고칼슘혈증을 보인 폐암환자에서 부갑상선호르몬(PTH)과 부갑상선호르몬관련단백질(PTHrP)이 동시에 증가된 1예 (Simultaneous Elevation of Serum Parathyroid Hormone(PTH) and Parathyroid Hormone-related Protein(PTHrP) in a Case of Lung Cancer with Hypercalcemia)

  • 악성질환에서 체액성 고칼슘혈증은 주로 부갑상선호르몬관련단백질(parathyroid hormone related hormone ; PTHrP)이 매개물질로 작용하여 발생한다. 이러한 PTHrP포 인한 고칼슘혈중이 발생한 경우에는 원발성부갑상선기능항진증을 동반한 경우를 제외하고는 모든 환자에서 부갑상선호르몬(PTH) 농도는 저하되며 PTHrP는 증가하는 것으로 알려져있다. 특히 부갑상선이외의 종양에서 이소성 부갑상선호르몬을 생성한다거나 PTH와 PTHrP를 동시에 산생하는 경우는 매우 드물다. 저자들은 고칼슘혈증을 동반한 편평상피세포 폐암환자에서 혈청 부갑상선호르몬(PTH)과 부갑상선호르몬관련단백질(PTHrP)이 동시에 증가된 1례를 경험하여 이에 보고한다. 61세 남자 환자로서 혈청 칼슘은 7.5 mEq/L로 증가되어 있었으며, iPTH와 PTHrP(C 말단 부위 : 109-138)는 각각 150 pg/mL, 99.1 pmol/L로 모두 상승되어 있었다. 골전이나 부갑상선 기능 장애의 동반여부를 알기 위해서 시행한 골스캔이나 경부초음파, 전산화단층촬영, Tc-99m MIBI 스캔 검사에서 모두 정상소견을 보였다. 결론적으로, 본 증례는 고칼슘혈증을 보인 편평상피상피세포 폐암환자에서 PTH와 PTHrP가 동시에 증가된 경우이다. 이러한 매개물이 동시에 증가한 원인으로 PTH와 PTHrP을 분비하는 타병변이 있을 수 있지만, PTH와 PTHrP를 동시에 분비할 수 있는 주된 기관인 부갑상선에 대한 상기 검사상 특이한 이상소견이 없었던점으로 미루어 보아 편평상피세포 폐암조직으로부터 이소성 PTH와 PTHrP가 동시에 분비되었을 가능성도 있다.

Brown Tumor of the Thoracic Spine : First Manifestation of Primary Hyperparathyroidism

  • Brown tumors also called as osteoclastomas, are rare nonneoplastic lesions that arise in the setting of primary or secondary hyperparathyroidism. Parathyroid adenomas or hyperplasia constitute the major Brown tumor source in primary hyperparathyroidism while chronic renal failure is the leading cause in secondary hyperparathyroidism. Most of the patients with the diagnosis of primary hyperparathyroidism present with kidney stones or isolated hypercalcemia. However, nearly one third of patients are asymptomatic and hypercalcemia is found incidentally. Skeletal involvement such as generalized osteopenia, bone resorption, bone cysts and Brown tumors are seen on the late phase of hyperparathyroidism. The symptoms include axial pain, radiculopathy, myelopathy and myeloradiculopathy according to their locations. Plasmocytoma, lymphoma, giant cell tumors and metastates should be ruled out in the differential diagnosis of Brown tumors. Treatment of Brown tumors involve both the management of hyperparathyroidism and neural decompression. The authors report a very rare spinal Brown tumor case, arisen as the initial manifestation of primary hyperparathyroidism that leads to acute paraparesis.

자연경색을 동반한 부갑상선암 1예 (A Case of Parathyroid Carcinoma with Spontaneous Infarction)

  • Parathyroid carcinoma is very rare malignant neoplasm, accounting for less than 0.005% of all cancers. Most parathyroid carcinoma is a functioning tumor that causes hyperparathyroidism, leading to hypercalcemia. We report a parathyroid carcinoma case that was suspicious for spontaneous infarction of cancer, leading to resolution of hypercalcemia. A 29-year-old male visited our hospital presenting with right neck swelling and pain. He has been experiencing frequent urolithiasis for four years but laboratory tests showed normal serum calcium level. Right vocal cord paresis was identified with laryngoscopy. Ultrasonography revealed a 3.7 × 3.5 cm mass in the right thyroid containing a focal cystic portion. Computed tomography confirmed the presence of a low-density right thyroid mass. Right thyroid lobectomy was performed and pathological evaluation revealed parathyroid carcinoma with central necrosis. We report this very rare case with a literature review.

94 마리 고칼슘혈증 개들에 대한 회고연구(2002-2004) (A Retrospective Study of 94 Hypercalcemic Dogs(2002-2004))

  • 2002년부터 2004년 까지 건국대학교 부속동물병원에 의뢰된 환축 중 고칼슘혈증견을 나타내는 94마리를 대상으로 원인질병을 조사하였다. 연구 기간 동안 고칼슘혈증은 총 94 마리, 19 견종에서 발견되었으며, 이들을 증례군에 포함시켰다. 대조군은 동일 연구기간 동안 고칼슘혈증이 발생되지 않은 총 94마리, 18 견종으로 구성되어졌다. 일반 품고 들에 대해서는 나이 분포를 제외하고는 증례군과 대조군 간에 현저한 차이가 없었다. 시츄(17.02%)와 요크셔 테리어(26.60%)가 증례군과 대조군에서 각각 가장 일반적인 품종이었다. 고칼슘혈증의 가장 일반적인 원인은 만성 신부전(14.89%) 이었으며, 그 다음으로 급성 신부전(14.89%), 신결석(6.38%)순이었다. 악성종양(림프종, 혈관육종, 만성 림프구성 백혈병, 유선 종양 및 다발성 골수종) 내분비계 질환(부신피질 기능항진증, 갑상선 기능항진증, 부신피질 기능저하증 및 갑상선 기능저하증)은 고칼슘혈증의 원인 중 8.5%와 6.4%를 각각 나타내었다.

Clinicopathology Profile and Bone Involvement of Multiple Myeloma Patients in Dharmais National Cancer Hospital, Indonesia

  • Background: Even though rarely found in Indonesia, the incidence of multiple myeloma (MM) is increasing every year. Bone involvement of MM is the most often a clinical disorder which leads to worsening clinical conditions and low quality of life of patients. Purpose: To determine the clinicopathology profile of bone involvement of MM patients in Indonesia. Materials and Methods: The cross-sectional study of MM conducted at Dharmais National Cancer Hospital (DNCH) by collecting data from medical records of MM patients who came to DNCH in period 2008-2012. Results: There were 39 MM patients all with age above 60 years. There were 56.4% male and 43.6% female patients. Most were diagnosed at stage III (32.4%), and 41% had obesity. The comorbid conditions were anemia (82.9%), hypoalbuminemia (60%), increased creatinine level (38.5%), increased ${\beta}2$ microglobulin level (94.1%), increased LDH level (23.1%) and plasmocytes above 30% (65%), but only 4.2% patients presented with hypercalcemia. Meanwhile, bone involvement occurred in 76.9% of MM patients with 4 lesions on average and a maximum of 16 lesions. The locations of bone lesions were spine (70%), skull (70%), pelvis (33.3%), humerus (30%), and femur (30%). Conclusions: The incidence of MM in Indonesia is increasing annually with bone involvement in more than three-fourths, but interestingly without hypercalcemia.

  • Introduction: A metastatic calcification is known for taking in bone scintigram medicine at metastatic calcification lesion due to abnormal distribution of the calcium and phosphorus. The one paper reports that a metastatic calcification occurs mainly at lung, stomach, kidney and myocardium. Index: The patient is seventy four years old man who is afflicted with clonic kidney disease, hypercalcemia, hypertension. Because of an ability of the multiple myeloma, we take a bone scan after intravenous injection $^{99m}Tc$-DPD 25 mCi in three hours. We found out homogeneous $^{99m}Tc$-DPD uptake at both lung and myocardium. Conclusions: Nothing unusual was found in other bone scan. We obtains a purity beyond 95 percent at $^{99m}Tc$-DPD vial. In spite of no evidence about a myocardial infarction, the patient has a $^{99m}Tc$-DPD uptake at both lung and myocardium.

골스캔상 신장의 미만성 방사능집적 증가소견의 임상적 의의 (Evaluation of Significance of Diffusely Increased Bilateral Renal Uptake on Bone Scan)

  • Unexpected renal abnormality can be detected on bone scan using Tc-99m-MDP. The purpose of the study is to evaluate the diagnostic significance of diffusely increased bliateral renal uptake on bone scarf. 1,500 boor scan were reviewed and 43 scans which showed diffusely increased bliateral renal uptake were selected for analysis. Laboratory findings for renal and liver function tests including routine urinalysis were reviewed in 43 patients. 26 of 43 case showed abnormality in urinalysis and renal function stud 20 of 43 cases showed abnormal liver function study and 3 of these cases were diagnosed as hepatorenal syndrome later. 13 of those 20 cases had liver cirrhosis with or without hepatoma. 12 of 43 cases showed abnormality both in renal and liver function studies. 2 of 43 cases showed diffusely increased bilateral renal uptake after chemotherapy for cancer but not on previous scans before chemotherapy. 2 of 43 cases showed hypercalcemia and 8 of 43 cases had multifocal bone uptake due to metastasis or benign bone lesion. but the latter showed no hypercalcemia at all. There was no significant correlatrion between increased renal uptake and MDP uptake in soft tissue other than kidneys. This study raised the possibility that the impaired liver and/or renal function may result in diffuse increase of bliateral renal uptake of MDP of unknown mechaninsm. It seems to need further study on this correlation.
