Research for the Mainly Cause of Safety-Management Sharp-type of Semiconductor Industry (To Center with Corporate Company) (반도체 산업의 안전관리 형태별에 따른 원인에 관한 연구 (협력업체 중심(中心)으로))

  • The study on semiconductor industrial accident in korea has been focused on frequencies of each type, employee, characteristics, cause and un-safety condition, behaviour and so on. Those attributes of semiconductor industrial accidents were usually analyzed independently, so that it was hard to provides a well-process and systematic guide lines for efficient safety management. There fore, there were a few studies based on comprehensive survey in terms of sharp-type of safe management. The questionnaire survey carried out for the workers(284) who were responsible for safety management in to center with corporate company with semiconductor industry the factor analysis showed that there were three factor of safety management. They were 1) Investment and operation and management for accident prevention, 2) Unsafe, safety management 3) General human error and behavior the industries of respondents were correlative with three group. Three Groups showed a statistically significant differences on the number of cases. Actually, the group with the larger investment and the better unsafe cause, human error a of accident prevention had a smaller cause of accident cases.

  • This research analyzes blasting accident cause that is happening construction and engineering works spot taking advantage of AHP (Analytic Hierarchy Process) techniques as metrical. Result that apply AHP with blasting accident that is happened the South Korea and Japan, appeared by thing which relative importance by human cause is highest. Specially, it is observance of safety rule that dominate the highest ratio among of human cause, and if observe a little, causes that prevention is possible are much. By result of this research, necessity of safety education is important first of all for prevention of blasting accident. Also, thorough safety control plan of during work and enough on-the-slut probe before work should be established. Because explosives uses gunpowder and explosive high energy, work by qualified person is essential. Ant it may become help to minimize dissipation of important life and property preventing beforehand explosion accident of gunpowder.

  • This research is purposed to find methods of treatment on allergy diseases, through summarizing thought on human and etiology, classification and treatment on diseases proposed in Sasang constitutional medicine 2. The cause of disease is classified into interior cause and exterior cause. Interior cause could be used in cause of disease, exterior cause could be used in prevention of illness, treatment of disease and preservation of health. 4. The treatment of disease proposed in ${\ulcorner}$Dongyi Soose Bowon Sasang Chobongyun(東醫壽世保元四象草本卷${\lrcorner}$ is that it is to recover 'Essential Qi of Constitution(體質正氣)' by medicine and management of 'Mind-Body(心身)' and that chronic disease is treated chiefly by management but acute disease is treated chiefly by medicine. 5. Allergy disease should be prevented by management of 'Mind-Body(心身)'. but if we suffer from allergy disease, we should treat disease through recovering 'Essential Qi of Constitution(體質正氣)' both medicine and management of 'Mind-Body(心身)'.

A Study on the Optimal Flight Time According to the Amount of Fatigue (피로누적에 따른 최적 비행시간 산출에 관한 연구)

  • Since the aircraft has a property of moving in the three-dimensional space, it may cause personally and financially critical damage in the case of an accident. Among the causes of aircraft accident, human factor has occupied about 70% of all accidents. Specially, fatigue among human's problems has been studied earlier than any other factor. Fatigue has been the cause of 75% of accidents that are related to human factor. So many studies have been conducted. But the direction of these studies mainly attach importance to the sleep loss and circadian rhythm. Limitation for flight time of ICAO is 8 hours per day, civil airlines in domestic line also adopt the limitation. But this rule is not based on human's performance but compromise between labor and management. The long-haul flight brings about a mental block to pilot. This mental block decreases performance of pilot and loses a lot of important information. So this may cause many accidents. This paper is to offer optimal flight time according to the amount of fatigue due to increasing flight time. The optimal flight time is searched through the field experiment. The experiment has adopted two methods. One is to examine pilot's objective fatigue accumulation rate through the critical fusion frequency, and another is to investigate pilot's subjective fatigue feeling through the fatigue subjective symptoms investigation table.

  • The purpose of this study is to develop the measuring instruments for evaluation criteria for Malcolm Baldrige National Quality Award(MBNQA), suitable for ICT Industries, and to analyze the cause-effect relationship between those criteria through aforementioned instruments. MBNQA is formed with seven categories: Leadership, Strategic planning, Focus on patients, other customers and markets, Measurement, analysis and knowledge management, Human resource focus, Process management and Results. As excluding the Human Resource Focus category, this study empirically examined the cause-effect relationship among six categories. In order to empirically examine the research model, this study calculated Cronbach's alpha and reliability index, thus examined the reliability and executed Exploratory Factor Analysis. Furthermore, Average Variance Extracted(AVE) is used to verify the discriminant validity. Lastly, the hypothesis testing was made complete through significance test on the paths between variables. The result of this study shows that both leadership and social responsibility have direct cause-effect relationship with Measurement, analysis and knowledge management, Human resource focus, Process management and also that this relationship has direct impact on Human resource focus, Measurement, analysis and knowledge management as well, consequently exerting influence on the result through Process management, Finance and Market data.

A Study on the Cause of the Accidents Influencing Factor based on the Safety Management Shape Types of the Cooperation Companies in Semiconductor Industry (협력업체의 안전관리 형태에 따른 반도체 산업의 사고의 영향요인에 관한 연구)

  • The studies on semiconductor industrial accident in korea have been focused on the frequencies of each type of safety management employee, characteristics, cause and unsafe conditions, acts and so on. Actually, the group with the larger investment and the more unsafe cause, human error of accidents prevention had a smaller causes of accident cases.

Puerperal septic shock and necrotizing fasciitis caused by Staphylococcus caprae and Escherichia coli

  • Puerperal sepsis is one of the leading causes of maternal morbidity and mortality worldwide. Postpartum pelvic infections can cause various complications, including wound infections and necrotizing fasciitis. Several microorganisms are known to cause such infections; however, no study has reported on Staphylococcus caprae, a coagulase-negative staphylococcus that is isolated frequently from animals and infrequently from human specimens, as a causative agent. Here, we report a rare case of septic shock complicated by necrotizing fasciitis after a cesarean section. This is the first report of a human isolate of S. caprae in association with puerperal sepsis and necrotizing fasciitis.

  • Chemical Process industry in Korea has over 30 year's of history and is likely to face potential incidents. The traditional risk analysis and control system in Chemical Process industry focuses on mechanical defects, overlooking the human performance control. Although development of automation technology and controlling technology was necessary, human decision factor is essential to preventing accidents in the Chemical Process. Almost all serious accidents take place when inappropriate humanperformance and mechanical defects of safety equipments simultaneously occurs. The AHRA(Advanced Human Reliability Analyzer) software has been developed to collect failure data and analyze human error probability (Reliability) in Chemical Process Industry in Korea. This paper describes the HRA analysis result of PIF(Performance Influencing Factor) evaluation, HEP(Human Error Probability) and root cause of accidents by applying a Chemical Process Industry related accident data. This analysis result should present a scheme that, by controlling human error factor other than putting safety management funds into the machinery in plants, can reduce cost and maximize the safety in Chemical Process Industry.

  • There have been many efforts to prevent accidents in Korea for the last 25 years. Many measures in the area of hardware sciences including electrical, mechanical, chemical engineering, etc. were applied to eliminate or at least reduce causes of accidents. However, the accidents rate has not been reduced much despite of these measures. This research aimed to find real causes of these accidents and to suggest a comprehensive model that can mainly be applied to industrial fields to find potential or existence of human errors during the pre-installation stage or after an accident. We tried to explain sequences of an operator's information process that might cause human errors on one hand, and life cycle stages of facilities involved when human errors occur on the other hand. With this comprehensive model presented in this research, one can follow up the sequence of human errors caused by operators. Further, errors made at the design stage which could be a main cause of accidents can be tracked. It is recommended that this comprehensive model should be used to prevent human errors in industrial fields since safety personnel can easily find out errors or error potentials through the life cycle stages of manmachine facilities.

A Study of Methodology to Examine Organizational Root Causes through the Retrospect Error Analysis of Railroad Accident Cases

  • Objective: This study proposes a systematic process to present the analysis methods and solutions of organizational root causes to human errors on the railroad. Background: In fact, organizational root cause such as organizational culture is an important factor in the safety concerns on human errors in the nuclear power plant, railroad and aircraft. Method: The proposed process is as follows: 1) define analysis boundary 2) select human error taxonomy 3) perform accident analysis 4) draw root causes with FGI 5) review root causes analysis with survey 6) chart analysis of root causes, and 7) propose alternatives and solutions. Results: As a result, root causes of the organizations like railroad and nuclear power plant came from the educational problems, violations, payoff system, safety culture and so forth. Conclusion: The proposed process does predict potential railroad accident through retrospect error analysis by building new human error taxonomies and problem solution. Application: This study would contribute to examination of the relationship between human error-based accidents and organizational root causes.
