The Trend in Household Catastrophic Medical Expenditure according to Healthcare Coverage Types and Its Associated Factors (의료보장 형태에 따른 연간 가구 과부담 의료비 지출 추이와 관련요인)

  • This study aims to examine the trend in household catastrophic medical expenditure according to the healthcare coverage types and its associated factors based on the raw data of the Korean Health Panel over the years 2008 to 2011. Correspondence analysis was used to investigate the trend in the incidence rates of annual catastrophic medical expenditure and generalized estimating equation to examine the factors influencing the incidence of catastrophic medical expenditure. The annual mean incidence rates of household catastrophic medical expenditure were 25.1%, 15.4%, 10.1%, 5.4% and 3.2% in the threshold levels of 10%, 15%, 20%, 30%, and 40% respectively. The incidence rate of household catastrophic medical expenditure was higher when the total annual household income was lower, the education level of the householder was lower, the healthcare coverage type was National Health Insurance, the householder had disability, the age of the householder was older, the number of household members was smaller, the subjective health status of household members was lower, and the prevalence rate of the chronic disease of the household was higher(p<0.05). Therefore, a policy for vulnerable households with older or patient members of chronic diseases should be established.

Assessment of the level and identification of airborne molds by the type of water damage in housing in Korea (국내 주택에서 물 피해 유형에 따른 부유곰팡이 농도 수준 평가 및 동정 분석)

  • Mold grows more easily when humidity is higher in indoor spaces, and as such is found more often on wetted areas in housing such as walls, toilets, kitchens, and poorly managed spaces. However, there have been few studies that have specifically assessed the level of mold in the indoor spaces of water-damaged housing in the Republic of Korea. We investigated the levels of airborne mold according to the characteristics of water damage types and explored the correlation between the distribution of mold genera and the characteristics of households. Samplings were performed from January 2016 to June 2018 in 97 housing units with water leakage or condensation, or a history of flooding, and in 61 general housing units in the metropolitan and Busan area, respectively. Airborne mold was collected on MEA (Malt extract agar) at flow rate of 100 L/min for 1 min. After collection, the samples were incubated at $25^{\circ}C$ for 120 hours. The cultured samples were counted and corrected using a positive hole conversion table. The samples were then analyzed by single colony culture, DNA extraction, gene amplification, and sequencing. By type of housing, concentrations of airborne mold were highest in flooded housing, followed by water-leaked or highly condensed housings, and then general housing. In more than 50% of water-damaged housing, the level of airborne mold exceeded the guideline of Korea's Ministry of Environment ($500CFU/m^3$). Of particular concern was the fact that the I/O ratio of water-damaged housing was greater than 1, which could indicate that mold damage may occur indoors. The distribution patterns of the fungal species were as follows: Penicillium spp., Cladosporium spp. (14%), Aspergillus spp. (13%) and Alternaria spp. (3%), but significant differences of their levels in indoor spaces were not found. Our findings indicate that high levels of mold damage were found in housing with water damage, and Aspergillus flavus and Penicillium brevicompactum were more dominant in housing with high water activity. Comprehensive management of flooded or water-damaged housing is necessary to reduce fungal exposure.

A Study of Insomnia and Depression of Elderly Welfare Facility Users in a City (일 도시 노인복지시설 이용자들의 불면증 및 우울증에 대한 연구)

  • Objectives : The aim of this study was to analyze the relationship between depressive symptoms and sociodemographic factors associated with geriatric depression and insomnia and to examine the effects of these factors on depression. Methods : The severities of insomnia and depression in elderly aged 60 and older lived in Gwangmyeong city were evaluated and the related sociodemographic factors were investigated. From April 20, 2016 to December 1, 2016, Gwangmyeong city Mental Health Sevices consignment by department of psychiatry of a university-affiliated general hospital conducted surveys and interviews for total 837 elderly peoples lived in Gwangmyeong city by visiting welfare center and wards located in the city. Structured interviews were conducted using Insomnia Severity Index(ISI) and Short form-Geriatric Depression Scale(S-GDS) to examine the relationship between sociodemographic factors and the severities of insomnia and depression. Results : There were significant differences in S-GDS mean scores and age(below 70 years old, 70s, 80s, above 90 years old), type of health care(health insurance, medical aid), type of residency(own, not own) and marital status(single, married, divorced or widowed).There was a significant difference in education level, especially between not educated(and/or) elementary graduates and college graduate(F=3.227, p=0.012). Also, there were significant differences in age, type of health care, type of residency, number of household on divided S-GDS score above and below 10 scores(p<0.05). Insomnia measured by ISI was not significantly associated with sociodemographic factors(p>0.05), but was significantly associated with depressive symptoms(p<0.05). These findings suggest more severe insomnia symptom indicated the higher probability of depression and elderly with depressive symptoms had more severe insomnia. Conclusions : Geriatric depression has significant relationships with age, type of health care, type of residency, marital status, education and number of households. In addition, insomnia which is main symptom of depression in elderly, has important role in predicting the severity and diagnosis of depression.

The Abolition Type and The Regional Characteristics of The Elementary Schools in Chungbuk Province (忠淸北道의 國民學校 廢校類型과 그 地域的 特性)

  • The migration of population into the city has been on the increase according as Korea has been industrialized repidly since the 1960's. And there is a steady decrease in rural population. Thus lack of the number of the students forced many elementary Schools to be abolished. The aim of this study is to grasp the abolition types and the regional characteristics with the subject region of Chungbuk province. From the viewpoint of the increasing abolition of the elementary schools, I think it is very important to understand how the elementary schools have been abolished so far and predict how the subject region will have been changed in geography. Data for this study are based on Annual Establishment-Abolition Situation of the Schools published by Chungbuk office of Education in 1992, and many Kinds of the statistical reports, and the interview with the related. The results are as follows: 1. By examining the change of the number of the elementary schools and students in Chnugbuk, the numder of the students had also decreased since 1969 and was less than the half in 1990. As the number of the schools began to decrease ten years later than the students began to, the abolition of the elementary schools has started in reality from 1980's. 2. The 72 elementary schools were aboilshed between 1980 and 1992: the principal school is 9.7%, the branch school is 90.3%. The most fifteen schools are abolished in Yongdong-county and Chechon-county, and the least one school is abolished in Chechon-city and Okchon-county, and there is no abolition in Chongju-city and Chungju-city: According to the type of the abolition process, the least seven principal schools are abolished, and the principal school is reorganized as a branch school and twenty eight branch schools are abolished, and the most thirty seven branch schools are abolished. 3. When special change of the abolition is classified into the first perio (1980-1986) and the second period (1987-1992), in the first period the principal and branch schools were abolished and they are 13.9% of total abolition. The abolition out of them by building a dam is 60%. The principal schools in the submerged area though they have many students, were abolished. In the second period sixty two branch schools are abolished and they are 86.1% of total abolition. The most fifteen schools are abolished in Yongdong-county, thirteen in Chechon-county, seven in Tanyang-county, six in Chongwon-county, five in Chungwon-county and Koesan-county. Unlike the first period, the schools were abolished in this period because the number of students was so small. In this period sixty branch schools were abolished. All the students in the abolished schools except six schools transfered to the principal schools. The 58 school authorities help the students attend school by bus or support the expenses for attending school after that. 4. The abolition types of city, county and myon are classified into five types by the number of the abolished schools. The most forty nine abolished schools in type II are 68.1 of the total abolition. The least three abolished ones in type I are 12.5%. Considering the relation between the abolition type ane the number of schools and students, the number of the schools, increased in type I, II, III, V except IV from 1980 and then have decreased by abolition since 1980, while the more students decreased than they did in 1970 and the more the abolished school increases, the less the students decreases. The average students per school decreased in every abolition type and the most students decreased in type IV. 5. Considering the relation between the abolition type and the regional characteristics, most abolished schools were located between 100m and 300m above the sea level and it is 71% of the total abolition. The region without the abolition is high in the ratio of the cultivate land, ratio of rice field, and the part-time farmer, but the region with many abolition is low in the ratio of cultivated land. As for the manufacturing there are the most city, county and myon in the abolition type in Youngdong-county and Chechon-county where the manufacturing ratio of employing is low but Chongju-city without the abolition is a region where the manufacturing ratio is high. Consequently the development of the manufacturing causes the population to emigation out and the decrease of the population leads the transport is difficult of access, the facilities sold after being abolished are not being used in many ways. 7. Take an example of Youndong-county where the most schools were abolished, I have examined the school district and the population characteristics of the abolition. Though there were more villages, households, populations in the region that is higher than low above the sea level, the schools were abolished. Therefore we know that above the sea level had a great effect on the abolition. As a result of the regional analysis of the abolition, many schools were abolished by the artificial buildings such as a dam in the early 1980's but the schools in the late 1980's were abolished ten years later after the students decreased. More schools were abolished in the region where the manufacturing industry didn't develop. And the higher the school position was above the sea level, the sooner the school was abolished. It is also proved that both the beautiful natural scenery and accessibility are the important factor in using the abolished facilities practically.

Significance Analysis of Facility Fires Though Spatial Econometrics Assessment (공간계량분석 방법에 따른 시설물 화재 발생 유의성 분석)

  • Recently, large and small fires have been happening more often in Korea. Fire is one of the most frequent disasters along with traffic accidents in korean cities, and this frequency is closely related to the land use and the type of facilities. Therefore, in this study, the significance of fires was analyzed by considering land use, facility types, human and social factors and using 10 years of fire data in Jinju city. Based on this, OLS (Ordinary Least Square) regression analysis, SLM (Spatial Lag Model) and SEM (Spatial Error Model) using space weights, were compared and analyzed considering the location of the fire and each factor, then a statistical model with high suitability was presented. As a result, LISA analysis of spatial distribution patterns of fires in Jinju city was conducted, and it was proved that the frequency of fires was high in the order as follow, central commercial area, industrial area and residential area. Multiple regression analysis was performed by integrating demographic, social, and physical variables. Therefore, the three models were compared and analyzed by applying spatial weighting to the derived factors. As a result of the significance test, the spatial error model was analyzed to be the most significant. The facilities that have the highest correlation with fire occurrence were second type neighborhood facilities, followed by detached house, first type neighborhood facilities, number of households, and sales facilities. The results of this study are expected to be used as significant data to identify factors and manage fire safety in urban areas. Also, through the analysis of the standard deviation ellipsoid, the distribution characteristics of each facility in the residential area, industrial area, and central commercial area among the use areas were analyzed. In, the second type neighborhood facility with the highest fire risk was concentrated in the center. The results of these studies are expected to be used as useful data for identifying factors and managing fire safety in urban areas.

  • Each ethnic group has a different cultural background and has developed its own culture in the name of a tradition. The interaction between different cultures is ever increasing through the process of acculturation or culture contact The purpose of this study is to provide baseline information about domestic living of Korean immigrants in Australia and Australians for a larger cross-cultural study project. As the first step, the usage of domestic space and seating styles of 52 Korean households in Melbourne were analyzed. Ethnographic research was conducted, utilizing a questionnaire. The findings of this research are as follows: 1. The most popular type of Living-Dining-Kitchen arrangement was the one which the kitchen is separated from the living and dining area. The level of satisfaction was the highest when the living room, dining room, and kitchen were all separated. 2. A laundry room was indispensable for Koreans in Australia, and they wanted to dry laundry and do ironing in there. Most people were satisfied with the toilet separated from the bathroom A drain hole on the floor of the bathroom was not indispensable for most Korean-Australians. 3. Korean-Australians tend to engage in various activities in their individual rooms, and they estimated that the size of most rooms were small. They also wanted to renovate the house to expand the rooms. 4. The seating style of Korean-Australians was mostly chair-seating. Yet, they often made beds on the floor for guests, and made kimchi sitting on the floor. Also, when they were relaxing, they were using both chair-seating and floor-seating. 5. Korean-Australians were not very satisfied with the use of floor carpets because of the dust collected on the carpets, and the difficulty to clean. It may be related to their preferred floor seating style, as well. 6. Almost all Korean-Australians take off their shoes inside of the house for hygienic reasons. They had a shoes cabinet at the entrance inside of the house. 7. The most popular heating system was ducted heating. The level of satisfaction about this was moderate because hot air contains lots of dust and it makes rooms very dry. Many were using electric blankets and their desired heating system was Ondol (heated floor). 8. Korean-Australians thought that the living room was the most important place for the family, and believed it should be decorated well to entertain guests. They also pointed out that the lighting was not bright enough in general.

The status quo of offering help in emergency room on pediatric patients with 119 rescuer - Focused on the Emergency Center of C National University Hospital - (119구급대를 이용한 소아환자의 응급실 내원 현황 - C대학병원 응급의료센터를 중심으로 -)

  • It is happening a lot that the pediatric patients who are offering help in emergency room with background 119 rescuer require emergency care compared to adults. Our study was conducted to have a clear grasp of the status quo of pediatric patients who are offering help in emergency room with 119 rescuer. METHOD & OBJECT pediatric patients under 8 age who were offering help was conducted as objects by 119 cases from January, 2006 to January 2007, with 119 rescuer at C university hospital in Gwangju metropolitan city, which were 87 cases for a boy(73.1%), 32 cases for a girl. Based on emergency diary of emergency members and the medical treatment record of emergency room in C university hospital, a disease and a type of an external wound were analyzed according to ages of these patients, a time zone and season. RESULT In terms of an age, 25 cases(21.0%) have less than 1 age extremelymuch, in case of emergency, 61 cases(51.3%), which were more than non-emergency, in terms of happened places of patients, there were households up to 78 cases(65.5%) extremely much. In terms of happened time, 13:00 to 18:00 have highest data up to 48 cases(40.3%). According to season, there were no big differences in 4 seasons(spring : 30.3%, summer : 24.4%, fall : 29.4%, winter : 16.0%). In case of being offered help at emergency room due to diseases, 1-2 ages have highest estimate with 55.7%, in terms of a time zone, 19:00-24:00 have highest with 33.7%, in terms of season, summer(6, 7, 8) have highest with 32.8%. in case of traffic accident, 7-8 age have highest with 47.1%, according to a time zone, 13:00 to 18:00 have highest with 88.2%, according to season, fall have highest with 64.7%. In case of an external wound, 3-4 age have highest with 38.2%, according to a time zone, 13:00-18:00 have highest with 41.2%, according to season, spring have highest with 38.2%. CONCLUSION It shows that in case of pediatric patients who were offering help at emergency room with 119 rescuer, a pediatric under 1 age was due to a disease, in case of an external wound, there were 3-4 age, in case of traffic accident, 7-8 age have highest, a case for emergency have highest.

A Study on the Value Analysis of School Forest (학교숲 속성별 가치평가 연구)

  • This study intends to analyze the value of school forests, one type of urban forest. For this purpose, four attributes of school forests were investigated, considering ecological, educational, social and economic values using a conjoint model as the stated preference. Based on literature reviews, the levels of the four attributes were selected, and a questionnaire survey was given to 279 urban residents divided into 2 groups: those impacted by school forests and those not. The study results suggest that the most important attribute of school forests is economic value, and next is ecological, social and educational value according to the part-worth model. The fitness level of the model is 0.900(total group) which is very significant. As for the economic value, free and 1,000 won are more critical factors than the other 2 levels, 5,000 won and 10,000 won and air pollution purification and making the school landscape are more critical factors than small habitats and microclimate factors. In addition, regarding the social value related to residents' leisure activities,the utility of nature observation is higher than walking and exercising. Finally, for educational value, understanding nature's importance is more critical than the emotions and learning of students. The estimated WTP per household/month is 3,580 won, the group related to school forestsis 3,650 won and the non-related group is 3,540 won. Based on these results, the estimated total economic value of all households per year is 6,820 hundred million won. The group related to school forests is 6,970 hundred million won and the non-related group is 6,750 hundred million won.

Comparison of Health-related Quality of Life of Elderly Aged more than 65 Years according to the Insurance Type (의료보장유형에 따른 65세 이상 노인의 건강관련 삶의 질 비교 연구)

  • In this study, we analyze the community health survey data for 3 years from 2011 to 2013 were analyzed for evaluation. This study attempts to comprehensively evaluate differences between elderly medical aid beneficiaries and elderly health insurance beneficiaries in terms of health-related quality of life, All of elderly medical aid beneficiaries who responded community health survey were included for study and elderly health insurance beneficiaries were randomly selected as a control group by matching gender, age and location. The findings show that the elderly medical aid beneficiaries can not receive sufficient health care services, even though they have pay a low socio-economic status, spouse or fewer households enemies and poorer health behavior and higher morbidity of chronic diseases than the elderly health insurance beneficiaries do. But elderly health insurance beneficiaries also showed that not enough care utilization is high morbidity of chronic diseases. These factors aggravate the medical aid beneficiaries' health-related quality of life This characteristic was analyzed to lower health-related quality of life of the elderly. Therefore, it is necessary to strive for the development of appropriate services and programs taking into account the characteristics of the elderly that may go though a limited resource and promote the quality of life for the elderly more effectively re-evaluation of social services systems and programs related to elderly.

  • The number of Aged Apartment units is expected to increase as time went on. Living standards are getting better and they want a new apartment space as the economy progresses. Therefore, it is necessary to prepare for the increasing remodeling market through the feasibility evaluation method that can be applied to the remodeling project of the apartment house. The purpose of this study is to analyze the social pricing factors affecting the Officially assessed individual House Price for the analysis model of commercial house remodeling. The collected samples were analyzed using multiple regression analysis of 350 prices included in 127 lots. Middle school level, high school level, total number of households, and floor area ratio were extracted. As a result of comparing the Officially assessed individual House Price by applying to the remodeling case, the difference between the existing Officially assessed individual House Price and the improvement Officially assessed individual House Price is different. The accessibility with the subway station is included in the land price, and there is no change in the number of stories and directions because it is customized remodeling. There was a difference in the disclosure price depending on the type of factor extraction by the evaluator in a batch application of the disclosure price factors. The research can be used as a model for future remodeling business feasibility analysis.
