인적행위개선 포스터 개발과 활용 (Development and Utilization of Posters for Human Performance Improvement)

  • As many as 111 reactor trips have occurred for recent 5 years('01-'05), and 26 cases of them have occurred due to human error. The trend of human error rate didn't decrease in 2004, so KHNP started to make efforts to decrease human errors. In 2006 KHNP bench marked excellent foreign nuclear power plants and introduced human error prevention tools. In addition, KHNP created as many as 40 posters for human performance improvement. The posters are based on the about 500 real incident reports collected through K-HPES from 1995. Therror preventive tols for the poster. This paper explains design of the posters and their application.

고속도로 건설현장의 인적오류 예방을 위한 실무자용 도구 개발 (Hands-on Tools to Prevent Human Errors in Highway Construction)

  • Objective: The aim of this study is to reclassify human errors and to develop hands-on tools to apply the new classification for preventing human error accidents in highway construction site. Background: The main cause of accidents in highway construction was reported as the carelessness of workers. However, such diagnosis could not help us operationally prevent accidents in real workplace. Method: The accidents in highway construction were reanalyzed and the causes of human error were reclassified in order to educate and improve the awareness of human error in highway construction. Field survey and interview with safety managers and workers were conducted to find the causal relationship between the actual accidents and the human errors. Results: The most frequently observed human errors in highway construction were classified into six categories such as mis-perception, distraction, memory fail, slip, cognition error and mis-judgment. In order to provide hands-on tools to increase the awareness of human error in construction field, the human error checklist and card sorting diary were developed. Especially, the card sorting diary was designed to increase the ability in human error inspection of safety manager at construction site. Moreover, posters were developed based on actual accident cases. Conclusion: We suggested that the improved awareness and analytical report on checklist, card sorting diary and posters for construction field could collectively prevent the accident. Application: The classification of human error, hands-on tools and posters can be directly applicable on highway construction site. This analytical and collective approach preventing human error-related accident could be extended to other construction workplaces.

상선 충돌사고 분석을 이용한 인적과실 예방 충돌회피모델 연구 (Study on the Human Error Prevention Collision Avoidance Model using Merchant Ship Collision Accident Analysis)

  • 본 연구의 목적은 과거 12년(2010~2021년)간 발생한 상선의 충돌사고 668건을 조사하여 충돌의 원인요인을 조사하고 이를 통계적으로 분석하여 항해사의 인적과실 예방 충돌회피(HEPCA) 모델을 제안하는 것이다. 중앙해양안전심판원의 통계연보 및 충돌사건 재결서를 조사하여 상선의 충돌 원인요인 데이터를 수집하고 통계분석 도구인 SPSS를 이용하여 빈도분석을 수행하였다. 1단계 분석으로 상선 충돌사고 668건을 대상으로 충돌원인을 분석하였고, 2단계 분석에서는 식별된 최대빈도 원인요인을 세부적으로 분석하였다. 분석결과, 충돌원인은 항해사의 인적과실이 98 %인 것으로 식별되었으며, 빈도 높은 요인 순서는 경계소홀 > 항행법규위반 > 조선 부적절 순이었다. 경계소홀의 원인 요인은 주로 상대선 초인 후 지속감시 소홀이었으며 상대선박의 존재를 인식하지 못한 원인은 주로 당직시간에 다른 작업을 한 요인이었다. 분석결과를 적용하여 인적과실 예방을 위한 HEPCA 모델을 제안하였고, 이를 재결서의 충돌사건에 적용해보았다. 본 연구결과는 해기사 교육기관 및 실무에서 항해사의 인적과실로 발생하는 충돌사고를 방지하기 위한 교육 자료로 활용이 가능할 것으로 기대된다.

업무수행도 추정을 위한 직무스트레스 평가표의 비교 분석 (Comparative Analysis of Work Stress Assessment Tools for Estimating Human Work Performance)

  • A man is a component of a large system how complex it may be so that human performance which can influence on the system safety should be included in system safety assessment. Meanwhile, human performance can vary over time due to lots of factors. Among them, stress is an indirect factor that may cause human error which can result in industrial accidents. To assess stress level of human workers, not a few assessment tools have been developed. However, it can be questionable to utilize them for human performance anticipation because they were mainly developed with the viewpoint of mental health, though stress assessment can be also required in the safety aspect. Therefore, this study aimed to survey the possibility of their application with safety purpose. About 10 kinds of work stress tools were collected and analyzed with reference to assessment items, assessment and analysis methods, and follow-up measures. The results showed that most tools focused their weights on Demands, Supports, and Relationships, in sequence. However, they, except only one tool developed by the Japanese researchers, merely advised to set up counterplans in PDCA cycle or risk management activities. In consequence, application of stress assessment tools mainly developed for mental health seemed impractical for safety purpose with respect to human performance anticipation so that it was concluded that development of a new assessment tools aimed to human performance variation and accident prevention would be inevitable.

  • As large-scale buildings, skyscrapers, and multi-purpose buildings recently increase in numbers dramatically the internal space of such buildings becomes more and more large and complicated accordingly. The Multi-purpose buildings are increasing rapidly in number, and concerns over the evacuation process in case of fire emergency are also increasing. However, A controller who is responsible for visitor's safety makes a decision about measures for visitor safety in human based decision making process. Many potential accidents that are caused by human error lurk in results of the process. The accidents can be decreased by changing the decision making process from human-based into technology-based. Through this study, the way to improve current disasters prevention system is presented and it can also improve shelter inducement methods effectively. SICS(Spatial Information Control System) which is being studied in other research is used as the tool for improvement.

선박충돌사고 원인과 관련된 인적과실 유발요인에 관한 연구 (A Study on Factors that Trigger Human Errors Related to Causes of Ship Collisions)

  • 본 연구의 목적은 실제 선박 충돌사고 사례를 조사하여 선박 충돌상황에서 충돌 원인별 선장 및 당직 해기사의 인적과실 유발요인을 통계적으로 분석하여 규명함으로써 해기사의 선박 충돌예방에 기여하고자 하는 것이다. 연구대상은 2010년부터 2016년까지 7년 동안의 상선과 상선, 상선과 어선 간의 선박 충돌사고 중 분석기준에 적합한 총 109건 218척(피항선109척, 유지선 109척)을 대상으로 하였으며 선종, 피항선과 유지선, 인적과실 측면의 충돌원인 등의 항목으로 구분하여 데이터를 수집하였고 상선에서 해기사의 충돌사고 유발 요인 규명에 중점을 두고 통계분석 도구인 SPSS를 이용하여 빈도분석과 교차분석을 실시하여 해기사의 인적과실 유발요인을 도출하였다. 분석결과 피항선에서는 레이다 감시를 포함한 경계소홀(74.3 %) 및 상대선 지속관찰 소홀(17.4 %) 순 이었으며, 유지선에서는 적절한 피항 협력동작 미 이행(63.3 %)이 주요 요인이었다. 특히 상선의 경계소홀 유형 대부분이 상대선 초인 후 지속관찰을 소홀히 한 점이며 미 경계 원인과 당직근무 태만의 공통요인은 항해당직 시간에 다른 업무에 치중하였기 때문인 것으로 나타났다.

A Study on the Observer Training and Implementation for Effective Normal Operation Safety Survey (NOSS)

  • Normal Operation Safety Survey (NOSS) is a safety management tool used by air traffic control organizations to measure data related to threats and errors to proactively prevent accidents before they occur. NOSS was developed to be applied to the air traffic control field according to the concept of Line Operation Safety Audit (LOSA) conducted by airlines to reduce human errors in the cockpits. Safety management based on data is crucial for the prevention of control-related accidents, and the competency of observers for this is important for the successful implementation of NOSS. Based on LOSA implementation experiences and expert interviews, this paper introduces the international NOSS methods and discusses the key factors required for effective observation and the implementation methods and implications of NOSS observer training.

A Systems Engineering Approach for Predicting NPP Response under Steam Generator Tube Rupture Conditions using Machine Learning

  • Accidents prevention and mitigation is the highest priority of nuclear power plant (NPP) operation, particularly in the aftermath of the Fukushima Daiichi accident, which has reignited public anxieties and skepticism regarding nuclear energy usage. To deal with accident scenarios more effectively, operators must have ample and precise information about key safety parameters as well as their future trajectories. This work investigates the potential of machine learning in forecasting NPP response in real-time to provide an additional validation method and help reduce human error, especially in accident situations where operators are under a lot of stress. First, a base-case SGTR simulation is carried out by the best-estimate code RELAP5/MOD3.4 to confirm the validity of the model against results reported in the APR1400 Design Control Document (DCD). Then, uncertainty quantification is performed by coupling RELAP5/MOD3.4 and the statistical tool DAKOTA to generate a large enough dataset for the construction and training of neural-based machine learning (ML) models, namely LSTM, GRU, and hybrid CNN-LSTM. Finally, the accuracy and reliability of these models in forecasting system response are tested by their performance on fresh data. To facilitate and oversee the process of developing the ML models, a Systems Engineering (SE) methodology is used to ensure that the work is consistently in line with the originating mission statement and that the findings obtained at each subsequent phase are valid.
