합곡(合谷)(LI4), 태충(太衝)(Liv3) 자침(刺鍼)이 정신적 스트레스를 가한 정상 성인의 심박변이도에 미치는 영향 (Effects of LI4, Liv3 Acupuncture for Mental Stress on Short-term Analysis of Heart Rate Variability)

  • Objectives : The objective of this study was to assess the effects of acupuncture applied at 114 and Liv3 points on heart rate variability (HRV) in normal subjects under mental stress. Methods: 36 healthy male and female subjects were recruited and randomized to a treatment group(18 subjects) or a control group (18 subjects). After 10-minutes rest period and instrumentation, both groups performed a mental stress test(Stroop color word test, Arithmetic test. four rule of arithmetic task) for l6-minutes. HRV was recorded before and after the mental stress. And then acupuncture needles were inserted on 114 and Liv3 for the study group. In the control group, they rested for l5-minutes. And Heart rate variability(HRV) was measured. Results: In the both groups, low frequency(LF) power, normalized low frequency power(LF(norm)) and ratio of low frequency to high frequency(LF/HF) was significantly increased after mental stress. Acupuncture induced significant decrease in low frequency(LF) power, normalized low frequency power(LF(norm)) and ratio of low frequency to high frequency (LF/HF) which was increased after mental stress test. In contrast, control group induced no changes. Conclusions : Acupuncture on 114 and Liv3 could be useful to decrease sympathetic activity and prevent the alteration of autonomic nervous system due to mental stress.

정신기능과 관련된 경혈에 대한 침 자극이 인체에 미치는 영향 (The Effects of Acupuncture Stimulation on Acupoints Related to Mind)

  • Objectives : This study aimed to understand the influence of acupuncture on the human body by comparing changes within human bodies before and after people in normal health are treated with acupuncture at the acupoints HT7 and PC9, which are related to mental functions. Methods : The study was performed from January 3, 2008 to March 5, 2008 on 60 healthy males and females in their 20s. HRV, EEG, skin conductance response, respiration and peripheral skin temperature were measured for 5 minutes before acupuncture simulation was applied to the acupoints HT7 and PC9. During 20 minutes of acupuncture treatment, the same items were continuously measured to determine whether there had been any changes, and they were then measured for 5 minutes after the removal of the acupuncture needles in order to implement a comparative analysis. Results : 1. The HRV measurement showed that in the course of before, during and after acupuncture stimulation, heart rate, HF and HF norm decreased significantly (P<0.05) at HT7. LF, LF norm, and LF/HF ratio increased significantly (P>0.05), while heart rate, HF and HF norm decreased significantly (P<0.05) at PC9. 2. Skin conductance response increased significantly (P<0.05) at PC9 during and after the acupuncture simulation periods, compared with the pre acupuncture period. 3. the peripheral skin temperature increased significantly (P<0.05) both at HT7 and PC9 in the course of before, during and after acupuncture stimulation. 4. Compared with the pre-acupuncture period, respiration rate increased both at HT7 and at PC9 during and after the acupuncture simulation periods, but not in a statistically significance. 5. In the EEG measurement, when compared with the pre-acupuncture period at HT7, mid ${\beta}$ wave decreased significantly (P<0.05) during acupuncture treatment. Compared with the measurements during acupuncture treatment at PC9, low ${\beta}$ wave increased significantly (P<0.05) after the acupuncture needles were removed. Conclusions : When acupuncture treatment is applied at the acupoints HT7 and PC9, the activation of parasympathetic nerves decreases and the activation of sympathetic nerves increases in the HRV measurement. It was determined that PC9 makes the sympathetic nerves become highly activated in a skin conduction response. The effect of stability in the brain wave seemed to bo shown at HT7 than PC9.

흉배부(胸背部)에 시행한 부항요법(附缸療法)이 자율신경계에 미치는 영향 - 심박변이도 측정을 통한 연구 - (The Effect of Dry Cupping Treatment Applied to Back-shu Points on the Autonomic Nervous System through HRV)

  • Objectives : This study was performed to evaluate the effect of dry cupping treatment applied to back-shu points on the autonomic nervous system. Methods : Two groups of sympathicotonia and normal with each 30 volunteers were set up for this experiment. The sympathicotonia group was selected by the criterion for sympathicotonia by the questionnaire composed of 11 items. After 10 minutes for environmental adaptation, the first HRV(heart rate variability) test was conducted, and then, dry cupping therapy was applied to back-shu points for 5 minutes to stimulate sympathetic ganglia lying along the spine. The second HRV test was carried out just after the cupping therapy under the same condition and then, the third test was repeated after two hours based on the first test time. Results : 1. In sympathicotonia group, SDNN(standard deviation of all normal R-R intervals), RMSSD(the square root of the mean of the sum of the squares of differences between adjacent normal R-R intervals), Ln(HF)(high frequency power), nmHF(normalized high frequency power) increased and mHR(mean heart rate), nmLF(normalized low frequency power) decreased significantly right after dry cupping therapy which means dry cupping affects on autonomic nervous system. The effect lasts in these items of nmLF, Ln(HF), nmHF, rLHF(rate ratio of LF/HF). 2. In normal group, SDNN, RMSSD increased and mHR decreased significantly right after dry cupping therapy, too. But, Ln(LF)(low frequency power), nmLF, rLHF unexpectedly increased and nmLF, rLHF stay increased up to 2 hours. Conclusions : The results suggest that the dry cupping therapy has effect on the autonomic nervous system. It is effective to stabilize hyper-sympathetic tone of people diagnosed as Sympathicotonia and activate parasympathetic tone.

Baseline heart rate variability in children and adolescents with vasovagal syncope

  • Purpose: This study aimed to evaluate the autonomic imbalance in syncope by comparing the baseline heart rate variability (HRV) between healthy children and those with vasovagal syncope. Methods: To characterize the autonomic profile in children experiencing vasovagal syncope, we evaluated the HRV of 23 patients aged 7-18 years and 20 healthy children. These children were divided into preadolescent (<12 years) and adolescent groups. The following time-domain indices were calculated: root mean square of the successive differences (RMSSD); standard deviation of all average R-R intervals (SDNN); and frequency domain indices including high frequency (HF), low frequency (LF), normalized high frequency, normalized low frequency, and low frequency to high frequency ratio (LF/HF). Results: HRV values were significantly different between healthy children and those with syncope. Student t test indicated significantly higher SNDD values (60.46 ms vs. 37.42 ms, P =0.003) and RMSSD (57.90 ms vs. 26.92 ms, P=0.000) in the patient group than in the control group. In the patient group, RMSSD (80.41 ms vs. 45.89 ms, P =0.015) and normalized HF (61.18 ms vs. 43.19 ms, P =0.022) were significantly higher in adolescents, whereas normalized LF (38.81 ms vs. 56.76 ms, P =0.022) and LF/HF ratio (0.76 vs. 1.89, P =0.041) were significantly lower in adolescents. In contrast, the control group did not have significant differences in HRV values between adolescents and preadolescents. Conclusion: The results of this study indicated that children with syncope had a decreased sympathetic tone and increased vagal tone compared to healthy children. Additionally, more severe autonomic imbalances possibly occur in adolescents than in preadolescents.

전중혈(膻中穴)($CV_{17}$) 습부항요법이 정상 성인의 심박변이도(HRV)에 미치는 영향 (The Effect of Venesection with Cupping Therapy at Jeonjung($CV_{17}$) on the Heart Rate Variability in Healthy Adults)

  • Objectives : This study was accomplished to investigate the effects of venesection with cupping therapy at Jeonjung($CV_{17}$) on the heart rate variability(HRV) in healthy adults. And based on that, we tried to find out how venesection with cupping therapy at Jeonjung($CV_{17}$) affects the activity and the balance of the autonomic nervous system. Methods : We investigated on 60 healthy volunteers consisted of 30 subjects in venesection with cupping therapy group and 30 subjects in control group. The study was established by randomized trial. venesection with cupping therapy was applied at Jeonjung($CV_{17}$) for 10 minutes in venesection with cupping therapy group. No treatment was executed in control group. We measured HRV 3 times : First, baseline measurement and two more times after treatment in venesection with cupping therapy group(after rest in control group). The SPSS 12.0 for Windows was used to analyze the data by paired t-test(in both group), independent-test(between the groups) and Chi-square test(between the sexs). Results : 1. In venesection with cupping therapy group, SDNN, Ln(TP), Ln(LF), Normalized LF, and LF/HF ratio increased significantly. Normalized HF decreased significantly. Mean HRT, RMSSD, PSI, Ln(HF), and Ln(VLF) were not affected. 2. In control group, Ln(HF) decreased significantly. Mean HRT, SDNN, RMSSD, PSI, Ln(TP), Ln(LF), Ln(VLF), Normalized LF, Normalized HF, and LF/HF ratio were not affected. Conclusions : This study suggests that venesection with cupping therapy at Jeonjung($CV_{17}$) increases the activity of the autonomic nervous system and has effects on the balance of the autonomic nervous system.

알레르기 비염 환자의 변증별 자율신경계 특성 분석 연구 (Autonomic Conditions in Allergic Rhinitis Depending on Various Pattern Identifications)

  • Objectives : We performed a clinical study to investigate autonomic conditions in persistent allergic rhinitis depending on various pattern identifications and the availability of heart rate variability (HRV) as a pattern identification diagnostic tool. Methods : 32 patients with persistent allergic rhinitis were asked to interview with doctor of Korean Medicine and perform the four pattern questionnaires (Cold-Heat Pattern, Phlegm Pattern, Yin Deficiency pattern, bloodstasis pattern). Then, they were examined their autonomic conditions with heart rate variability test. Results : Patients were classified as three pattern groups (Lung-stomach heat, Lung qi deficiency cold, Lung-spleen qi deficiency) by doctor. In the Lung qi deficiency cold group, Total power of the HRV (TP) and the power of the low frequency component (LF) significantly higher than in the Lung-stomach heat or Lung-spleen qi deficiency group (P < 0.05). Also, Patients were classified as 8 pattern groups (Cold/Heat, Phlegm/Non-phlegm, Yin deficiency/Non-yin deficiency, Bloodstasis/Non-bloodstasis) by four pattern questionnaires. Only in the Yin deficiency group, the power of the low frequency component (LF) significantly lower than in the Non-yin deficiency group (P < 0.05). There were not any significant differences in the rest groups. Conclusions : The result may provide that HRV doesn't reflect well the differences in the various pattern groups, and the HRV's availability is low. Continuous studies are needed to develop the objective and standardized pattern identification diagnostic tool for allergic rhinitis.

지체장애인의 운동참여와 심박변이도(HRV), 표정정서인식력과의 관계 (The Relationship between Physically Disability Persons Participation in Exercise, Heart Rate Variance, and Facial Expression Recognition)

  • 본 연구는 지체장애인들의 운동참여와 심박변이도(HRV), 표정정서인식력의 인과관계를 검증하는데 목적이 있다. 이를 위해 목적표집법(purposive sampling)을 이용하여, 장애인 볼링 및 론볼대회에 참여한 참가자와 충남지역의 생활체육 운동 프로그램에 참여하는 지체장애인 139명을 대상으로 하였다. 경기장과 클럽활동 시설에 직접 방문하여 연구목적을 구체적으로 설명을 한 후, 연구 참여에 동의한 대상자에 한해 운동량, 심박변이도(HRV), 표정정서인식력을 측정하였다. 측정된 결과는 SPSS 18.0, Amos 18.0 프로그램을 이용하여 평균 및 표준편차, 상관분석, 구조방정식모형을 분석되었고, 그 결과, 지체장애인들의 운동량은 자율신경계의 교감활성도와 부교감 활성도에 긍정적인 영향을 미치는 것으로 나타났다. 지체장애인들의 운동경력은 LF/HF에 긍정적인 영향을 미치는 것으로 나타났고, 부교감 활성도에는 부정적인 영향을 미치는 것으로 나타났다. 지체장애인의 교감활성도는 기쁨 정서인식력에 긍정적인 영향을 미치는 것으로 나타났으며, 운동량은 슬픔 정서인식력에 부정적인 영향을 미치는 것으로 나타났다. 이러한 연구결과가 지체장애인의 자율신경계와 표정정서인식력에 어떻게 관련성이 있는지 그 메카니즘에 대해 논의되었다.

심박변이도를 이용한 적응적 뉴로 퍼지 감정예측 모형에 관한 연구 (Implementing an Adaptive Neuro-Fuzzy Model for Emotion Prediction Based on Heart Rate Variability(HRV))

  • 감정을 정확히 예측하는 것은 환자중심의 의료디바이스 개발 및 감성관련 산업에서 매우 중요한 이슈이다. 감정예측에 관한 많은 연구 중 감정 예측에 심박 변동성과 뉴로-퍼지 접근법을 적용한 연구는 없다. 본 연구는 HRV를 이용한 ANFEP(Adaptive Neuro Fuzzy system for Emotion Prediction)을 제안한다. ANFEP의 핵심 기능은 인공 신경망과 퍼지시스템을 통합해 예측 모델을 학습하는 ANFIS(Adaptive Neuro-Fuzzy Inference System)에 기반한다. 제안 모형의 검증을 위해 50명의 실험자를 대상으로 청각자극으로 감정을 유발하고, 심박변이도를 구하여 ANFEP 모형에 입력하였다. STDRR과 RMSSD를 입력으로 하고 입력변수 당 2개의 소속함수로 하는 ANFEP모형이 가장 좋은 결과를 나타났다. 제안한 감정예측 모형을 선형회귀 분석, 서포트 벡터 회귀, 인공신경망, 랜덤 포레스트와 비교한 결과 본 제안모형이 가장 우수한 성능을 보였다. 연구 결과는 보다 적은 입력으로 신뢰성 높은 감정인식이 가능함을 입증했고, 이를 활용해 보다 정확하고 신뢰성 높은 감정인식 시스템 개발에 대한 연구가 필요하다.

담배 흡연에서 가열담배 사용으로의 단기간 전환에 따른 심장 자율신경 반응 (Short-term Effects of Switching from Cigarette Smoking to Using Heated Tobacco Products on Cardiac Autonomic Regulation)

  • 가열담배(heated tobacco products, HTPs) 에어로졸 내 유해 성분들의 수준은 담배 연기와 비교해 현저히 낮은 것으로 보고되었지만, HTPs 사용이 담배 흡연과 연관된 심혈관 위험을 완화할 수 있는지는 불분명하다. 현재 연구는 습관성 흡연자들을 대상으로 담배 흡연에서 HTPs 사용으로의 단기간 전환이 심장 자율신경조절(cardiac autonomic regulation, CAR)에 미치는 영향을 탐색하기 위한 목적으로 수행되었다. 건강한 20대 남성 흡연자 7명은 5일 동안 담배 흡연(cigaette smoking, CS), 3가지 서로 다른 HTPs 사용(IQOS use, IQ; lil SOLID use, LS; lil HYBID use, LH) 혹은 흡연 중단(non-smoking, NS)으로 구성된 개방표지, 무작위 교차설계 시험을 완료했다. 각 세션은 1주의 세척 기간으로 분리되었으며, 세션당 할당된 제품 사용 전과 사용 후 24, 48, 72, 96 및 120시간에서 생체 이물[CO (carbon monoxide) and COHb (carboxyhemoglobin)], 혈류역학적 변인(systolic and diastolic blood pressure) 및 CAR 조절을 반영하는 HRV (heart rate variability) 지표들의 수준이 평가되었다. 호기 CO 및 COHb 수준은 NS에서만 통계적으로 유의하게 감소했다. 모든 세션 내 혈류역학적 변인에서 통계적 변화는 없었다. 그러나r HRV 스펙트럼 분석에서 lnHF (log-transformed high frequency, lnHF)는 NS를 포함한 IQ와 LS에서 유의하게 증가하였으며, HFnu (normalized HF)는 NS와 LH에서 통계적으로 증가하였다. lnHF와 HFnu는 각각 세션 간 시간에 따른 유의한 상호작용효과를 나타냈다. 현재 연구는 습관성 흡연자들에게서 HTPs 사용으로의 단기간 전환이 담배 흡연에 의한 심장 미주신경 긴장도 저하를 개선하여 CAR에 영향을 미칠 수 있음을 시사한다.

제주 곶자왈숲 삼림욕 후에 심박동변이에 미친 영향 (Effects of Heart Rate Variability after Visiting the Gotjawal Forest in Jeju)

  • 제주 곶자왈 삼림욕 후 혈관연령타입, 혈관연령지수, 심박세기, 혈관탄성도, 심장의 잔혈량 및 평균맥박의 비율 변화를 조사하기 위하여 대조군은 14명과 실험군은 49명을 대상으로 손끝의 맥파 측정기로 측정하여 변화를 통계적으로 분석 연구하는 것이 목적이었다. 연구결과에서는 혈관연령타입, 혈관연령지수, 심박세기(b/a 비율), 심장의 잔혈량(d/a 비율), 평균맥박의 비율의 변화는 대조군은 사전·사후의 평균치 차이는 유의성이 없었으나, 실험군에서는 유의성이 있었으며(p<.011), 사후의 분산분석에서는 그룹 내 및 간에서도 유의한 차이를 보였다(p<.011). 혈관탄성도(c/a 비율)변화는 대조군과 실험군은 사전 사후의 평균치 차이는 유의성이 없었으며, 사후의 평균 측정치의 분산분석에서는 그룹 내에서는 유의한 차이가 있었으나, 그룹 간에서는 유의성이 없었다. 평균맥박의 변화는 그룹 내에서는 유의성이 없었으나, 그룹 간에는 유의성이 있었다. 결론적으로 제주 곶자왈숲에서 2시간 이상의 삼림욕 후에는 심박동변이(HRV)가 유의성이 높게 나타나서 b/a, c/a, d/a, e/a 비율 변이가 HRV의 변화 측정에 유용한 도구라 생각하며, 삼림욕이 심혈관의 건강 유지에 우수한 자연요법이라 판단한다.
