The purpose of this study was to determine the incidence of malnutrition among patients on admission to hospital, to monitor changes in their nutritional status during hospitalization, and to determine the factors which might affect changes in nutritional status. The subjects for the study were patients who were admitted to general medicine for more than one week. Patients suffering from cardiovascular. renal disease, or dehydration were excluded. Nutritional assessment of the patients was performed on admission and nutritional status was reassessed one week and two weeks after admission. The nutritional assessment tool consisted of subjective history taking and anthropometric measurements. Biochemical measurements were performed only on admission. For anthropometric assessment : patients' body weight, subcutaneous skinfolds thickness, % of body fat, body mass index, and lean body mass were measured using caliper or Bio impedance Analyzer. Factors which might influence current nutritional status, like dietary intake, anorexia, nausea, vomiting, diarrhea, sleep disturbance, and number of days of NPO for diagnostic examinations were analyzed. The results are as follows : 1. Of the 59 patients who were studied, 61% were male and 39% female. The nutritional status of all of the 59 subjects was reassessed one week after admission, but it was only done for 22 subjects at two weeks. 2. The anthropometric measurements. including weight body mass index, lean body mass, body fat. and skin fold thickness. were all significantly decreased at one week after admission compared to the values at admission. On the other hand, two weeks after admission, only body weight and abdominal skinfolds thickness were decreased. 3. The subjects reported anorexia for an average of two days, sleep disturbance for two days, and no food intake due to diagnostic test for one day. In the second week of hospitalization, almost none of the patients complained of gastrointestinal symptoms or sleep disturbance except anorexia. Food consumption which was measured based on rice intake was 60% of the food served during the first week of hospitalization, and 66% during the second week of hospitalization. 4. There was no correlation between the subjective nutritional assessment and anthropometric assessment. 5. There was no statistical significance in anthropometric measurements among the patients with various diseases whereas sleep disturbance and no food intake due to various diagnostic test was prominent in patients with gastrointestinal diseases.
Background : The purpose of this research was to evaluate the appropriateness of preoperative hospital days in a tertiary care hospital and to examine the reasons of the inappropriateness, so as to provide basic information and policy for enhancing appropriateness of preoperative hospitalization and benefit of patients and hospital. Methods : The subjects of the research were the 344 patients who received operation among discharged patients during January, 1996 in surgical departments including general surgery, neurosurgery, orthopedic surgery, plastic surgery and ophthalmology. Their medical records were reviewed and appropriateness of hospital days was evaluated by the Appropriateness Evaluation Protocol. Result : The results of evaluating the appropriateness of preoperative hospitalization showed that inappropriate hospital days were 80.8%. The reasons of inappropriate hospital stays were the tests or preparation which could be done in outpatient basis' followed by 'possible tests or preparation on the operation day' and 'cancelation of operation'. Conclusion : In order to shorten the inappropriate preoperative length of stay, it is recommended that lengthening of laboratory running time and doing most of tests necessary for operations on the outpatient basis prior to admission should be considered. In addition, the operation at the same day of hospitalization and usage of day surgery should be encouraged. Finally there should be changes in the inpatient management system and attitudes and behaviors of surgeons to shorten unnecessary preoperative and maximize the benefit for patients and hospital.
Objectives : This study was performed to evaluate the status of psychiatric consultation at a newly opened university hospital, to explore problems related to psychiatric consultation and to contribute for future consultation to be more systematic and more efficient. Methods: The subjects of this study were 284 patients hospitalized at Inha University Hospital between September 1, 1996 and August 31, 1997, who were referred for psychiatric consultation during hospitalization. This study was evaluated demographical characteristics of referred patients, resons for referral, recommendations by psychiatric consultant, clinical diagnoses, psychiatric diagnoses, relationship between referral time and hospitalization following the day of consultation and consultation referral types based upon psychiatrists' reports and patients' medical charts. Results : Most common patients referred to consultation were females aged 20s who visited at the department of internal medicine and related to the risk of suicidal attempts. Mood disorder was the most frequent psychiatric diagnosis, and the most common recommendations were to use psychopharmacological treatment along with outpatient visits. The 30% of subjects were referred to consultation within a day after hospitalization. In the case of consultation referral timing within two days and after three days, there was significant difference between early consultation and shorter hospitalization following the day of consultation. Most frequent type of consultation(29.6%) was categorized as IIIa type(physical complication type as a "Mending" request type). Conclusion : In review of consultation referral timing and consultation referral type of the subjects, who were referred for psychiatric consultation for one year at a newly opened University Hospital, referrals were found to be made within two days after their admission to hospital(50.7%), which was observed to be early request. We found that there was significant association between early consultation and shorter hospitalization following the day of consultation. The physical complication type(29.6%) which belongs to "Mending" request type was found to be the most frequent consultation referral type. This result indicates that more systematic psychiatric consultation is needed for the future.
Every patient, who enters the hospital has a potential for becoming anxious. The control of hospitalization anxiety experienced by hospital patients is considered to be an important factor in the process of recovery from illness. This study was conducted to investigate the relationship between informativeness and hospitalization anxiety in order to give basic data for psycho-social aspect of nursing care for hospitalized patients. One hundred patients admitted to Kyungpook National University Hospital during the period of Jan. to June 1975 were sampled and divided into two groups; fifty of experimental and fifty of control group. The set of informations prepared by the investigator were given additionally to experimental group while the control group only received routine informations. Both groups were rated according to the Hospitalization Informativeness Scale which consisted of 24 questionaries and 4 categories and Hospitalization Anxiety Scale one or two days prior to discharge from the hospital. The results of the study were as follows; 1 . Mean values of Hospitalization Informativeness Scale revealed significant differences at 0.01 level with experimental group showing higher mean value. Age, sex ana educational states aid not influence the mean values of Hospitalization Informativeness Scale in both groups. 2 . The length of hospitalization did not influence significantly on the mean value of Hospitalization Informativeness Scale in both groups. 3. Rank difference correlation coefficiently between mean value of Hospitalization Informativeness Scale and the importance of information the patient's perceived were revealed significant at 0.01 level in all 4 categories such as admission discharge, treatment and nursing activities, diagnostic test, diagnosis and prognosis, health teaching for the patient and family in experimental group. While, only two categories such as treatment and nursing activities and health teaching for the patient and family in control group were revealed significant correlation. 4. Mean value of Hospitalization Anxiety Scale revealed significant difference at 0.05 level with the experimental group showing lower Hospitalization Anxiety Scale. 5. Correlation coefficiently between Hospitalization Informativeness Scale and Hospitalization Anxiety Scale were revealed significant at 0.01 level in experimental group but there was no significant correlation in control group.

Objective: Hospitalization of patients with delirium after visiting the emergency department (ED) is often required. However, the readmission risk after discharge from the ED should also be considered. This study aimed to explore whether (i) immediate hospitalization influences the readmission risk of patients with delirium; (ii) the readmission risk is affected by various risk factors; and (iii) the healthcare cost differs between groups within 28 days of the first ED visit. Methods: Using the National Health Insurance Research Database, the data of 2,780 subjects presenting with delirium at an ED visit from 2000 to 2008 were examined. The readmission risks of the groups of patients (i.e., patients who were and were not admitted within 24 hours of an ED visit) within 28 days were compared, and the effects of the severities of different comorbidities (using Charlson's comorbidity index, CCI), age, gender, diagnosis and differences in medical healthcare cost were analyzed. Results: Patients without immediate hospitalization had a higher risk of readmission within 3, 7, 14, or 28 days of discharge from the ED, especially subjects with more severe comorbidities ($CCI{\geq}3$) or older patients (${\geq}65years$). Subjects with more severe comorbidities or older subjects who were not admitted immediately also incurred a greater healthcare cost for re-hospitalization within the 28-day follow-up period. Conclusion: Patients with delirium with a higher CCI or of a greater age should be carefully considered for immediate hospitalization from ED for further examination in order to reduce the risk of re-hospitalization and cost of healthcare.

Purpose: This study was to examine the change of stress and nursing needs in preterm labor women after hospitalization. Method: Forty seven women with preterm labor who were admitted in two university hospitals participated in the survey and they were completed the questionnaires on admission day and 5th day after admission. Questionnaires included in modified Antepartum Hospital Stressor Inventory (AHSI) and Nursing Needs for Preterm Labor. The collected data were analyzed by paired t-test and Pearson correlation test. Result: The stress level was not decreased significantly after hospitalization. Among them, the stresses about fetus and medical staff were decreased significantly after hospitalization, but those about pregnant women, husband and environment were increased significantly after hospitalization. The difference in the level of nursing needs between the two different days was not significant except the physical needs. The total level of stress and nursing needs were correlated significantly on the 5th day after hospitalization. Conclusion: These results indicate that stress and nursing needs of preterm labor women undergo changes after hospitalization. Therefore, development of the program being followed by stressors which change from day to day and further studies are required in order to examine the decreasing of the stress.
Purpose: The purpose of this study was to describe pregnant women's lived experiences of hospitalization due to preterm labor in Korea. Methods: This qualitative study adopted a phenomenological approach. Individual in-depth interviews were conducted with nine participants, over the age of 20 years, who had been hospitalized for more than 1 week after being diagnosed with preterm labor. All interviews were audio-taped and verbatim transcripts were made for analysis. The data were analyzed following Colaizzi's phenomenological method. Results: The participants' ages ranged from 26 to 36 years, and all were married women. They were hospitalized for 13.1 days on average. Five thematic clusters emerged from the analysis. 'Withstanding hospitalization for the fetus's well-being' describes women's feelings during preterm labor and their endurance during their prolonged hospitalization, rooted in their conviction that the fetus comes first. 'Endless frustration in the hospital' encompasses women's emotions while lying in bed and quietly thinking to themselves. 'Unmet physiological needs' describes participants' awareness of their inability to independently handle human physiological needs given the need for careful and limited movement. 'Gratitude for the support around oneself' reflects the support from family and medical staff. 'Shifting perceptions and accepting one's circumstances' describes accepting hospitalization and making efforts to spend their remaining time in the hospital in a meaningful way. Conclusion: The findings in this study provide a deeper understanding and insights into the experiences of Korean women with preterm labor during hospitalization, underscoring the need to develop interventions for these patients.
Objectives: The purpose of this study was to report the treatment progress of a pregnant women who were treated with Korean medical hospitalization due to threatened abortion in first trimester and plecenta previa in second trimester during pregnancy. Methods: A 36-year-old pregnant woman was hospitalized twice at 9 and 19 weeks of pregnancy due to threatened abortion in first trimester and plecenta previa in second trimester. The patient was prescribed Korean Medicine(Gamidanggui-san, Gamibosaeng-tang) and treated with acupuncture, moxa. Global Assessment(G/A) of bleeding volume and frequency, Numerical Rating Scale(NRS) of abdominal pain, Visual Analogue Scale(VAS) of symptoms was assessed during hospitalization days. Results: Amounts of bleeding, abdominal pain decreased, and other symptoms improved during hospitalization days. The patient maintained her pregnancy until 31st gestational week and gave a birth by Cesarean section. After giving birth, mother and infant are both in good health. Conclusions: This study suggests Korean medicine treatment has effectively reduced uterine bleeding and helped maintain pregnancy.
Purpose: This study was conducted to compare postoperative complications, hospitalization days and treatment expenses to postoperative prophylactic antibiotics administrated to hysterectomy or not. Methods: A retrospective survey study was performed with 128 cases in which elective hysterectomy had undergone. They were divided into two groups by identifying whether postoperative prophylactic antibiotics was administered for hysterectomy: a) one group who received postoperative prophylactic antibiotics and; b) those who did not. Data were collected using the electric medical record at a hospital and analyzed by SPSS 23.0 for $x^2$ test, t-test and ANCOVA. Results:Postoperative complications including wound infection (p=1.000), pneumonia (p=.496), hematoma (p=.530), and pneumoperitoneum (p=.496) showed no significant differences between two groups. Hospitalization days for the prophylactic antibioticsadministrated group were significantly longer than the non-administered for prophylactic antibiotics (p=.004). The treatment expenses of the prophylactic antibiotics-administrated group were significantly higher than those of the non-administered prophylactic antibiotics (F=4.31, p=.040). Conclusion: These results can be provided for the evidence of administrating postoperative prophylactic antibiotics to hysterectomy. Additionally, it can contribute to decreasing the medication errors caused by infrequently administrating postoperative prophylactic antibiotics as well as to lessening likelihood of infection of intravenous injection site.

This study analyzed change of nurse image after hospitalization experience. The data were collected though questionnaire survey on 87 patients, who were, for the first time, hospitalized for 5~7 days at five general hospitals in J city. The survey was performed during July 3~August 2, 2000. The nurse image was analyzed through the instrument conceived by Il-Sim Yang(1998) on the basis of four dimensions ; traditional, social, professional and personal. The collected data were processed through SPSS/WIN to examine t-test, ANOVA and paired t-test. The study results were summarized as follows ; 1. Before hospitalization, patients' score of nurse image was the highest in professional dimension, followed by personal, traditional and social in order. 2. The nurse image before hospitalization showed statistically significant differences in age(p=.009), sex(p=.027) and marital status(p=.000). 3. After hospitalization experience, the score of nurse image was the highest in personal dimension, followed by professional, traditional and social one. 4. The nurse image after hospitalization showed statistically significant differences in marital status(p=.002) only. 5. The difference of nurse image before and after hospitalization experience showed statistical significance in traditional (p=.007) and social (p=.037) dimensions. 6. The score of nurse image was improved in all dimensions after hospitalization experience. In conclusion, hospitalization experience helps improve the nurse image. Therefore, for better improvement of nurse image, it is necessary for nurses to offer their best care to hospitalized patients. In addition, efforts should be made to improve the social image of nurse, which showed lowest score.
