현대의 고도화, 자동화된 시스템이 파생한 휴먼에러에 관한 이론적 고찰을 통한 리스크 대응전략 설정 (A Study on Countermeasure Strategy on Risk of Human Errors driven by Advanced and Automated Systems Through Consideration of Related Theories)

  • This paper provides an integrated view on human and system interaction in advanced and automated systems, which adopting computerized multi-functional artifacts and complicated organizations, such as nuclear power plants, chemical plants, steel and semi-conduct manufacturing system. As current systems have advanced with various automated equipments but human operators from various organizations are involved in the systems, system safety still remains uncertain. Especially, a human operator plays an important role at the time of critical conditions that can lead to catastrophic accidents. The knowledge on human error helps a risk manager as well as a designer to create and control a more credible system. Several human error theories were reviewed and adopted for forming the integrated perspective: gulf of execution and evaluation; risk homeostasis; the ironies of automation; trust in automation; design affordance; distributed cognition; situation awareness; and plan delegation theory. The integrated perspective embraces human error theories within three levels of human-system interactions such as affordance level, psychological logic level and trust level. This paper argued that risk management process should dealt with human errors by providing (1) reasoning improvement; (2) support to situation awareness of operators; and (3) continuous monitoring on harmonization of human system interaction. This approach may help people to understand risk of human-system interaction failure characteristics and their countermeasures.

  • A mechanical system controlled by human operator, such as master-slave system, includes human dynamics in the whole system and such a system is called a human-machine system. In the system, operator's skill is required considerably in order to realize a meaningful operation. In this paper, a new concept and design strategy of compensator that improves the operativity of human-machine system are proposed. The compensator is called "collaborater "that is named after "collaborator" who works together with people. We mean not to design the automatic controller but the compensator that works together with a machine so that human feels the fulfillment in the operation. Our aim is to realize cooperation of people and a machine on a higher level.

  • Human-machine serial systems must be normal in both systems. Though the failure of machine is irreducible by itself, the human errors are of recurring type. When the human performance is described quantitatively, non-homogeneous Poisson Process model of human errors can be developed. And the model parameters can be estimated by maximum likelihood estimation and numerical analysis method. System reliability is obtained by multiplying machine reliability by human reliability.

  • Present computing power and enhanced technology is progressing at a dramatic rate. These systems can unravel complex issues, assess and control processes, learn, and-in many cases-fully automate production. There is no doubt that technological advancement is improving many aspects of life, changing the landscape of virtually all industries and enhancing production beyond what was thought possible. However, the human is still a part of these systems. Consequently, as the advancement of systems transpires, the role of humans within those systems will unavoidably continue to adapt as well. Due to the human tendency for error, this technological advancement should compel a persistent emphasis on human error reduction as part of maximizing system efficiency and safety-especially in the context of the nuclear industry. Within this context, as new systems are designed and the role of the human is transformed, human error should be targeted for a significant decrease relative to predecessor systems and an equivalent increase in system stability and safety. This article contends that optimizing the roles of humans and machines in the design and implementation of new types of automation in nuclear facility systems should involve human error reduction without ignoring the essential importance of human interaction within those systems.

  • In this paper, the human error contributions to the system unavailability are calculated and compared to the mechanical failure contributions. The system unavailability is a probability that a system is in the failed state at time t, given that it was the normal state at time zero. It is a function of human errors committed during maintenance and tests, component failure rates, surveillance test intervals, and allowed outage time. The THERP (Technique for Human Error Rate Prediction), generally called "HRA handbook", is used here for evaluating human error rates. This method treats the operator as one of the system components, and human reliability is assessed in the same manner as that of components. Based on the calculation results, the human error contribution to the system unavailability is shown to be more important than the mechanical failure contribution in the example system. It is also demonstrated that this method is very flexible in that it can be applied to any hazardous facilities, such as gas valve stations and chemical process plants.ss plants.

  • This paper reviews and assesses the analogy between the human immune system and network intrusion detection systems. The promising results from a growing number of proposed computer immune models for intrusion detection motivate this work. The paper begins by briefly introducing existing intrusion detection systems (IDS's). A set of general requirements for network-based IDS's and the design goals to satisfy these requirements are identified by a careful examination of the literature. An overview of the human immune system is presented and its salient features that can contribute to the design of competent network-based IDS's are analysed. The analysis shows that the coordinated actions of several sophisticated mechanisms of the human immune system satisfy all the identified design goals. Consequently, the paper concludes that the design of a network-based IDS based on the human immune system is promising for future network-based IDS's

  • Applying human engineering simulation to analyzing work capability and movements of operators during manufacturing is highly demanded. However, difficulty in modeling digital human required for simulation makes engineers to be reluctant to utilize human simulation for their tasks. This paper addresses such problem on human engineering simulation by developing the technology to automatize human modeling with multiple Kinects at different depths. The Kinects enable us to acquire the movements of digital human which are essential data for implementing human engineering simulation. In this paper, we present a system for modeling automation of digital human. Especially, the system provides a way of generating the digital model of workers' movement and position using multiple Kinects which cannot be generated by single Kinect. Lastly, we verify the effects of the developed system in terms of modeling time and accuracy by applying the system to four different scenarios. In conclusion, the proposed system makes it possible to generate the digital human model easily and reduce costs and time for human engineering simulation.

HUMAN-MACHINE INTERACTION IN NUCLEAR POWER PLANTS

  • Advanced nuclear power plants are generally large complex systems automated by computers. Whenever a rare plant emergency occurs the plant operators must cope with the emergency under severe mental stress without committing any fatal errors. Furthermore, The operators must train to improve and maintain their ability to cope with every conceivable situation, though it is almost impossible to be fully prepared for an infinite variety of situations. In view of the limited capability of operators in emergency situations, there has been a new approach to preventing the human error caused by improper human-machine interaction. The new approach has been triggered by the introduction of advanced information systems that help operators recognize and counteract plant emergencies. In this paper, the adverse effect of automation in human-machine systems is explained. The discussion then focuses on how to configure a joint human-machine system for ideal human-machine interaction. Finally, there is a new proposal on how to organize technologies that recognize the different states of such a joint human-machine system.

  • In human-mechanical cooperative systems, a significant issue is to improve the control performance and the maneuvering feeling of human operation. However, since it is not easy to evaluate the feeling of operators numerically, control engineers design controllers only through experience. Thus, in this paper, a new evaluation method for control performance of human-mechanical cooperative system is proposed based on the reserge waveform. Various distortions of waveform represent deteriorations of control performance and maneuvering feeling. In some cases, since there is a tradeoff between the control performance and the maneuvering feeling, it is difficult to compensate for both of them by usual feedback controllers. To overcome this situation, the two degrees of freedom control system is applied to human-mechanical cooperative system. Some numerical simulation results for an electric power steering system are shown to confirm the effectiveness of proposed control design method.

  • This paper investigates a mathematical model of a system composed of two non-identical unit parallel system with common-cause failure, critical human error, non-critical human error, preventive maintenance and two type of repair, i.e. cheaper and costlier. This system goes for preventive maintenance at random epochs. We assume that the failure, repair and maintenance times are independent random variables. The failure rates, repair rates and preventive maintenance rate are constant for each unit. The system is analyzed by using the graphical evaluation and review technique (GERT) to obtain various related measures and we study the effect of the preventive maintenance preventive maintenance on the system performance. Certain important results have been derived as special cases. The plots for the mean time to system failure and the steady-state availability A(${\infty}$) of the system are drawn for different parametric values.
