손위생 교육프로그램이 간호대학생의 손위생 지식, 손위생 인식, 비강 내 황색포도알균 집락 및 손위생 이행에 미치는 효과 (The Effect of the Hand Hygiene Education Program on Hand Hygiene Knowledge, Hand Hygiene Perception, Nasal Staphylococcus aureus Colonization and Hand Hygiene Adherence in Nursing Students)

  • Purpose: This study was conducted to test the effect of the Hand Hygiene Education Program on hand hygiene knowledge, hand hygiene perception, nasal Staphylococcus aureus colonization and hand hygiene adherence in nursing students. Methods: A non-equivalent pre-post test of quasi-experimental design was used. 87 second grade nursing students participated in the study with 43 in the experimental group and 44 in the control group. We used the Hand Hygiene Education Program which was held 5 times over 5 weeks, taking 60 minutes per session. For the analysis, descriptive statistics, chi test, and t-test were used for statical analysis with SPSS 19.0. Results: There were significant increases in hand hygiene knowledge (p=.004) and hand hygiene adherence (p=.002) and there was a significant decrease in nasal Staphylococcus aureus colonization (p=.026) in the experimental group compared to the control group. However, hand hygiene perception (p=.543) was not significantly changed. Conclusion: Findings of this study suggest that the Hand Hygiene Education Program may be effective in enhancing hand hygiene knowledge and hand hygiene adherence. Also this program was effective in reducing nasal Staphylococcus aureus colonization in nursing students. Further studies are needed to evaluate the effects of the Hand Hygiene Education Program on hand hygiene perception in nursing students.

Effects of Infection Control Training on Dental Hygienists' Health Beliefs and Practices of Infection Control

  • This study aims to determine the changes in health beliefs and practices of dental hygienists on infection control after having received special training on that subject. The study population consisted of dental hygienists working at the dental institutions located in Seoul and Gyeonggi areas. The intervention group and the control group each had randomly assigned 26 dental hygienists. The intervention group received training courses on infection control once a week for 3 hours, for a total of 4 training sessions. We used a survey tool to find the changes between the two groups regarding their health beliefs and practices on infection control measures. The survey was conducted prior to the training sessions, 3 months and 2 weeks after the training. The study results revealed statistically significant increases in perceived sensitivity, perceived seriousness, and perceived benefits after the training courses in the intervention group (p<0.05). A statistically significant decrease in perceived barriers was also noted in the intervention group (p<0.05). Additionally, there was a statistically significant increase in the practice of infection control in the intervention group as compared to the control group (p<0.05). Therefore, we conclude that infection control training is crucial in bringing about positive changes to the health beliefs of dental hygienists, and a structured system is necessary for continuous infection management along with training in order to improve infection control practices.

요양보호사의 구강건강관리실태 및 구강건강관리교육 요구도 조사 (Oral health care status and educational needs of care workers)

  • Introduction: This study intended to identify the current oral health care status and demand of care workers for oral health education. Methods: A survey was distributed to care workers working in 11 nursing homes for older people located in the Chungcheongdo Province. Of those distributed, 217 questionnaires were collected and analyzed. To analyze the collected data, a frequency analysis, t-test, and one-way analysis of variance(ANOVA) were performed using SPSS version 18.0. Results: The demand for an educational course on the 'Management of Oral Health Care for the Aged People' had a score of 4.22 points(full marks were 5.0 points), whereas the score for the necessity for control of oral health was 4.29 points. The control of oral health for the aged people suffering dysphagia scored 4.27 points, whereas the control of oral health for older people who have dementia was 4.27 points. The score for a course on the nutritional control for aged people having difficulties in masticating foods was 4.27. Conclusion: It is clear that the development of educational courses and standardized manuals for care workers on aspects of oral health care is necessary. Therefore, it would be desirable to develop institutional infrastructure for dental hygienists to educate care workers on oral health.

전국 치위생과 학생들의 B형간염과 AIDS에 관한 지식도 조사 (A study on Hepatitis B and knowledge about AIDS of dental hygiene student in Korea)

  • The purpose of this was to evaluate the level of knowledge about Hepatitis B and AIDS among dental hygiene students at six dental hygiene educational programs in nationwide, compare the findings to those of Song's study. 1703 dental hygiene students surveyed by mail and answered a questionnaire, using the questionnaire used developed by DiClemente el al., on Hepatitis B vaccination, antibody production, experience of unintentional needle stick injuries, experience of surgical operation and blood transfusion, personal risk factors for HBV and HIV, management of HBV and HIV postexposure management et al. The data indicated that dental hygiene students (79.0%) in this study had more HIV vaccination than dental hygiene(74.3%) in Song's study. As for recognition of antibody production after Hepatitis B vaccination, only 34.6% of respondents answered they knew having antibody production and 95% of respondents knew not having antibody production. Over one-half of the respondents (55%) answered they didn't even recognize having antibody production. 52.3 percent of dental hygiene students experienced unintentional needle stick injuries. The findings on the knowledge about Hepatitis B and AIDS were that dental hygienists (76.5 points) received higher mean score than those of dental hygiene students (71.0 points) and that the distribution of right answer rate also showed wider range in dental hygiene students (27.7~97.5%) than those of dental hygienist (41.2~99.5%). There were no statistical differences among dental hygiene education programs. Finding of this study support that the curriculum of dental hygiene program should include instruction on sources and methods of transmission of infectious diseases, risk of virus transmission in the workplace and principles of infection control. Furthermore, infection-control practices should be utilized routinely and their application to dental personnel, management of HBV and HIV postexposure management. Furthermore, for preventing the further spread of infectious diseases (HBV, HIV and AIDS${\cdots}$) caused by bloodborne viruses, imposing an obligation rather than recommendation on Hepatitis B vaccination to all dental personnel and routinely utilizing infection-control guidelines for all dental patients in dental practices (include dental educational programs) should be accomplished by coordination the government agency.

Effect of Micronutrient Supplementation on the Growth of Preschool Children in China

  • The purpose of this study was to investigate the effects of micronutrient supplementation on the growth of preschool children in China. A double-blind, placebo-controlled trial was conducted on 156 growth retarded preschool children who were randomly assigned to the following five groups : supplemental control (S-control; n=28); zinc supplementation (+Zn; 3.5mg Zn/day, n=34); zinc and calcium supplementation (+ZnCa; 3.5mg Zn + 250mg Ca/day, n=37); zinc, calcium and vitamin A supplementation (+ZnCaVA; 3.5mgZn + 250mgCa + 200gVA/day, n=28); and calcium and vitamin A supplementation (+CaVA; 250mgCa + 200gVA/day, n=29). Another 34 children of normal height were selected as a normal control (N-control). Supplementation continued for twelve months. After supplementation, the height gains in the +Zn group (7.84cm per year) and the +ZnCa group (7.70 cm per year) were significantly higher than that in the S-control group (6.74 cm per year, P<0.05). The weight gain in the +ZnCaVA group (2.55kg per year) and the +CaVA group (2.57 kg per year) was also significantly higher than that in the S-control group (2.19 kg per year, P<0.05). The average number of days of illness in each group taking supplements was lower than that in the S-control group (13 days per year compared with 23 days per year). No significant differences in bone maturity were observed between the groups. In conclusion, in this study Zinc and Zinc + Calcium supplementation improved the height gain, and vitamin A improved the weight gain, in growth retarded preschool children, but these supplements did not affect the maturity of bone. Micronutrient supplementation also lowered the morbidity of these children.

치과감염관리 영향 요인에 대한 연구 -치과근무자의 손세척 및 장갑사용 실태를 중심으로- (A Study on Affect Factors in Dental Infection Control: Focus on Wash Hand and Put Gloves on of a Dental Step)

  • 치과위생사의 감염관리 교육, 감염관리에 필요한 보호장비의 구비 및 착용 등이 모두 치과병원장과 관련된다. 따라서 본 연구는 치과 감염관리에서 치과병원장의 중요성에 대하여 살펴보고자 하였다. 1. 치과감염관리에 대한 추가교육이 치과위생사의 94.2%가 필요하다고 응답하였으며, 교육의 주체는 치과위생사협회가 되어야 한다고 47.1%가 응답하였다. 2. 치과의사의 관심도가 높을수록(높은편이다 이상 36.7%, 낮은편이다 이하 16.0%) 치과위생사가 교육경험이 높고, 치과의사의 관심도가 낮을수록(높은편이다 이상 18.8%, 낮은편이다 이하 31.3%) 치과위생사의 교육경험이 낮은 것으로 나타났다. 따라서 치과의사의 관심은 곧 치과위생사의 감염관리 교육과 관련있음을 알 수 있었다. 그러나 통계적으로 유의하지는 않았다. 3. 치과위생사의 의료용 장갑 사용은 매환자 사용이 42.4%, 필요시 51.8%로 통계적으로 유의한(<.001)것으로 조사되었다. 치과의사는 진료전 70.7%만이 항상 손세척을 하며 필요시 22.0%, 안하는 경우도 7.3%로 조사되었다. 이 결과는 통계처리 결과 유의한 것으로 조사되었다(<.001). 4. 감염관리에서 가장 의식개혁이 필요한 사람은 치과병원장이 66.5%, 치과위생사 31.9%로 조사되었다. 5. 감염관리가 잘 되기 위해서는 병원장의 의지가 37.2%로 가장 중요한 것으로 조사되었다. 다음으로 치과위생사의 26.2%, 법적규제와 의료보험 수가에 반영이 각각 20.4%, 15.2%로 조사되었다.

임상실습 중인 간호학생의 손 위생 시점 동영상 실습교육의 효과 (Effect of Video Education on the Moment of Hand Hygiene among Nursing student in Clinical Practicum.)

  • 목적 : 본 연구는 임상실습 중인 간호학생들에게 손 위생 시점에 관한 동영상 프로그램을 적용한 후 손 위생 시점지식과 수행을 검정하는 유사실험연구로 비동등성 대조군 전후 설계이다. 방법 : 2017년 3월 2일부터 6월 30일까지 3개 간호대학 92명의 학생들을 실험군 44명, 대조군 48명으로 나누어 자료를 수집했다. 결과 : 1) 손 위생 시점 동영상 실습교육프로그램 교육 후 손 위생 지식은 실험군이 대조군보다 높았다(t =6.30, p <.001). 2) 손 위생 시점 동영상 실습교육프로그램 교육 후, 손 위생 시점 지식은 실험군이 대조군보다 높았다(t =6.34, p <.001). 3) 손 위생 시점 동영상 실습교육프로그램 교육 후, 손 위생 수행은 실험군이 대조군보다 높았다(t =3.82, p <.001). 4) 손 위생 시점에 관한 지식과 손 위생 수행은 상관관계가 있었다(r =.458, p <.001). 결론 : 손 위생 시점 동영상 교육 프로그램은 손 위생 시점에 대한 지식과 손 위생 수행 향상에 효과적인 중재임을 알 수 있었다.

Effects of Professional Toothbrushing and Instruction in the Elderly: A Randomized Trial

  • The purpose of this study was to identify the effect of providing toothbrushing by professionals as part of oral hygiene education for the elderly. We randomly visited 12 centers in the metropolitan city and analyzed the data of 114 participants among 310 early registrants aged >65 years, who participated in the study at all 3 time points. The subjects were categorized into an experimental group (odd-numbered visits) and a control group (even-numbered visits). Oral hygiene practices were provided for both experimental and control groups, but professional toothbrushing was performed in a different manner in the experimental group. Differences in plaque index (PI) according to the subjects' general characteristics and oral health status were assessed using the t-test, and the effect of PI difference between the experimental and control groups was assessed by repeated measure two-way analysis of variance. A stepwise multiple regression analysis was used to analyze factors affecting the PI. At baseline, the mean overall PI was 61.82. In both experimental and control groups, the PI significantly reduced from the baseline (p<0.01). At 5 weeks, the experimental group showed a decrease of 27.16 points from the baseline, and the 10-week PI was similar to the 5-week PI. The control group showed a decrease of 14.87 points from the baseline, and the PI increased by 5.74 points at 10 weeks. PI-related factors were gender and self-xerostomia. The PI was lower in the female group and the group with self- xerostomia (p<0.01). It is important to select an appropriate method to educate elderly subjects on proper removal of dental plaque and to habituate correct behavior, which requires inclusion of a direct toothbrushing intervention by a specialist.

Antibacterial Effects of Tea Tree Oil and Mastic Oil to Streptococcus mutans

  • Background: Tea tree oil has antiviral, antimicrobial and antifungal effects and Mastic oil has antifungal and anticancer effects. For synergistic effects of oils, blending oil containing a mixture of two to three oils is recommended. This study aimed to determine the antibacterial effects of Tea tree oil, Mastic oil, and Blending oil containing the two oils in a mixture, to verify and suggest the potential use of these oils as a substance to prevent dental caries. Methods: Tea tree oil, Mastic oil, and Blending oil with a 1:1 blend of the two oils were diluted in liquid medium to 0% (negative control), 0.5%, 1.0%, and 2.0%. Streptococcus mutans was applied to each experimental group of the three diluted oils and after 8 h culture, the optical density (OD) was measured and the growth inhibition rate for S. mutans was estimated. Results: Tea tree oil had significantly low OD values across all concentrations (p<0.05) without significant variation among different concentrations (p>0.05). Mastic oil did not significantly vary in OD compared to the negative control across all concentrations (p>0.05) without significant variation among different concentrations (p>0.05). Blending oil, compared to the negative control, did not significantly vary in OD at 0.5% (p>0.05) but significant variation was found as the concentration increased (p<0.05). Additionally, for Tea tree oil and Mastic oil, the growth inhibition rate showed no significant variation according to concentration (p>0.05), whereas for Blending oil, the growth inhibition rate for S. mutans showed a significant difference at 1.0% (p<0.05) and at higher concentrations. Conclusion: Blending oil containing a Tea tree oil and Mastic oil demonstrated a significant growth inhibition effect on S. mutans from the concentration of 1.0%, which suggested its potential use as an effective antibacterial agent for dental caries.

치과위생사의 일회용 치과진료용품 감염관리 인지도와 재사용자율의 차이 (Differences in dental hygienists' infection control awareness and re-user rate of disposable dental care supplies)

  • Objectives: The purpose of this study is to identify the difference between the awareness and reuser rate of infection control t for disposable dental care supplies (DDCS) according to general characteristics and infection management-related characteristics. Methods: A questionnaire was used for 277 dental hygienists to check the general characteristics, infection management-related characteristics, awareness of infection control disposable dental care products, syringe needle, prophylaxis cup, prophylaxis brush, plastic saliva ejector, orthodontic bracket, and gloves reuse rate. Results: The awareness of infection control for DDCS differed according to 'hospital type', 'average number of patients per day', 'presence or absence of infection control guidelines', and 'experience in infection management training in the last two years' (p<0.05). Reuser rates of disposable dental care products differed according to 'hospital type', 'average number of patients per day', 'presence or absence of infection control guidelines', and 'experience in infection management training in the last two years' (p<0.05). Conclusions: In order to manage infection of DDCS, the level of infection control system in the workplace is improved and support for related education is needed. In addition, guidelines and regulations on prohibition of reuse and classification criteria for various DDCS should be prepared.
