입원아동이 지각한 건강과 질병개념에 관한 연구 (A Study on Health/Illness Concepts in Hospitalized Children)

  • The purpose of this study was to explore the health and illness concepts of hospitalized children. The subjects were 129 hospitalized children from 3 to 12 years old in one general hospital. Data were collected through semistructured interviews by authors. This study was conducted from Jun. 1, 2000 to Dec. 31, 2000. Data were coded and categorized by content analysis. The results were as follows : 1. Perceived health concept were physical well-being, food, exercise, powerfulness, emotional stability, obeidence, cleanliness, sleep and ability of social adaptation. 2. Perceived health behavior to maintain health were food, treatment, exercise, cleanliness, obeidence, sleep, emotional stability, power-fulness and psychological stability, physical well-being. 3. Perceived prevention of illness were food, cleanliness, treatment, exercise, obedience, sleep, powerfulness, psychological stability, emotional stability, recreation and ability of social adaptation. 4. Perceived causes of illness were illness, trauma and food. 5. Perceived treatment of illness were treatment, sleep, rest, food, obedience, emotional stability, psychological stability, cleanliness, exercise and powerfulness.

Clinical characteristics and outcomes among pediatric patients hospitalized with pandemic influenza A/H1N1 2009 infection

  • Purpose: The purpose of this article is to describe the clinical and epidemiologic features and outcomes among children hospitalized with pandemic influenza A/H1N1 2009 infection. Methods: We retrospectively reviewed the charts of hospitalized pediatric patients (<18 years) diagnosed with pandemic influenza A/H1N1 2009 infection by reverse-transcriptase polymerase chain reaction at a tertiary hospital in Seoul, Korea, between September 2009 and February 2010. Results: A total of 72 children were hospitalized with pandemic influenza A/H1N1 2009 infection (median age, 6.0 years; range, 2 months to 18 years). A total of 40% had at least 1 underlying medical condition, including asthma (17%), malignancies (19%), and heart diseases (17%). Of the 72 patients, 54 (76%) children admitted with H1N1 infection showed radiographic alterations compatible with pneumonia. There was no significant difference in pre-existing conditions between pandemic influenza A/H1N1 infected patients with or without pneumonia. Children with pandemic influenza A/H1N1 pneumonia were more likely to have a lower lymphocyte ratio (P=0.02), higher platelet count (P=0.02), and higher level of serum glucose (P=0.003), and more commonly presented with dyspnea than did those without pneumonia (P=0.04). Conclusions: No significant differences in age, sex, or presence of preexisting conditions were found between children hospitalized with pandemic influenza A/H1N1 H1N1 influenza infection with pneumonia and those without pneumonia. Higher leukocyte count, higher glucose level, and a lower lymphocyte ratio were associated with the development of pandemic A/H1N1 2009 influenza pneumonia.

호흡기 감염성 질환아 가족의 교육 요구 및 가족이 지각한 간호사의 교육 수행 정도 (The Educational Needs of Families with Respiratory Infected Children and the Degree of Nurses′ Educational Performances Perceived by Families)

  • This study was performed to compare the educational needs of the families of the hospitalized children with respiratory problems and the degree of nurses' educational performances perceived by the families. The subjects consisted of 120 families with respiratory infected children in one university- affiliated hospital in Daegu. Data were collected through self-report questionnaires which were developed to investigate the educational needs of the families of the hospitalized children with respiratory problems and nurses' educational performances perceived by the families. The data were analyzed by an SPSS program. The results were as follows: 1. Among the educational needs of the families, the item of the diagnostic test was ranked as the highest one. Among the degree of nurses' educational performances perceived by the families, the item of the diagnosis was ranked as the highest one. 2. The educational needs of the families were very high, but the degree of nurses' educational performances perceived by families was relatively low. 3. The educational needs of the families were not influenced by the general characteristics of the subjects. These results suggest that the educational needs of the families was different to the degree of nurses' educational performances perceived by the families of the hospitalized children with respiratory problems. Therefore the nursing education program should be based on the educational needs of the families.

입원환아 가족의 간호요구도와 만족도 (Nursing Need and Satisfaction of Families with their Hospitalized Children)

  • This study was done to evaluate the need and satisfaction for nursing care of the families with their hospitalized children. The data were collected through the questionnaire from March 15, 2002 to April 7. Subjects were the 103 families caring for their hospitalized children at pediatric ward in two university hospitals in Daegu. The nursing need instrument was developed by Seo(1999) and modified by the researcher of this study based on the classification of nursing care area(nursing assessment, direct nursing, education and counseling, and facilities and environment). The nursing satisfaction instrument was developed by Wandelt & Ager(1974) and modified by Park(1994) based on classification of nursing care area (psychosocial care, physical care, general care, professional care, and communicative care). The data were analyzed for mean, percent, t-test, ANOVA, and Pearson correlation coefficient using SAS program. The results are summarized as follows: 1. The scores in the nursing need showed in the Direct nursing(3.41±.42), Facilities and Environment(3.38±.46), Education and Counseling (3.35±.40), and Nursing Assessment(3.14±.41) area in order. 2.The scores in the nursing satisfaction showed in the Psychosocial care(3.70±.74), Commu- nicative care(3.60±.72), General care(3.42±.76), Professional care(3.38±.82), and Physical care(3.32±.70) area in order. 3.General characteristics of families which influence on the satisfaction showed a significant difference according to their educational(F=5.63, p=.001) and economical level(F=4.47, p=.006), and hospitalized experience(t=2.30, p=0.02). 4. There was no correlation between the nursing need and the nursing satisfaction.

입원아동 돌봄을 위한 가족중심 순회의 통합적 고찰 (An Integrative Review on Family-Centered Rounds for Hospitalized Children Caring)

  • Purpose: Involvement of families in rounds is one strategy to implement patient- and family-centered care to help families get clear information about their child, and be actively involved in decision making. The purpose of this paper was to identify the major concepts of family-centered rounds for hospitalized children. Methods: We searched five electronic databases for relevant articles and used Whittemore and Knafl's integrative review methods to synthesize the literature. Articles published between June 2003 and January 2016 were reviewed and through full text screening 24 peer-reviewed articles were found that met the selection criteria for this review. Results: Through in-depth discussion and investigation of the relevant literature, four overarching components emerged: (a) cognition of parents and medical staff, (b) effective communication, (c) collaboration of family and medical staff, (d) coaching of medical staff. Conclusion: For successful family-centered rounds positive cognition is important. Appropriate communication skills and consideration of multi-cultural family can lead to effective communication. Offering consistent and transparent information is important for collaboration between family and medical staff. Prior education on family-centered rounds is also important. Four major components have been identified as basic standards for implementing family-centered rounds for hospitalized children.

  • Purpose: The purpose of this study is to develop educational material for parents of hospitalized preschool children who receive nebulizer therapy treatment. The educational material is aimed at providing information on the correct use of nebulizer. Methods: The development of educational material was based on the ADDIE model and consists of an A4-sized leaflet and a video. Results: The leaflet included basic information, procedures, and cautionary information for nebulizer therapy, as well as frequently asked questions. The one minute and fifty second video is accessed via QR code and provides step-by-step instruction on nebulizer therapy. Conclusion: This study recommends that the educational leaflet and video developed for nebulizer therapy use be provided to parents of preschool children hospitalized in the pediatric ward.

학령기 아동의 입원생활 적응 (School aged Children's Adjustment to Hospital Life)

  • Purpose: The study was done to identify adjustment to hospital life of school aged children. This research was designed as a descriptive study. Method: A convenience sample of 186 patients who were hospitalized children, 6~12 years old. The instrument in this study were developed by researcher and constructed to include 5 sub categories about adjustment to hospital life. They ask children to rate each item on 5 Likert scale. The data were analyzed by SPSS Win Program. Result: The mean average score of adjustment to hospital life was 91.75(SD 11.22); the social support revealed the highest value, followed by self-esteem, stress on disease, stress on hospitalization, and defense strategies. There was differences on bibliographic data; hospital date, disease type. The relationship between adjustment and sub categories was significant, especially in the stress on hospitalization and disease, social support and self-esteem. The relationship between coping strategies and stress on diseases was reversed. Conclusion: These results suggest that pediatric nurses need to be aware of the adjustment to hospital life. A development of nursing intervention program may be useful and critical for hospitalized school aged children.

Comparison of four nutritional screening tools for Korean hospitalized children

  • BACKGROUND/OBJECTIVES: Several nutritional screening tools were recently developed to screen the risk of malnutrition in hospitalized children, but have not been validated in Asia. We compared four nutritional screening tools for pediatric patients in evaluating nutritional risks in newly hospitalized children. SUBJECTS/METHODS: Medical records of newly admitted pediatric patients between June 2016 and May 2017 at two tertiary hospitals were reviewed. Initial information by nurses and hospital records by doctors on baseline demographic, clinical, and anthropometric data at admission were collected in all subjects. Nutritional risks were evaluated using four nutritional screening tools including the pediatric nutritional risk score (PNRS), the screening tool for the assessment of malnutrition in pediatrics (STAMP), the paediatric Yorkhill malnutrition score (PYMS), and the screening tools for risk of nutritional status and growth (STRONGkids). RESULTS: A total of 559 patients (310 boys and 249 girls, mean age $6.3{\pm}5.5years$) were recruited. Patients in medical and surgical departments were 469 (83.9%) and 90 (16.1%), respectively. The prevalence of patients at risk of malnutrition were 31.1% for low risk, 52.2% for medium risk, and 16.6% for high risk by PNRS; 11.4%, 39.7%, and 48.8% by STAMP; 26.5%, 25.4%, and 48.1% by PYMS; and 35.6%, 58.9%, and 5.5% by STRONGkids. PNRS versus STRONGkids and STAMP versus PYMS showed moderate agreement (kappa = 0.566 and kappa = 0.495, respectively). PYMS and STAMP revealed a relatively high sensitivity of 87.8% and 77.6% for wasting. CONCLUSION: Different nutritional screening tools revealed considerably different results in evaluating nutritional risks in newly hospitalized children. Since pediatric patients are at risk of malnutrition at admission and during hospitalization, screening tools should be applied properly according to the situation of each hospital.

공공기관 의무기록부에서 관찰된 기아(棄兒)에 관한 고찰 (A Study on Abandoned Children through the Analysis of Medical Records in a Public Hospital)

  • It is the purpose of this study to provide the basic data to stimulate the social understanding of an abandoned child from the medical viewpoint and the viewpoint of administrative management. The data are based on the medical dossier of 305 patients which were hospitalized in 'A' hospital (of Seoul municipal hospital) between 1992 and 1996. The method of analysis is SPSS statistical package and the following results can be obtained 1. The abandoned children were frequently found in the Spring by season, in April by month, on Friday by day, and especially in the night and before an individual home and an medical institution in many cases. 2. The children found in an individual home were younger, handicapped smaller, and hospitalized for shorter period. As they were healthy relatively, parents might hope that they were accepted as foundling foster children. Contrary to above case, the children found in an medical institution were worse, handicapped more, and hospitalized for longer period. Accordingly, parents might hope that they were protected in an public institution. 3. The $69.8\%$ of abandoned children had caught a disease at that time and the $53.8\%$ of them had been handicapped. The most serious disease and handicap were the heart-lung trouble and the cerebral palsy, respectively. 4. The only $60.8\%$ of them were adopted and the most of them were healthy relatively. The $10.2\%$ which have been dead had caught four diseases on the average. As the results of above, the followings can be proposed : 1) It is required that the system connected to administrative management, which can provide the abandoned children turned for the better with the family relations to voluntary servants, be developed. 2) It is required that the system of registration management and plan for support economy, which can support the handicapped birth as the congenital malformation in national aspect, be developed. 3) It is required that to protect abandoned children, the health educational program for childbirth-people be prepared in the unit of a public health center.

정맥주사 시 입원환아가 경험하는 동통에 대한 환아, 어머니, 간호사의 지각 (The Pain Perception of Children, Mothers & Nurses Experienced by Hospitalized Children at the Time of Intravenous Injection)

  • Purpose: The purpose of this study was to provide basic data for hospitalized children under the condition of painful procedure. Method: The data were collected from July 15th to Oct 30, 2003 from 68 hospitalized children suffer from acute disease, 68 mothers take care of children, 14 pediatric ward nurses. To evaluate the degree of pain perception of children, mothers & nurses, face pain rating scale and behavior characteristic were used. Result: The result were as follows. 1) The pain perception score of children, mothers & nurses. child perceived highest as the score of 3.60 2) The most frequent behavioral characteristic children were 'crying(47.1%)' mothers were 'soothing the child(54.4%)' and nurses were 'support not to move(52.9%)' 3) The degree of pain perception of children, mothers and nurses according to subjects' general characteristics and painful procedure, there were stastical difference according to children's age, mothers age and duration of painful procedure 4) There were significant correlation between the degree of pain perception and behavioral characteristics of children, mothers & nurses. Conclusion: Nurses should not underestimate children's pain and need to provide knowledge and information to the child and their parents regarding painful procedure.
