Weight Reduction Management of Patients with Idiopathic Intracranial Hypertension: A Case Series Report (특발성 두개내 고혈압 환자에서 체중 감량을 동반한 치료 증례 시리즈)

  • Idiopathic intracranial hypertension (IIH) is characterized by increased intracranial pressure without detectable causes. Its main symptoms include headache, vision problems, and pulsatile tinnitus. Obesity is a major risk factor for IIH; therefore, weight reduction is required to ameliorate these symptoms in patients with obesity and IIH. We present four cases of obesity-related IIH whose symptoms improved through weight reduction. In three patients, the dose of IIH medications, such as acetazolamide, decreased. Based on these cases, we report that weight reduction contributes to the improvement of symptoms in patients with IIH as the main part of treatment. Further clinical research is necessary to evaluate the effects of weight reduction on IIH treatment in Korean patients.

  • There is a need to develop a data quality management algorithm in order to improve the quality of health care data. In this study, we developed a data quality control algorithms associated diseases related to diabetes in patients with hypertension. To make a data quality algorithm, we extracted hypertension patients from 2011 and 2012 discharge damage survey data. As the result of developing Data quality management algorithm, significant factors in hypertension patients with diabetes are gender, age, Glomerular disorders in diabetes mellitus, Diabetic retinopathy, Diabetic polyneuropathy, Closed [percutaneous] [needle] biopsy of kidney. Depending on the decision tree results, we defined Outlier which was probability values associated with a patient having diabetes corporal with hypertension or more than 80%, or not more than 20%, and found six groups with extreme values for diabetes accompanying hypertension patients. Thus there is a need to check the actual data contained in the Outlier(extreme value) groups to improve the quality of the data.

Effect on postoperative pulmonary arterial pressure with preoperative hemodynamic data in VSD (심실중격결손증에서 수술전 혈류역학치가 수술후 폐동맥압력에 미치는 영향)

  • The prognosis of patients with VSD and pulmonary hypertension is at least partially related to the reversibility of pulmonary hypertension after surgery. To predict postoperative pulmonary arterial pressure, immediate postbypass values were compared with preoperative hemodynamic data in 18 surgically closed VSD patients aged 6 to 80 months. The following results were obtained. 2] There was a good correlation between preoperative PP/PS and postoperative PP/PS in patients aged over 24 months [r=0.685, p<0.05], and in patients whose QP/QS were over 2.0 [r=0.686, p<0.01]. 3] There was also a good correlation between pump time and postoperative PP in total patients [r=0.697, p<0.005]. 4] Direct correlations were found between preoperative PP/PS and immediate postbypass PP/PS in patients aged over 24 months and in patients whose QP/QS>2.0, but the effect of CPB would participate in some degree. So it may be necessary to reevaluate their relationship after the effect of CPB have been disappeared.

The Experiences of Young Adult with Hypertension (성인 전기 고혈압 환자의 경험)

  • Purpose: The purpose of this study was to explore the experiences of young adults with hypertension, using phenomenology in order to describe the essence of such experiences and to understand them from the patients' point of view. Methods: Participants in this study were 4 hypertension patients with ages ranging from 20 to 35 years old, living in Seoul city. Data were collected from in-depth interviews done from October to November in 2005. Colazzi method(1978) was used to analyze the data. Results: The 12 themes were extracted in this study ; 'Discovery by accident', 'Denial of the diagnosis', 'A disease unworthy of the young men', 'Guilty feeling toward their parents', 'Fear of death', 'Unconvinced reason', 'Worry about the future', 'Attribute all symptoms to the disease', 'Will of not life-long medication', 'Concern for fertility problem', 'Not convinced with current treatment method', 'Becoming unimportant'. Conclusion: The results of this study show that the young adult with hypertension will be in a high risk group of non compliance, so helpful program for them should be developed and the nurses need to pay more attention to the psychosocial aspect of nursing.

The Effect of Yoga Program on Reduced Blood Pressure in Elderly′s Essential Hypertension (요가 프로그램이 본태성 고혈압 노인환자의 혈압하강에 미치는 효과)

  • The purpose of this study was to evaluate the effect of a Yoga program on decreasing blood pressure in elderly patients with essential hypertension and to suggest a yoga program effective as a nursing intervention tool to reduced blood pressure with increasing life satisfaction. Method: The subjects of this study were 24 elderly's essential hypertension, who practiced yoga by three times a week for 8 weeks. In order to evaluate the effect of the yoga program, blood pressure, physiological parameters (Total cholesterol, HDL, LDL, triglycerides) and level of life satisfaction were measured before and after the training. Collected date were analyzed by SPSSWIN program. Result: 1) There were significant reduction in systolic and diastolic blood pressure. 2) There were significant reductions in total cholesterol, LDL, triglycerides but no significant increased in HDL. 3) Blood pressure changes were time specific : Both of systolic and diastolic blood pressures were significantly reduced after 2weeks. 4) There was a significant increase in life satisfaction. Conclusion: The results proved that a yoga program was an effective nursing intervention to reduce blood pressure and to increased life satisfaction for elderly patients with essential hypertension.

Non-cirrhotic portal hypertension in an ankylosing spondylitis patient

  • Idiopathic non-cirrhotic portal hypertension (INCPH) is a disease with an uncertain etiology consisting of non-cirrhotic portal hypertension and portal pressure increase in the absence of liver cirrhosis. In INCPH, patients exhibit normal liver functions and structures. The factors associated with INCPH include the following: Umbilical/portal pyremia, bacterial diseases, prothrombic states, chronic exposure to arsenic, vinyl chloride monomers, genetic disorders, and autoimmune diseases. Approximately 70% of patients present a history of major variceal bleeding, and treatment relies on the prevention of complications related to portal hypertension. Autoimmune disorders associated with INCPH are mainly systemic sclerosis, systemic lupus erythematosus and rheumatoid arthritis. To the best of our knowledge, a case of ankylosing spondylitis (AS) associated with INCPH has not been reported thus far. Therfore, we report our experience of a patient with AS accompanied by INCPH, who showed perisplenic varices with patent spleno-portal axis and hepatic veins along with no evidence of cirrhosis on liver biopsy, and provide a brief literature review.

The need assessment of visiting health services in a rural community (농촌지역의 방문보건서비스에 대한 요구조사)

  • The aim of this study was to assess the people's need for visiting health services in a rural area. In recent years, the great concern for the visiting health services has aroused in Korea. Stratified cluster sample for a household survey was used to select 1,255(8.4%) Households from Yonchon county. This study was undertaken from July 26 to August 7 in 1993. Medically defined need, usually expressed by the prevalencies or rates of specific disease, was evaluated with the use of criteria established by medical and nursing professors and expressed by the percent of specific objects for the visiting health services. Perceived need represented by the acceptability for the visiting services and willingness to paying for it, also, evaluated. The major results were as follows : 1. Of the 348 patients with hypertension, 201 were the non-compliant patients, the rate of the non-compliant hypertension patients in 4,577 study population was 7.4%. 2. Of the 141 diabetic patients, 73 were the non-compliant patients, the rate of the non-compliant hypertension patients in 4,577 sample population was 2.7%. 3. The number of patients with severe musculo-skeletal disease was 24, the rate was 0.9% above the age 30. 4. Of the 514 elderly, 33 were the elderly without any family member, the rate was 8.4%. Those with severely decreased activity of daily living were 13. 5. Infants with high risks were 12, pregnant women and neonates were 5, patients discharged within 1 week and with special equipments such as peritoneal dialysis, stoma, TPN etc. were 17, and patients with cancer were 5. Total number of the objects needed visiting health services was 752(18.43%) of the 4,577 study population. Perceived need evaluated were as follows; The acceptance rate of visiting health services was 74.9%, The kinds of visiting health services such as family health protection and promotion, early detection of hypertension, physiotherapy, health education and counseling were needed in the order of high rate The price willing to pay for visiting health services per visit was about 3,000-5,000Won. In conclusion, Visiting health services programme to be developed should have priority to the prevention of complications of chronic disease such as hypertension, diabetes milletus, elderly disease and health promotion.

Elastin Degradation and Collagen III Deficiency in the Superficial Temporal Arteries of Patients with Intracranial Aneurysms

  • Objective : We present the difference of histopathologic changes of the internal elastic lamina (IEL) and collagen III in the superficial temporal artery (STA) between aneurysmal patients and non-aneurysmal patients. Also, the pathologic data with clinical features by comparative methods to validate the risk factor of the intracranial aneurysm are presented. Methods : Samples of the STA were harvested form 38 patients including aneurysmal and non-aneurysmal patients undergoing craniotomy. Paraffin-embedded sections were examined, using hematoxylin and eosin, van Giebson and mouse anti-collagen III staining techniques. Histopathological observations were ana lysed and correlated with clinical features such as presence of aneurysm, hypertension, age, and sex. Results : Twenty-seven patients had the intracranial aneurysm. Of these 24 patients were 50 years old or older. Nineteen patients had a history of hypertension. Twenty patients were female. Histopathological study demostrated the derangement of IEL and the deficiency of type III collagen were prominent in aneurysmal patients (p < 0.05). Fifty years old or older patients did not show correlation with the deficiency of type III collagen, but with the derangement of IEL (p < 0.05). The female sex was not correlated with the derangement of IEL but with the deficiency of type III collagen (p < 0.05). However, Hypertension was not correlated with these pathologic data. Conclusion : Patients with intracranial aneurysms have severe histopathologic changes of the arterial wall showing the derangement of IEL and the deficiency of type III collagen. In the clinico-pathologic study, the advanced age and female sex were considered as risk factors of the intracranial aneurysm.

Correlation Between NT-proBNP and Pulmonary Arterial Pressure in COPD Patients (만성 폐쇄성 폐질환 환자에서 뇌나트륨이뇨펩티드와 폐동맥압과의 상관관계에 관한 연구)

  • Background: Pulmonary hypertension is one of the cardiovascular complications of in COPD. However, a diagnosis of pulmonary hypertension requires an invasive test, such as right heart catheterization. NT-proBNP is a cardiac hormone that is elevated when a cardiac volume or pressure overload is present. It was hypothesized that NT-proBNP might play a role in detecting of pulmonary hypertension in COPD patients. Method: The 31 COPD patients, who underwent all of NT-proBNP, echocardiography, and spirometry in Seoul National University Bundang Hospital during the period from November 2003 to July 2005, were retrospectively analyzed. Result: Of the 31 COPD patients, 9 patients had pulmonary hypertension. A significant positive correlation was observed between the NT-proBNP and pulmonary arterial pressure (r=0.589, p=0.002). However, there was no significant correlation observed between the $FEV_1$ and NT-proBNP and $FEV_1$ and pulmonary arterial pressure. Conclusion: NT-proBNP might indicate the presence of pulmonary hypertension in COPD patients.

Effects of an Experiences Sharing Self-management Program for Hypertension and Diabetes Patients in Primary Public Health Center (보건진료소 중심 고혈압, 당뇨병 환자의 경험공유 자가관리 프로그램 효과)

  • This study examined the effects of an Experience Sharing Self-management Program (ESSP) on physiological indexes, knowledge, and self-management implementation among hypertension and diabetes patients in a primary public health center in G city. A quasi-experimental research was conducted with a non-equivalent control group pretest-posttest design. The experimental group (n=30) participated in a 12-week ESSP, and the control group (n=25) received only general services of a primary public health center between May 1 and October 31 in 2018. Data were analyzed with x2 test (Fisher's exact test), Mann-Whitney test, and Ranked ANCOVA using SPSS/WIN 22.0 program. The results showed significant differences in systolic blood pressure (F=14.56, p<.001), diastolic blood pressure (F=7.05, p=.012), knowledge (F=14.30, p=.001), self-management implementation (F=29.73, p<.001) of hypertension patients, knowledge (F=7.92, p=.010), and self-management implementation (F=4.54, p=.044) of diabetes patients between the two groups. Therefore, the ESSP was effective for reducing the blood pressure of hypertension patients and improving the knowledge and self-management implementation among hypertension and diabetes patients in a primary public health center.
