Interreationship between the Subclinical Hypothyroidism and Serum Total Cholesterol, LDL-Cholesterol (불현성 갑상선 기능저하증과 혈청 총 콜레스테롤, 저밀도 콜레스테롤 농도와의 상관성)

  • This study was carried out in 14,443 subjects(8,706 men, 5,737 women, mean age $41.8{\pm}9.16$) who visited the health promotion center of one university hospital. Serum $T_3$, $T_4$ and TSH concentrations were measured with CIA using commercial kits on ADVIA Centaur Assay and Serum total cholesterol, triglyceride, and HDL-cholesterol were measured by auto-analyzer Hitach 7600 in enzyme method. 1. The prevalence of subclinical hypothyroidism was 12.6/1,000 population(men 6.0/1,000, women 22.7/1,000) and as high as about 4 times in women than in men. 2. In subclinical hypothyroidism, the values of total cholesterol and LDL-cholesterol were significantly high by 5.5 mg/dL, 4.6 mg/dL compared with normal group(p<0.05). 3. In all the tested hypercholesterolemia and hyperLDL-cholesterolemia, were correlated with TSH. There is higher correlation in women group than men group. These results indicate that the subclinical hypothyroidism and dyslipemia significantly correlated. Especially, it is very important to find that the hypercholesterolemia and hyperLDL-cholesterolemia are the high risk factors of atherosclerosis. Therefore, it is essential for the group of subclinical hypothyroidism to take the cholesterol, LDL-cholesterol and serum lipid test as well as thyroid function test.

The Effects of Woogwi-eum(Yougui-yin) on the Hypothyroidism of Rats (우귀음이 백서의 갑상선기능저하에 미치는 영향)

  • Objectives : In order to study experimentally the effects of Woogwi-eum(Yougui-yin) on hypothyroidism, ${T_3}-uptake$ content, $T_3$ content, $T_4$ content, TSH content, hematological values, enzyme activity, total cholesterol content, $Na^{+}$ content change in the serum and body weight of rats induced by thiouracil were measured after medication of extract of Woogwi-eum (Yougui-yin). Methods : Sparque-Dawley rats were used in this experiment and the rats were divided into normal, control and experimental groups. The hypothyroidism of control and experimental groups was induced by thiouracil, Woogwi-eum(Yougui-yin) extract was orally administered in the experimental groups for 20 days Results : In this experiment, the Woogwi-eum(Yougui-yin) treated group showed a significant increase in the serum ${T_3}-uptake$, TSH, RBC and hematocrit content, whereas a significant decrease in the serum CPK, ALP, LDH, total cholesterol content and the body weight was measured. Conclusions : According to the above results, it is suggested that Woogwi-eum(Yougui-yin) is partially effective to the hypothyroidism induced by thiouracil in rats. Therefore, Woogwi- eum(Yougui-yin) seems to be applicable to hypothyroidism in clinical practice.

A Case of Thyroxine Binding Globulin Deficiency with Hypothyroidism (갑상선기능저하증을 동반한 티록신 결합글로불린 결핍증 1례)

  • A child diagnosed with congenital hypothyroidism after newborn screening and follow up thyroid function test at 1 month of life in another general hospital demonstrated euthyroid state with thyroxine( $T_4$) supplementation until the age of 22 months of life, when he was transferred to our hospital, where he was diagnosed as thyroxine binding globulin(TBG) deficiency with low $T_4$ and TBG. Withdrawal of $T_4$ at age of 26 months was associated with hyperthyrotropinemic hypothyroidism. This patient is a case of TBG deficiency associated with hypothyroidism, and in rare instances, TBG deficiency may lead to hypothyroidism requiring hormone supplementation.

Effects of Maternal Hypothyroidism on the Pubertal Development in Female Rat Offspring

  • The present study was performed to investigate the effect of maternal hypothyroidism and puberty onset in female rat pups. To do this, we employed propylthiouracil (PTU) to prepare a hypothyroid rat model. Pregnant rats were treated with PTU (0.025%) in drinking water from gestational day 14 to postnatal day 21 of offspring. Comparison of general indices such as body and tissue weights and puberty indices such as vaginal opening (VO) and tissue histology between control and PTU-treated rats were conducted. There was no significant difference in the date of VO between control and PTU group. The body weights of the PTU group were significantly lower, only 36.8% of the control group (p<0.001). Although the absolute thyroid weight was not changed by PTU treatment, the relative weight increased significantly about 2.8 times (p<0.001), indicating that hypothyroidism was successfully induced. On the other hand, the absolute weights of the ovary and uterus were markedly decreased by PTU administration (p<0.001), and the relative weight was not significantly changed. The ovarian histology of PTU group revealed the advanced state of differentiation (i.e., presence of corpora lutea). Inversely, the uterine histology of PTU group showed underdeveloped structures compared those in control group. Taken together, the present study demonstrates that our maternal hypothyroidism model resulted in minimal effect on pubertal development symbolized by VO despite of huge retardation in somatic growth. More sophisticatedly designed hypothyroidism model will be helpful to achieve a better understanding of pubertal development and related disorders.

Effects of Yijung-tang (YJT) on Experimental Hypothyroidism in Mice (이중탕(理中湯)이 생쥐에 유발된 갑상선기능저하증에 미치는 영향)

  • Objectives: The present study was carried out to investigate the effects of YJT on experimental hypothyroidism induced by sodium perchlorate and methimazolein in mice. Methods: 30 one-month-old C57BL6 mice were decided into 4 groups: 1) normal (n=6), 2) sodium perchlorate and methimazolein-induced hypothyroidism control (n=8), 3) hypothyroidism mice treated with low YJT (n=8), 4) hypothyroidism mice treated with high YJT (n=8). Sodium perchlorate and methimazolein were administered for 4 weeks, YJT (low and high) was administered for 2 weeks after sodium perchlorate and methimazolein were initiated for a total duration of 2 week. The changes were observed : weight of body, T3, T4, TSH, follicular cells in the thyroid tissues, LDL cholesterol, HDL cholesterol, triglyceride and free fatty acid, FBG, AST, ALT and so on. Results: YJT did not affect body weight gain. YJT restored free T4 level decreased by sodium perchlorate and methimazolein and prevented shrinking of follicles and proliferation of follicular cells in the thyroid tissues. In addition, YJT lowered total and LDL cholesterol levels elevated by sodium perchlorate and methimazolein respectively and ameliorated distribution of fat in liver tissues. In addition, the effect on fasting blood glucose (FBG), aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were also investigated. Conclusions: These data suggest that YJT can be used to treat woman patients which are accompanied with hypothyroidism relatively safely.

Risk Factors of Hypothyroidism after Hemithyroidectomy for Benign Thyroid Disease in Korea (양성 갑상선 질환으로 편측 갑상선 절제술을 시행한 후 발생되는 갑상선 기능 저하증에 대한 위험인자)

  • Objectives:There are various reports for incidence and risk factors of hypothyroidism after hemithyroidectomy for benign thyroid disease. The reported incidence rate varies from 5.0 to 41.9%. Moreover, there are few reports about this in Korea. So, our objective is to identify the incidence and possible factors contributing to hypothyroidism after hemithyroidectomy in Korea. Subjectives and Methods:We reviewed medical records of patients who underwent hemithyroidectomy between 1999 and 2007 years. We enrolled 80 patients. The incidence of postoperative hypothyroidism was based on thyrotropin values and clinical symptoms. And all patients were evaluated for age, gender, serum thyrotropin(TSH) levels, serum free T4, weight of resected thyroid tissue and associated thyroiditis. Results:Nine of 80 patients(11.3%) became biochemically hypothyroid postoperatively. Six of 9 hypothyroid patients were diagnosed within 2 months of operation. The mean preoperative TSH level was $2.12{\pm}0.96mIU/L$ in hypothyroid and significantly different from $1.33{\pm}0.77mIU/L$ in euthyroid patients(p<0.05). There were no significant differences in age, gender, weight of resected thyroid tissue, preoperative free T4 and associated thyroiditis. Conclusion:A minority of patients became hypothyroid after hemithyroidectomy. Hypothyroidism following hemithyroidectomy occurred in patients with higher preoperative TSH levels.. The screening test for postoperative hypothyroidism should be performed from month to year after operation.

Newborn screening of inherited metabolic disease in Korea (유전성 대사질환의 신생아 스크리닝)

  • In 1991, the Ministry of Health & Social affairs adopted a nationwide service program for neonatal screening of phenylketonuria, galactosemia, maple syrup urine disease, homocystinuria, histidinemia & congenital hypothyroidism for newborns delivered from low class pregnant women registered in health centers. Government decreased the test items from six to two, PKU & congenital hypothyroidism to increase test numbers with same budget from 1995. Government decided to test PKU & hypothyroidism for all newborns from 1997. 78 laboratories wanted to participate for neonatal screening test in 1999. Government didn't decide laboratory center for a certain district and placed responsibility on free competition. Government are planning to test 573,000 newborns from 1998, Government decided to screen 6 items PKU, congenital hypothyroidism, maple syrup urine disese, homocystinuria, galactosemia and congenital adrenal hyperplasia from 2006. 17 laboratores are participating now. The cost of screening test is supported by both the federal government and local government on a 40-60 basis. In case a patient with an inherited metabolic disease is diagnosed by screening of government program, special milk is provided at government's expense. Interlaboratory quality control was started 6 times a year from 1994. According to the government project, 3,707,773 newborns were screened. 86 PKU, 718 congenital hypothyroidism were detected. So incidence of PKU is 1/43,114 and congenital hypothyroidism is 1/4,612. Maeil dairy company produced new special formula for PKU, MMA and PA, MSUD, urea cycle disorder, homocystinuria, isovaleric acidemia from Oct. 1999. The cost benefit of performing screening procedures coupled with treatment has been estimated to be as high as 1.77 times in PKU, 11.11 times in congenital hypothyroidism than cost without screening. We are trying to increase the budget to test all newborns for Tandem mass sereening & Wilson disease from 2008. Now it is a very important problem to decrease laboratory numbers of neonatal screening in Korea. So we are considering 4-5 central laboratories which cover all newborns and are equipped with tandem mass spectrometer & enzyme immunoassay for TSH, 17OHP & enzyme colorimetric assay for galactose.

Effects of Hyangsayangyi-tang Aqueous Extracts on the Hypothyroidism Related Hepatic Damages induced by PTU in Rats (香砂養胃湯이 PTU로 유발된 Rat의 갑상선기능저하와 간손상에 미치는 영향)

  • To evaluate the benefits of Hyangsayangyi-tang aqueous extracts (HSYYT) on the propylthiouracil (PTU)-induced Rat hypothyroidism. 6 groups, each consisting 8 Rats were used in the present study - Intact vehicle control, PTU control, LT4, HSYYT 500, 250 and 125 ㎎/㎏ treated groups. HSYYT was given, once day for 42 days from 2 weeks before start of PTU treatment as an oral dose of 500, 250 and 125 ㎎/㎏(body weight), and for 28 days while the PTU 10 ㎎/㎏ by daily subcutaneous treatment induced hypothyroidism. Compared the results with LT4 0.5 ㎎/㎏ intraperitoneally treated rats in this experiment. Results of the PTU treatment included; decreases of body weight, increase in thyroid weight, decrease in liver weight, in serum T3, and T4 level decrease with increase of serum TSH levels, in serum HDL increase and in TG content decrease, decrease in liver antioxidants defense system, increase of serum AST levels were observed. However, these PTU induced hypothyroidism and related hepatic damages were dose-dependently inhibited by treatment of HSYYT 500 and 250 ㎎/㎏, and they also effectively regulated the PTU-induced abnormal antioxidant defense system changes in liver. Therefore, in comparison with the PTU control group, it was effective and advantageous changes were not observed in HSYYT 125 ㎎/㎏ treated rats on the PTU induced hypothyroidism and related hepatic damages. In this experiment, HSYYT 500 and 250 ㎎/㎏ dose-dependently inhibited PTU-induced hypothyroidism and related liver damage in rats but not in HSYYT 125 ㎎/㎏.

Effects of Bupleuri Radix on the Rat Hypothyroidism Induced by PTU (6-n-propyl-2-thiouracil) (시호가 PTU (6-n-propyl-2-thiouracil)로 유발된 Rat 갑상샘 기능저하증에 미치는 영향)

  • The object of this study was to evaluate the effect of Bupleuri Radix, aqueous extracts of the root part of Bupleurum falcatum on the 6-n-propyl-2-thiouracil (PTU)-induced rat hypothyroidism. Aqueous extracts of Bupleuri Radix (BR; yield = 11.73%) were administered, once day for 42 days from 2 weeks before start of PTU treatment as an oral dose of 300 and 150 mg/kg (body weight), and hypothyroidism was induced by daily subcutaneous treatment of PTU 10 mg/kg for 28 days. The changes on the body weight, thyroid gland weights, serum thyroid hormone - thyroid stimulating hormone (TSH), triiodothyronine ($T_3$) and thyroxine ($T_4$), serum lipid profiles - total cholesterol, low density lipoprotein (LDL), high density lipoprotein (HDL) and triglyceride, liver antioxidant defense system - lipid peroxidation, $H_2O_2$, superoxide dismutase (SOD) and catalase (CAT), serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were observed with histopathology of thyroid glands. Results were compared with $LevoT_4$ 0.5 mg/kg treated rats. As results of PTU treatment, marked decreases of body weights, triglyceride contents, liver CAT activities and changes of serum thyroid hormone levels were observed with increases of serum AST, HDL contents, liver $H_2O_2$ and SOD activities and thyroid gland weight. In addition, marked hyperplasia of follicular cells with decreases of follicular colloid contents and sizes were demonstrated at histopathological inspections. However, these PTU induced hypothyroidism were dose-dependently inhibited by treatment of BR extracts, and BR extracts effectively regulated the hypothyroidism related changes on the antioxidant defense system. The results obtained in this study suggest that BR extracts have favorable effects on the thyroid hormone productions with beneficial effects on the hypothyroidism mediated by the modulatory effects on the antioxidant defense system.

The Effects of Aconiti Radix on Thyroid Function in Hypothyroidism Rat Model Induced by 6-propyl-2-thiouracil(PTU). (부자(附子)가 6-propyl-2-thiouracil(PTU)로 유발된 rat의 갑상선 기능저하증에 미치는 영향)

  • Objective : Although hypothyroidism is a common disease in the endocrinology system, it is reported that there are a lot of difficulties in treating it effectively. Aconiti Ra얕 traditionally has been used in treatment of coldness, fatigue, and bradycardia. In this study, we investigated the therapeutic effects of Aconiti Radix on hypothyroidism rat model induced by 6-propyl-2-thiouracil (PTU). Methods : Two-month-old rats were used with administration of PTU which induced hypothyroidism in the rats. After 2 weeks, Aconiti Radix and thyroxine were administered, respectively. The body weights were measured every week. After 4 weeks, the blood samples of all rats were taken from their hearts. They were analyzed biochemically and $T_4$ (thyroid hormone) & TSH (thyroid stimulating hormone) was measured by ELISA kits. Results : In comparison with normals, controls showed hypothyroidism with significantly low $T_4$ and high TSH the statistics. In Aconiti Radix administration groups significantly increased $T_4$ was observed in the statistics and its effects were dose-dependent. There was no difference statistically in TSH of Aconiti Radix treatment groups from controls, nor were statistical differences observed significantly in biochemical labs and weight of each group. Conclusions : These findings suggest that Aconiti Radix protects thyroid cells and makes thyroid cells produce thyroid hormones. It is also very safe in the view of liver, kidney function, and other metabolism. It may be a useful agent for treating hypothyroidism.
