A Case Report of Menopausal Syndrome treated by Herbal medicine based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 준거하여 한약 투여 후 호전된 갱년기 증후군 1례 임상 보고)

  • Objective : This case report aims to demonstrate the effect of Herbal medicine on menopausal syndrome. Methods : A 54 years woman suffered from menopausal syndrome with hot flush, insomnia and dyspepsia. According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung and treated by Sosiho-tang decoction at first. But the next time the patient visited, she was treated by Oryeong-san decoction. The result of administrations were evaluated by Kupperman's index(KI), Menopause rating scale(MRS). Results : After administration of Sosiho-tang decoction for 35 days, KI was decreased from 50 to 19, MRS was decreased from 9 to 4. But her chronic dyspepsia wasn't getting better. So the KMD prescribed Oryeong-san decoction. After administration of Oryeong-san decoction for 14 days, KI was decreased from 19 to 0, MRS was decreased from 4 to 1. And her chronic dyspepsia was functionally improved. Conclusions : The patient completely recovered from menopausal syndrome and dyspepsia as treated by Sosiho-tang decoction and Oryeong-san decoction according to Diagnostic system based on Shanghanlun provisions.

A Clinical Study of Climacteric Symptoms after Hysterectomy (자궁적출술 후에 갱년기 증후군으로 이행된 환자의 임상보고)

  • Purpose : The purpose of this study was to investigate the clinical application of Traditional Korean Medicine(TKM) treatment to climacteric symptoms after hysterectomy. Methods : The patient was a 51-years-old woman who suffered from hot flush, arthralgia, fatigue et al after hysterectomy. The patient was treated by TKM such as herb medication and acupuncture therapy. And her health status was evaluated by Kupperman's index and Medical Outcome Study Short Form 36( SF-36). Results : Climacteric symptoms after hysterectomy was reduced( Kupperman's index from 40 to 20). It got better that Physical Functioning(PF), General health(GH), Bodily pain(BP), Role limitation-Emotion(RE) and Mental Health(MH). So, it means that her health status was improved. Conclusion : TKM is expected to have positive effects on climacteric symptoms after hysterectomy, and can be improve woman's health status after hysterectomy.

An analysis of the Relationship between Depression and Menopausal Syndroms in Women at Midlife (중년여성의 우울증과 폐경증후군과의 관계)

  • This study was done for the purpose of analysing the relationship between menopausal syndrome and depression in the pre and post menopausal women's group. Data were collected by a questionnaire from November, 1995 to December, 1996. The subject were 244 midlife women(94 subjects were in pre-menopausal period, 150 subject were in post-menopausal period). The instruments used for this study were the menopausal syndrome scale by Neugarton and depression scale by Zung. Data were analyzed with the SPSS/PC using frequency, t-test, and Pearson correlation coefficients. The results of this study were as follows ; 1. The most serious menopausal symptom in the midlife women was 'back pain and joint pain'. 2. The item of 'Hot flush'(t=-2.78, p<0.01), 'sweating'(t=-2.31, p<0.05) and 'nervousness'(t=-2.13, p<0.05) as menopausal syndromes were statistically significant in the two groups ; Post menopausal group were higher then premenopausal group. 3. Postmenopausal group were a little higher then premenopausal group as depression level. It was none statistically significant in the two groups. 4. The relationship between menopausal syndromes and the level of depression was statistically significant in the group of post-menopausal group(r=0.2083, p<0.01).

The efficacy and safety of a Huanglian-jie-du decoction on Hwa-byung patients: A study protocol for a randomized controlled trial

  • Objective: Hwa-byung is one of the cultural concept of distress in Korea resulted from chronic accumulated anger. It is characterized by various symptoms like stuffy in the chest, hot or heat sensation, something pushing up in the chest, feeling of mortification, and a flush of anger. This protocol aims to explore the efficacy and safety of Huanglian-jie-du decoction on various somatic symptoms and insomnia in patients with Hwa-byung. Methods: This is study protocol for a randomized, double-blind, placebo-controlled trial. A total of 44 patients will be randomly assigned to the experimental group or the placebo group in a 1:1 ratio. All medications will be taken orally 3 times per day for 7 consecutive days. The primary outcomes are the mean changes in Patient Health Questionnaire of physical symptoms (PHQ-15) and Insomnia Severity Index (ISI) after the 7 days of administration. The secondary outcomes include the scales to assess stress response, symptoms of Hwa-byung, and state anger. Conclusion: The results of this study will provide high quality and explorative evidence to investigate the effect of Huanglian-jie-du decoction on Hwa-byung.

A case of enlarged uterine myoma on postmenopausal women those who taking H.R.T (호르몬 대체요법 중 증대된 자궁근종 환자 1례의 임상에 관한 연구)

  • Recently we observed and treated a 49-year-old postmenopausal female with enlarged uterine myoma who taking through the hormone replacement therapy(H.R.T). We regard the cause of this case as insufficiency of kidney-yin(腎陰虛), stagnation or the phlegm and pathologic blood(痰瘀阻滯) and H.R.T. ln general cases, myomas grow in size if estrogen and progesterone is present and do not increase in size if estrogen and progesterone levels are low. So if a woman can get to menopause without having symptoms from the myomas, then it is likely that she will never have problems from the growths that require treatment. But Taking postmenopausal estrogen and progestin replacement therapy can cause myomas to grow. So we recommended her to stop the H.R.T. gave herbal-medicine and acupuncture therapy. Thus we could get a good result as follows. The uterine myomas were significantly declined but no change on it's characters. Hot flush and general conditions were also significantly improved And body weight was lessened for about 4.7kg. But vaginal dryness was appeared, So we planed another therapy for that symptom.

  • This paper analyzes file access characteristics of smartphone apps and finds that a large portion of file writes are performed only once. Based on this observation, we present a new buffer cache management scheme that considers this characteristics. Buffer cache improves storage performance by maintaining hot file data in memory thereby servicing subsequent requests without storage accesses. However, it should flush modified data to storage in order to resist system crashes. The proposed scheme evicts cache data that has been written only once upon flushes, thus improving cache space utilization. Simulation experiments show that the proposed scheme improves cache hit ratio by 5-33% and power consumption by 27-92%.

  • The purpose of this study is to examine the medicalizing process of menopanse with literature review, and then is to explore the knowledge of 'menopausal syndrome' critically, focused on four critics on the biomedical model which were suggested by Mishler. Although menopause is a natural biological phenomenon, the view of many medical researchers and practitioners is that menopause is a disease. After synthetic estrogen was developel in 1938, physicians did agree on two basic assumptions : menopausal women should be managed by physicians, and medical intervention should be given. Menopouse was defined as a deficiency disease (estrogen difficiency) by Wilson in 60's and is redefined as a cause of disease(for example, osteoporosis, heart disease) at the present. But the other view of non-medical researcher is that 'menopausal syndrome' as a disease is constructed medically. It was reported that Only hot flush and sweating of physical symptoms experienced by menopausal women, were associated with menopause. Symptoms of menopausal syndrome are also related with symptons of aging. So, it cann't conclude that menopausal syndrome is resulted from menopause, and it cann't be only explained that menopausal syndrome is related causally to estrogen deficiency, and only treatment by ERT or HRT is best relevant. It cann't assume that menopausal syndrome is a common phenomenon to all menopause women, because culture affected on women's experience of menopause.

Symptom Clusters in Women with Gynecologic Cancer (부인암 여성의 증상 클러스터(Symptom Cluster))

  • Purpose: Women with gynecologic cancer often experience various physical and psychological symptoms relating to the cancer and its treatment. The purpose of this study was to identify symptom clusters. Method: A survey was conducted on 184 women with diagnoses of cervical, ovarian or endometrial cancer. Fifty symptoms were assessed for prevalence, severity and interference, and symptom clusters were identified. Cluster analysis was done using SPSS version 12.0. Results: Fatigue was identified as the most prevalent symptom (81.52%), lack of vaginal lubrication (2.26) as the most severe symptom, and lack of vaginal lubrication as the most interfering one (2.15). Identified six clusters were: Anorexia-pain cluster (loss of appetite, taste change, weight loss, appearance change, alopecia, weakness, pain), Fatigue cluster (lack of concentration, lack of memory, fatigue, dry mouth), Urinary-bowel distress cluster (urinary difficulty, constipation), Abdominal discomfort cluster (lower abdominal pain, abdominal pain, bloating), Emotional distress (sadness, anxiety-worry, nervousness, restlessness), and Menopausal cluster (sweating, hot flush, fever). Conclusion: The result of this study provides fundamental data to health care professionals in developing interventions for effective symptom management for women with gynecologic cancer by understanding identified 6 symptom clusters.

A Case Report of Sanhupung Patient Diagnosed of Postpartum Thyroiditis (산후 갑상선염 진단을 받은 산후신통(産後身痛) 환자 치험 1례)

  • Objectives: Postpartum thyroiditis patient's symptoms are very similar to symptoms of postpartum disease which can be called sanhupung. This study is to report the effect of herbal medicine on sanhupung patient diagnosed of postpartum thyroiditis. Methods: We treated a 32-year-old female patient who had symptoms of postpartum thyroiditis and sanhupung with Jaeumganghwa-tang and Jayumkeonbi-tang. Results: After herbal medicine treatment the symptoms of postpartum thyroiditis such as general weakness, hot flush on the palms and soles of the feet, fever pattern of malaria and symptoms of sanhupung such as arthralgia, depression, imsomnia, sweating were improved. Conclusions: This case report shows that herbal medicine can be used to treat patient with postpartum thyroiditis and Jaeumganghwa-tang, Jayumkeonbi-tang is effective for mitigating the symptoms of postpartum thyroiditis. However, patient is still in thyroxine treatment, and additional care is required.

A Study on Characters of Heart Rate Variability in Postpartum Disease Patients (산후풍 환자의 심박변이도 특성 관찰 연구)

  • Purpose: During postpartum period, many women complain multiple pain, sweating, cold hypersensitivity, abdominal pain, hot flush and so on. We called the postpartum disease as San Hu Pung. The San Hu Pung could related with dysautonomia. So we studied the character of the postpartum disease patients as heart rate variability(H.R.V) Methods: The subjects were 63 women who visited Kangnam Kyunghee hospital for postpartum management from February 2006 to June 2007. From history taking, 42 symptomatic women was divided at the postpartum disease group, 21 asymptomatic women was divided at the control group. All women was measured HRV, we studied the difference of HRV between two group. Results: Compared with the control group, SDNN of the disease group was significant low. TP, LF and HF of the disease group were significantly lower than the control group. Conclusion: The difference of HRV between the disease group and the control group was significant. The results of HRV on the disease group could mean that San hu pung is related dysautonomia. The more studies to diagnose San hu pung would be needed.
