A Study on Regional Medical Utilization Variation of Hospital Inpatients in Korea (우리나라 병원급 이상 의료기관 입원환자의 지역별 의료이용의 변이에 관한 연구)

  • The purpose of this study was to analyze regional medical utilization variation of hospital inpatients and to suggest policy for the allocation of medical service in Korea. We analyzed the relationship among medical service, city size, income level and regional medical utilization variation of hospital inpatients. The patient survey report of 2005 year in Ministry of Health and Welfare was used for this study. To adjust on the factor age and sex, we used direct standardization method. Findings of the research were summarized as follows ; First, standardization discharge rate on patient was different in the district. city type, and number of beds. Second, standardization length of stay of patient was different in region, city type, and number of beds.

Effect of private health insurance on health care utilization in a universal health insurance system: A case of South Korea (민간 의료 보험 가입이 의료 이용에 미치는 영향)

  • Purpose: this study investigates the effect of private health insurance on healthcare utilization. Methodology: For the analysis, we employed the three level nested two part model. Findings: the private health insurance adoption was associated with higher health care utilization. In particular, indemnity and fixed insurances adoption was associated with higher probability of outpatient visit, the number of outpatient visit and outpatient cost. While indemnity insurance adoption was associated with higher inpatient admission probability and inpatient days, fixed insurance adoption was associated only with higher inpatient admission probability. Practical Implications: indemnity and fixed insurance adoption were related with the adverse selection as well as moral hazard.

  • Objectives: This study aimed to analyze the healthcare utilization behavior of patients visiting the department of Korean internal medicine in the Korean medicine hospital of Wonkwang University in Jeon-ju from July 2018 to June 2021. Methods: We retrospectively analyzed the medical records of 26,108 patients and sorted the data by period, month, visiting types, new or returning types, sex, and age group. IBM SPSS 26.0 and the R 4.05 'changepoint' package were used with various statistical methods, such as Independent t-test, Mann-Whitney test, Chi-square test, Simple regression analysis. The P-value was set at 0.05. Results and Conclusions: Females outnumbered males regardless of period, and the ratio of females fell after COVID-19. Regardless of visiting types, patients in their 50s, 60s, and 70s outrated any other age group. The average number of females among the returning patients decreased significantly after COVID-19, but did not in males. Outpatients under 10 and in their 10s decreased significantly after COVID-19, as did inpatients in their 40s and 60s. The average duration of hospitalization was extended significantly after COVID-19. The number of outpatients and inpatients decreased as time passed after COVID-19. We expect that the results of this study will be used as reference materials in analyzing the effects of COVID-19 on healthcare utilization.

Variation of the Medical Service Utilization of the Dead by Cancers (암 사망자의 의료이용 변이)

  • This study focused on finding the variation of medical service utilization and medical payments of the patients died by three, cancers, stomach, breast, and colon cancer. For this study, data of the one-year episodes of the health insurance subscribers died in 2004 were selected. The frequency of medical visits, the lengths of slays, the days of outpatient visits, the total period of medical services and the total medical payments were compared by the characteristics of the suppliers and utilizers. The data of the patients died by cerebrovascular diseases and cancer in 2004 were selected. To select the dead by cerebrovascular diseases and cancer in 2004, were matched the 2004 reimbursement data of all medical institutions to the data of funeral fee payment by the National Health Insurance Corporation from January 2004 to May 2005 for the death in 2004. The results of the analysis were as follow. The variation of medical service utilization of the dead by cancers were not small in Korea. The current study found that the variation of medical care utilization was influenced by the factors of suppliers, such as types and locations of medical institutions and the factors of users, such as sex and age. It was suspected that the reimbursement by fee-for-service contributed to the variation quite a lot, but we could not compare the variation between the different reimbursement systems in Korea. The results of the study suggested that tile factors of suppliers and utilizers should he reviewed to reduce the under use and over use expressed by variations of medical service utilization. The processes of care, effective communication and management system should be investigated for the equity of medical service utilization. Additionally, prospective payment could he recommended to reduce the high variation of medical service Use. To find the variation caused by under use and over use, further study need to control the severity of diseases, socio-economic status of the users and the system factors.

Functional Status and Health Care Utilization among Elders with Hip Fracture Surgery from a Fall (낙상후 대퇴골절 수술 노인의 신체기능, 의료이용 및 낙상의 두려움)

  • Purpose: The purposes of this study were to examine 1) functional status at 2 months after hip fracture surgery 2) health care utilization after a fall episode and 3) fear of falling experienced during first 2 months after a fall episode. Method: With a convenient sample of 99 elderly from six university or general hospitals with hip fracture from a fall, data were collected at 2-3 days before discharge and at 2 months after hip fracture surgery. Result: 1) At 2 months after hip fracture from a fall, significant proportion (25.3%) of elderly was not able to walk indoors. 2) Average length of hospital stay was 27.6 days with a range of 8 to 86 days. About 51% subjects received physical therapy during hospital stay, and only 6.1% subjects received physical therapy following discharge from the hospital. 3) Significant proportion (72.7%) had fear of falling after the fall episode. About 51% reported that they restricted their activities because they had fear of falling. Conclusion: Fall is a dreaded event which result in loss of independence and restriction of activity. Development and application of fall prevention program is critical especially for those with risk factors of fall.

Split orthodontic airway plate: An innovation to the utilization method of conventional orthodontic airway plate for neonates with Robin sequence

  • Since the emergence of neonatal infant orthodontics for treatments of cleft lip and palate with or without Robin sequence (RS) in Europe in the 1950s, advancements in design and scope of its application have been remarkable. As the first institution to adopt orthodontic airway plate (OAP) treatment in the United States in 2019, we saw a need for innovation of the original design to streamline the most labor-intensive and time-consuming aspects of OAP utilization. A solution is introduced using a systematic split expansion mechanism to re-size the OAP periodically to accommodate the neonate's maxillary growth. To date, seven RS patients have received this modified treatment protocol at our institution. Each patient completed full treatment using only one OAP. This innovative utilization method is aptly named the split orthodontic airway plate (S-OAP). Details of the S-OAP and its modifications from conventional OAP are reported.

Changes on Hospital-based Home Care Services Utilization After Long-term Care Insurance Launch (노인장기요양보험제도 도입 후 의료기관 가정간호 이용실태 변화)

  • This study was to address changes on hospital-based home care utilization after long-term care insurance(LTCI) was launched. National electronic data information(EDI) on hospital-based home care from Health Insurance Review Agency in 2007.7~2008.6(prior to LTCI) and in 2009(posterior to LTCI) was analyzed. After the launch of long-term care insurance, 40 hospital-based home health care agencies(HHCA) were diminished and regions not having any HHCA were increased from 53% to 59%. Hospital-based home care utilization was decreased in the elderly(clients 13.4%, visits 20.9%) as well as non-elderly(clients 3.5%, visits 3.9%). It is presumed that diminished HHCAs result in decreased accessibility to hospital-based home health care for non-elderly. The clients, visits, and reimbursed cost per agency were not changed. It is presumed that small agencies were closed already. The total reimbursed cost per agency in 2009 was 121,850,000 won. Results suggest that the government has to give support to open more HHCA to increase the accessibility for non-elderly. Also, hospital-based home care services utilization has to be monitoring regularly.

Developing Innovation Index of Hospital Service Using 6 Sigma and SERVQUAL (6 시그마와 SERVQUAL을 활용한 병원서비스 혁신지표 개발)

  • Purpose: The purpose of this study is to develop innovation index of hospital service integrating 6 sigma and SERVQUAL. Methods: This study used DMA(Define, Measure and Analysis) from 6 sigma and 5 Factors from SEVQUAL. To test data, chi-squire text, association analysis and behavior analysis was conducted. Results: This study indicated the management index through CTQ (Critical to Quality) and Chosen few X using 6 sigma process. Finally, And this study developed 5 Factors; Equipment Utilization in Tangibility, Ratio of Patients/Disease/Behavior/Treatment in Reliability, Survival RAte, Canselation Rate of Reservation, Churn Rate, Interval of Treatment and Confidence in Responsiveness, Frequency of Patients/Disease/Behavior/Treatment in Assurance and Contrast to Best Department/Best Doctor/Best Doctor in Faculty/Average of Mine in Empathy. Conclusion: This study developed innovation index of hospital service. Managing this index, hospital is able to achieve the decline of total treatment cycle, adjustment of patients behavior and increase of equipment utilization. Ultimately, hospital is able to accomplish innovation of healthcare service.

Determinants Influencing the Utilization of the Rural Health Sub-centers (농촌지역 보건지소 이용에 영향을 미치는 요인 분석)

  • To analyze of determinants influencing the utilization of the rural health sub-centers (HSCs), 116 of 144 HSCs in Kyong Gi Do, were selected for this study. The self-administered questionnaire covering the environment and the characteristics of doctors working in the HSCs was sent to HSCs by mail. 105 questionnaires were returned of which 88 were completed and use in the study. The dependant variable was the total number of medical care visits to the HSCs from January 1, 1990 to March 31, 1990. Data was analyzed by multiple regression analysis. The results were as follows : First, the more time required to set from the HSCs to nearest hospital or clinic, the higher the utilization of the HSCs. Second, the more geographically accessible the HSCs was, the more utilization the HSCs. Third, the older of the HSCs doctors were, the more utilization of the HSCs. Fourth, the higher frequency of bus service from the HSCs to town, the more utilization of the HSCs. Fifth, the more time required from the HSCs to town, the more utilization of the HSCs. Therefore, the determinants influencing the utilization of the rural HSCs in Kyong-Gi Do, are mostly geographical accessibility of medical care.
