갱년기 여성 안면홍조의 주관적 측정과 객관적 측정의 상관성 (Correlation between Subjective and Objective Measurement of Climacteric Women's Hot Flashes)

  • Purpose: The purposes of this study were to explain the phenomena of hot flashes in climacteric women by using Mexameter, Skin Thermometer, Corneometer, and Laser Doppler Perfusion Imager (LDPI) objectively and to identify the interrelation between the subjective and objective measurements of hot flashes by comparing the two as reported in retrospective questionnaires. Methods: The participants were one hundred women (45-60 yr) who were not currently on hormone therapy, and had reached hot flash scores of 10 or higher. Hot flashes were measured in a temperature and humidity controlled room for 7 hr from 10 am to 5 pm. Hot flashes were measured subjectively and recorded via the Hot Flash Diary Report. When participants felt the hot flashes, they were measured objectively by Mexameter, Skin Thermometer, Corneometer, and LDPI. Results: The frequency of hot flashes in participants ranged from 1 to 7 times. When hot flashes occurred in participants, the erythema, skin temperature, skin hydration, and blood perfusion showed statistically significant changes in all measurements. But, the subjective and objective measurements of hot flashes showed only weak correlations. Conclusion: Results indicate a need for future research with subjective and objective measuring instruments chosen depending variations identified for the study.

  • This case study reports on the effectiveness of Korean medicine treatment for menopausal hot flashes in stroke patients. During hospitalization, a female patient with medial medullary infarction was diagnosed with menopausal hot flashes and received treatment with traditional Korean medicine and acupuncture. The assessment was performed by monitoring daily habits to relieve hot flashes. The hot flash index, menopause rating scale (MRS), menopause-specific quality of life (MENQoL) questionnaire, and Kupperman index (KI) were also used. After 28 days of treatment, the hot flash index improved from 27 to 2, MRS improved from 11 to 2, MENQoL improved from 40 to 21, and KI improved from 22 to 6. Thus, this case suggests that Korean medicine treatment may be effective for menopausal hot flashes in stroke patients.

A Randomized trial to investigate the effect of Asparagus racemosus root tablet in Relieving Postmenopausal Hot Flashes

  • Background and Objectives: As per the available data, 75% peri and postmenopausal women experience hot flashes having negative effect on their quality of life. Using herbal medicines are stepping forward to alleviate hot flashes as the available hormonal medications have been reported the presence of side effects. This study was planned with the intention to investigate the effect of Asparagus racemosus root in the treatment of postmenopausal hot flashes in comparison to the placebo. Methods: This prospective patient blinded clinical trial randomly assigned 40 postmenopausal women with symptoms of hot flashes to receive either test drug (n=20) comprising Asparagus racemosus tablet of 1g in the dosage of 2 tablet thrice in a day for the duration of 60 days continuously or to receive placebo tablet prepared from roasted wheat flour in the same dosage and duration as test drug. The efficacy was determined by reduction in the frequency and intensity of hot flashes assessed by daily diary along with the improvement in quality of life assessed through Hot Flash-Related Daily Interference Scale. Statistical analysis was accomplished by student t test and Chi-square/Fisher Exact test. Results: Test drug significantly (p <0.001) reduced the intensity and frequency of hot flashes as well as improved the quality of life without showing any side effect. Discussion & Conclusion: According to Unani scriptures, the treatment is to treat the underlying cause, such as aberrant temperament and psychological and environmental issues. Unani drugs have emmenagogue, anti-inflammatory, analgesic, and neuroprotective qualities that can help alleviate premenstrual symptoms. Unani herbs such as C. sativus, Vitex, agnus castus, P. vulgare, N. Jatamansi, M. officinalis, and Z. officinalis have been clinically demonstrated to be effective in PMS. Thus, traditional knowledge authentication and conservation are vital for future research and appreciated for application in the modern day. Furthermore, randomised controlled trials, comprehensive reviews, and meta-analyses are suggested.

  • Purpose: This research investigated the degree and predictors of hot flashes and hypogonadism symptoms in patients with prostate cancer receiving hormone replacement therapy. Methods: The subjects were 111 patients with prostate cancer receiving hormone replacement therapy in two university hospitals located in D city. The measurement tools included Hot Flash Diary and AMS (Aging Male's Symptoms rating scales). The data were analyzed using t-test, ANOVA, and binary logistic regression analysis. Results: The percentage of patients who experienced hot flashes among the participants was 14.4%. The predictors for hot flashes were eating irregularly, having coffee frequently and the types of hormone. The average score of hypogonadism symptom was 2.16 out of five-point scale and the highest score of hypogonadism symptom was the sexual symptoms (2.77 out of five-point scale). The predictors for hypogonadism symptom were eating habits and years of having the illness. Conclusion: These findings provide the information that irregularly eating habit was an important factor in hot flashes and hypogonadism symptoms of the participants. Therefore the development of a nutritional education encouraging regular meals is necessary for the given population.

유방암 환자의 호르몬 보조 요법 중 발생한 안면홍조에 대한 침 치료의 고찰 (A Review of Acupuncture on Hot Flashes during Adjuvant Endocrine Therapy in Breast Cancer Patients)

  • Objects: This study was conducted to examine the effect of acupuncture on hot flashes symptoms that occurred after adjuvant endocrine therapy in breast cancer patients. Methods: A total of 7 electronic databases, including Pubmed, Cochrane Central, Embase, and CNKI, were used to search for randomized controlled trials studies on acupuncture treatment for hot flashes that occurred after adjuvant endocrine therapy in breast cancer patients. Studies published from January 2014 to March 2024 were targeted, and Breast Cancer, Hot Flashes, Acupuncture, and Electroacupuncture were used as search keywords. The intervention method, treatment period and follow-up period, results, side effects, and acupuncture points were analyzed. Results: According to the inclusion criteria, 7 studies were finally selected. Acupuncture, electroacupuncture, acupuncture and enhanced self-care were combined as intervention methods of the treatment group, and the most common control group was the use of sham acupuncture. The selected study was evaluated by the symptoms of hot flashes, quality of life, depression, and quality of sleep. In terms of hot flashes, quality of life, and depression, a significant number of treatment groups showed improvement results than control group. The most commonly used acupuncture points in the study were 百會 (GV20), 關元 (CV4), 氣海 (CV6) each of which was used twice. Conclusions: According to this study, the improvement of symptoms was confirmed when acupuncture and electroacupuncture treatment were used for hot flashes that occurred during breast cancer adjuvant endocrine therapy.

  • Objectives: The purpose of this study is to find diagnostic points and define the cut off values of hot flashes by using digital infrared thermographic imaging. Methods: Thermographic images of 75 patients with hot flashes (HF, n=35) and non-hot flashes (NHF, n=40) were retrospectively reviewed. We used the temperature difference between Ex-HN3 and CV17, LU4, CV12, CV4 for diagnosing hot flashes. The temperature differences of between two groups were analysed using independent samples t-tests. The cut off values were calculated by received operating characteristic curve analysis. Analyses were undertaken using SPSS version 17.0. and p-value of <0.05 was considered significant. Results: The temperature difference Ex-HN3 and LU4 were the most significantly different between groups (p<0.001). Using receiver operating characteristic curve analysis, the sensitivity, specificity, and area under the curve were 65.7%, 72.5%, 0.729, respectively. The optimum cut off value was defined as $1.00^{\circ}C$. Conclusions: These results suggest that the digital infrared thermographic imaging is a reliable instrument for estimating hot flashes.

안면홍조를 호소하는 뇌경색 환자 한방치료 치험 1례 (A Case Study of a Stroke Patient with Hot Flushes Treated with Traditional Korean Medicine)

  • Methods: A stroke patient with hot flashes was treated for 4 weeks with acupuncture, herbal acupuncture, and herbal medicine. We then evaluated the frequency, intensity, and score (frequency×intensity) of the hot flashes, as well as the Pittsburgh Sleep Quality Index (PSQI), which shows subjective sleep quality. Results: After 4 weeks of traditional Korean medicine (TKM) treatment, the patient showed reductions in the frequency of hot flashes from a score of 7 to 4, the intensity of hot flashes from a score of 4 to 2, the scoring (frequency×intensity) of hot flashes from 28 to 4, and the PSQI from 13 to 4. Conclusions: This study validated the effect of TKM as a treatment for hot flashes in patients with stroke.

  • Objectives: Hormone replacement therapy may relieve many symptoms of menopause; however, hormone replacement therapy may increase the risk of breast cancer, stroke, and dementia. This case study describes the effect of Hoechunboeum-tang on the menopausal disorder of hot flashes, which has been well documented as a cause of distress to patients undergoing menopause. Methods and Results: Two female patients suffering from severe hot flashes related to menopausal disorders were administered the herbal medicine, Hoechunboeum-tang. Measurements included hot flash frequency over a 24 hour period, hot flash severity (visual analogue scale), and hot flash-related daily interference and were recorded during the last week before herbal medicine and again after 7 and 15 days of herbal treatment. The patients were asked about their hot flash symptoms 6 months later and both reported that the symptoms had been successfully reduced after herbal treatment and were still reduced after 6 months. Conclusions: Hoechunboeum-tang could have a beneficial effect on menopausal disorder with hot flashes.

  • Objectives: To report the effect of Korean medicine on a patient with insomnia from severe hot flashes. Methods: The patient suffered from extreme hot flashes for months. Symptoms were so severe that the patient attempted suicide. After starting psychiatric medication, symptoms persisted and hospitalization began. During hospitalization, herbal medicine, acupuncture, and psychotherapy were conducted. For evaluating therapeutical effect, Digital Infrared Thermal Imaging was performed twice during the treatment process. It was divided into major facial area and back area. The facial area was divided into two small units to measure the difference in temperature between two points. The back area was measured in the same way. Results: The temperature difference between the two points decreased over time and the patient's subjective symptoms reduced. Conclusions: Korean medicine therapy can improve symptoms of patients with insomnia accompanied by hot flashes.

  • This study reported a case of hot flashes in a patient after coronavirus disease-2019 (COVID-19) infection. The patient in this case was hospitalized for eight days and improved after the administration of Korean medicine, mainly Yangkyuksanwha-tang. We evaluated the patient's subjective discomfort daily, and the patient kept a diary of hot flashes every day. After treatment, the degree and frequency of hot flashes were greatly improved and continued two weeks after discharge. In addition, the accompanying hyperhidrosis improved from Grade 2 to Grade 0. This study suggests that Korean medicine might be effective in combating the hot flashes of soyangin-type patients.
