Screening of Antioxidative, Anti-atherosclerotic Effect of Alisma Rhizome Extracts (택사 열수 및 에탄올 추출물의 항산화활성과 human LDL 산화억제 및 ACE 저해효과에 미치는 영향)

  • Objectives : The study was conducted to evaluate antioxidative, anti-atherosclerotic and anti-hypertensive effects of natural remedies. Alisma Rhizome (AR) has been used for a long time in Asia in folk remedies for treatment of hypertension and stroke and has been used in Korean traditional medicine for the treatment of glycosuria, gonorrhea, hypercholesterolemia, hypertension, and jaundice and its diuretic effect. These pharmacological effects of AR might come from antioxidant properties of phytochemicals in these materials. Methods : In this study, the antioxidant activity of extract from AR was studied with in vitro methods by measuring the antioxidant activity by TEAC, measuring the scavenging effects on reactive oxygen species (ROS) [superoxide anion, hydroxyl radical] and on reactive nitrogen species (RNS) [nitric oxide and peroxynitrite] as well as measuring the inhibitory effect on $Cu^{2+}$ induced human LDL oxidation and on ACE. Results : The AR extracts were found to have a potent scavenging activity, as well as an inhibitory effect on LDL oxidation and on ACE against all of the reactive species tested, with the water extract showing particularly strong antioxidant activities. Conculsions : The AR extracts have antioxidative, anti-atherosclerotic and anti-hypertensive effects in an in vitro system, which can be used for developing pharmaceutical drugs against oxidative stress and atherosclerosis.

Rosuvastatin Induces ROS-mediated Apoptosis in Human Prostate Cancer PC-3 Cells (Rosuvastatin이 유도하는 ROS가 전립선암 PC-3 세포주의 세포사멸 유도에 미치는 영향)

  • Statins, the inhibitors of 3-hydroxy 3-methylglutaryl coenzyme A (HMG-CoA) reductase, are widely used in treatments of hypercholesterolemia and newly known as anti-cancer effect of various cancer cells. Recently, several studies suggested that reactive oxygen species (ROS) play a critical role on cell death signaling. However, mechanism of ROS by rosuvastatin is currently unclear. This study aimed to explore the molecular mechanism of apoptosis by rosuvastatin in human prostate cancer PC-3 cells. Cell viability and apoptosis-related protein expression were measured by MTT assay and western blotting, respectively. In addition, the levels of apoptosis and ROS were analyzed. The results showed that rosuvastatin dramatically reduced cell viability in a dose- and time-dependent manner. We confirmed that rosuvastatin induced apoptosis through reduction of procaspase-3 and cleavage of poly (ADP-ribose) polymerase (PARP) in PC-3 cells. In addition, rosuvastatin stimulated ROS production in a dose-dependent manner and pre-treatment with N-acetylcysteine (NAC), a ROS scavenger, significantly recovered rosuvastatin-induced ROS and apoptosis. Thus, we concluded that rosuvastain induces apoptosis through generation of ROS in human prostate cancer PC-3 cells and provides a promising approach to improve the efficacy of cancer therapy.

Evaluation of Potential Drug-Drug Interactions in Patients Taking HMG CoA-reductase Inhibitors (HMG CoA-reductase inhibitors를 복용하는 환자의 잠재적 약물상호작용 연구)

  • Objective: The 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) are frequently prescribed medications worldwide for the treatment of hypercholesterolemia. Statins are considered to be well tolerated; however, they have a potential for myotoxicity. Concomitant drugs that inhibit cytochrome P450 3A4 can increase the concentration of statins and thus the risk of developing myotoxicity. The purpose of this study was to evaluate risk factors associated with potential drug-drug interactions in patients receiving statins. Methods: The subjects of this study were patients aged more than 18 years who received at least one prescription of statins in a general hospital located in Chuncheon-si, Korea, between January 1, 2018, and March 31, 2018. Data regarding statin use and baseline characteristics was collected from the computerized hospital database. Logistic regression analysis was used to identify risk factors associated with potential drug-drug interactions. Results: A total of 1061 patients were finally included in the study. The incidence of potential drug-drug interactions was 45% in all subjects. According to the results of the multivariate logistic regression analysis, myocardial infarction as the indication of statin, arrhythmia or heart failure as a comorbidity, and aspartate aminotransferase levels higher than 40 IU/L were significant risk factors for potential drug-drug interactions in study subjects. Diltiazem was the most commonly co-prescribed drug that caused potential drug-drug interactions with statins. Conclusion: There was a considerable rate of potential drug-drug interactions in patients receiving statins. Health care professionals should attempt to reduce potential drug-drug interactions during statin administration.

Effect of Rhynchosia Nulubilis Ethanolic Extract on DOPA Oxidation and Melanin Synthesis (서목태 주정 추출물이 DOPA 산화와 멜라닌 합성에 미치는 영향)

  • Melanin is a polymer substance that plays an important role in the determination of hair growth and skin color in vivo. However, melanin, which is over-produced by reactive oxygen species, is known to cause stains, freckles, and hypercholesterolemia, which are associated with aging. Previous studies have shown that polyphosphate, one of the components of Rhynchosia Nulubilis, inhibits skin aging induced by ultraviolet rays. The aim of this study is to investigate the direct effect of Rhynchosia Nulubilis ethanolic extract (RNEE) on melanin synthesis. In this study, RNEE showed no antioxidative effects on scavenging activity of DPPH radical in addition to reducing power. The cytotoxicity of RNEE was increased in a dose-dependent manner in an MTT assay. In addition, RNEE increased tyrosinase activity and melanin synthesis in DOPA-oxidation experiments. RNEE did not promote the conversion L-DOPA into melanin in live cells, but melanin production was promoted in the RNEE-treated group after H2O2 pretreatment compared to the control group in which melanin production was reduced by treatment with H2O2. In addition, RNEE increased the expression level of tyrosinase related protein-2 (TRP-2) and increased the expression level of tyrosinase related protein-1 (TRP-1) at a concentration of $16{\mu}g/ml$. In particular, it was found that RNEE increased the expression level of SOD-3, by which superoxide anion is converted to hydrogen peroxide, higher than the control and ${\alpha}$-MSH used as a positive control at a concentration of more than $16{\mu}g/ml$. The results suggest that RNEE can induce melanogenesis related to black hair.

Effects of White Mulberry (Morus alba) Leaves on Blood Vessel Reactivity in Hyperchloesterolemic Rats

  • In atherosclerosis, blood vessels become sensitive to vessel-constricting agents leading to reduced control in the event of abrupt blood pressure changes. Mulberry trees (Morus alba L., MA) have been claimed to contain various bioactive principles that could possibly prevent atherosclerosis development caused by high cholesterol consumption. In order to examine whether MA feeding can prevent the sensitization of blood vessels, MA leaves were fed to rats for 8 weeks and pressor responses to vasoconstricting agents were assessed. Animals were pithed before blood pressure assessments to eliminate reflex compensation in vessel responses. Feeding diets containing high levels of cholesterol led to potentiated pressor responses to sympathetic nerve stimulation, or to injection of norepinephrine, phenylephrine, angiotensin II and vasopressin in pithed rats. These potentiated pressor responses were prevented in rats fed MA leaf-containing diets at 2 or 10% levels. It was also examined in anesthetized non-pithed rats whether similar cholestrol-related sensitization and MA prevention could be observed. However, high cholesterol-induced sensitization in pressor responses were not observed, suggesting that destruction of central cardiovascular control by pithing must have revealed the sensitization responses. It was concluded that MA leaves seem to be active in preventing abnormal blood vessel reactivity caused by hypercholesterolemia.

Early Result of Coronary Artery Bypass Surgery (관상동맥 우회술의 조기성적)

  • From July 1994 to August 1995, 32 patients underwent coronary artery bypass surgery. There were 14 men and 18 women. The mean age was 59 years (range from 37 to 81 years). Preoperatively 26 patients had unstable angina pectoris and 6 patients had stable angina pectoris. Nine patients had previous myocardial infarction hi tory. Five patients had preoperative left ventricular ejection fraction of 40% or less, The involved risk factors were as follows ; smoking 19 cases, hypertension 16 cases, hypercholesterolemia 14 cases, diabetes mellitus 6 cases, and obesity 3 cases.21 patients had three-vessel disease, 7 patients had two-vessel disease, 2 patients had one-vessel disease and 2 patients had left main coronary artery disease. We performed 103 distal bypasses out of 32 cases, and the mean number of grafts per patients is 3.22. We used arterial grafts (left internal mammary artery,)1, radial artery; 2) in 32% of total grafts. Postoperative complications were low cardiac output, perioperative myocardial infarction, respiratory failure and atrial fibrillation, etc. Early mortality was 6.25% (2/32). The causes of deaths were low cardiac output (1), and perioperative myocardial infarction(1).

Risk Factors of Coronary Artery Bypass Grafting According to Ventricular Function (좌심실기능에 따른 관상동맥우회술의 위험인자)

  • Patients with coronary artery disease and depressed ventricular function have better longterm benefits after coronary artery bypass grafting compared with medical therapy. But operative mortality remains high. This study was designed to identify the risk factors for coronary artery bypass grafting according to ventricular function. The records of 103 patients who underwent coronary artery bypass grafting from truly 1994 to June 1996 were analysed. The patients were divided into two groups based on preoperative ejection fraction: Low EF group(Ejection fraction < 40%, n=24) and Normal EF group(Ejection fraction $\geq$ 40%, n=79). The indication of operation was significantly different between the two groups (p=0.00003). Postinfarction angina was frequent in Low EF group but unstable angina was frequent in Normal EF group. The frequency of cardiomegaly(p=0.0012), serum creatinine abnormality(p=0.0473) and preoperative use of IABP(Intra Aortic Balloon Pump, p=0.0095) were higher in Low EP group. The left internal thoracic artery was used less frequently in Low EP group(p=0.00416). The operative mortality was 8.3% in Low EF group and 5.1% in Normal EF group, but without statistical difference(p=0.5492). In Normal EF group, age (p=0.041) was identified as a significant risk factor for operative mortality. In Low EF group, age(p=0.018), preopertive use of IABP(p=0.0036), hypercholesterolemia(p=0.0007), and emergency of operation(p=0.0037) were identified as significant risk factors. Postoperative morbidity was 50% in Low EF group and 33olo in Normal EP group, but without s atistical significance(p=0.1007). These results suggest that in patients with coronary artery disease and depressed ventricular function, more aggresive coronary artery bypass grafting is needed to improve the symptom and long-term benefit.

Effects of Angelica keiskei on Lipid Metabolism in Rats (Angelica keiskei가 흰쥐의 지질대사에 미치는 영향)

  • To investigate the effects of Angelica keiskei Koidz on cholesterol metabolism, male Sprague Dawley rats were fed diets containing 5% of A. keiskei flour for 6 weeks. A. keiskei decreased plasma contents of total cholesterol, LDL-cholesterol and triglyceride while showing no change in HDL-cholesterol, resulting in reduction of atherosclerotic index. Decrease in liver concentration of triglyceride and increases in fecal excretion of cholesterol, total neutral steroid and bile acid were also observed. Those results suggest that A. keiskei improves hypercholesterolemia through, at least in part, reducing the absorption of cholesterol and enhancing the catabolism of cholesterol to bile acids.

Relationship of Level of Stress, Life Style, Subjective Symptoms and Clinical Diagnosis in Clients taken Multiphasic Screening Program (종합검진 수검자의 스트레스 수준과 생활습관, 자각증상 및 임상적 진단의 관련성)

  • To improve wellness and quality of life by recognizing the health efforts of stress, the author estimated the relationships between stress, subjective symptoms and clinical diagnosis through a questionnaire and a battery of specified laboratory tests; electrocardiography, blood pressure, cholesterol, aspartate aminotransferase(AST), alanine aminotransferase(ALT), gamma glutamyl transferase$(\gamma-GTP)$, fasting blood sugar, gastro-endoscopy or UGI, abdominal sonography, etc. The data was gathered from 337 clients who were undergoing multiphasic screening program at a University Hospital from January to March 1998. The mean age of subjects was $46.5{\pm}11.2$ years and the mean of body mass index was $24.0{\pm}3.7kg/m^2$. The mean vol of stress was $18.5{\pm}6.0$ expressed as the score out of 40. By general characteristics and lift style among male, mean level of stress was significantly higher in case of lower socioeconomic status, habitual drug use, longer daily working time(>10 hours), no regular exercise, drinkers, irregular meal, skip-ping breakfast(p<0.05). In case of female, that was significantly higher in case of lower education, lowe. socioeconomic status, longer daily working time(>10 hours), no regular exercise, drinkers, smokers, irregular meal, skipping breakfast(p<0.05). Significant correlations were observed between stress and subjective symptoms in all kinds of organ system (p<0.01). Correlation coefficients of stress among male were relatively high with neuro-psychiatric symptom$(\gamma=0.476)$ and cardio-vascular symptom$(\gamma=0.361)$ in order, and correlation coefficients of stress among female was highest with neuro-psychiatric symptom$(\gamma=0.371)$. The prevalence of the diagnosis through the battery of laboratory tests was high in upper gastrointestinal disorders and hypercholesterolemia in order in both sex group. Among male the mean score of stress was significantly high in ulcerative peptic disorder of upper gastrointestine and hepatopathy in order (p<0.05) . Among female that was significantly high in diabetes mellitus. In summary, it is likely that there are associations between stress, subjective symptoms and clinical diagnosis. To promote wellness and quality of life it would be of value that periodic stress evaluation program and stress management including apropriate control of smoking and drinking, regular exercise and meal.

Quantitative Analsysis of Flavanone Glycosides and Peroxynitrite Scavenging Effect of the Five Oriental Medicinal Drugs (Aurantii nobilis Pericarpium, Citrii unshiu Pericarpium, Citrii unshiu Semen, Aurantii Fructus, Poncirii Fructus) (5종 생약(진피, 청피, 귤핵, 지실, 지각)의 Flavanone Glycoside 함량분석과 Peroxynitrite 소거효과)

  • Overproduction of peroxynitrite ($ONOO^-$) causes a variety of disease such as atherosclerosis, hypercholesterolemia, diabetes mellitus or obesity. Peroxynitrite scavenging activities and HPLC analysis on the five Oriental medicinal drugs belonging to the genus Citrus, Aurantium or Poncirus (Rutaceae family) and HPLC analysis were taken to evaluate flavanone glycosides with peroxynitrite scavenging activity. The $IC_{50}s$ of the five crude drugs were shown as follows: Aurantii nobilis Pericarpium (Jinpi, 18.3 ${\mu}g$/ml), Citrii unshiu Pericarpium (Chungpi, 7.50${\mu}g$/ml), Citrii unshiu Semen (Gyulhaek, >50.0${\mu}g$/ml), Aurantii Fructus (Jigak, 18.3${\mu}g$/ml), and Poncirii Fructus (Jisil, >50.0${\mu}g$/ml) where Korean crude drug's names are noted in the parenthesis. Peroxynitrite scavenging effect of flavanones or their glycosides usually contained in Citrus species were observed as follows: hesperetin (1.89 ${\mu}g$/ml), naringenin (7.77 ${\mu}g$/ml), hesperidin (8.44 ${\mu}g$/ml), poncirin (>50.0 ${\mu}g$/ml)and ponciretin(>50.0 ${\mu}g$/ml). The activities of naringin and poncirin with ${\alpha}$-L-rhamnopyranosyl($1{\rightarrow}2$)-${\beta}$-D-glucopyranosyl moiety were weak. HPLC analytical data revealed that Jinpi (the peels of mature fruits of Citrus unshiu) and Chungpi (the peels of immature fruits of C. unshiu) had high quantities of hesperidin as the value of 142.1${\pm}$0.21 and 104.51${\pm}$1.10 mg/g dried weight, respectively. Poncirin was clearly detected in only Jisil and naringenin and naringin were not observed on the HPLC chromatogram of the five crude drugs.
