Malaria Risk Factors in Kaligesing, Purworejo District, Central Java Province, Indonesia: A Case-control Study

  • Objectives: Malaria remains a public health concern worldwide, including Indonesia. Purworejo is a district in which endemic of malaria, they have re-setup to entering malaria elimination in 2021. Accordingly, actions must be taken to accelerate and guaranty that the goal will reach based on an understanding of the risk factors for malaria. Thus, we analysed malaria risk factors based on human and housing conditions in Kaligesing, Purworejo, Indonesia. Methods: A case-control study was carried out in Kaligesing subdistrict, Purworejo, Indonesia in July to August 2017. A structured questionnaire and checklist were used to collect data from 96 participants, who consisted of 48 controls and 48 cases. Univariate, bivariate, and multivariate analyses were performed. Results: Bivariate analysis found that education level, the presence of a cattle cage within 100 m of the house, not sleeping under a bednet the previous night, and not closing the doors and windows from 6 p.m. to 5 a.m. were significantly ($p{\leq}0.25$) associated with malaria. Of these factors, only not sleeping under a bednet the previous night and not closing the doors and windows from 6 p.m. to 5 a.m. were significantly associated with malaria. Conclusions: The findings of this study demonstrate that potential risk factor for Malaria should be paid of attention all the time, particularly for an area which is targeting Malaria elimination.

  • Air pollution in large cities is reduced through the environmental health policies, but due to increased population and automobile, some pollutants are still a problem. These air pollutants are known to cause asthma and respiratory diseases. According to an OECD report, the number of premature deaths will increase. Hazardous air pollutants should be managed through a systematic monitoring, risk assessment, and many studies are in progress. In order to manage hazardous air pollutants, transformation of policy for the protection of human health is required. management policy through the calculation of the excess number of deaths that occur from hazardous air pollutants for the public health is necessary. Korea has put a lot of efforts for air quality, but health risk assessment should be more considered.

Multifaceted Risk Factors for Mental Health Problems of Adolescents in Correctional Facilities: Toward an Integrated Implication for Rehabilitation and Reintegration into Society

  • This study explores a model that examines how multidimensional risk factors explain mental health problems of young offenders. One hundred and ninety six students aged 13 to 15 in correctional facilities were assessed for mental health symptoms that examined the effects of multifaceted risk factors on mental health conditions. Consistent with the hypothesis of this study, secure unit students appear to have experienced various forms of risk factors and those factors have influenced mental health conditions. Results show that correctional facilities should reframe therapeutic and preventive approaches to disadvantaged students and develop integrated services and programs for rehabilitation and reintegration into society. It is recommended that those involved in treatment plans in correctional facilities consider that different treatment plans are necessary for each young offender.

염화비닐(Vinyl Chloride)의 인체 및 수생태계 영향 연구 (Effects of Vinyl Chloride to Human Health and Aquatic Ecosystems)

  • VC는 국제적인 발암등급 체계인 IRIS, IARC, ESIS에 의해 인체발암물질로 평가되고 있으며, 경구, 호흡, 경피 노출 경로를 통해 간세포 다형성 등을 유발하는 것으로 보고된 바 있다. 또한 VC는 자연계에서 쉽게 분해되지 않는 유기 염소계용제로서, 대기, 지표수, 지하수, 먹는물, 토양으로 유출되어 인체를 포함한 환경을 오염시킬 가능성이 높은 것으로 나타났다. 현재 우리나라의 수질환경기준과 먹는물 기준에서는 VC에 대한 규제 항목 및 기준치가 제시되어 있지 않으나, 인체 및 수생태계 위해 우려가 있어 특정수질유해물질 및 수질오염물질로 지정된 바 있다. US EPA, 일본, WHO 등 선진국에서는 법적 항목 및 기준치로 VC를 제도적으로 관리하고 있다. 특히 우리나라에서는 2004년 한강 및 낙동강 유역에서 $0.8\sim4.7{\mu}g\;L^{-1}$ (검출한계 $0.1{\mu}g\;L^{-1}$),개별배출업소 유출수에서 $10.6\sim668{\mu}g\;L^{-1}$의 농도로 VC가 검출된 사례가 있으며, 독일, 일본, 미국 등의 국외에서 검출된 $0.03\sim566{\mu}g\;L^{-1}$ 농도보다 국내 수계 내 VC의 분포 수준이 높은 것으로 나타났다. 국내 인체 위해성 평가 결과 VC에 대한 위해 우려가 높은 것으로 평가된 바 있으며, 생태 위해성 평가는 자료 부족으로 보류된 바 있다. 이 때 활용된 모니터링 자료는 일부 지점 및 단회 기반의 신뢰성 다소 낮은 자료에 국한되므로 지속적인 모니터링 사업을 통해 신뢰성 높은 자료로 대체될 필요가 있다. 또한 물 환경 내 생태독성연구는 일부 시험증에 국한되어 있고, 실험 설계도 VC의 수체 내 농도 감소를 고려하지 않아 결과치가 과다 산정되었을 가능성이 높다. 따라서 지속적으로 다양한 수서생물을 활용한 VC의 생태 독성 자료가 생성되어야 추후 VC에 대한 신뢰도가 높은 생태 위해성 평가가 수행될 것으로 사료된다.

  • PCBs are classified as B2 (Probable human carcinogen) based on the induction of hepatocellular carcinomas in rats and mice from IRIS (Integrated Risk Information System). About 20 years ago, PCBs were phased out for electrical use in Korea, but PCBs were continuously used in the other field. Lately, there has been increasing concern on possible effects of contaminated soil to the other environment and human health. The purpose of this study is to determine PCBs level in soil at some site and to assess the human exposure doses according to exposure routes for people living within sites which expected to be exposed to PCBs. Pollution level of PCBs on the site was monitored using gas liquid chromatography. To assess the transport of PCBs in soil to plant and to air, various transfer factors(diffusion coefficient, bioconcentration factor etc.) were considered in simple calculations. To calculate the residential exposure doses by routes, some equations were considered using assumption value, which define inhalation, ingestion (soil, plant) and derreal uptake pathway. Computated results will be used as risk assessment information for human health evaluation on contaminated soil.

대기중 유해 화학 물질의 인체 위해도 우선순위 선정 연구 (Human Health Risk based Priority Ranking for Hazardous Air Pollutants)

  • Although it is suggested that risk -based management plan is needed to manage air pollution effectively, we have no resources enough to evaluate all aspects of substances and set priorities. So we need to develop a logical and easy risk-based priority setting method. However, it if impossible that only one generic system that is consistent with all the use is developed. In this study, we proposed a human health risk based priority-setting method for hazardous air pollutants, and ranked priorities for this method. First of all, after investigating previous chemical ranking and scoring systems, we chose appropriate indicators and logics to goal of this study and made a chemical priority ranking method using these. As results, final scores in priority ranking method were derived for 25 substances, and ethylene oxide, acrylonitrile and vinyl chloride were included in high ranks. In addition, same substances were highly ranked when using default values like when using no default, but the scores of hydrofluoric acid and ryan and compounds were sensitive to default values. This study could be important that priorities were set including toxicity type and quality and local inherent exposure conditions and we can set area-specific management guidelines and survey plans as a screening tool.

환경영향평가에 있어 위해성분석 기법의 도입 (Application of the Risk-Based Analysis to EIA)

  • In generally speaking, the purpose of Environmental Impact Assessment(EIA) is to give the environment its due place in the decision-making process by clearly ealuating the environmental consequence of a proposed activity before action is taken. The introduction of conventional EIA is to be seen as an end product of a very long evolutionary process, starting with rudimentary but evolving pollution control measures for air, water, noise, land and chemicals, each governed by separate, and separately administered pieces of legislation. In EIA process, the measures of status, scoping, proposed mitigation and communication have not been very quantitative in their significancy. Of course, the determinations have uncertainity in the implications for significant impacts. To improve the determination of significant impacts, some more comprehensive methodologies of EIA has been proposed with the concepts of risk analysis in the proposed projects. The concepts of risk analysis has been introduced to the expression of human health impairment due to environmental pollutants since the early 1980's. The risk analysis being meant by the statistical significance of impact has a process quantitatively considering uncertainities and importances of ecological systems and human health as well. The process of risk analysis shows assessment, doseresponse in toxicity, exposure assessment and risk characterization. With the risk assessment, it could be suggested for the proper measurements against their anticipated risk in the EIA. This paper deals the priciples developing process and application of the risk-based analysis in EIA.

국내 폐금속 광산지역에서의 토양, 지하수, 쌀의 중금속 노출에 따른 인체 위해성평가 (Risk Assessment for Heavy Metals in Soil, Ground Water, Rice Grain nearby Abandoned Mine Areas)

  • BACKGROUND: The objectives of this study are to investigate the contamination levels of heavy metals in soil, ground water, and agricultural product near the abandoned Boeun and Sanggok mine areas in Korea and to assess the health risk for these local residents exposed to the toxic heavy metals based on analytical data. METHODS AND RESULTS: By the results of human health risk assessment for local residents around Boeun and Sanggok, human exposure to cadmium, copper, arsenic from soil and to lead, cadmium, and arsenic from rice grain were higher in Sanggok, but human exposure to zinc and arsenic from ground water was higher in Boeun. By the results of hazard index (HI) evaluation for arsenic, cadmium, copper, lead, and zinc, HI values in both areas were higher than 1.0. This result indicated that the toxicity hazard through the continuous exposure to lead, cadmium, arsenic from rice, ground water, and soil would be likely to occur to the residents in the areas. Cancer risk assessment for arsenic, risks from the rice were exposed to one to two out of 10,000 people in Boeun and one of 1,000 people in Sanggok. These results showed that the cancer risks of arsenic in both areas were 10~100 times greater than the acceptable cancer risk range of US EPA ($1{\times}10^{-6}{\sim}1{\times}10^{-5}$). CONCLUSION(S): Therefore, if these two local residents consume continuously with arsenic contaminated soil, ground water, and rice, the adverse health effects (carcinogenic potential) would be more increased.

(구)장항제련소 매입구역의 비소 오염도와 생물학적접근성을 반영한 위해성평가 및 정화수준 결정에 관한 연구 (Determination of Human Health Risk Incorporated with Arsenic Bioaccessibility and Remediation Goals at the Former Janghang Smelter Site)

  • Metal concentrations in the former Janghang smelter area were determined and human health risk of arsenic (As) with bioaccessibility was investigated. Site investigation of the area within 1.5 km from the Janghang smelter showed the As concentrations of 4.8~169.8 mg/kg (avg. 37.8 mg/kg). For 85 samples out of 126 samples, As concentrations were higher than the Worrisome Level of the Korean Soil and Environment Conservation Act, and seven samples exceeded the Countermeasure Standard. Risk assessment for As incorporated with the bioaccessibility revealed that potential human health risk of the carcinogenic ($1.8{\sim}5.0{\times}10^{-5}$) was above the acceptable risk range ($10^{-5}{\sim}10^{-6}$) while the risk of the non-carcinogenic was not found. Remediation goals based on risk incorporated with bioaccessibility of As ranged from 10.8 to 20.0 mg/kg. Such difference in the remediation goals resulted from various bioaccessibility of As (i.e., between 8.7~66.3%) at the study site.

Breastfeeding and its Relationship with Reduction of Breast Cancer: A Review

  • In this review, we describe the patterns of known immunological components in breast milk and examine the relationship between breastfeeding and reduced risk of breast cancer. The top risk factors for breast cancer are a woman's age and family history, specifically having a first-degree relative with breast cancer. Women that have a history of breastfeeding have been shown to have reduced rates of breast cancer. Although the specific cause has not been elucidated, previous studies have suggested that breastfeeding reduces the risk of breast cancer primarily through two mechanisms: the differentiation of breast tissue and reduction in the lifetime number of ovulatory cycles. In this context, one of the primary components of human milk that is postulated to affect cancer risk is alpha-lactalbumin. Tumour cell death can be induced by HAMLET (a human milk complex of alpha-lactalbumin and oleic acid). HAMLET induces apoptosis only in tumour cells, while normal differentiated cells are resistant to its effects. Therefore, HAMLET may provide safe and effective protection against the development of breast cancer. Mothers should be encouraged to breastfeed their babies because the complex components of human milk secretion make it an ideal food source for babies and clinical evidence has shown that there is a lower risk of breast cancer in women who breastfed their babies.
