Maximum level of hazardous chemicals in foods can be settled after overall investigation of toxicological database, the representative exposure assessment, risk level on the present exposure level, the need of maximum level establishment, making of maximum level scenario proposed under consumer protection and verification of contamination reduction method and review of application efficiency on maximum level for risk management. The maximum level should be needed when the risk value of specific chemical caused by food ingestion was high and chronic human exposure was predicted continuously unless the maximum level exists. The key role of the risk assessment is to improve the efficiency of the risk management to reduce the present risk level and make the risk management policy scientifically by setting the representative result of the risk assessment.
Background: Concerns have been raised regarding the criteria of groundwater, in particular in Gwangwon-do Province where many residents drink groundwater due to the poor supply of tap water and a high nonconformity rate with water quality criteria nationwide. Objectives: Water quality monitoring and risk assessment were conducted for groundwater in Hoengseong, Gangwon-do Province. Methods: A total of 46 items required for meeting drinking water criteria were analyzed from 258 samples collected from March 2017 through August 2018 (152 sites in 2017 and 106 sites in 2018). Risk assessment was conducted for two non-carcinogens (F- and NO3-N), and one carcinogen (i.e., arsenic) based on their high nonconformity to water quality criteria. Results: Water quality analysis revealed that the total proportion of nonconformities was determined to be 27.9%. The nonconformity rate for each content item is as follows: total colony counts (1.6%), total coliform (6.2%), Escherichia coli (1.2%), F- (8.1%), arsenic (4.7%), NO3-N (8.1%), pH (1.2%), manganese (0.4%), and turbidity (5.8%). Risk assessment indicated that fluoride induced a hazard quotient greater than 1 with the 95% UCL (Upper Confidence Limit) concentration of the total 258 sites and average, median, and 95% UCL concentrations of nonconformity sites. For NO3-N, there was no human health risk. For arsenic, the excess cancer risk exceeded the acceptable cancer risk of 1×10-6 with the average and 95% UCL concentrations of total 258 sites and average, median, and 95% UCL concentrations of nonconformity sites. Conclusions: This study suggests that it is necessary to expand water quality monitoring of groundwater and conduct a more detailed risk assessment in order to establish a health care plan for the residents of Hoengseong, Gangwon-do Province.
In this article, we reviewed the literature on risk assessment (RA) models with and without molecular genomic markers and the current utility of the markers in the pharmacogenetic field. Epidemiological risk assessment is applied using statistical models and equations established from current scientific knowledge of risk and disease. Several papers have reported that traditional RA tools have significant limitations in decision-making in management strategies for individuals as predictions of diseases and disease progression are inaccurate. Recently, the model added information on the genetic susceptibility factors that are expected to be most responsible for differences in individual risk. On the continuum of health care, from diagnosis to treatment, pharmacogenetics has been developed based on the accumulated knowledge of human genomic variation involving drug distribution and metabolism and the target of action, which has the potential to facilitate personalized medicine that can avoid therapeutic failure and serious side effects. There are many challenges for the applicability of genomic information in a clinical setting. Current uses of genetic markers for managing drug therapy and issues in the development of a valid biomarker in pharmacogenetics are discussed.
Chlorpyrifos is an organophosphate insecticide and one of the most commonly and widely used insecticide. However, a little known about the dermal risk of chlorpyrifos on human being. Therefore, this study was conducted for the dermal risk assessment after exposure to chlorpyrifos in Korean farmers. First, skin irritation by chlorpyrifos (10 mg/$\textrm{cm}^2$, 50 mg/$\textrm{cm}^2$, 100 mg/$\textrm{cm}^2$, 250 mg/$\textrm{cm}^2$ in acetone) was determined in rabbits for 5 days considering the usage of chlorpyrifos short term highly exposure. The index of skin irritation by chlorpyrifos was increased in each dose and length of exposure dependent manners. Next, using benchmark dose (BMD$_{5}$) approach, the dose-response relationship was assessed to calculate the reference dose (RfD). The value of RfD was 2.84 $\mu\textrm{g}$/kg/day from 142.16 $\mu\textrm{g}$/kg/day BMD5 value divided uncertainty factor 50. Finally, we assessed human dermal risk of chlorpyrifos with exposure level and RfD. Skin absorbed levels were assumed with several exposure scenarios encounting the circumstances of exposure that application method, protection equipment and cloth, exposure time and exposure frequency during chlorpyrifos spraying. By the comparison of skin absorbed dose with the reference dose, it was identified that risk values (risk index) to skin chlorpyrifos exposure were 0.958 from the point of above results and it was recommended that the occurrence of hazard effect (skin irritation toxicity) of chlorpyrifos would not be expected. Risk index was smaller than 1 in the case of spraying vehicle mounted application, 1hour exposure time and wearing protective cloth exposure. Whereas, risk index was above 1 in the case of hand-held application, 2hour exposure time and wearing common cloth. Comparing two kinds of application method, total risk index of the hand held application (1.67) was higher than vehicle mounted (0.27). Therefore, chlorpyrifos skin exposure was mainly affected by application equipment and applied form. The results of risk assessment on the human dermal toxicity of chlorpyrifos should be required to control in keeping safety rules, skin surface area available for contact, spraying time ,and spraying frequency.y.
Arsenic (As) is a ubiquitous element found in several forms in foods and water. Although certain foods, such as marine fish, contain substantial levels of organic arsenic forms, they are relatively low in toxicity compared to inorganic forms. in contrast, arsenic in drinking water is predominantly inorganic and highly toxic. Chronic ingestion of arsenic-contaminated drinking water is therefore the major pathway posing potential risk to human hearth. since the early 1990s in Bangladesh ozone, arsenic exposure has caused more than 7,000 deaths and uncounted thousands shout symptoms of long-term arsenic poisoning. Significant portion of world populations are exposed to low to moderate levels of arsenic of parts per billion (ppb) to hundreds of ppb. As a consequence, the World Health Organization (WHO) and U.S. environmental health agencies, such as the Environmental Protection Agency (EPA) made arsenic their highest priority. Recently, the WHO, European Union (EU), and US. EPA lowered an acceptable level of 10 ppb for arsenic in drinking water In this article, various health effects of arsenic in drinking water were reviewed and the current status for risk assessment to regulate arsenic in drinking water was discussed.
Nitrate contamination problems from groundwater supplies have been reported throughout many countries in the world, including Korea. Nitrate salts can induce methemoglobinemia and possibly human gastric cancer. To reduce human health risk from nitrate in groundwater supplies, several nitrate risk-management strategies can be developed based on the acceptable level of human health risk, the reasonableness of nitrate-control cost, and the technical feasibility of nitrate-control methods. However, due to a lack of available information, assessing risk, cost and technical feasibility contains elements of uncertainty. In the present paper, a nitrate risk-management methodology using fuzzy sets in combination with a multiobjective decision-making (MODM) technique is developed to assist decision makers in evaluating, with uncertain information, various nitrate risk-management strategies in order to decide a proper strategy.
This study is carried out in order to propose a drought risk assessment methodology. This methodology is required to deal with practical questions that a variety of stakeholder often raise in the course of discussions on mitigation measures. With a focus on the socioeconomic aspect of drought, more particularly, residents' hardship from water scarcity, it suggests basic concepts and a system of methods in order to assess hazard, exposure, vulnerability and risk. The case study shows a considerable possibility of the methodology in evaluating potential levels of damages in a certain area, in identifying the boundary of districts where risk is disproportionately concentrated, and also in understanding the underlying risk factors of those districts. The authors think that the proposed methodology is able to offer risk information in terms of socioeconomic damages, and therefore contribute to reducing information gaps that policy-makers are currently encountered with.
Recently, it is continuously rising to concern about the health risk being induced by microorganisms in food such as Escherichia coli O157:H7 and Listeria monocytogenes. Various organizations and regulatory agencies including U.S.FPA, U.S.DA and FAO/WHO are preparing the methodology building to apply microbial quantitative risk assessment to risk-based food safety program. Microbial risks are primarily the result of single exposure and its health impacts are immediate and serious. Therefore, the methodology of risk assessment differs from that of chemical risk assessment. Microbial quantitative risk assessment consists of tow steps; hazard identification, exposure assessment, dose-response assessment and risk characterization. Hazard identification is accomplished by observing and defining the types of adverse health effects in humans associated with exposure to foodborne agents. Epidemiological evidence which links the various disease with the particular exposure route is an important component of this identification. Exposure assessment includes the quantification of microbial exposure regarding the dynamics of microbial growth in food processing, transport, packaging and specific time-temperature conditions at various points from animal production to consumption. Dose-response assessment is the process characterizing dose-response correlation between microbial exposure and disease incidence. Unlike chemical carcinogens, the dose-response assessment for microbial pathogens has not focused on animal models for extrapolation to humans. Risk characterization links the exposure assessment and dose-response assessment and involve uncertainty analysis. The methodology of microbial dose-response assessment is classified as nonthreshold and thresh-old approach. The nonthreshold model have assumption that one organism is capable of producing an infection if it arrives at an appropriate site and organism have independence. Recently, the Exponential, Beta-poission, Gompertz, and Gamma-weibull models are using as nonthreshold model. The Log-normal and Log-logistic models are using as threshold model. The threshold has the assumption that a toxicant is produce by interaction of organisms. In this study, it was reviewed detailed process including risk value using model parameter and microbial exposure dose. Also this study suggested model application methodology in field of exposure assessment using assumed food microbial data(NaCl, water activity, temperature, pH, etc.) and the commercially used Food MicroModel. We recognized that human volunteer data to the healthy man are preferred rather than epidemiological data fur obtaining exact dose-response data. But, the foreign agencies are studying the characterization of correlation between human and animal. For the comparison of differences to the population sensitivity: it must be executed domestic study such as the establishment of dose-response data to the Korean volunteer by each microbial and microbial exposure assessment in food.
Metal concentrations in the former Janghang smelter area were determined and human health risk of arsenic (As) with bioaccessibility was investigated. Site investigation of the area within 1.5 km from the Janghang smelter showed the As concentrations of 4.8~169.8 mg/kg (avg. 37.8 mg/kg). Large doses of nicotinic acid are associated with a number of adverse effects in human. The effects include flushing, skin itching, nausea, vomiting and gastrointestinal disturbance. This study was conducted to estimate daily intake of niacin by ingestion of food and multivitamin, and to identify risk value related with side effects, which can be caused by large dose intake in population having a typical lifestyle. Induced risk values by comparing only dietary intake level and intake level from both food and multivitamin with upper level as 35 mg/day were 0.53 and $0.81\~6.24$ respectively. Hazard Index over 1 means that occurrence of side effects would be expected in some population. When people intake multivitamin and functional food including niacin, risk value may increase more than risk value identified in this study.
