리어종 개에서 발생한 임신성 당뇨병)

  • A 3 year-old pregnant female Yorkshire terrier dog was presented with major complaints of polyuria and polydipsia. Laboratory tests found persistent hyperglycemia and glucosuria. Abdominal ultrasound study found 2 dead fetuses. Ovarian hysterectomy was performed to remove the dead fetuses. After ovarian hysterectomy, the level of blood glucose was returned to normal range. In addition, the clinical signs and urine glucose were all disappeared. Based on findings on laboratory tests and follow-up study, the case was diagnosed as gestational diabetes mellitus. To our best knowledge, this is the first case report describing a gestational diabetes mellitus of dog in Korea.

  • A benign metastasizing pulmonary leiomyomatosis(BMPL) is a rare and uncommon variety of leiomyoma. BML affects middle-aged women who had a previous hysterectomy due to a histologically benign appearing leiomyoma. BMPL is characterized by asymptomatic multiple smooth muscle nodules with a less aggressive course than a leiomyosarcoma. A 45-year-old woman who had a hysterectomy for a uterine leiomyoma 4 years prior to this study presented with multiple variable-sized lung nodule. A percutaneous needle biopsy confirmed the benign metastasizing pulmonary leiomyomatosis. Here, we present this case with a review of the relevant literatures.

Change Patterns on Subjective Distress Level in Gynecological Postoperative Patients with Different Levels of State Anxiety (불안수준에 따른 부인과 수술환자의 불편감지각의 변화 - 통증 자가 조절기 사용 중인 환자를 중심으로 -)

  • Purpose: A descriptive study was conducted to identify the change patterns on the level of perceived distress during the early postoperative period with regard to state anxiety in patients using patient controlled analgesia. Method: One hundred women who underwent elective hysterectomy procedures or other gynecologic surgeries completed a series of questionnaires measuring state anxiety, and subjective distress assessed by visual analog scales at 8, 24, & 48 hours postoperatively. Data were analyzed with frequencies, percentages, means, ANOVA, Repeated Measures ANCOVA, and Scheff'e post test utilizing SPSS WIN 11.0. Result: There was a gradual decrease in levels of total distress and pain over the three points in time after surgery regardless the levels of state anxiety. However, women with higher levels of anxiety their pain curve rose at 48 hours post-op. In addition, over the first two points in time, women in the higher anxiety group showed higher levels of distress than those in the lower anxiety group, but no such group differences were observed in the levels of pain, revealing higher pain levels only at 8 hours post-op in both groups. Conclusion: Irrespective of effective pain management modality, most postoperatively experienced distress in gynecological patients was derived from anxiety and pain. These findings have implications for pain management, especially in patients with emotionally charged surgeries like hysterectomy.
